Dupuytren’s Disease

Dupuytren’s disease is a benign contractile disorder of the hand. The condition commonly affects older men of Celtic descent. Although fibroproliferation and collagen alteration play a role in its etiology, defining a cause remains elusive. Nonoperative intervention for advanced disease has shown only short-term benefit. Therefore, open fasciectomy has become the mainstay of treatment. Associated morbidity and recurrence have prompted investigation into less invasive techniques, including needle aponeurotomy and enzymatic fasciotomy. Data from phase III studies using injectable collagenase are changing treatment algorithms. Postoperative rehabilitation includes nighttime splinting and immediate active range of motion exercises to facilitate return to function.

Existential formulations of therapeutic practice

Existential therapy is characterized by a strong tradition of sceptical engagement with the fundamental and basic beliefs of psychotherapy. Existential therapy's critical stance towards much that is practised under the title of \"developing formulations\" arises also from its commitment to forms of practice that privilege understanding over explanation. Existential therapists have frequently taken issue with the foundational belief that the essence of therapeutic practice is the provision of a specific treatment to a client in order to achieve a specified outcome. Rather than notions of \"treatment\" or \"intervention\", existential therapy has attempted to maintain a focus upon the essence of therapy as \"attending to\". Therapy from an existential perspective is primarily an encounter or meeting between two individuals where the focus remains on the exploration, clarification, and challenge of the client's experience of living. E. Spinelli employs the term \"worlding\" to describe the interrelationally based, process-like, and ever shifting experience of existence that forms the focus for existential exploration.

A review of the research literature reveals that there is no generally agreed upon definition of treatment failure. Historically, many of the types of problems and difficulties that have been associated with treatment failure or drop out have been understood with reference to the concept of “resistance”. A contemporary Cognitive Behavioural Therapy (CBT) view on the nature of therapeutic resistance strongly support a view of these phenomena as “understandable”, “adaptive” and as “making sense” from the perspective of the client at some level. The procedural competence of being able to employ CBT theory to devise tentative hypotheses or formulations of what might be maintaining the resistance is an important aspect of developing appropriate flexibility and artistry as a CBT therapist. Difficulties with resistance and therapy interfering behaviours often escalate to the point that a rupture in the working alliance occurs.

Hot Cognition in Sexual Attraction

This chapter discusses ′Beckian′ cognitive behavioural therapy (CBT). Dilemmas around sexual attraction between therapist and client are far more likely to become activated in therapies that are of a long‐term duration. Theoretical and research base of CBT has continued to be developed, and the approach has been extended to work with clients presenting with more complex and enduring difficulties. A primary metaphor for CBT has been the ‘mind as computer’ as it has also been for cognitive science more broadly. The contribution of existential‐phenomenological philosophy to CBT practitioners is the foregrounding of the inevitable aspects of human existence itself. The chapter also discusses emotional response to sexual attraction in therapy, the training methods to develop effective CBT therapists, and the clinical strategies for the management of sexual attraction in therapy. The move towards some of the so‐called third‐wave CBT approaches opens the model towards a more Dionysian perspective.
