Hospital-Based Clinical Profile and Management Patterns of Keratoconus in Riyadh City, Saudi Arabia: A Multi-Center Cross-Sectional Study
Background and Objectives: Keratoconus (KC) is a progressive ectatic corneal disease that can cause irregular astigmatism and visual impairment. To describe the demographic and clinical profile of KC patients attending major eye care centers in Riyadh City, Saudi Arabia, and to explore associations with laterality, disease severity, and management patterns. Materials and Methods: This multi-center hospital-based cross-sectional study enrolled consecutive patients with a confirmed diagnosis of KC (new or follow-up) presenting between April 2022 and April 2023. All participants underwent standardized ophthalmic assessment and Scheimpflug tomography (Pentacam). Disease severity was categorized as early, moderate, or advanced using Pentacam-derived keratoconus staging, and ocular parameters (refraction, keratometry, pachymetry, and higher-order aberrations) were compared across severity categories. Results: A total of 157 patients (264 eyes) were included (mean age 31.8 years; 56.7% female), with bilateral KC in 68.2%. Eye rubbing (67.8%) and allergic symptoms (61.7%) were common. Keratometric indices and higher-order aberrations differed significantly by severity grade (p < 0.001). Management patterns differed by sex and laterality, with corneal cross-linking and glasses reported more frequently in males, and soft contact lens use concentrated among bilateral cases. Conclusions: In this hospital-based Riyadh sample, KC was often associated with eye rubbing and allergic symptoms and showed clear stage-dependent worsening of tomographic indices and optical quality. These findings support early detection and targeted counseling on modifiable behaviors, while population-based studies with non-diseased comparators are needed to quantify incidence and prevalence in Riyadh.
Myopia Progression Among School-Aged Children in the COVID-19 Distance-Learning Era
To investigate the effect of online learning and other environmental factors on myopia progression during the Coronavirus pandemic (COVID-19). A retrospective cohort study from 2018 to 2021. Data from children aged 6-14 were gathered during three visits: pre-pandemic, at the beginning, and during the pandemic. Demographics (hours spent on screens for educational, recreational purposes, outdoors, and type of screen), best-corrected distance visual acuity (BCDVA), uncorrected distance visual acuity (UCDVA), and cycloplegic refraction were gathered. Of 150 patients, 70 [47%] were boys. The mean age was 11 (2.4) years. Participants mainly used mobile phones (62%) and had insufficient outdoor play (88%). Of the 300 eyes, 221 (74%) showed myopia progression. A significant difference in spherical equivalent (SE) was found between pre-pandemic and post-pandemic periods (-0.29 (0.23) D vs -0.40 (0.11) D; p =0.023). Additionally, UCDVA showed a difference between the initial and 1st follow-up visits (0.57 (0.37) vs 0.64 (0.36), p =0.001), and the first and 2nd follow-up visits (0.64 (0.36) vs 0.70 (0.36), p =0.001). Significant hazard ratio for change in SE in patients with higher age (>9 years), (HR [95% confidence interval (CI)], 0.71 [0.51-0.84]), greater recreational screen usage (HR [95% CI], 1.26 [1.15-1.66]), and insufficient outdoor time (HR [95% CI], 1.45 [1.35-1.67]). Myopia progression was accelerated during the COVID-19 pandemic. Younger age, prolonged screen use, and insufficient outdoor time contributed to increased myopia progression. However, the type of device used had no significant effect.
Impact of Using Blackboard on Vocabulary Acquisition: KKU Students’ Perspective
This research aims to investigate the impact of employing the learning management system (Blackboard) on attitudes of King Khalid University (KKU) students. The research examines the students' knowledge and skills in employing the Blackboard in their learning process, and investigates the elements, which influence the students' attitude and performance in using Blackboard in classroom instruction from their perspectives. To this end, the author has selected a sample of 34 of KKU students, where they have been requested to complete a 14-item questionnaire. Findings of the current study revealed that KKU students faced several difficulties in learning English language vocabulary. Consequently, the students have developed negative attitudes towards English vocabulary. The study found that Blackboard had a little influence on the attitude and performance of the KKU students in vocabulary learning. The study also showed that there were some limitations in the use of Blended Learning. It needs an action plan in order to make supportive learning opportunities for  the university students.
