Behavioral and transcriptomic effects of the cancer treatment tamoxifen in mice

Tamoxifen is a common treatment for estrogen receptor-positive breast cancer. While tamoxifen treatment is generally accepted as safe, there are concerns about adverse effects on cognition.
We used a mouse model of chronic tamoxifen exposure to examine the effects of tamoxifen on the brain. Female C57/BL6 mice were exposed to tamoxifen or vehicle control for six weeks; brains of 15 mice were analyzed for tamoxifen levels and transcriptomic changes, and an additional 32 mice were analyzed through a battery of behavioral tests.
Tamoxifen and its metabolite 4-OH-tamoxifen were found at higher levels in the brain than in the plasma, demonstrating the facile entry of tamoxifen into the CNS. Behaviorally, tamoxifen-exposed mice showed no impairment in assays related to general health, exploration, motor function, sensorimotor gating, and spatial learning. Tamoxifen-treated mice showed a significantly increased freezing response in a fear conditioning paradigm, but no effects on anxiety measures in the absence of stressors. RNA sequencing analysis of whole hippocampi showed tamoxifen-induced reductions in gene pathways related to microtubule function, synapse regulation, and neurogenesis.
These findings of the effects of tamoxifen exposure on fear conditioning and on gene expression related to neuronal connectivity suggest that there may be CNS side effects of this common breast cancer treatment.

Androgen deprivation therapy (ADT) for prostate cancer is carried out in a palliative approach in symptomatic geriatric patients. Testosterone synthesis is suppressed using a dual therapy principle with luteinising hormone-releasing hormone (LH-RH) analogues or antagonists of LH-RH, in addition to modern antiandrogens. Additional administration of taxanes is initially possible in cases of high tumor burden and aggressive tumor biology, which is otherwise reserved for castration-resistant prostate cancer. The ADT is a systemic therapy for all testosterone-dependent processes and can lead to hot flushes, gynecomastia, osteoporosis, sarcopenia, anemia, falls, cognitive decline, depression, metabolic syndrome, increased cardiovascular events and many drug interactions. In patients identified as geriatric, ADT should therefore only be used after a thorough risk-benefit analysis.

HIV-associated neurocognitive disorder (HAND) persists in people living with HIV (PLWH) despite antiretroviral therapy. HAND is characterized by synapto-dendritic damage, yet the cause of this pathology is still under investigation. Various viral proteins, including the envelope protein gp120, have been proposed to be the leading neurotoxic agents underlying HIV-mediated neuronal degeneration. Gp120 has been shown to bind to neuronal microtubules (MTs) and impair their functions. The dynamic properties of MTs are modulated by microtubule-associated proteins (MAP), including MAP2, which is particularly abundant in dendrites. This review article explores how gp120 could be altering the function of the neuronal cytoskeleton by affecting MAP2. These effects may serve as a causal link between viral proteins and HAND pathology.

Androgendeprivative Therapie des Prostatakarzinoms aus geriatrischer Sicht

Zusammenfassung
Eine androgendeprivative Therapie (ADT) des Prostatakarzinoms (PCA) erfolgt beim symptomatischen geriatrischen Patienten im palliativen Ansatz. Die Testosteronsynthese wird mithilfe eines dualen Therapieprinzips mit Luteinisierendes-Hormon-Releasing-Hormon(LH-RH)-Analoga oder Antagonisten des LH-RH unterdrückt; zusätzlich wird mit modernen Antiandrogenen therapiert. Prinzipiell ist initial bei hoher Tumorlast und aggressiven Tumoren die additive Gabe von Taxanen möglich; diese bleibt sonst dem kastrationsresistenten Prostatakarzinom vorbehalten. Eine ADT stellt eine Systemtherapie aller testosteronabhängigen Prozesse dar und kann zu Hitzewallungen, Gynäkomastie, Osteoporose, Sarkopenie, Anämie, Stürzen, kognitivem Abbau, Depression, metabolischem Syndrom, vermehrten kardiovaskulären Ereignissen und einer Fülle von Medikamenteninteraktionen führen. Sie sollte bei einem als geriatrisch identifizierten Patienten nur nach sorgfältiger Risiko-Nutzen-Analyse angewendet werden.

