Neoadjuvant chemotherapy plus camrelizumab for locally advanced cervical cancer (NACI study): a multicentre, single-arm, phase 2 trial
Locally advanced cervical cancer constitutes around 37% of cervical cancer cases globally and has a poor prognosis due to limited therapeutic options. Immune checkpoint inhibitors in the neoadjuvant setting could address these challenges. We aimed to investigate the efficacy and safety of neoadjuvant chemo-immunotherapy for locally advanced cervical cancer. In this single-arm, phase 2 trial, which was done across eight tertiary hospitals in China, we enrolled patients aged 18–70 years with untreated cervical cancer (IB3, IIA2, or IIB/IIIC1r with a tumour diameter ≥4 cm [International Federation of Gynecology and Obstetrics, 2018]) and an Eastern Cooperative Oncology Group performance status of 0 or 1. Eligible patients underwent one cycle of priming doublet chemotherapy (75–80 mg/m2 cisplatin, intravenously, plus 260 mg/m2 nab-paclitaxel, intravenously), followed by two cycles of a combination of chemotherapy (cisplatin plus nab-paclitaxel) on day 1 with camrelizumab (200 mg, intravenously) on day 2, with a 3-week interval between treatment cycles. Patients with stable disease or progressive disease received concurrent chemoradiotherapy, and patients with a complete response or partial response proceeded to radical surgery. The primary endpoint was the objective response rate, by independent central reviewer according to Response Evaluation Criteria in Solid Tumours, version 1.1. Activity and safety were analysed in patients who received at least one dose of camrelizumab. This study is registered with ClinicalTrials.gov, NCT04516616, and is ongoing. Between Dec 1, 2020, and Feb 10, 2023, 85 patients were enrolled and all received at least one dose of camrelizumab. Median age was 51 years (IQR 46–57) and no data on race or ethnicity were collected. At data cutoff (April 30, 2023), median follow-up was 11·0 months (IQR 6·0–14·5). An objective response was noted in 83 (98% [95% CI 92–100]) patients, including 16 (19%) patients who had a complete response and 67 (79%) who had a partial response. The most common grade 3–4 treatment-related adverse events during neoadjuvant chemo-immunotherapy were lymphopenia (21 [25%] of 85), neutropenia (ten [12%]), and leukopenia (seven [8%]). No serious adverse events or treatment-related deaths occurred. Neoadjuvant chemo-immunotherapy showed promising antitumour activity and a manageable adverse event profile in patients with locally advanced cervical cancer. The combination of neoadjuvant chemo-immunotherapy with radical surgery holds potential as a novel therapeutic approach for locally advanced cervical cancer. EPHA2 mediates PDGFA activity and functions together with PDGFRA as prognostic marker and therapeutic target in glioblastoma
Platelet-derived growth subunit A (PDGFA) plays critical roles in development of glioblastoma (GBM) with substantial evidence from TCGA database analyses and in vivo mouse models. So far, only platelet-derived growth receptor α (PDGFRA) has been identified as receptor for PDGFA. However, PDGFA and PDGFRA are categorized into different molecular subtypes of GBM in TCGA_GBM database. Our data herein further showed that activity or expression deficiency of PDGFRA did not effectively block PDGFA activity. Therefore, PDGFRA might be not necessary for PDGFA function.To profile proteins involved in PDGFA function, we performed co-immunoprecipitation (Co-IP) and Mass Spectrum (MS) and delineated the network of PDGFA-associated proteins for the first time. Unexpectedly, the data showed that EPHA2 could be temporally activated by PDGFA even without activation of PDGFRA and AKT. Furthermore, MS, Co-IP, in vitro binding thermodynamics, and proximity ligation assay consistently proved the interaction of EPHA2 and PDGFA. In addition, we observed that high expression of EPHA2 leaded to upregulation of PDGF signaling targets in TCGA_GBM database and clinical GBM samples. Co-upregulation of PDGFRA and EPHA2 leaded to worse patient prognosis and poorer therapeutic effects than other contexts, which might arise from expression elevation of genes related with malignant molecular subtypes and invasive growth. Due to PDGFA-induced EPHA2 activation, blocking PDGFRA by inhibitor could not effectively suppress proliferation of GBM cells, but simultaneous inhibition of both EPHA2 and PDGFRA showed synergetic inhibitory effects on GBM cells in vitro and in vivo. Taken together, our study provided new insights on PDGFA function and revealed EPHA2 as a potential receptor of PDGFA. EPHA2 might contribute to PDGFA signaling transduction in combination with PDGFRA and mediate the resistance of GBM cells to PDGFRA inhibitor. Therefore, combination of inhibitors targeting PDGFRA and EHA2 represented a promising therapeutic strategy for GBM treatment.

Enhancing english oral translation through cross-modal learning and synchronous optimization
Oral translation in English serves as a critical conduit for international communication and cultural exchange. However, the prevalent variations in pronunciation and the rapid pace of spoken language currently impede the efficacy of synchronous translation methods. To improve the quality and efficiency of synchronous oral translation, this paper explores the integration of cross-modal semantic understanding and synchronous enhancement specifically for English oral translation. This exploration commences with the implementation of a cross-modal translation scenario. Subsequently, the text sequence derived from this process is amalgamated with the original speech features via Bidirectional Encoder Representations from Transformers (BERT). The cross-information between modalities is explored, and linear transformation optimization is performed on the self-attention mechanism in Transformer to achieve context-awareness and understanding of oral-transcribed text. In conclusion, the integration of dynamic time warping (DTW) enhances real-time synchronization between speech and text, thereby improving translation fluency. Experimental results reveal that, when compared to the existing bilingual attention neural machine translation (NMT) model and the context-aware NMT model, the model proposed in this study yields an average bilingual evaluation understudy (BLEU) score that is 9.3% and 26.9% higher, respectively. Furthermore, its synchronization speed surpasses that of the other two models by 17.9% and 16.8%, respectively. These findings suggest that the fusion model, which incorporates context-awareness and an attention mechanism in cross-modal translation, can significantly elevate the quality and efficiency of English oral translation, offering a novel approach to the synchronous translation of spoken English.

