The Effectiveness of Nutritional Interventions on Maternal and Childhood Anaemia in Sindh, Pakistan

Background/Objectives: In Pakistan, 41.7% of women of reproductive age and 53.7% of children aged 6–59 months are anaemic. This study aimed to evaluate the effectiveness of a nutritional supplementation programme, underpinned with behaviour change communication and implemented through Lady Health Workers (LHWs), in reducing anaemia among pregnant and lactating women as well as 6–59-month-old children. Methods: This study used a quasi-experimental design with an intervention and a control group. A total of 2821 and 2410 pregnant and lactating women and 3397 and 3277 children aged 6–59 months participated in the baseline and endline surveys, respectively. The study areas were matched for demographic and economic characteristics, and both had routine public-sector health and nutrition programmes, including iron–folic acid supplementation during pregnancy. The participants in the intervention group received additional nutritional support. Throughout the pregnancy and in the first six months of exclusive breast feeding, women were provided 5 kg (165 g/day) of wheat soya blend (WSB) per month. Children aged 6–23 months received lipid-based nutrient supplements (LNS), and those aged 24–59 months received micronutrient powder. We applied a difference-in-difference (DID) analysis with kernel propensity score matching to assess the impact on anaemia, a secondary outcome of a stunting prevention programme. Results: Maternal anaemia (both pregnant and breastfeeding women) declined substantially in the intervention areas from 80.4% to 62.6% compared with a smaller reduction in the control areas (80.0% to 72.9%). The DID estimate showed a 10.7-percentage-point (pp) greater decline in maternal anaemia (p ≤ 0.001). Both pregnant and lactating women benefited, with DID reductions of 16.7 pp (p ≤ 0.001) and 9.4 pp (p = 0.005), respectively. The largest gains occurred among women in higher-wealth quintiles, with reductions up to 22.6 pp (p ≤ 0.001). In contrast, childhood anaemia showed no overall significant difference between intervention and control areas (DID = −0.7 pp, p = 0.73). However, significant improvements were observed among male children (DID = −10.3 pp, p = 0.001) and those in higher-wealth quintiles (DID = −15.0 pp, p = 0.002). Conclusions: Overall, the findings suggest that LHWs can effectively reduce maternal anaemia when iron–folic acid supplementation is complemented with WSB and targeted behaviour change communication.

A mixed methods study to assess the effectiveness of food-based interventions to prevent stunting among children under-five years in Districts Thatta and Sujawal, Sindh Province, Pakistan: study protocol

Background
Maternal and child malnutrition is widely prevalent in low and middle income countries. In Pakistan, widespread food insecurity and malnutrition are the main contributors to poor health, low survival rates and the loss of human capital development. The nutritional status trends among children exhibit a continuous deteriorating with rates of malnutrition exceeding the WHO critical threshold. With the high prevalence of maternal and child malnutrition, it is important to identify effective preventative approaches, especially for reducing stunting in children under-five years of age. The primary aim of this study is to assess the effectiveness of food-based interventions to prevent stunting in children under-five years.
Methods
A mixed methods study design will be conducted to evaluate the effectiveness of food-based interventions to prevent stunting among children under-five years in districts Thatta and Sujawal, Sindh Province, Pakistan. The study will include cross sectional surveys, a community-based cluster randomized controlled trial and a process evaluation. The study participants will be pregnant women, lactating mothers and children under-five years. The cross-sectional surveys will be conducted with 7360 study participants at baseline and endline. For the randomized control trial, 5000 participants will be recruited and followed monthly for compliance of food-based supplements, dietary diversity, pregnancy outcomes, and maternal and child morbidity and mortality. Anthropometric measurements and hemoglobin levels will be measured at baseline, quarterly and at endline. The interventions will consist of locally produced lipid-based nutrient supplement (Wawamum) for children 6–23 months, micronutrient powders for children 24–59 months, and wheat soya blends for pregnant and lactating mothers. Government lady health workers will deliver interventions to participants. The effectiveness of the project will be measured in terms of the impact of the proposed interventions on stunting, nutritional status, micronutrient deficiencies, and other key indicators of the participants. The process evaluation will assess the acceptability, feasibility and potential barriers of project implementation through focus group discussions, key informant interviews and household surveys. Data analysis will be conducted using STATA version 12.
Discussion
There is considerable evidence on the effectiveness of food-based interventions in managing stunting in developing countries. However, these studies do not account for the local environmental factors and widespread nutrient deficiencies in Pakistan. These studies are often conducted in controlled environments, where the results cannot be generalized to programs operating under field conditions. The findings of this study will provide sufficient evidence to develop policies and programs aimed to prevent stunting in children 6–59 months and to improve maternal and child health and growth outcomes in poor resource settings.