Pharmakotherapie der Harninkontinenz im Alter

Die Prävalenz und Inzidenz der Harninkontinenz nimmt mit dem Alter zu. Frauen sind häufiger von einer Belastungs- und gemischten, Männer häufiger von einer Dranginkontinenz betroffen. Bei älteren Menschen müssen neben pathophysiologischen auch physiologische Alternsprozesse der unteren Harnwege und eine zugrunde liegende Multimorbidität berücksichtigt werden. Bei der Belastungsinkontinenz ist unter anderem ein insuffizienter Harnröhrenverschluss ursächlich beteiligt, bei der Dranginkontinenz sind dies häufig unwillkürliche Detrusorkontraktionen. Diese können durch Medikamente und neurologische Veränderungen zusätzlich negativ beeinflusst werden. Die medikamentöse Therapie ist nur ein Teil im Gesamtkonzept der Inkontinenztherapie. Bei der Belastungsinkontinenz ist es mit dem Serotonin-Noradrenalin-Wiederaufnahmehemmer Duloxetin möglich, den urethralen Verschlussdruck zu erhöhen und Inkontinenzepisoden um bis zu 64 % zu reduzieren. Bei einer Dranginkontinenz bewirken Muskarinrezeptorantagonisten eine Relaxierung des Blasendetrusors, was eine signifikante Abnahme der Miktionsfrequenz und der Inkontinenzepisoden sowie eine Zunahme der Blasenkapazität zur Folge hat. Darifenacin, Fesoterodin, Oxybutynin, Propiverin, Solifenacin, Tolterodin und Trospiumchlorid sind in Deutschland zugelassene antimuskarinerge Medikamente. Hinzugekommen ist im Sommer letzten Jahres für kurze Zeit der β3-Rezeptor-Antagonist Mirabegron mit direkter Wirkung im Rahmen der Füllungsphase an der Blase. Aufgrund fehlender Preisfindung wurde der Wirkstoff in Deutschland vom Markt genommen.

A multidisciplinary German expert group met in 2012 to discuss the current status and prospects of health care of geriatric patients with urinary incontinence in Germany. The purpose of this position paper is to raise awareness among health care providers for the challenges associated with adequate management of urinary incontinence in frail elderly. The experts agree that a multidisciplinary collaboration is essential for the successful treatment of urinary incontinence symptoms which are often associated with loss of autonomy and social isolation. For most geriatric patients, usually the general practitioner is the first contact when seeking help. Hence, the general practitioner plays a crucial role in the coordination of diagnosis and treatment. The involved health care providers should have adequate education and training in their respective disciplines and should be networked allowing quick turnaround times. Non-pharmacological treatments (e.g. behavioural interventions) should have been tried before any pharmacotherapy is initiated. If pharmacological treatment of urinary incontinence involves the use of anticholinergic agents, cognitive performance should be monitored regularly. If indicated, anticholinergic agents with a documented efficacy and safety profile, explicitly assessed in the elderly population, should be preferred.

40 Jahre Deutsche Gesellschaft für Geriatrie (DGG) – gefragt, gereift, gestärkt

Die Deutsche Gesellschaft für Geriatrie (DGG) begeht im Jahr 2025 ihren 40. Geburtstag. Sie wurde 1985 gegründet, um Geriatrie in Wissenschaft und Politik zu positionieren, Wissen zu vermitteln, einen Facharztstatus Geriatrie zu erreichen sowie eine eigenständige Altersmedizin in Diagnostik, Therapie und Rehabilitation zu etablieren. Wie konnte sie diese Ziele erreichen? Es werden 3 Methoden eingesetzt: (1) eine halbstrukturierte und offene Befragung (n = 28), (2) ein sehr umfassendes Quellenstudium mit Identifizierung von 4 Meilensteinen auf Basis der historischen Ethnographie sowie (3) eine Inhaltsanalyse aller Protokolle (n = 36) der Mitgliederversammlungen über 40 Jahre. Die DGG erreicht fast alle Ziele – und darüber hinaus. In den Themen Ernährung, Alterstraumatologie und Assessment erarbeitet sie eigenständig Evidenz. Durch hervorragende internationale Vernetzung kann man wissenschaftliche Exzellenz bei ihr finden. Allerdings ist das Ziel eines Facharztes Geriatrie noch nicht erreicht. Nur ein Fachgebiet wie die Geriatrie mit der DGG kann den komplexen Erfordernissen älterer Menschen gerecht werden.

