The San Giovanni Baptistery in Florence (Italy): Assessment of the State of Conservation of Surfaces and Characterization of Stone Materials
During the last restoration campaign of the Baptistery of San Giovanni in Florence (Italy), the assessment of the state of conservation of external surfaces was performed, with a multi-analytical approach, in order to support the conservation and restoration treatments. Black crusts, red staining, sulphation, and organic patinas were identified. Moreover, a complete characterization of marble cladding, bricks, and mortars was carried out. A geochemical and minero-petrographic approach was used in order to classify and assess the provenance of marbles, and define raw materials and technologies of the production for bricks and mortars. Provenances from Italy (Carrara and Lasa marbles) and from Greece (Hymettus and Pentelicum marbles) were identified and attributed to different construction phases, restoration interventions, and re-use of ancient materials. For mortars, the obtained data suggested the use of local materials and traditional technologies for the production. Overall, two different minero-petrographic typologies were identified for the analyzed bricks.
Decay Process of Serpentinite: The Case of the San Giovanni Baptistery (Florence, Italy) Pavement
Serpentinite is a low-grade metamorphic rock derived from the transformation of ultramafic rocks. Mainly because of its aesthetic characteristics it has been widely used as a building and ornamental stone. “Verde di Prato” is the most common local name used in Tuscany to refer to this type of rock, historically quarried in this area and used for many centuries in a large number of monuments of this region. In this paper, we report the results of a study carried out on the serpentinite from the pavement of the Florence baptistery, to properly characterize it from a physical point of view, describe the rock conservation state, and understand the phenomena responsible for its decay. The studied rock displays numerous forms of decay including fractures, loss of material, erosion, discolouration and efflorescence. X-ray diffractometer analyses of the efflorescence revealed the presence of numerous salts whose formation can be imputed to multiple, possibly concomitant, causes such as the high relative humidity and the variation of inside temperature, the presence of concrete and/or cementitious mortars in the subsoil, atmospheric pollution and the burial ground existing close the baptistery.
Saint John Baptistery in Florence (Italy): Studies for Conservation of the External Marble Cladding
Saint John’s Baptistery in Florence (Italy), dating back to the XI century, represents one of the most outstanding historical buildings in the city, and has been under the UNESCO patronage as Cultural Heritage since 1982. In recent years, in the frame of a conservation project, detailed studies and mapping of all the tiles covering the Baptistery have been developed. Based on a laser-scan survey, a detailed wireframe model of all the external sides, reporting all the tiles and decor, has been developed. This model was implemented into a 2D-GIS, georeferenced in real scale and spatial position. An in situ survey of all the tiles, ashlars, inlays and columns, made in contradiction by experts in historical ornamental stones, allowed the recognition of several types of marble in place. All these marbles have been analyzed and characterized as geometric, geological and historical data, and the information implemented into a GIS for obtaining a spatial geodatabase representing a “box” to store all information achieved. All these data are manageable by web through smartphone, tablet and PC for querying or updating, thus representing an effective management tool for further conservation of such important historical cultural buildings.
Educational Intervention of Healthy Life Promotion for Children with a Migrant Background or at Socioeconomic Disadvantage in the North of Italy: Efficacy of Telematic Tools in Improving Nutritional and Physical Activity Knowledge
The aim of the “Smuovi La Salute” (“Shake Your Health”) project was to implement an integrated and comprehensive model to prevent and treat overweight and obesity in low socioeconomic status (SES) and minority groups living in three different districts in the north of Italy. An app and a cookbook promoting transcultural nutrition and a healthy lifestyle were developed, and no-cost physical activities were organized. Healthy lifestyle teaching was implemented in 30 primary school classrooms. Learning was assessed through pre- and post-intervention questionnaires. At the Obesity Pediatric Clinic, overweight and obese children of migrant background or low SES were trained on transcultural nutrition and invited to participate in the project. Primary school students increased their knowledge about healthy nutrition and the importance of physical activity (p-value < 0.001). At the Obesity Pediatric Clinic, after 6 months, pre–post-intervention variation in their consumption of vegetables and fruit was +14% (p < 0.0001) and no variation in physical activity habits occurred (p = 0.34). In this group, the BMI z-score was not significantly decreased (−0.17 ± 0.63, p= 0.15). This study demonstrates the feasibility and efficacy of telematic tools and targeted community approaches in improving students’ knowledge with regard to healthy lifestyle, particularly in schools in suburbs with a high density of migrants and SES families. Comprehensive and integrated approaches provided to the obese patients remain mostly ineffective.
