Chronic energy deficiency and associated factors among older population in Ethiopia: A community based study

Chronic energy deficiency is an important public health problem among older (aged ≥ 65 years) population. Globally, one in seven older people has a medium to high risk of malnutrition. The situation of chronic energy deficiency among older people is quite poorly known in Ethiopia. Therefore, this study was aimed to determine the magnitude of chronic energy deficiency and associated factors among elders' aged ≥ 65 years, in Aykel town administration, Amhara Regional State, Northwest Ethiopia.
A community based cross-sectional survey was carried out from March 28th to April 20th, 2018. Study participants were recruited by a census technique. Both bivariate and multivariate logistic regression analysis used to identify factors associated with chronic energy deficiency. All variables with p-values of < 0.2 in the bivariate analysis were remarked for the multivariable analysis. Both Crude Odds Ratio (COR) and Adjusted Odds Ratio (AOR) were computed to determine the strength of association. In the multivariate analysis, all variables at p-values of < 0.05 were considered as statistically significant.
A total of 892 participants recruited for the study. The prevalence of chronic energy deficiency was 17.6% (95%CI: 15.00, 20.20) among the study community. It was significantly associated with female sex (AOR: 1.58; 95%CI: 1.04, 2.41), age (AOR: 3.90; 95%CI: 1.85, 8.25), household food insecurity (AOR: 1.95; 95%CI: 1.16, 3.00), poor household wealth status (AOR: 1.77; 95%CI: 1.07, 2.94), loss of appetite due to illness (AOR: 2.93, 95%CI: 1.92, 4.48) and poor dietary diversity score (AOR: 5.51; 95%CI: 2.89, 10.52).
The magnitude of chronic energy deficiency was high in the study area. It was significantly associated with female sex, age, poor dietary diversity score, loss of appetite due to illness, household food insecurity and poor wealth status. Therefore, there is a need to design and implement programs and strategies to improve nutritional status particularly focusing on female older population in improving dietary practices and food security. In addition, improving household economic and living standards is an essential measure to address the burden of CED among the older community.

Anemia is a global public health problem affecting 305 million school children (SC) worldwide. It has deleterious effects in SC, including lower school achievement due to impaired cognitive development and physical growth, fatigue and poor attention span, and increased morbidity because of reduced resistance to infection. Hence, the aim of this study was to assess the prevalence and associated factors of anemia among SC attending public primary schools in Gondar town, northwest Ethiopia.
A school-based cross sectional study was conducted on a total of 523 SC aged from 6-14 years old. Multi-stage sampling followed by systematic random sampling techniques were employed to select study participants. Data on socio-demographic and socio-economic characteristics, and dietary status of children were collected using pre-tested structured questionnaire through face-to-face interview of children's caregivers. Anthropometric measurements were taken. Hemoglobin (Hb) concentration was determined by using HemoCue 301+analyser. Stool and blood samples were collected in the school premises, and examined for intestinal and hemoparasites, respectively. Data were entered into Epi Info version 3.5.3 and transferred to SPSS version 20 for analysis. Bivariate and multivariate binary logistic regression models were fitted to identify associated factors of anemia. P-value < 0.05 was considered as statistically significant.
Of the total SC participated in the study, 269 (51.4%) were males. The median (inter quartile range (IQR)) age was 12 (10-13) years, and 332 (63.5%) of them were in the age group 11-14 years. About 81 (15.5%; 95%CI: 12.4%, 18.7%) of them were anemic: 56 (69.1%) and 25 (38.9%) of them were mildly and moderately anemic, respectively. Low maternal education (AOR = 2.30; 95%CI: 1.11, 4.78), stunting (AOR = 2.22; 95%CI: 1.30, 3.80), severe food insecurity (AOR = 5.11; 95%CI: 1.53, 17.13), and soil-transmitted helminthic (STH) infection (AOR = 7.13; 95%CI: 3.16, 16.86) were found significantly associated with anemia.
Anemia among SC was found to be a mild public health problem. It was strongly associated with low maternal education, food insecurity, stunting and intestinal helminthic infection. Focused policies and strategies towards the above factors should be designed to reduce anemia among SC.

