Sustaining the drive to overcome the global impact of neglected tropical diseases : second WHO report on neglected tropical diseases
The second WHO report on neglected tropical diseases builds on the growing sense of optimism_x000D__x000D_ generated by the 2012 publication of the WHO Roadmap. Commitments on the_x000D__x000D_ part of ministries of health in endemic countries global health initiatives funding_x000D__x000D_ agencies and philanthropists have escalated since 2010 as have donations of_x000D__x000D_ medicines from pharmaceutical companies and the engagement of the scientific_x000D__x000D_ community. _x000D__x000D_ This report marks a new phase and assesses opportunities and obstacles in the_x000D__x000D_ control elimination and eradication of several of these diseases. Unprecedented_x000D__x000D_ progress over the past two years has revealed unprecedented needs for_x000D__x000D_ refinements in control strategies and new technical tools and protocols. The_x000D__x000D_ substantial increases in donations of medicines made since the previous report_x000D__x000D_ call for innovations that simplify and refine delivery strategies. _x000D__x000D_ However some diseases including especially deadly ones like human African_x000D__x000D_ trypanosomiasis and visceral Leishmaniasis remain extremely difficult and costly_x000D__x000D_ to treat. The control of Buruli ulcer Chagas disease and yaws is hampered by_x000D__x000D_ imperfect technical tools although recent developments for yaws look promising._x000D__x000D_ The report highlights progress against these especially challenging diseases _x000D__x000D_ being made through the development of innovative and intensive management_x000D__x000D_ strategies. _x000D__x000D_ Innovations in vector control deserve more attention as playing a key part in_x000D__x000D_ reducing transmission and disease burden especially for Dengue Chagas disease_x000D__x000D_ and the Leishmaniases. _x000D__x000D_ Achieving universal health coverage with essential health interventions for_x000D__x000D_ neglected tropical diseases will be a powerful equalizer that abolishes distinctions_x000D__x000D_ between the rich and the poor the young and the old ethnic groups and women_x000D__x000D_ and men.
COVID-19 Vaccine Acceptance among Low- and Lower-Middle-Income Countries: A Rapid Systematic Review and Meta-Analysis
Widespread vaccination against COVID-19 is critical for controlling the pandemic. Despite the development of safe and efficacious vaccinations, low-and lower-middle income countries (LMICs) continue to encounter barriers to care owing to inequitable access and vaccine apprehension. This study aimed to summarize the available data on COVID-19 vaccine acceptance rates and factors associated with acceptance in LMICs. A comprehensive search was performed in PubMed, Scopus, and Web of Science from inception through August 2021. Quality assessments of the included studies were carried out using the eight-item Joanna Briggs Institute Critical Appraisal tool for cross-sectional studies. We performed a meta-analysis to estimate pooled acceptance rates with 95% confidence intervals (CI). A total of 36 studies met the inclusion criteria and were included in the review. A total of 83,867 respondents from 33 countries were studied. Most of the studies were conducted in India (n = 9), Egypt (n = 6), Bangladesh (n = 4), or Nigeria (n = 4). The pooled-effect size of the COVID-19 vaccine acceptance rate was 58.5% (95% CI: 46.9, 69.7, I2 = 100%, 33 studies) and the pooled vaccine hesitancy rate was 38.2% (95% CI: 27.2–49.7, I2 = 100%, 32 studies). In country-specific sub-group analyses, India showed the highest rates of vaccine acceptancy (76.7%, 95% CI: 65.8–84.9%, I2= 98%), while Egypt showed the lowest rates of vaccine acceptancy (42.6%, 95% CI: 16.6–73.5%, I2= 98%). Being male and perceiving risk of COVID-19 infection were predictors for willingness to accept the vaccine. Increasing vaccine acceptance rates in the global south should be prioritized to advance global vaccination coverage.