Non-Invasive SWIR Monitoring of White Marble Surface of the Cathedral of Santa Maria del Fiore (Florence, Italy)
The monitoring of stone alteration represents a key factor in the knowledge and prediction of the status of conservation of building stones in the urban framework. A continuous monitoring requires a non-destructive analytical approach and, possibly, a simple, low-cost and effective tool to study the decay processes. Previous studies demonstrated the capability of the SWIR hyperspectral technique to gain information on the degree of sulfation of carbonate stone surfaces. In this study we aim at setting up a protocol to investigate on-site the sulfation degree of the white marble cladding surfaces of the worldwide-famous Santa Maria del Fiore Cathedral in Florence (Italy). The proposed protocol couples information by SWIR hyperspectral and colorimetric techniques. We have proved that, in selected areas investigated at a distance of nine years, the colour and the mineralogical changes (i.e., sulfation) are significantly greater than the relative uncertainties of the two methods. Moreover, the proposed protocol results rapid, repeatable and fully not invasive.
Evaluation of serum miRNAs expression in frail and robust subjects undergoing multicomponent exercise protocol (VIVIFRAIL)
Background Frailty, defined as physical performance impairment, is a common condition in older adults and can anticipate the development of sarcopenia, a geriatric syndrome characterized by loss of muscle strength and mass. microRNAs (miRNAs) are short molecules of RNA endowed with the ability to modulate gene expression; miRNAs are present in serum and are considered potential biomarkers for several diseases. Serum concentration of miR-451a, miR-93-5p, miR-155-5p, miR-421-3p, miR-425-5p, miR-495-3p and miR-744-5p was recently shown to be altered in sarcopenic patients. Methods We verified if a particular miRNAs pattern could be detected in frailty as well by analyzing these molecules in 50 frail and 136 robust subjects. Additionally, a subgroup of these subjects (15 frail and 30 robust) underwent a 12-week program based on a multicomponent exercise protocol (VIVIFRAIL) consisting of resistance training, gait retraining, and balance training. After the program, serum miRNAs concentration was measured again, to verify whether the physical activity had an effect on their concentration. Moreover, clinical characteristics and indicators of physical performance of all subjects were compared before and after intervention to verify the effect of the VIVIFRAIL program. Results At the end of the multicomponent exercise program, Short Physical Performance Battery (SPPB) score as well right and left handgrip (p < 0.05) were significantly increased in frail subjects; right and left handgrip significantly were increased also in robust subjects (p < 0.05). Interestingly, the variation of SPPB was significantly higher in frail compared to robust subjects (p < 0.0001). Moreover, at the end of the program, in frail compared to robust subjects: miR-451a serum concentration was significantly increased (frail: 6.59 × 10 4 ; 1.12 × 10 4 –2.5 × 10 5 c/ng; robust: 2.31 × 10 4 ; 1.94 × 10 3 –2.01 × 10 5 c/ng) (p < 0.05); and 2) miR-93-5p and miR-495-3p serum concentration was reduced, whereas that of miR-155-5p was significantly increased (p < 0.05 in both cases). Serum concentration of miR-93-5p and miR-495-3p was decreased, and that of miR-155-5p was increased at the end of the program in robust subjects alone, statistical significance being reached for miR-93-5p alone (p = 0.02). Conclusion These results suggest that serum miR-451a should be investigated as a potential biomarker for frailty and show that the VIVIFRAIL multicomponent program modulates circulatory miRNAs expression, at least in older adults.
Sarcopenia associates with SNAP-25 SNPs and a miRNAs profile which is modulated by structured rehabilitation treatment
Background Sarcopenia is a loss of muscle mass and strength causing disability, morbidity, and mortality in older adults, which is characterized by alterations of the neuromuscular junctions (NMJs). SNAP-25 is essential for the maintenance of NMJ integrity, and the expression of this protein was shown to be modulated by the SNAP-25 rs363050 polymorphism and by a number of miRNAs. Methods We analysed these parameters in a cohort of sarcopenic patients undergoing structured rehabilitation. The rs363050 genotype frequency distribution was analyzed in 177 sarcopenic patients and 181 healthy controls (HC). The concentration of seven miRNAs (miR-451a, miR-425-5p, miR155-5p, miR-421-3p, miR-495-3p, miR-744-5p and miR-93-5p), identified by mouse brain miRNome analysis to be differentially expressed in wild type compared to SNAP-25 ± heterozygous mice, was analyzed as well by droplet digital PCR (ddPCR) in a subgroup of severe sarcopenic patients undergoing rehabilitation. Results The SNAP-25 rs363050 AA genotype was significantly more common in sarcopenic patients compared to HC (p c  = 0.01); miR-451a was significantly up-regulated in these patients before rehabilitation. Rehabilitation modified miRNAs expression, as miR-155-5p, miR-421-3p, miR-451a, miR-425-5p, miR-744-5p and miR-93-5p expression was significantly up-regulated (p < 0.01), whereas that of miR-495-3p was significantly down-regulated (p < 0.001) by rehabilitation. Notably, rehabilitation-associated improvement of the muscle-skeletal SPPB score was significantly associated with the reduction of miR-451a expression. Conclusion These results support the hypothesis of a role for SNAP-25 in sarcopenia and suggest SNAP-25-associated miRNAs as circulatory biomarkers of rehabilitative outcome for sarcopenia.
