Petroleum refining and petrochemical industry integration and coordination under uncertainty
Petroleum refining and the petrochemical industry account for a major share in the world energy and industrial market. In many situations, they represent the economy back-bone of industrial countries. Today, the volatile environment of the market and the continuous change in customer requirements lead to constant pressure to seek opportunities that properly align and coordinate the different components of the industry. In particular, petroleum refining and petrochemical industry coordination and integration is gaining a great deal of interest. However, previous research in the field either studied the two systems in isolation or assumed limited interactions between them. The aim of this thesis is to provide a framework for the planning, integration and coordination of multisite refinery and petrochemical networks using proper deterministic, stochastic and robust optimization techniques. The contributions of this dissertation fall into three categories; namely, (a) Multisite refinery planning, (b) Petrochemical industry planning, and (c) Integration and coordination of multisite refinery and petrochemical networks. The first part of this thesis tackles the integration and coordination of a multisite refinery network. We first address the design and analysis of multisite integration and coordination strategies within a network of petroleum refineries through a mixed-integer linear programming (MILP) technique. The integrated network design specifically addresses intermediate material transfer between processing units at each site. The proposed model is then extended to account for model uncertainty by means of two-stage stochastic programming. Parameter uncertainty was considered and included coefficients of the objective function and right-hand-side parameters in the inequality constraints. Robustness is analyzed based on both model robustness and solution robustness, where each measure is assigned a scaling factor to analyze the sensitivity of the refinery plan and the integration network due to variations. The proposed technique makes use of the sample average approximation (SAA) method with statistical bounding techniques to give an insight on the sample size required to give adequate approximation of the problem. The second part of the thesis addresses the strategic planning, design and optimization of a network of petrochemical processes. We first set up and give an overview of the deterministic version of the petrochemical industry planning model adopted in this thesis. Then we extend the model to address the strategic planning, design and optimization of a network of petrochemical processes under uncertainty and robust considerations. Similar to the previous part, robustness is analyzed based on both model robustness and solution robustness. Parameter uncertainty considered in this part includes process yield, raw material and product prices, and lower product market demand. The Expected Value of Perfect Information (EVPI) and Value of the Stochastic Solution (VSS) are also investigated to numerically illustrate the value of including the randomness of the different model parameters. The final part of this dissertation addresses the integration between the multisite refinery system and the petrochemical industry. We first develop a framework for the design and analysis of possible integration and coordination strategies of multisite refinery and petrochemical networks to satisfy given petroleum and chemical product demand. The main feature of the work is the development of a methodology for the simultaneous analysis of process network integration within a multisite refinery and petrochemical system. Then we extend the petroleum refinery and petrochemical industry integration problem to consider different sources of uncertainties in model parameters. Parameter uncertainty considered includes imported crude oil price, refinery product price, petrochemical product price, refinery market demand, and petrochemical lower level product demand. We apply the sample average approximation (SAA) method within an iterative scheme to generate the required scenarios and provide solution quality by measuring the optimality gap of the final solution.
Sequencing, Analysis, and Annotation of Expressed Sequence Tags for Camelus dromedarius
Despite its economical, cultural, and biological importance, there has not been a large scale sequencing project to date for Camelus dromedarius. With the goal of sequencing complete DNA of the organism, we first established and sequenced camel EST libraries, generating 70,272 reads. Following trimming, chimera check, repeat masking, cluster and assembly, we obtained 23,602 putative gene sequences, out of which over 4,500 potentially novel or fast evolving gene sequences do not carry any homology to other available genomes. Functional annotation of sequences with similarities in nucleotide and protein databases has been obtained using Gene Ontology classification. Comparison to available full length cDNA sequences and Open Reading Frame (ORF) analysis of camel sequences that exhibit homology to known genes show more than 80% of the contigs with an ORF>300 bp and approximately 40% hits extending to the start codons of full length cDNAs suggesting successful characterization of camel genes. Similarity analyses are done separately for different organisms including human, mouse, bovine, and rat. Accompanying web portal, CAGBASE (http://camel.kacst.edu.sa/), hosts a relational database containing annotated EST sequences and analysis tools with possibility to add sequences from public domain. We anticipate our results to provide a home base for genomic studies of camel and other comparative studies enabling a starting point for whole genome sequencing of the organism.