Strengthening Primary Health-Care Services to Help Prevent and Control Long-Term (Chronic) Non-Communicable Diseases in Low- and Middle-Income Countries
The prevalence of long-term (chronic) non-communicable diseases (NCDs) is increasing globally due to an ageing global population, urbanization, changes in lifestyles, and inequitable access to healthcare. Although previously more common in high- and upper-middle-income countries, lower-middle-income countries (LMICs) are more affected, with NCDs in LMICs currently accounting for 85– 90% of premature deaths among 30– 69 years old. NCDs have both high morbidity and mortality and high treatment costs, not only for the diseases themselves but also for their complications. Primary health care (PHC) services are a vital component in the prevention and control of long-term NCDs, particularly in LMICs, where the health infrastructure and hospital services may be under strain. Drawing from published studies, this review analyses how PHC services can be utilized and strengthened to help prevent and control long-term NCDs in LMICs. The review finds that a PHC service approach, which deals with health in a comprehensive way, including the promotion, prevention, and control of diseases, can be useful in both high and low resource settings. Further, a PHC based approach also provides opportunities for communities to better access appropriate healthcare, which ensures more significant equity, efficiency, effectiveness, safety, and timeliness, empowers service users, and helps healthcare providers to achieve better health outcomes at lower costs.
Impact of climate change on cereal production: evidence from lower-middle-income countries
This study empirically examines the impact of climate change on cereal production in selected lower-middle-income countries with a balanced panel dataset spanning 1971–2016. The study uses average annual temperature and rainfall to measure climate change. Besides this, CO 2 emissions, cultivated land under cereal production, and rural population are used as the control variables. Second-generation unit root tests, i.e., CIPS and CADF, are used to test the stationarity of the variables. Feasible generalized least square (FGLS) and fully modified ordinary least square (FMOLS) models are used to achieve the objective. Pedroni cointegration test confirms the presence of cointegration between cereal production and climate change variables. The findings show that a rise in the temperature reduces cereal production in lower-middle-income countries. In contrast, rainfall and CO 2 emissions have a positive effect on cereal production. For robustness purpose, the Driscoll-Kraay standard regression and dynamic ordinary least square (DOLS) models have also found similar results. Dumitrescu-Hurlin test has found the bidirectional causality of cereal production with temperature and CO 2 emissions. Also, unidirectional causality is running from rainfall and rural population to cereal production. The adverse effects of temperature on cereal production are likely to pose severe implications for food security. The paper recommends that governments of the sample countries should research and develop heat-resistant varieties of cereal crops to cope with the adverse effects of temperature on cereal production and ensure food security.
Prevalence of osteoarthritis in lower middle- and low-income countries: a systematic review and meta-analysis
Evidence from the Global Burden of Disease studies suggests that osteoarthritis (OA) is a significant cause of disability globally; however, it is less clear how much of this burden exists in low-income and lower middle-income countries. This study aims to determine the prevalence of OA in people living in low-income and lower middle-income countries. Four electronic databases (MEDLINE, EMBASE, CINAHL and Web of Science) were systematically searched from inception to October 2018 for population-based studies. We included studies reporting the prevalence of OA among people aged 15 years and over in low-income and lower middle-income countries. The prevalence estimates were pooled across studies using random effects meta-analysis. Our study was registered with PROSPERO, number CRD42018112870.The search identified 7414 articles, of which 356 articles were selected for full text assessment. 34 studies were eligible and included in the systematic review and meta-analysis. The pooled prevalence of OA was 16·05% (95% confidence interval (CI) 12·55–19·89), with studies demonstrating a substantial degree of heterogeneity (I2 = 99·50%). The pooled prevalence of OA was 16.4% (CI 11·60–21.78%) in South Asia, 15.7% (CI 5·31–30·25%) in East Asia and Pacific, and 14.2% (CI 7·95–21·89%) in Sub Saharan Africa. The meta-regression analysis showed that publication year, study sample size, risk of bias score and country-income categories were significantly associated with the variations in the prevalence estimates. The prevalence of OA is high in low-income and lower middle-income countries, with almost one in six of the study participants reported to have OA. With the changing population demographics and the shift to the emergence of non-communicable diseases, targeted public health strategies are urgently needed to address this growing epidemic in the aging population.