Association of dietary patterns derived by reduced-rank regression with colorectal cancer risk and mortality

Purpose
Unhealthy dietary patterns contribute to an increased risk of colorectal cancer (CRC). Limited prior research has used reduced rank regression (RRR) to assess dietary patterns relative to CRC risk. This study aimed to identify dietary patterns derived by RRR and assess their associations with CRC risk and mortality.
Methods
We used data from the multicentre Prostate, Lung, Colorectal, and Ovarian Cancer Screening (PLCO) trial. Dietary intake was assessed using a Dietary History Questionnaire. In the RRR intake of fibre, folate, and the percentage of energy from carbohydrates, saturated and unsaturated fatty acids were used as response variables. Cox models and competing risk survival regression, with age as the time scale, were used to estimate hazard ratios (HRs) and 95% confidence intervals (CIs) for CRC risk and mortality, respectively.
Results
The median follow-up time for CRC risk (
n
= 1044) and mortality (
n
= 499) was 9.4 years (Interquartile Range: 8. 0, 10.1) and 16.9 years (11.9, 18.6), respectively. Two dietary patterns were identified: the first was characterised by high carbohydrate, folate and low fatty acid intake, and the second by high fibre and unsaturated fatty acid. Compared to participants in the first tertile of the high fibre and unsaturated fatty acid pattern, those in the third tertile had a lower risk of CRC (HR = 0.88; 95% CI: 0.76, 1.03), and colon cancer (HR = 0.85; 95% CI: 0.72, 1.01). Conversely, the high carbohydrate, high folate and low fatty acid pattern had no association with CRC outcomes. None of the dietary patterns showed associations with rectal cancer or CRC mortality.
Conclusion
A diet enriched with high fibre and unsaturated fatty acids may reduce the risk of CRC. These results highlight the potential protective effect of adequate fibre intake in conjunction with high consumption of unsaturated fatty acids against CRC.

Poor dietary diversity, wealth status and use of un-iodized salt are associated with goiter among school children: a cross-sectional study in Ethiopia

Background
Globally, more than two billion people are at risk of iodine deficiency disorders, 32% of which are school children. Iodine deficiency has been recognized as a severe public health concern in Ethiopia, however little is known about the problem. Therefore, this study aimed to assess the prevalence of goiter and associated factors among school children (6 to 12 years) in Dabat District, northwest Ethiopia.
Methods
A school-based cross-sectional study was conducted from February 21 to March 31, 2016. A total of 735 school children were included in the study. A stratified multistage sampling followed by systematic sampling technique was employed to select the study participants. Thyroid physical examination was done and classified according to the World Health Organization recommendations as grade 0, grade 1, and grade 2. The level of salt iodine content was determined using the rapid field test kit. The value 0 parts per million (PPM), <15 PPM and ≥15 PPM with the corresponding color chart on the rapid test kit were used to classify the level of iodine in the sampled salt. A multivariable logistic regression analysis was employed to identify factors associated with goiter. Adjusted Odds Ratio (AOR) with a 95% Confidence Interval (CI) was calculated to show the strength of association. In multivariable analysis, variables with a

Results
In this community, the overall prevalence of goiter was 29.1% [95% CI: 25.9, 32.6], in which about 22.4 and 6.7% had goiter grade 1 and grade 2, respectively. The age of children (AOR = 1.13; 95% CI: 1.01, 1.26), being housewife mother (AOR = 1.49; 95% CI: 1.08, 2.15), use of unprotected well water source for drinking (AOR = 6.25; 95% CI: 2.50, 15.66), medium household wealth status (AOR = 1.78; 95% CI: 1.18, 2.92), use of inadequately iodized salt (AOR = 2.79; 95% CI: 1.86, 4.19), poor dietary diversity score of the child (AOR = 1.92;95% CI: 1.06, 3.48) and medium maternal knowledge (AOR = 0.65; 95% CI: 0.42, 0.94) were significantly associated with goiter.
Conclusions
The prevalence of goiter is higher in Dabat District, which confirmed a moderate public health problem. Therefore, regular monitoring of household salt iodine content, improving access to safe water, promoting the importance of diversified food for children is recommended to address the higher burden of iodine deficiency.