In Pakistan, the 2018 National Nutrition Survey reported that 40% of children under five years old were stunted. This study assessed the effectiveness of nutritional supplementation in reducing stunting among children under five years old in two rural districts in Sindh, Pakistan. This was a mixed-method quasi-experimental study comprising intervention and control populations, with 3397 and 3277 children under five years old participating in the baseline and end-line surveys, respectively. The study areas were similar in terms of demographic and economic circumstances. In the intervention group, pregnant and lactating women (first six months post-partum) received wheat soy blend, children 6–23 months old received Wawamum (lipid-based supplement), and children 24–59 months old received micronutrient powders, all through lady health workers. This was underpinned by nutrition behaviour change communication for appropriate complementary feeding practices and hygiene promotion targeted at primary caregivers. The control group received no intervention. The impact was assessed using the difference-in-difference analysis with kernel propensity score matching to adjust the differences among the control and intervention populations. The overall DID analysis indicated that the intervention did not significantly reduce the prevalence of stunting (under 5 years) [DID = −5.1, p = 0.079]. The adjusted DID indicated a significant decrease of 13% [DID = −13.0, p = 0.001] in the number of stunted children 24–59 months of age at the endline survey. A significant reduction in underweight among children 24–59 months old was also observed (DID = −9.4%, p = 0.014). In conclusion, this evidence further establishes that nutrient uptake through an intervention for a short duration cannot effectively reduce stunting. It requires continuous nutritional supplementation for mothers during the pregnancy and an initial six months of lactation and then nutritional supplementation for children 6–59 months of age underpinned by effective behaviour change communication targeting mothers and other caregivers for improving complementary feeding practices and hygiene promotion.

Safety and efficacy of side-by-side versus stent-in-stent stenting for malignant hilar biliary obstruction: a systematic review and meta-analysis

Objectives:
Stenting of the malignant hilar biliary obstruction (MHBO) area for bile drainage is challenging. Bilateral stenting techniques of stent-in-stent (SIS) and stent-by-stent (SBS) have shown promising results. This study evaluates the efficacy of different stenting methods for MHBO.
Design:
A meta-analysis was performed to determine the efficacy of SIS and SBS stenting strategies for MHBO.
Data sources and methods:
Medical databases such as PubMed, Web of Science, Embase, and Scopus, were searched up to August 2023. We selected eligible studies reporting the data on technical and clinical success, adverse events, and incidence of re-obstruction (RO) of SBS and SIS groups in MHBO patients. We compared the outcomes of SBS and SIS groups.
Results:
A total of 9 studies comparing the data of 545 patients (268 in the SBS group and 277 in the SIS stenting group) were analyzed. There was no significant difference (p > 0.05) in the odds ratio (OR) of Re-obstruction (RO) 0.87 (95% confidence interval (95% CI) 0.6–1.25), technical success 0.58 (95% CI 0.16–2.11), clinical success 1.13 (95% CI 0.62–2.07), and adverse events 1.53 (95% CI 0.88–2.64). The mean difference in procedure time was −12.25 min (95% CI −18.39, −6.12), and the hazard ratio of stent patency was 1.22 (95% CI 1.01–1.47), favoring SBS, with high heterogeneity (I2 = 94%). There was no significant difference in HR for survival 1.05 (95% CI 0.95–1.16) with high heterogeneity (I2 = 84%).
Conclusion:
Compared with SIS, SBS showed better stent patency with comparable technical and clinical success and adverse events.

Control of equine nematodes has relied on benzimidazoles (BZs), tetrahydropyrimidines and macrocyclic lactones. The intensive use of anthelmintics has led to the development of anthelmintic resistance (AR) in equine cyathostomins and Parascaris equorum. Field studies indicate that BZ and pyrantel resistance is widespread in cyathostomins and there are also increasing reports of resistance to macrocyclic lactones in cyathostomins and P. equorum. The unavailability of reliable laboratory-based techniques for detecting resistance further augments the problem of nematode control in horses. The only reliable test used in horses is the fecal egg count reduction test; therefore, more focus should be given to develop and validate improved methodologies for diagnosing AR at an early stage, as well as determining the mechanisms involved in resistance development. Therefore, equine industry and researchers should devise and implement new strategies for equine worm control, such as the use of bioactive pastures or novel feed additives, and control should increasingly incorporate alternative and evidence-based parasite control strategies to limit the development of AR. This review describes the history and prevalence of AR in equine nematodes, along with recent advances in developing resistance diagnostic tests and worm control strategies in horses, as well as giving some perspective on recent research into novel control strategies.

