Randomised controlled trial to assess the impact of hospital–community pharmaceutical care on drug-related problems in oncology practice for at-risk outpatients treated with oral anticancer drugs—a French Society for Oncology Pharmacy (SFPO) study: DROP-SFPO study protocol
IntroductionDrug-related problems (DRPs) associated with oral anticancer drugs are frequent and require a new healthcare organisation to manage them on an outpatient basis. The aim of this article is to present the study protocol of the Drug Related problems in Oncology Practice (DROP) randomised controlled trial (RCT), endorsed by the French Society for Oncology Pharmacy. The main objective of the DROP RCT is to measure the impact at 6 months of the DROP community/hospital pharmaceutical intervention programme, compared with usual treatment, on the mean number of DRP (ie, adverse effects, drug–drug interactions, medication errors) related to oral anticancer drugs in at-risk outpatients.Methods and analysisThe DROP protocol is a prospective, multicentre controlled clinical trial, with individual randomisation, comparing in parallel and in open, two groups of outpatients treated with oral anticancer drugs. The interventional group benefits from the DROP multidisciplinary intervention on oral anticancer treatment. The control group receives usual care. The primary outcome of the DROP RCT is the number of DRP due to oral anticancer drugs, per patient, identified between the inclusion of the patient and 6 months after inclusionEthics and disseminationApproval to conduct this study was obtained for all participating centres from an Ethics Committee (Comité de Protection des Personnes Sud-Méditerranée V) in August 2018 in accordance with French law. The trial results will be disseminated at clinical conferences and published in peer-reviewed journals.Trial registration numberClinicalTrials.gov: NCT03257969, recruitment started in June 2019. The current protocol version is V.9, 13 December 2023.

Craft teaching and learning are evolving today at an unprecedent pace, driving the need for skills that increasingly fuse tradition with innovation. However,  mapping craft skills is complex, due to both the multiplicity of factors determining skill development and the uncertainty surrounding future developments. In this paper, we present the methodological approach designed for the MOSAIC project, an ambitious Erasmus+ that addresses the need for skills in arts and crafts through research, creation of innovative craft training, and the establishment of a craft observatory. Firmly grounded within the field of craft science research, the methodology is rooted in the theory of practice and deploys a participatoryprocess to further explore crafts skill needs. However, to the purpose of this article, we focus on a particular methodological aspect, that is, how we use cultural mapping to explore the complex craft skillecosystem and analyse the rich data sets collected through a mixed set of instruments. Cultural mapping involves the systematic collection, documentation, and analysis of information about cultural assets – here: craft skills – within specific geographic areas or communities. In MOSAIC, we use cultural mappingas an approach to establishing an analytical continuum between top-down (e.g. legislation) and bottom-up (e.g. industry) approaches to the analysis of skill needs in MOSAIC partner countries. This approach isa good example of a context-appropriate research strategy that reaches beyond the borders of disciplines. As such, it fosters a holistic approach to craft science research that speaks to craft researchers, educators, and policymakers.
Ovarian cancer in the older patient: where are we now? What to do next?
In recent years, major advances have been made toward the individualization of epithelial ovarian cancer care, leading to an overall improvement of patient outcomes. However, real-life data indicate that the oldest populations do not benefit from this, due to aspects related to cancer (more aggressive histopathological features), treatment (i.e. frequently suboptimal), and the host (increased toxicities in patients with lower physiological reserve). A specific risk-benefit perspective should therefore be taken when considering surgery, chemotherapy, and maintenance treatments: the decision for cytoreductive surgery should include geriatric vulnerability and surgical complexity, neo-adjuvant chemotherapy being an option when primary surgery appears at high risk; carboplatin paclitaxel association remains the standard even in vulnerable older patients; and bevacizumab and poly(ADP-ribose) polymerase inhibitors maintenance are interesting options provided they are prescribed according to their indications with a close monitoring of their toxicities. Future studies should aim to individualize care without limiting access of older patients to innovation. A specific focus is needed on age-specific translational analyses (focusing on tumor mutational burden and impaired biological pathways), a better patient stratification according to geriatric parameters, an adaptation of both oncological treatment and geriatric interventions, and treatment adaptations not a priori but according to formal pharmacokinetic data.