Background
Nutritional status of children influences their health status, which is a key determinant of human development. In Ethiopia, 28% of child mortality is caused by under nutrition. There is also some controversial evidence about the association between maternal characteristics and nutritional status of under five children. This study was aimed to assess the association between maternal characteristics and nutritional status among 6–59 months of children in Ethiopia.
Methods
This was furtheranalysis ofthe 2016 Ethiopian Demographic and Health Surveyusing7452 children.. Generalized estimating equations was used to quantify the association of maternal factors with stunting and wasting. Results
The higher odds of stunting were found among children whose mothers had no education (AOR = 1.58; 95%CI: 1.25, 2.0) and primary education (AOR = 1.42; 95%CI: 1.13, 1.78), underweight nutritional status (AOR = 1.59; 95%CI: 1.27, 2.0), and anemia (AOR = 1.16; 95%CI: 1.04, 1.30). Similarly, higher odds of wasting were observed among children whose mother had underweight nutritional status (AOR = 2.34; 95%CI: 1.65, 3.38), delivered at home (AOR = 1.31; 95%CI: 1.07, 1.60), and lower than 24 months birth interval (AOR = 1.31; 95%CI: 1.04, 1.64).
Conclusion
Maternal education, nutritional status, and anemia were associated with child stunting. Also maternal nutritional status, place of delivery, and preceding birth interval were associated with wasting. Therefore, there is needed to enhance the nutritional status of children by improving maternal underweight nutritional status, maternal educational and maternal anemia status, prolonging birth interval, and promoting health facility delivery.

Burden and Trends of Diet-Related Colorectal Cancer in OECD Countries: Systematic Analysis Based on Global Burden of Disease Study 1990–2021 with Projections to 2050

Background: An unhealthy diet is a major risk factor for colorectal cancer (CRC). This study assessed the diet-related CRC burden from 1990 to 2021 in Organisation for Economic Co-operation and Development (OECD) nations and estimated the burden until 2050. Methods: Data for OECD countries on diet-related CRC disability-adjusted life years (DALYs) and deaths were obtained from the Global Burden of Disease 2021 study. The estimated annual percent change (EAPC) was calculated to analyse the CRC burden attributable to dietary factors. A generalised additive model with a negative binomial distribution was used to predict the future burden of CRC attributable to dietary factors from 2021 to 2050. Results: In 2021, the age-standardised percentages of diet-related CRC DALYs and deaths were 39.1% (95% uncertainty interval (UI): 9.3, 61.3) and 39.0% (95% UI: 9.7, 60.9), respectively, in the OECD countries. Between 1990 and 2021, the age-standardised DALYs decreased from 185 to 129 per 100,000, and deaths decreased from 8 to 6 per 100,000 population for OECD countries. Similarly, the EAPC in the rates showed a downward trend (EAPCdeaths = −1.26 and EAPCDALYs = −1.20). The estimated diet-related CRC DALYs and deaths are projected to increase to 4.1 million DALYs and 0.2 million deaths by 2050. There is a downward trend in CRC deaths (EAPC = 1.33 for both sexes) and in DALYs (−0.90 for males and −1.0 for females) from 1990 to 2050. Conclusions: The diet-related CRC burden remains significant. Implementing nutrition intervention programmes is necessary to promote access to affordable and nutritious foods and raise awareness about the importance of a healthy diet in reducing CRC risk.