Parents' Knowledge of and Attitude Toward the Human Papillomavirus Vaccine in the Western Region of Saudi Arabia
 Human papillomavirus (HPV) is the most common cause of cervical cancer among Saudi females in reproductive age. Parents' awareness of and attitude toward vaccination against HPV in young females are very crucial to prevent the development of cervical cancer. This study aimed to assess the knowledge of and attitude toward the HPV vaccine among Saudi parents in the Saudi western region. An analytical cross-sectional questionnaire-based study was conducted that included 343 parents randomly selected from in the Saudi western region. The online questionnaire was disseminated via WhatsApp and Telegram groups of parents in schools and among visitors of parents in primary health care. The studied parents had poor knowledge of HPV and its vaccine. About a third (32.9%) knew about the HPV vaccine and the most common source of their knowledge was physicians (38%) while the most frequent barrier for vaccination was their confidence of being not at risk (75.2%). About 90.0% of parents having a good level of knowledge were willing to vaccinate their children. This study reveals a poor level of knowledge about HPV infection and its vaccine among both male and female parents in the Saudi Arabian western area. As a consequence, only 7.2% of them had vaccinated their female children. The majority of the parents having good knowledge about the HPV vaccine were willing to vaccinate their children. Therefore, this study highlights the necessity of educating women on cervical cancer risk factors, as well as the importance of screening programs.
Incidence Rate of Cardiovascular Events in Rheumatoid Arthritis: An Observational Cohort Study in Saudi Arabia
Rheumatoid arthritis (RA) doubles the morbidity of cardiovascular disease (CVD) and leads to a 50% increase in mortality compared to the general population. This study aims to estimate the CVD incidence among RA patients in Saudi Arabia (SA), vital for assessing CVD burdens within this group. This retrospective study took place at two centers in the Eastern Province of SA, including all adult RA patients who visited the rheumatology clinic from 2016 to 2021 and were prescribed disease-modifying antirheumatic drugs (DMARDs). CVD incidence was determined by the diagnosis of ischemic heart disease (IHD), stroke/transient ischemic attack (TIA), venous thromboembolism (VTE), heart failure (HF), and arrhythmia post-RA diagnosis. Additional data collected included demographics, CVD risk factors, comorbidities, RA-related factors, and medication usage. The study comprised 651 patients, 80.5% of whom were females with an average age of 51. The overall CVD incidence was 11.2 per 1000 person-years, with males experiencing five times more incidents than females. The prevalence of CVD risk factors included 18.7% with hypertension, 7.8% with hyperlipidemia, 18.9% with diabetes, and 42.9% with obesity. Significant predictors of CVD were male gender and RA duration, with adjusted odds ratios (aOR) of 3.17 (95% CI 1.10 to 9.14, P=0.033) and 64.81 (95% CI 3.68 to 1140.6, P=0.004), respectively. This unique study from SA examined the CVD incidence in RA patients, identifying long disease duration and male gender as significant predictors. Effective reduction of CVD risk in RA patients requires aggressive management of modifiable risk factors and regular risk assessments.
Comparing Gastroesophageal Reflux Disease (GERD) and Non-GERD Patients Based on Knowledge Level of Acute Myocardial Infarction Symptoms, Risk Factors and Immediate Action Taken in Eastern Province, Saudi Arabia
A major cause of death globally is cardiovascular disease (CVD). Chest pain, nausea, vomiting, and heartburn are common symptoms of acute myocardial infarction (AMI). Chest pain is also the main symptom of gastroesophageal reflux disease (GERD). Therefore, the differential diagnosis of chest pain can become more challenging when GERD and AMI coincide. This study evaluated and compared the knowledge of the signs, symptoms, and immediate action that must be taken regarding AMI among GERD and non-GERD patients. An observational cross-sectional study using an online questionnaire was created and published between October and November 2022 to collect data from Saudi males and females 18 or older willing to participate in the study. Participants who were not Saudi had declined to complete the survey or had not fully completed it was excluded. The questionnaire contained three sections; after collecting informed consent, it made inquiries regarding respondents' GERD status, demographic information, and knowledge and attitudes regarding GERD. This study included 691 responses from 300 non-GERD participants and 391 GERD participants. The study showed a high level of awareness (75.5%) of GERD, with significant differences in the level of awareness according to marital status, education levels, and occupation status. There was no significant difference in the level of awareness according to gender and GERD diagnosis, where the p-value > 0.05. The most common source of information about AMI was the Internet, followed by health care professionals. The most commonly known symptoms of AMI were sudden pain or discomfort in the chest, followed by a sudden shortness of breath. Additionally, there was no significant association between the diagnosis of GERD and known risk factors. The association between GERD and other diseases (chi-square = 46.94, p-value 0.01). Obesity and smoking were the two main risk factors for heart attacks. This study demonstrated that there was no significant difference between GERD and non-GERD participants regarding the knowledge and awareness level of AMI. Moreover, it showed that there was a lack of general knowledge and awareness of AMI in Saudi Arabia. The authors recommend initiating more awareness programs in Saudi Arabia to inform people about AMI and cardiovascular disease. More research is required to determine whether other patients are aware of AMI.
