الكتاب يتناول الفكرة ضمن اجزاء عدة خصصه في الحديث عن الجنسانية كتجربة فريدة تاريخيا يفترض القيام بكتابة جينيالوجيا للذات الراغبة، وبالتالي العودة ليس إلى بدايات التقليد المسيحي وحسب، وإنما أيضا إلى الفلسفة اليونانية القديمة. وهكذا، فبانطلاقة من المرحلة الحديثة وبرجوعه، فبما وراء المسيحية، إلى العهد القديم، كان ميشيل فوكو يصطدم يسؤال بسيط جدا وعام جدا في آن واحد، لماذا يشكل السلوك الجنسي، والأنشطة والمتع المتعلقة به، موضوع انشغال أخلاقي ؟ لماذا هذا الهم الأخلاقي الذي ظهر، حسب لحظات مختلفة، أكثر أو أقل اهمية من الانشغال الأخلاقي الذي انصب على ميادين أخرى من الحياة الفردية أو الجماعية، مثل السلوكات الغذائية أو القيام بالواجبات المدنية ؟ إن هذه الأشكلة للوجود.