Dietary diversity and meal frequency among infant and young children: a community based study

Background
Insufficient quantities, frequencies, and inadequate quality of complementary feedings have a negative effect on child health and growth, especially in the first two years of life. Therefore, the aim of this study was to assess the minimum dietary diversity, meal frequency and its associated factors among infants and young children aged 6–23 months at Dabat District, northwest, Ethiopia.
Methods
A community- based cross-sectional study was conducted from February 15 to March 10, 2016. The simple random sampling method was used to select study participants. An interviewer- administered structured questionnaire was used to collect data. Both Crude and Adjusted Odds Ratio with the corresponding 95% confidence interval were calculated to show the strength of association. In the multivariable analysis, variables with less than 0.05

Results
The proportion of children who met the minimum dietary diversity and meal frequency were 17% (95% CI: 14.9, 19.4%) and 72.2% (95% CL: 69.3, 75%), respectively. Satisfactory media exposure (AOR = 2.79; 95% CI: 1.74, 4.47), postnatal care visits (AOR = 1.96; 95% CI: 1.32, 2.88), participation in child growth and monitoring follow ups (AOR = 1.65; 95% CI: 1.14, 2.39), age of children (AOR = 2.34; 95% CI: 1.33, 4.11) and age of mothers (AOR = 1.89; 95% CI: 1.09, 3.27) were positively associated with dietary diversity. Similarly, age of children (AOR = 2.38; 95% CI: 1.56, 3.65), household wealth status (AOR = 1.84; 95% CI: 1.27, 2.68), residence (AOR = 3.02; 95% CI: 1.41, 6.48), sources of information (AOR = 1.72; 95% CI:1.14, 2.59) and participation in child growth monitoring folow ups (AOR = 1.57; 95% CI: 1.13, 2.19) were significantly associated with meal frequency.
Conclusion
In this study, the proportion of children who received the minimum dietary diversity and meal frequency were low. Media exposure, age of children, postnatal care visits, and participation in child growth and monitoring follow-ups were significantly associated with dietary diversity. Likewise, wealth status and residence had a significant association with meal frequency. Thus, encouraging all mothers to participate in child monthly growth monitoring programs, intensive media advertising and strengthening counseling of mothers, and postnatal care visit are highly recommended for achieving the recommended dietary practices.

Background
The absence of proper infant and young child feeding practice results in malnutrition. Intimate Partner Violence (IPV) is potentially a major factor affecting child feeding practices. However, there is limited evidence about the effect of intimate partner violence (IPV) on a minimum acceptable diet. Therefore, in this study, we hypothesized that IPV will be associated with a lack of a minimum acceptable diet among children aged 6–23 months.
Methods
We conducted a cross-sectional analysis using the Ethiopian Demographic and Health Survey (EDHS) 2016. All child-mother pairs that participated in EDHS 2016 from all regions of Ethiopia were included. The analysis included mother-child pairs where 6–23 months aged children with mothers who were ever in a committed partnership and interviewed for domestic violence were involved. The data were weighted considering enumeration areas as a cluster and place of residence as a stratum. A binary logistic regression analysis was done to identify factors independently associated with a minimum acceptable diet.
Result
Totally, 1307 observations were included in the final analysis. The mean age of mothers was 29 years (standard deviation ±6.54 years), the mean age of children was 14. ± 5.02 months, and 32% of women had intimate partner violence (IPV). Of the children, 8% had a minimum acceptable diet (minimum acceptable diet), 15% had a minimum dietary diversity, and 43% had a minimum meal frequency. Having intimate partner violence decreases children minimum acceptable diet by 65% (AOR:
0.35
; 95% CI:
0.16, 0.77)
. The other factors associated with the minimum acceptable diet were caregivers attaining a secondary level of education (AOR: 4.01; 95% CI: 1.04, 15.45), currently working (AOR: 2.26; 95% CI: 1.01, 5.11), and undecided fertility desire (AOR: 4.72; 95% CI: 1.37, 16.28).
Conclusion
Intimate partner violence against women had a negative association with the minimum acceptable diet children have received. Decreasing violence against women, educating, and increasing work opportunities for them would help in improving child feeding practice and reducing malnutrition and its consequences. Further studies that focus on possible community-based interventions aiming to decrease IPV are recommended.