Air Pollution During Pregnancy and Childhood Cognitive and Psychomotor Development: Six European Birth Cohorts
Background: Accumulating evidence from laboratory animal and human studies suggests that air pollution exposure during pregnancy affects cognitive and psychomotor development in childhood. Methods: We analyzed data from 6 European population-based birth cohorts—GENERATION R (The Netherlands), DUISBURG (Germany), EDEN (France), GASPII (Italy), RHEA (Greece), and INMA (Spain)—that recruited mother-infant pairs from 1997 to 2008. Air pollution levels—nitrogen oxides (NO2, NOx) in all regions and particulate matter (PM) with diameters of <2.5, <10, and 2.5–10 μm (PM2.5, PM10, and PMcoarse, respectively) and PM2.5 absorbance in a subgroup—at birth addresses were estimated by land-use regression models, based on monitoring campaigns performed primarily between 2008 and 2011. Levels were back-extrapolated to exact pregnancy periods using background monitoring sites. Cognitive and psychomotor development was assessed between 1 and 6 years of age. Adjusted region-specific effect estimates were combined using random-effects meta-analysis. Results: A total of 9482 children were included. Air pollution exposure during pregnancy, particularly NO2, was associated with reduced psychomotor development (global psychomotor development score decreased by 0.68 points [95% confidence interval = −1.25 to −0.11] per increase of 10 μg/m3 in NO2). Similar trends were observed in most regions. No associations were found between any air pollutant and cognitive development. Conclusions: Air pollution exposure during pregnancy, particularly NO2 (for which motorized traffic is a major source), was associated with delayed psychomotor development during childhood. Due to the widespread nature of air pollution exposure, the public health impact of the small changes observed at an individual level could be considerable.
Social Withdrawal Behaviour at One Year of Age Is Associated with Delays in Reaching Language Milestones in the EDEN Mother-Child Cohort Study
The aim of the study was to examine the relationship between social withdrawal behaviour at one year and motor and language milestones. One-year old children from the EDEN French population-based birth cohort study (Study on the pre- and postnatal determinants of the child's development and prospective health Birth Cohort Study) were included. Social withdrawal at one year was assessed by trained midwives using the Alarm Distress BaBy (ADBB) scale. Midwives concurrently examined infants' motor and language milestones. Parents reported on child's psychomotor and language milestones, during the interview with the midwife. After adjusting for potential confounding factors, social withdrawal behaviour was significantly associated with concurrent delays in motor and language milestones assessed by the midwife or the parents. Higher scores on social withdrawal behaviour as assessed with the ADBB were associated with delays in reaching language milestones, and to a lesser extent with lower motor ability scores. Taking the contribution of social withdrawal behaviour into account may help understand the unfolding of developmental difficulties in children.
Trauma-induced coagulopathy: impact of the early coagulation support protocol on blood product consumption, mortality and costs
Introduction Hemorrhage is the principal cause of death in the first few hours following severe injury. Coagulopathy is a frequent complication of critical bleeding. A network of Italian trauma centers recently developed a protocol to prevent and treat trauma-induced coagulopathy. A pre-post cohort multicenter study was conducted to assess the impact of the early coagulation support (ECS) protocol on blood products consumption, mortality and treatment costs. Methods We prospectively collected data from all severely injured patients (Injury Severity Score (ISS) >15) admitted to two trauma centers in 2013 and compared these findings with the data for 2011. Patients transfused with at least 3 units of packed red blood cells (PRBCs) within 24 hours of an accident were included in the study. In 2011, patients with significant hemorrhaging were treated with early administration of plasma with the aim of achieving a high (≥1:2) plasma-to-PRBC ratio. In 2013, the ECS protocol was the treatment strategy. Outcome data, blood product consumption and treatment costs were compared between the two periods. Results The two groups were well matched for demographics, injury severity (ISS: 32.9 in 2011 versus 33.6 in 2013) and clinical and laboratory data on admission. In 2013, a 40% overall reduction in PRBCs was observed, together with a 65% reduction in plasma and a 52% reduction in platelets. Patients in the ECS group received fewer blood products: 6.51 units of PRBCs versus 8.14 units. Plasma transfusions decreased from 8.98 units to 4.21 units ( P <0.05), and platelets fell from 4.14 units to 2.53 units ( P <0.05). Mortality in 2013 was 13.5% versus 20% in 2011 (13 versus 26 hospital deaths, respectively) (nonsignificant). When costs for blood components, factors and point-of-care tests were compared, a €76,340 saving in 2013 versus 2011 (23%) was recorded. Conclusions The introduction of the ECS protocol in two Italian trauma centers was associated with a marked reduction in blood product consumption, reaching statistical significance for plasma and platelets, and with a non-significant trend toward a reduction in early and 28-day mortality. The overall costs for transfusion and coagulation support (including point-of-care tests) decreased by 23% between 2011 and 2013.