Resistance associated metabolite profiling of Aspergillus leaf spot in cotton through non-targeted metabolomics

Aspergillus tubingensis is an important pathogen of economically important crops. Different biotic stresses strongly influence the balance of metabolites in plants. The aim of this study was to understand the function and response of resistance associated metabolites which, in turn are involved in many secondary metabolomics pathways to influence defense mechanism of cotton plant. Analysis of non-targeted metabolomics using ultra high performance liquid chromatography-mass spectrometry (UPLC-MS) revealed abundant accumulation of key metabolites including flavonoids, phenylpropanoids, terpenoids, fatty acids and carbohydrates, in response to leaf spot of cotton. The principal component analysis (PCA), orthogonal partial least squares discriminant analysis (OPLS-DA) and partial least squares discriminant analysis (PLS-DA) score plots illustrated the evidences of variation between two varieties of cotton under mock and pathogen inoculated treatments. Primary metabolism was affected by the up regulation of pyruvate and malate and by the accumulation of carbohydrates like cellobiose and inulobiose. Among 241 resistance related (RR) metabolites, 18 were identified as resistance related constitutive (RRC) and 223 as resistance related induced (RRI) metabolites. Several RRI metabolites, identified in the present study were the precursors for many secondary metabolic pathways. These included phenylpropanoids (stilbenes and furanocoumarin), flavonoids (phlorizin and kaempferol), alkaloids (indolizine and acetylcorynoline) and terpenoids (azelaic acid and oleanolic acid). Our results demonstrated that secondary metabolism, primary metabolism and energy metabolism were more active in resistant cultivar, as compared to sensitive cultivar. Differential protein and fatty acid metabolism was also depicted in both cultivars. Accumulation of these defense related metabolites in resistant cotton cultivar and their suppression in susceptible cotton cultivar revealed the reason of their respective tolerance and susceptibility against A. tubingensis.

Environmentally Friendly Utilization of Wheat Straw Ash in Cement-Based Composites

The open burning of biomass residue constitutes a major portion of biomass burning and leads to air pollution, smog, and health hazards. Various alternatives have been suggested for open burning of crop residue; however, each of them has few inherent drawbacks. This research suggests an alternative method to dispose wheat straw, i.e., to calcine it in a controlled environment and use the resulting ash as a replacement of cement by some percentage in cement-based composites. When wheat straw, an agricultural product, is burned, it is very rich in SiO2, which has a pozzolanic character. However, the pozzolanic character is sensitive to calcination temperature and grinding conditions. According to the authors’ best knowledge, until now, no systematic study has been devised to assess the most favorable conditions of burning and grinding for pozzolanic activity of wheat straw ash (WSA). Hence, a systematic experimental program was designed. In Phase I, calcination of WS was carried out at 500 °C, 600 °C, 700 °C, and 800 °C for 2 h. The resulting ashes were tested for color change, weight loss, XRD, XRF, Chapelle activity, Fratini, and pozzolanic activity index (PAI) tests. From test results, it was found that beyond 600 °C, the amorphous silica transformed into crystalline silica. The WSA calcined at 600 °C was found to satisfy Chapelle and Fratini tests requirements, as well as the PAI requirement of ASTM at 28 days. Therefore, WSA produced at 600 °C (WSA600) showed the best pozzolanic performance. In Phase II, WSA600 was ground for various intervals (15–240 min). These ground ashes were tested for SEM, Blaine fineness, Chapelle activity, Fratini, and PAI tests. From test results, it was observed that after 120 min of grinding, there was an increase of 48% in Blaine surface area, with a consequence that WSA-replaced cement cubes achieved a compressive strength almost similar to that of the control mix. Conclusively, wheat straw calcined at 600 °C and ground for 120 min was found to be the most effective way to use pozzolanic material in cement-based composites. The addition of WSA in cement-based composites would achieve manifold objectives, i.e., aiding in the production of environmentally friendly concrete, the use of wheat straw as fuel for electricity production, and adding economic value to wheat straw.