Perception regarding the causes of schizophrenia and associated factors among Feresbet district residents: a community based study

Background
A wide variety of beliefs exist in the public towards schizophrenia. Community perception about the causes of schizophrenia can affect the way of seeking help, treatment outcomes, and community integration of individuals with schizophrenia. Therefore, assessing the community perception and associated factors about the causes of schizophrenia is vital.
Method
A cross-sectional study was conducted among Feresbet district residents through a multi- stage sampling technique. A causal model questionnaire for schizophrenia (CMQS) was used to assess the perceived causes of schizophrenia. The collected data were explored to SPSS version 20 for analysis. Bi-variable and multi variable logistic regression were computed to identify factors associated with the traditional perception about the causes of schizophrenia and the level of significance were determined at a

- value < 0.05 with 95% CI.
Results
Out of the total study participants, about 73.7% had the traditional perception regarding the causes of schizophrenia. According to multivariate analysis, female sex, no formal education, age ≥ 25 years, living in the extended family system, and being unemployed had a significant association with the traditional perception of the cause of schizophrenia.
Conclusions
The traditional perception of the cause of schizophrenia is higher than the bio-psycho-social view. Female sex, no formal education, age ≥ 25 years, living in an extended families and unemployed had a significant association with the traditional perception of the causes of schizophrenia. Therefore, giving special attention to females, uneducated and unemployed individual is crucial. In addition, older age and individuals living in extended family system need attention regarding the possible causes of schizophrenia.

Sero-epidemiology and associated factors of HIV, HBV, HCV and syphilis among blood donors in Ethiopia: a systematic review and meta-analysis

Background
Transfusion transmissible infections (TTIs) remain a major public health problem in developing countries including Ethiopia. In Ethiopia, comprehensive information about sero-epidemiology of major TTIs is lacking at the national level. Therefore, this systematic review and meta-analysis was aimed at providing the pooled estimate of human immunodeficiency virus (HIV), hepatitis B virus (HBV), hepatitis C virus (HCV) and syphilis among blood donors in Ethiopia.
Methods
Relevant studies published until May 31, 2019 were searched through PubMed/Medline, EMBASE, SCOPUS, HINARI, Cochrane database library, Web of Science, Google Scholar and Google. The methodological quality of articles was assessed using Joanna Brigg’s Institute critical appraisal checklist for prevalence and analytical studies. The pooled sero-epidemiology of HIV, HBV, HCV and syphilis were determined using the random-effects model. Heterogeneity between the studies was assessed using the I

statistics. Publication bias was assessed by visual inspection of the funnel plot and Egger's statistics.
Results
A total of 7921 articles were retrieved, and 7798 were screened for eligibility after duplicates removed. Forty-nine full-text articles were assessed for eligibility; of which 45 were eligible for qualitative and quantitative synthesis: categorized as 36, 34, 31 and 23 studies for estimations of HBV, HIV, HCV and syphilis, respectively. In the random-effects model, the pooled sero-epidemiology of HBV, HIV, HCV and syphilis was 5.20, 2.83, 0.93 and 1.50%, respectively. Moreover, being a male blood donor was significantly associated with HBV and syphilis infection, whereas being a replacement blood donor was significantly associated with a high burden of HIV, HBV and HCV infections.
Conclusion
The pooled sero-epidemiology of major TTIs among blood donors was high. Therefore, there is a need to design prevention and control strategies in a comprehensive approach to reduce the burden.
