Evaluation of a Question Prompt List About Cardiovascular Disease Risk and Prevention After Hypertensive Pregnancy: A Pilot Study

Introduction The aim of this study was to pilot test a question prompt list (QPL) about cardiovascular disease (CVD) risk reduction after hypertensive pregnancy (HDP). Methods In a prospective cohort study of adult women who had HDP given the QPL before and surveyed after a physician visit, we assessed perceived person‐centred care, self‐efficacy for self‐management, perceived self‐management and QPL feasibility. Results Twenty‐three women participated: 57% of diverse ethno‐cultural groups, 65% < 40 years of age and 48% immigrants. Most scored high for person‐centred care (mean 4.1 ± 0.2/5); and moderately for self‐efficacy (mean 7.4 ± 0.6/10) and self‐management (mean 3.1 ± 0.3/5). Most appreciated QPL design and reported QPL benefits: helped them to prepare for the visit and know what to ask; increased confidence to ask questions, knowledge of the link between HDP and CVD and lifestyle behaviours to reduce CVD risk. Most reported that physicians were receptive to discussing QPL questions. Conclusion Women appreciated the QPL and knowledge about self‐management was high but self‐efficacy for or perceived self‐management was moderate. It appears feasible to share a QPL with ethno‐culturally diverse women who can share it with physicians to facilitate discussions about post‐pregnancy HDP‐related CVD risk. Patient or Public Contribution This study involved women who experienced HDP and engaged ethno‐culturally diverse women with lived experience of HDP as study advisors in all stages of the research.

Adolescent Sexually Transmitted Infections and Risk for Subsequent HIV

Objectives. We estimated the risk of HIV associated with sexually transmitted infection (STI) history during adolescence. Methods. We retrospectively studied a cohort of adolescents (n = 75 273, born in 1985–1993) who participated in the Philadelphia High School STD Screening Program between 2003 and 2010. We matched the cohort to STI and HIV surveillance data sets and death certificates and performed Poisson regression to estimate the association between adolescent STI exposures and subsequent HIV diagnosis. Results. Compared with individuals reporting no STIs during adolescence, adolescents with STIs had an increased risk for subsequent HIV infection (incidence rate ratio [IRR] for adolescent girls = 2.6; 95% confidence interval [CI] = 1.5, 4.7; IRR for adolescent boys = 2.3; 95% CI = 1.7, 3.1). Risk increased with number of STIs. The risk of subsequent HIV infection was more than 3 times as high among those with multiple gonococcal infections during adolescence as among those with none. Conclusions. Effective interventions that reduce adolescent STIs are needed to avert future STI and HIV acquisition. Focusing on adolescents with gonococcal infections or multiple STIs might have the greatest impact on future HIV risk.

Igfn1 is a complex locus that codes for multiple splicing variants of Immunoglobulin- and Fibronectin-like domain containing proteins predominantly expressed in skeletal muscle. To reveal possible roles for Igfn1, we applied non-selective knock-down by shRNAs as well as specific targeting of Igfn1 exon 13 by CRISPR/Cas9 mutagenesis in C2C12 cells. Decreased expression of Igfn1 variants via shRNAs against the common 3'-UTR region caused a total blunting of myoblast fusion, but did not prevent expression of differentiation markers. Targeting of N-terminal domains by elimination of exon 13 via CRISPR/Cas9 mediated homologous recombination, also resulted in fusion defects as well as large multinucleated cells. Expression of IGFN1_v1 partially rescued fusion and myotube morphology in the Igfn1 exon 13 knock-out cell line, indicating a role for this variant in myoblast fusion and differentiation. However, in vivo overexpression of IGFN1_v1 or the Igfn1 Exon 13 CRISPR/Cas9 targeting vector did not result in significant size changes in transfected fibres.

The poison grapevine: How destructive are gossip and rumor in the workplace?

The dysfunctional system, at home or at work, is governed by a characteristic set of rules that are unspoken but clearly understood by everyone. The first and most obvious rule is to maintain the status quo. Two other characteristics of the dysfunctional system are lack of trust and lack of closure. Stressful situations prompt dysfunctional behavior in the workplace. To maintain the status quo through control, power, and security, dysfunctional bosses often engage in gossiping and backbiting. Although individuals from dysfunctional family systems come into the workplace with unhealthy ways of communicating, there are measures that organizations may take to avoid being trapped in unhealthy relationships. Enabling need-based behavior will decrease the need to rely on the grapevine as a primary source of information and strengthen horizontal and vertical communication within the organization.

Milankovitch forcing of Early Jurassic wildfires

The Early Jurassic was characterized by major climatic and environmental perturbations which can be seen preserved at high resolution on orbital timescales. The Early Jurassic is a period of overall global warmth, and therefore serves as a suitable modern-day analogue to understand changes in the Earth System. Presently, Earth’s climate is warming and the frequency of large wildfires appears to be increasing. Recent research has indicated that Quaternary deposits reveal that wildfires respond to orbital forcings; however, to date no study has been able to test whether wildfire activity corresponds to changes over Milankovitch timescales in the deep past. A high-resolution astrochronology exists for the Upper Pliensbachian in the Llanbedr (Mochras Farm) borehole (NW Wales). Ruhl et al. (2016) show that elemental concentration recorded by hand-held X-ray fluorescence (XRF), changes mainly at periodicities of ~21,000 year, ~100,000 year and ~400,000 year, and which can be related to visually described sedimentary bundles. We have quantified the abundance of fossil charcoal at a high resolution (10-15 cm) to test the hypothesis that these well-preserved climatic cycles influenced fire activity throughout this globally warm period. Preliminary results suggest that variations in charcoal abundance are coupled to Milankovitch forcings over periods of ~21,000 and ~100,000 years. We suggest that these changes in fire relate to changes in seasonality and monsoonal activity that drove changes in vegetation that are linked to variations in the orbital forcing. Supplementary to the charcoal record, a high-resolution clay mineralogy dataset has been generated to further explain the climatic cyclicity observed in the wildfire record regarding the hydrology on land.