Irregular measurements may occur during the drilling process due to unconsolidated formation resulting in poor signal recordings by the logging tool. This affects the quality of data acquisition and the accuracy of elastic logs, such as density and velocity profiles, in reservoir characterization. It is of paramount importance to ensure the stability of the wireline-logging tool and to prevent compromising measurements of the formation's physical properties. While previous literature focused on the application of different machine learning (ML) algorithms for well logging, their application in a particular domain implied a narrow methodological utility for researchers. Therefore, this study combined two superior techniques of ML, supervised and unsupervised, for enhancing the elastic log response to ultimately help us to enhance reservoir characterization and interpretation. First, the density-based spatial clustering of applications with noise (DBSCAN) was used for outlier detection, and then, feature selection was used to identify highly correlated logs, which helped in rebuilding the density log. After successful ranking, the scaled-down features were carried forward to construct a regression model for density logs rebuilding. The comparative results confirmed the high accuracy of porosity estimated from rebuilt density log compared to that of core data. Consequently, it reduces cumbersome human efforts and time.

This article reports the simulation of ZnO-based polymer solar cell (PSC) device parameters. The Solar Cell Capacitance Simulator (SCAPS-1D) software was used to analyze the cell parameters. Two different device configurations of PSCs were simulated with ZnO (an inverted device) and without ZnO (a reference device). ) resistances on cell performance were investigated. Comparing simulated and experimental results, a good correlation between PSCs was observed. The efficiencies of reference PSCs are 0.87% (simulated) and 0.85% (experimental) and for inverted PSCs are 2.33% (simulated) and 2.32% (experimental). In addition, the structure maintains an efficiency of 2.33–2.14% as the temperature goes up from 300 to 330 K. Comparing the efficiencies of reference and inverted devices, the role of the ZnO layer between the indium tin oxide (ITO) substrate and the device active layer (AL) was quite significant. The efficiencies of PSCs were well-matched, achieving a quite nice similarity between the simulated and experimental devices.

The current study investigated the potential of selenium nanoparticles (SeNPs) and mannan-oligosaccharide (MOS) supplementation in ameliorating high stocking density (HSD) stress in broilers. A total of 392 day-old male chicks were divided into seven groups with eight replicates (n = 7): NSD [basal diet (BD) + normal stocking density: 10 bird/m2], HSD [BD + high stocking density: 16 bird/m2], Se–HSD [BD + Selenium (Se) 0.15 mg/kg], MOS–HSD (BD + MOS 5 gm/kg), Se–MOS–HSD (BD + Se 0.15 mg/kg and MOS 5 gm/kg), SeNPs–HSD (BD + SeNPs 0.15 mg/kg) and SeNPs–MOS–HSD (BD + SeNPs 0.15 mg/kg and MOS-5 gm/kg). HSD stress decreased (p < 0.05) weekly body weight and body weight gain and increased (p < 0.05) FCR compared to the NSD group. Supplementation with SeNPs and the SeNPs–MOS combination improved (p < 0.05) the weekly body weight and FCR in HSD-stressed broilers during the 5th and 6th weeks. On day 21, HSD stress decreased (p < 0.05) duodenal villus height (VH) and villus surface area (VSA) and increased (p < 0.05) serum corticosterone and cholesterol compared to the NSD group. Supplementation with the SeNPs–MOS combination increased (p < 0.05) duodenal VH and VH:CD, and jejunal total goblet cell (TGC) density and decreased (p < 0.05) serum corticosterone and cholesterol and ileal intra-epithelial lymphocyte (IEL) density in HSD-stressed broilers. On day 42, HSD stress decreased (p < 0.05) duodenal and jejunal VH, VSA, VH:CD, PCNA positive cell density and TGC density, Ileal VSA and TGC density, and increased (p < 0.05) serum cholesterol and ileal IEL density compared to the NSD group. Supplementation with the SeNPs–MOS combination increased (p < 0.05) spleen and bursa absolute weights, duodenal VH, VSA, VH:CD, PCNA positive cell density and jejunal VH, VH:CD, and decreased (p < 0.05) serum cholesterol and ileal IEL density in HSD-stressed broilers. Our findings signify that HSD is stressful for broilers particularly during the finishing phase. Supplementation with the SeNPs–MOS combination mitigated HSD stress by partially improving the gut microarchitecture, gut barrier function and performance indicators.