A Clinical Study Comparing the Efficacy of SesameOil with Desensitizing Tooth Paste in Reducing Dentinal Hypersensitivity: A Randomized Controlled Trial
Context. Several treatments are available for management of dentinal hypersensitivity, most routinely recommended one being desensitizing tooth paste with variable results. This is a clinical trial to study the therapeutic effectiveness of sesame oil which is an essential oil, a commonly available household traditional medicine with several health benefits. Objectives. The objective of this study was to evaluate the efficacy of sesame oil therapy in reduction of dentin hypersensitivity, as compared to desensitizing dentifrice. Design, setting, participants. We conducted a single blinded randomized controlled trial in 100 patients reported to Diagnostic Department of King Khalid University-College of Dentistry between March 2018 and December 2019. Interventions. Patients were given desensitizing tooth paste or sesame oil to apply for the specified time. Main outcome measures. A Visual Analogue Scale was used to record sensitivity scores for controlled air stimulus and tactile method at the first visit and after 8th week of treatment. Measured outcome was reduction in dentinal hypersensitivity. Results. Hypersensitivity degree before treatment in case of desensitizing tooth paste was 6.90 ± 1.04, and posttreatment showed a score of 4.70 ± 1.37. In case of sesame oil groups, subjects included had a score of 7.14 ± 0.90 which showed a drop to a score of 4.52 ± 1.16. Conclusions. Desensitizing tooth paste showed 30.5% reduction in sensitivity, whereas sesame oil application showed 36.2% reduction. The belief of considering oil therapy in oral health is just a placebo effect and may not be considered anymore. Efficacy can be established with many more studies including long follow-up and varying time periods.
Nasolacrimal duct obstruction following radioactive iodine 131 therapy in differentiated thyroid cancers: review of 19 cases
Radioactive iodine 131 ((131)I) therapy has long been used in the treatment of differentiated thyroid cancers (DTC). While salivary and lacrimal glandular complications secondary to (131)I therapy are well documented, there is little in the literature addressing nasolacrimal duct obstruction (NLDO). We aimed to evaluate the frequency of (131)I therapy-acquired NLDO, its correlation to (131)I therapy doses, and the surgical treatment outcome of this rare side effect. From 2000-2012, a retrospective review of 864 among 1,192 patients with confirmed DTC who were treated with (131)I therapy was performed to examine the frequency of NLDO, its causative factors, as well as imaging, surgical intervention, and outcomes. Nineteen (2.2%) patients were identified with NLDO. The mean age was 51.9±10.5 years (range: 39-72 years). Fifteen (78.9%) were female and four were male (21.1%). The mean individual (131)I doses were 311.1±169.3 millicurie (mCi) (range: 150-600 mCi). The mean duration between the date of (131)I therapy and the occurrence of NLDO was 11.6±4.1 months (range: 6.5-20). Fourteen (73.7%) patients had bilateral epiphora. Computed tomography dacryography allowed for the detection of all NLDO. Eighteen (94.7%) patients underwent dacryocystorhinostomy. Complete recovery was obtained in 14 (73.7%) patients. Age >45 years and (131)I therapy doses >150 mCi were significantly correlated with NLDO (P=0.02 and P=0.03, respectively). NLDO is an underestimated complication of (131)I therapy in DTC patients. Clinicians should be aware of this rare complication for prompt intervention.
Skull base metastasis from papillary thyroid carcinoma: a report of three cases
Skull base metastasis from differentiated thyroid carcinoma, including papillary and follicular thyroid carcinoma, is a rare manifestation. Herein, we present three cases of skull base metastasis of papillary thyroid carcinoma. The mean age of the patients was 68.6 (65-74) years, and the mean interval between initial diagnosis and skull base metastasis was 56.3 (28-89) months. Cranial nerve palsies were seen in all patients. Intensity modulated radiation therapy to deliver 6,000-6,600 cGy to the skull base metastasis was given to all patients, in addition to partial resection in one patient. At the time of last follow-up, all skull base metastases were well controlled.