A multicentre, randomised, double-blind, placebo-controlled trial with the interleukin-1 receptor antagonist anakinra in patients with systemic-onset juvenile idiopathic arthritis (ANAJIS trial)
Objectives To assess the efficacy of the interleukin 1 receptor antagonist anakinra in systemic-onset juvenile idiopathic arthritis (SJIA). Methods A multicentre, randomised, double-blind, placebo-controlled trial was conducted. The primary objective was to compare the efficacy of a 1-month treatment with anakinra (2 mg/kg subcutaneous daily, maximum 100 mg) with a placebo between two groups each with 12 patients with SJIA. Response was defined by a 30% improvement of the paediatric American College of Rheumatology criteria for JIA, resolution of systemic symptoms and a decrease of at least 50% of both C-reactive protein and erythrocyte sedimentation rate compared with baseline. After month 1 (M1), patients taking placebo were switched to anakinra. Secondary objectives included tolerance and efficacy assessment for 12 months, and analyses of treatment effect on blood gene expression profiling. Results At M1, 8/12 responders were receiving anakinra and 1 responder receiving placebo (p=0.003). Ten patients from the placebo group switched to anakinra; nine were responders at M2. Between M1 and M12, six patients stopped treatment owing to an adverse event (n=2), lack of efficacy (n=2) or a disease flare (n=2). Blood gene expression profiling at enrolment and at 6 months' follow-up showed one set of dysregulated genes that reverted to normal values in the clinical responders and a different set, including interferon (IFN)-inducible genes, that was induced by anakinra. Conclusions Anakinra treatment is effective in SJIA, at least in the short term. It is associated with normalisation of blood gene expression profiles in clinical responders and induces a de novo IFN signature. Trial Registration Number: NCT00339157.
Effect of a Roux-en-Y Gastric Bypass on the Pharmacokinetics of Oral Morphine Using a Population Approach
Background and Objectives Obesity and opioid use for chronic pain in obese individuals are both important public health concerns. The pharmacokinetics of oral morphine after Roux-en-Y gastric bypass (RYGB) are unknown. Therefore, we aimed to study the pharmacokinetics of oral morphine in morbidly obese patients before and after RYGB surgery, to identify the effects of RYGB and the subsequent reversal of morbid obesity on the pharmacokinetic parameters of morphine. Methods The pharmacokinetics of oral morphine (30 mg) were studied in 30 obese patients before (Visit 1) and then 7–15 days (Visit 2) and 6 months (Visit 3) after RYGB. A population pharmacokinetic model was used to describe the time course of the plasma morphine concentration, to study the effect of RYGB on morphine pharmacokinetics and to estimate inter-patient variability. Results The oral morphine time to maximum plasma concentration ( t max ) was twofold lower and maximum plasma concentration ( C max ) was 1.7 times higher at Visit 2, and t max was 7.5 times lower and C max 3.3 times higher at Visit 3 than at Visit 1. The mean oral morphine area under the plasma concentration–time curve (AUC) increased significantly (1.55-fold) between Visits 1 and 3. Changes in body mass index (BMI) after RYGB were clearly associated with decreased apparent oral morphine clearance and apparent central and peripheral morphine volumes of distribution. None of the other anthropometric parameters explained the inter-subject variability in morphine exposure better than BMI. Conclusion RYGB and the BMI reduction that followed it dramatically increased the rate of morphine absorption and slightly increased morphine exposure. The dose of immediate-release forms of morphine may be divided in obese patients after RYGB to prevent adverse events due to early and high morphine plasma peaks.
L'Art nouveau, un long temps pour mourir et disparaitre: L'Exemple du Vase aux lys tigres de 1927/Art Nouveau: A Long Time to Die and Disappear. The Example of the Vase aux lys tigrés of 1927
The Vase aux lys tigrés, made at the Daum factory and housed in the Fine Arts Museum in Nancy, is a remarkable piece. Although it is an Art Nouveau design, befitting the factory's glory days from 1897 to 19141 it was not made until 1927, almost 20 years after the supposed end of the movement. This type of piece is unusual but not unknown for Daum or for many other manufacturers of the time. It demonstrates the economic importance of this production and the complexity of the arts industry, which attempted to strike a balance between the expectations of consumers, aesthetic demands, and the industry's own restrictions. The vase also shows what appealed to the public, whose tastes were less definitive and categorical than those of art critics. (Author abstract)