Optimizing Japanese Entrepreneurship for Startup Growth and Sustainability in ASEAN Lower-Middle-Income Economies
This study investigates how Japanese entrepreneurship contributes to the strengthening of startup ecosystems in lower-middle-income ASEAN economies. Even with the dynamic expansion of these economies, they continue to grapple with historic challenges such as the Middle-Income Trap, institutional weaknesses, and limited access to capital. Japanese firms contribute to the enhancement of the scalability and competitiveness of ASEAN startups by implementing production management strategies—including lean concepts, digitalization, and cross-cultural collaboration. Through a qualitative examination of Japanese-ASEAN partnerships, the study identifies key success factors as technology transfer, structured business processes, and adaptation to markets. While Japanese investment and management expertise increase the effectiveness of operations, cultural disparities and regulatory mismatches remain significant obstacles to seamless collaboration. To address the challenges, the study proposes integrative solutions for conciliating Japanese models of production with ASEAN business culture with a view to enabling sustainable entrepreneurial growth. Applying the Grounded Theory Method, the research adds theory to international entrepreneurship and production management literature while also offering practitioner implications. By optimizing Japanese investment trends and institutional alignment, the study offers policymakers, investors, and entrepreneurs’ actionable recommendations to strengthen ASEAN startup ecosystems. Enhancing Japanese-ASEAN economic collaboration is imperative to foster innovation and long-term economic development in the region.
Systemic Inflammation and the Increased Risk of Inflamm-Aging and Age-Associated Diseases in People Living With HIV on Long Term Suppressive Antiretroviral Therapy
The ART program in low- and middle-income countries (LMIC) like India, follows a public health approach with a standardized regimen for all people living with HIV (PLHIV). Based on the evidence from high-income countries (HIC), the risk of an enhanced, and accentuated onset of premature-aging or age-related diseases has been observed in PLHIV. However, very limited data is available on residual inflammation and immune activation in the populations who are on first-generation anti-HIV drugs like zidovudine and lamivudine that have more toxic side effects. Therefore, the aim of the present study was to evaluate the levels of systemic inflammation and understand the risk of age-associated diseases in PLHIV on long-term suppressive ART using a large number of biomarkers of inflammation and immune activation. Blood samples were obtained from therapy naïve PLHIV (Pre-ART, = 43), PLHIV on ART for >5 years (ART, = 53), and HIV-negative healthy controls (HIVNC, = 41). Samples were analyzed for 92 markers of inflammation, sCD14, sCD163, and telomere length. Several statistical tests were performed to compare the groups under study. Multivariate linear regression was used to investigate the associations. Despite a median duration of 8 years of successful ART, sCD14 ( < 0.001) and sCD163 ( = 0.04) levels continued to be significantly elevated in ART group as compared to HIVNC. Eleven inflammatory markers, including 4E-BP1, ADA, CCL23, CD5, CD8A, CST5, MMP1, NT3, SLAMF1, TRAIL, and TRANCE, were found to be significantly different ( < 0.05) between the groups. Many of these markers are associated with age-related co-morbidities including cardiovascular disease, neurocognitive decline and some of these markers are being reported for the first time in the context of HIV-induced inflammation. Linear regression analysis showed a significant negative association between HIV-1-positivity and telomere length ( < 0.0001). In ART-group CXCL1 ( = 0.048) and TGF-α ( = 0.026) showed a significant association with the increased telomere length and IL-10RA was significantly associated with decreased telomere length ( = 0.042). This observation warrants further mechanistic studies to generate evidence to highlight the need for enhanced treatment monitoring and special interventions in HIV-infected individuals.
Awareness, Risk Perception, and Stress during the COVID-19 Pandemic in Communities of Tamil Nadu, India
The health and economic consequences of the COVID-19 pandemic is expected to disproportionately impact residents of lower-middle income countries. Understanding the psychological impact of the pandemic is important to guide outreach interventions. In this study, we examined people’s awareness of COVID-19 symptoms, risk perception, and changes in behaviors and stress levels during the lockdown in peri-urban Tamil Nadu India. Field workers conducted phone call surveys (included n = 2044) in 26 communities from 20–25 May 2020. The majority perceived no (60%) or low (23%) level of risk of personally contracting coronavirus. Common fears were related to health and economic concerns, including loss of income (62%), inability to travel freely (46%), and becoming sick (46%). Residents were well aware of the common symptoms of COVID-19, such as fever (66%) and dry cough (57%), but not the asymptomatic transmission (24%). The majority experienced increased stress about finance (79%) and the lockdown (51%). Our findings emphasize the need to develop context-adequate education and communication programs to raise vigilance about asymptomatic transmission and to sustain preventative behaviors. The evidence on fear and changes in stress levels could inform designing coping strategies and programs focused on mental well-being.