Comparative clinicopathological and outcome analysis of differentiated thyroid cancer in Saudi patients aged below 60 years and above 60 years
The aim of this study was to evaluate the treatment outcomes of differentiated thyroid cancer in Saudi patients aged above 60 years. Comparative analysis was performed in 252 patients aged 46-60 years (Group A) and 118 patients aged above 60 years (Group B), who had thyroidectomy, radioactive iodine-131, and thyroid-stimulating hormone suppression therapy between July 2000 and December 2012. Different clinicopathological features, treatment, complications, disease-free survival, and overall survival rates were compared. Mean age of patients in Group A was 51.9 years (range: 46-60), and mean age of those in Group B was 68.6 years (range: 62-97). Group B patients had higher positive lymph nodes (43.2%), P=0.011. The frequency of extrathyroidal extension, multifocality, and lymphovascular space invasion was seen more in Group B than in Group A. Postsurgical complications (permanent hypoparathyroidism, bleeding, and wound infections) were also seen more in Group B (P=0.043, P=0.011, and P=0.021, respectively). Group B patients experienced more locoregional recurrences (11.0%, P=0.025); similarly, more distant metastases were observed in Group B (15.3%, P=0.003). The 10-year disease-free survival rates were 87.6% in Group A and 70.8% in Group B (P<0.0001). Differentiated thyroid cancer in patients aged above 60 years are more aggressive biologically and associated with a worse prognosis, and the morbidity is significantly high as compared to patients aged below 60 years.
Calvarium and dura mater as delayed sites of distant metastasis from papillary thyroid carcinoma
Skull calvarium and dura mater are rare sites of distant metastasis, and mostly have been reported in lung, breast, and prostate malignancies. However, the calvarial and dural metastases from papillary thyroid cancer (PTC) are rare entities and pose diagnostic and therapeutic challenges. To date, only seven cases of calvarial metastasis with intracranial extension from PTC have been reported in literature. However, true dural metastasis from PTC has not yet been reported. A 65-year-old Saudi woman presented with 6 months history of painful posterior scalp lump, 7 years after initial diagnosis of PTC. Computed tomography and magnetic resonance imaging showed occipitoparietal calvarial lesion with massive intracranial extension. Another para-sagittal lesion was found at the top of corpus callosum mimicking a meningioma. Histopathology of para-sagittal lesion and the biopsy of calvarial mass confirmed the diagnosis of metastatic PTC. After surgical resection, residual tumors were treated with postoperative intensity-modulated radiation therapy. At 13 months of follow-up, patient was alive and without any signs of recurrence. Calvarial and dural metastases from PTC are extremely rare clinical entities. Surgical resection followed by postoperative radiotherapy is the treatment of choice. However, for such cases, multidisciplinary approach can prolong the treatment outcome and survival.
UPLC-MS/MS determination of doxazosine in human plasma
A sensitive, selective and rapid method for the analysis of doxazosine (DOX) in human plasma based on ultra-performance liquid chromatography tandem mass spectrometry (UPLC-MS/MS) is described. DOX and tamsulosin, used as an internal standard (IS), were extracted by liquid-liquid extraction, and the chromatography was performed on a C18 UPLC column packed with 1.7 μm particles. The total run time was 2 min. Detection was achieved by the multiple reaction monitoring of the following transitions: m/z 452[rightward arrow]344 and m/z 409[rightward arrow]228 for DOX and IS, respectively. Transitions of m/z 452[rightward arrow]247 and m/z 409[rightward arrow]271 were also collected for confirmation purposes. The calibration curve based on peak area ratio was linear up to at least 100 ng ml-¹, with a detection limit of 0.02 ng ml-¹ (a signal-to-noise ratio of 3). The method showed satisfactory reproducibility, and the short-term stability of the analyte was assessed. The method was successfully applied to the analysis of DOX in human plasma.
