Overview
BackgroundCardiovascular disease (CVD) remains a leading cause of mortality in the UK, with higher prevalence among ethnically diverse and socioeconomically disadvantaged populations. Early identification of individuals at risk is critical but often limited by accessibility barriers in traditional care settings.AimThis proof-of-concept, non-interventional study evaluated the feasibility of using the PocDoc Healthy Heart point-of-care screening platform to assess CVD risk in real-world community settings across the UK. The primary goal was to identify individuals with elevated 10-year CVD risk (QRISK3 ≥10%) and those eligible for targeted interventions such as weight loss, smoking cessation, and hypertension management.MethodsThis proof-of-concept, non-interventional study screened individuals across the UK in workplaces, homes, shopping centres, leisure centres, churches, mosques, football stadiums, women’s groups, foodbanks, asylum seeker forums, farmers markets and traveller events.Results4,256 individuals were included in the Primary Cohort (mean age 49 years; 57% male; 31% BMI ≥30 kg/m²), with 1,970 in the Targeted Outreach and 756 in the Workplace sub-cohorts.In the Primary Cohort, 35% had high total cholesterol (>5.0 mmol/L), 10% were smokers, 10% qualified for weight loss services, and 19% were eligible for hypertension management. 20% in the Primary Cohort had a QRISK3 score ≥10%, compared to 26% in Targeted Outreach and 6% in the Workplace cohort. Notably, 60% of high-risk individuals in the Primary Cohort were not on lipid-lowering therapy.ConclusionsThis study supports a new community-based clinical pathway using point-of-care CVD screening to identify at-risk individuals outside traditional healthcare settings. Early detection in community and workplace environments may reduce CVD incidence and ease the burden on primary care services, offering a scalable, preventative model for population health management.