Exploring Perceptions and Acceptance of Minimally Invasive Tissue Sampling among Bereaved Relatives and Health-Care Professionals in Rwanda
In most low- and lower middle-income countries (LMICs), minimally invasive tissue sampling (MITS) is a relatively new procedure for identifying the cause of death (CoD). This study aimed to explore perceptions and acceptance of bereaved families and health-care professionals regarding MITS in the context of MITS initiation in Rwanda as an alternative to clinical autopsy. This was a qualitative phenomenological study with thematic analysis. Participants were bereaved relatives (individual interviews) and health-care professionals (focus-group discussions) involved in MITS implementation. It was conducted in the largest referral and teaching hospital in Rwanda. Motivators of MITS acceptance included eagerness to know the CoD, noninvasiveness of MITS, trust in medics, and the fact that it was free. Barriers to consent to MITS included inadequate explanations from health-care professionals, high socioeconomic status, lack of power to make decisions, and lack of trust in medics. Health-care professionals perceived both conventional autopsy and MITS as gold-standard procedures in CoD determination. They recommended including MITS among hospital services and commended the post-MITS multidisciplinary discussion panel in CoD determination. They pointed out that there might be reticence in approaching bereaved relatives to obtain consent for MITS. Both groups of participants highlighted the issue of delay in releasing MITS results. Both health-care professionals and bereaved relatives appreciate that MITS is an acceptable procedure to include in routine hospital services. Dealing with barriers met by either group is to be considered in the eventual next phases of MITS implementation in Rwanda and similar sociocultural contexts.
Health and economic burden of respiratory syncytial virus (RSV) disease and the cost-effectiveness of potential interventions against RSV among children under 5 years in 72 Gavi-eligible countries
Background Respiratory syncytial virus (RSV) frequently causes acute lower respiratory infection in children under 5, representing a high burden in Gavi-eligible countries (mostly low-income and lower-middle-income). Since multiple RSV interventions, including vaccines and monoclonal antibody (mAb) candidates, are under development, we aim to evaluate the key drivers of the cost-effectiveness of maternal vaccination and infant mAb for 72 Gavi countries. Methods A static Multi-Country Model Application for RSV Cost-Effectiveness poLicy (MCMARCEL) was developed to follow RSV-related events monthly from birth until 5 years of age. MCMARCEL was parameterised using country- and age-specific demographic, epidemiological, and cost data. The interventions’ level and duration of effectiveness were guided by the World Health Organization’s preferred product characteristics and other literature. Maternal vaccination and mAb were assumed to require single-dose administration at prices assumed to align with other Gavi-subsidised technologies. The effectiveness and the prices of the interventions were simultaneously varied in extensive scenario analyses. Disability-adjusted life years (DALYs) were the primary health outcomes for cost-effectiveness, integrated with probabilistic sensitivity analyses and Expected Value of Partially Perfect Information analysis. Results The RSV-associated disease burden among children in these 72 countries is estimated at an average of 20.8 million cases, 1.8 million hospital admissions, 40 thousand deaths, 1.2 million discounted DALYs, and US$611 million discounted direct costs. Strategy ‘mAb’ is more effective due to its assumed longer duration of protection versus maternal vaccination, but it was also assumed to be more expensive. Given all parameterised uncertainty, the optimal strategy of choice tends to change for increasing willingness to pay (WTP) values per DALY averted from the current situation to maternal vaccination (at WTP > US$1000) to mAB (at WTP > US$3500). The age-specific proportions of cases that are hospitalised and/or die cause most of the uncertainty in the choice of optimal strategy. Results are broadly similar across countries. Conclusions Both the maternal and mAb strategies need to be competitively priced to be judged as relatively cost-effective. Information on the level and duration of protection is crucial, but also more and better disease burden evidence—especially on RSV-attributable hospitalisation and death rates—is needed to support policy choices when novel RSV products become available.