يتناول كتاب (مبادئ الحكم في الصين القديمة) والذي قامه بتأليفه (وي تشينغ) في حوالي (211) صفحة من القطع المتوسط موضوع (تاريخ الصين قديم) مستعرضا المحتويات التالية : قبل أكثر من ألف وربعمائة عام، أصدر الإمبراطور تاي تسونغ إمبراطور أسرة تانغ الملكية (618-907) مرسوما بأن يجمع أربعة من أشهر علماء الصين وسياسييها وأمهرهم وقتها المعارف التاريخية حول مبادئ الحكم الإمبراطوري من الكتب الكلاسيكية القديمة الستة، والمجموعات التاريخية الأربع، والمئات من مؤلفات المدارس الفكرية الصينية، وأن يرتبوها ويستخرجوا منها الدروس الاكثر اهمية حول تهذيب النفس وإصلاحها، وإدارة العائلة، والحكم الجيد للبلاد، وسبل جلب السلام للعالم. وكانت النتيجة مجموعة عنوانها \"تشيونشو تشيياو\" التي أقتبست بعناية من أربعة عشر ألف كتاب، وتسع وثمانين ألف مخطوطة من الكتابات القديمة، بإجمالي خمسمائة ألف فقرة مكتوبة، تغطي خمسة وشتين صنفا من الكتب. وقد ساعد هذا الكتاب الإمبراطور تاي سونغ والكثير من الأباطرة من بعده في أمور الحكم، ويعود الفضل في تحقيق الرخاء والسلام في المراحل اللاحقة بالصين القديمة بنسبة كبيرة إلى هذا الكتاب.
Deep spatio-temporal graph convolutional network for police combat action recognition and training assessment
Traditional police combat training relies heavily on subjective evaluation by human instructors, which lacks consistency and comprehensive coverage of complex movement patterns in real-world scenarios. This paper presents an enhanced deep spatio-temporal graph convolutional network (ST-GCN) framework specifically designed for automated police combat action recognition and quality assessment. The proposed method incorporates adaptive graph topology learning mechanisms that dynamically adjust spatial connectivity patterns based on action-specific joint relationships, multi-modal fusion strategies combining skeletal and RGB video data for robust recognition under diverse environmental conditions, and comprehensive quality assessment algorithms providing objective evaluation of technique execution. The enhanced ST-GCN architecture features attention-guided feature extraction, curriculum learning-based training strategies, and real-time processing capabilities suitable for practical deployment in training facilities. Experimental validation on a comprehensive police combat dataset demonstrates superior performance with 96.7% recognition accuracy across twelve action categories and real-time processing at 42.8 frames per second. The multi-dimensional evaluation framework successfully assesses action completion, standardization compliance, and movement fluency, providing immediate feedback for skill development. The proposed system offers significant improvements over conventional approaches, enabling standardized evaluation criteria, data-driven curriculum development, and enhanced training effectiveness for law enforcement personnel.

A phase 1, open-label study of LCAR-B38M, a chimeric antigen receptor T cell therapy directed against B cell maturation antigen, in patients with relapsed or refractory multiple myeloma
Background Chimeric antigen receptor (CAR) T cell therapy has demonstrated proven efficacy in some hematologic cancers. We evaluated the safety and efficacy of LCAR-B38M, a dual epitope-binding CAR T cell therapy directed against 2 distinct B cell maturation antigen epitopes, in patients with relapsed/refractory (R/R) multiple myeloma (MM). Methods This ongoing phase 1, single-arm, open-label, multicenter study enrolled patients (18 to 80 years) with R/R MM. Lymphodepletion was performed using cyclophosphamide 300 mg/m 2 . LCAR-B38M CAR T cells (median CAR+ T cells, 0.5 × 10 6 cells/kg [range, 0.07 to 2.1 × 10 6 ]) were infused in 3 separate infusions. The primary objective is to evaluate the safety of LCAR-B38M CAR T cells; the secondary objective is to evaluate the antimyeloma response of the treatment based on the general guidelines of the International Myeloma Working Group. Results At data cutoff, 57 patients had received LCAR-B38M CAR T cells. All patients experienced ≥ 1 adverse events (AEs). Grade ≥ 3 AEs were reported in 37/57 patients (65%); most common were leukopenia (17/57; 30%), thrombocytopenia (13/57; 23%), and aspartate aminotransferase increased (12/57; 21%). Cytokine release syndrome occurred in 51/57 patients (90%); 4/57 (7%) had grade ≥ 3 cases. One patient reported neurotoxicity of grade 1 aphasia, agitation, and seizure-like activity. The overall response rate was 88% (95% confidence interval [CI], 76 to 95); 39/57 patients (68%) achieved a complete response, 3/57 (5%) achieved a very good partial response, and 8/57 (14%) achieved a partial response. Minimal residual disease was negative for 36/57 (63%) patients. The median time to response was 1 month (range, 0.4 to 3.5). At a median follow-up of 8 months, median progression-free survival was 15 months (95% CI, 11 to not estimable). Median overall survival for all patients was not reached. Conclusions LCAR-B38M CAR T cell therapy displayed a manageable safety profile and demonstrated deep and durable responses in patients with R/R MM.