Acute poisoning in old and very old patients: a longitudinal retrospective study of 5883 patients in a toxicological intensive care unit

Although morbidity and mortality of acute poisoning are increased in elderly compared to younger patients, little has been published on this topics in the last years (Medline search). To investigate the influence of age on the clinical course of acute poisoning with different toxic agents, a longitudinal retrospective study at the Toxicological Intensive Care Unit (ICU) of the 2nd Department of Internal Medicine (Klinikum Nürnberg, Germany) was performed.A total of 5883 patients treated at our toxicological ICU were enrolled into the study, including all patients of the years 1982, 1992, and 1997. These three years were selected to investigate possible time-dependent changes of intoxication characteristics and quality of therapy at our ICU over a time span of 15 years. For each patient the following data were obtained from a standardized toxicological record: age, gender, toxic agents responsible for acute poisoning, and length of stay at the toxicological ICU. For a subgroup of 3740 patients, the cause of acute poisoning and the clinical outcome was also recorded. As compared with younger patients, mean length of stay at the ICU, indicating a more serious course of acute poisoning, was prolonged in elderly and, i. e., in very old patients (p <0.001). However, this prolongation of time at the ICU was only observed in elderly patients poisoned with drugs or with mixed poisoning including drugs, while mean length of stay was not prolonged in elderly patients poisoned with alcohol, with illegal drugs, chemicals, animal/plant poison, or other toxic agents. Patients with the highest risk of dying in the ICU after acute poisoning were elderly patients attempting suicide with drugs. Mortality in 3740 patients with acute poisoning was 0.24%, while it was 2.17% in the 184 patients being 65 years old or older. Thus, mortality was 9-fold higher in the elderly. Mean length of stay at the ICU decreased significantly from 1982 to 1992 and to 1997 (p <0.001) indicating an improvement of the therapeutical ICU management of acute poisoning and/or less dangerous toxic agents (i. e. less barbiturates). The age-dependent increase of the length of stay at the ICU until very old age (> 80 years) was most pronounced in 1982 and also declined markedly until 1997.Age, suicide attempt, and ingestion of (multiple) drugs seem to be risk factors for a higher mortality and a prolonged stay in the ICU after acute poisoning. Although in general the clinical course after poisoning has more complications and an impaired prognosis in old age, each category of toxic agents (drugs, alcohol, chemicals, etc.) has its own special \"risk profile\" for elderly patients. However, due to advances in modern ICU medicine the general prognosis of acute poisoning is good in old and even in oldest old patients.

Das Delir bei geriatrischen Patienten in der Urologie

HintergrundDas Delir ist mit 14–56 % eine der häufigsten Komplikationen in Krankenhäusern in der Altersgruppe über 70-jähriger Patienten. Auch bei postoperativ zu versorgenden Patienten ist eine passager oder auch länger anhaltende Verwirrtheit eine häufig anzutreffende Komplikation. In diesem Zusammenhang wird vielfach der Zusammenhang zwischen der Verordnung von Medikamenten mit anticholinerger Nebenwirkung und Störung der kognitiven Funktionen beschrieben.MethodikAnhand der Beschreibung der klinischen Zeichen eines Delirs, der formalen Kriterien zur Diagnosestellung und allgemeingültiger Einschätzungsskalen soll der Arzt befähigt werden, diese Komplikation im besten Fall vermeiden zu helfen oder frühzeitig zu erkennen und gezielt zu behandeln.ErgebnisseDas plötzliche Auftreten fluktuierender Symptome mit Störung der Aufmerksamkeit, des Bewusstseins und des Denkens sind typische Symptome des Delirs. Häufig wird eine organische Ursache für das Entstehen eines Delirs nicht oder zu spät erkannt. Die TUR-Prostata (transurethrale Resektion) oder der Blase können nicht nur das Risiko für eine Blutung erhöhen, sondern auch für ein Delir und das TUR-Syndrom. Als einfaches Instrument um die Zeichen eines Delirs zu erkennen gilt der („clinical assessement method“) CAM und z. B. die „nurses delirium screeening scale“ (NuDesc) als gut untersucht. Die Behandlung mit nichtpharmakologischen Interventionen reduzieren die Nebenwirkungen eines postoperativen Delirs.SchlussfolgerungPräoperatives Screening und die Durchführung eines geriatrischen Basisassessments zur Detektion vulnerabler Älterer kann ein erster Schritt zur Reduktion von Komplikationen sein. Das regelhafte Screening auf Symptome eines Delirs reduziert im weiteren Verlauf die prä-, peri- und postoperativen Komplikationen in Hinblick auf ein Delir.
