Overview
Alteplase, a recombinant form of tissue plasminogen activator, has been the sole thrombolytic treatment available since the 1990s. The study included patients with very mild stroke (the median National Institutes of Health Stroke Scale [NIHSS] score at baseline was 6) who were relatively young (median age 63 years) compared with similar trials in predominantly White participants. Head-to-head comparisons between reteplase and tenecteplase for acute ischaemic stroke should now be considered, ideally with more precise imaging-based selection, and the optimal reteplase dose should be further investigated.
