Microhabitat use by three species of egret (Pelecaniformes, Ardeidae) in southern Brazil
In the present study, we examined the role of different habitat components and their relationship with microhabitat use by three species of egret: Cocoi heron (Ardea cocoi), Great egret (Ardea alba), and Cattle egret (Bubulcus ibis), in wetlands of southern Brazil. Ardea alba and A. cocoi were not habitat-specific (e.g. vegetation cover and flooding level). Conversely, B. ibis was associated with drier microhabitats. Relative air humidity and air temperature were the main variables which correlated with the occurrence of these species and supported the plasticity of B. ibis and its predominance in drier habitats. No presente estudo foi avaliado o papel de diferentes componentes do habitat e sua relação quanto ao uso do microambiente por três espécies de Ardeidae: garça-moura (Ardea cocoi), garça-branca-grande (Ardea alba) e garça-vaqueira (Bubulcus ibis), em banhados do extremo sul brasileiro. Aparentemente, A. alba e A. cocoi não apresentaram especificidade a algum tipo de habitat (e.g. cobertura vegetal e nível de alagamento). Por outro lado B. ibis revelou associações a microambientes mais secos. A temperatura do ar e umidade relativa do ar foram as principais variáveis correlacionadas com a ocorrência dessas espécies, além disso, reforçou-se a plasticidade de B. ibis e sua predominância em ambientes secos.
Foraging activity of the snail kite, Rostrhamus sociabilis (Aves: Accipitridae) in wetlands of southern Brazil
The snail kite (Rostrhamus sociabilis) is widely distributed in the American continent. Its specialised diet consists mostly of the gastropod mollusk Pomacea sp and its foraging strategy probably varies depending on the season, prey availability, and climate factors, which can be reflected in its semi-nomad behaviour. This study was aimed at examining the hunting strategy of the snail kite, and its association with climate factors and habitat heterogeneity. Direct observations of birds between January 2010 and March 2011 in southernmost Brazil revealed that hunting was still the predominant foraging strategy (79% of records) to capture mollusks. Despite morphological specialisations to extract mollusks from the shells, the handling time (average = 92.4 s) was twice as much the time between prey search and capture (average = 55 s). The increase in the number of mollusks ingested apparently occurs when the resting time on perches or any other substrates near the hunting sites decreases between successive unsuccessful attempts. The correlation between the number of consumed preys and the climatic variables examined was low. Regarding habitat heterogeneity, our findings suggest that birds forage preferentially in marshes with low vegetation, which may increase the access to mollusks. The hunting efficiency of the snail kite was high (76 % successful attempts) compared to those of other birds of prey. O gavião-caramujeiro (Rostrhamus sociabilis) possui ampla distribuição no continente americano e apresenta dieta especializada no molusco gastrópode Pomacea sp. Possivelmente, a estratégia de forrageio da espécie responde às variações sazonais na disponibilidade de presas, bem como aos fatores climáticos, o que pode refletir em seu comportamento seminômade. O objetivo desse estudo foi avaliar a estratégia de caça do gavião-caramujeiro, buscando suas associações quanto aos aspectos climáticos e à heterogeneidade do habitat. Observações diretas das aves, feitas entre janeiro de 2010 e março de 2011 no extremo sul brasileiro, revelaram que, para a captura dos moluscos, as aves usaram predominantemente a estratégia de caça do tipo still hunting (79% dos registros). Apesar de apresentar especializações morfológicas para a extração dos moluscos da concha, a avaliação do tempo empregado na manipulação (média = 92,4 s) foi quase o dobro do desprendido entre a procura e a captura (média = 55 s) das presas. Aparentemente, o aumento no número de moluscos ingeridos ocorre por meio de redução no tempo de repouso entre sucessivas investidas de captura mal sucedidas, tal como pelo uso de poleiros ou quaisquer outros substratos próximos aos locais de captura. O número de presas consumidas apresenta baixa relação com as variáveis climáticas testadas. Quanto à heterogeneidade do habitat, os dados sugerem que as aves forrageiam preferencialmente em banhados com cobertura vegetal rasteira, o que poderia aumentar a acessibilidade aos moluscos. O gavião-caramujeiro apresentou alta taxa de eficiência de caça (76% das investidas tiveram êxito), revelando a eficiência da estratégia de caça desta ave quando comparada a outras aves de rapina.

Venous thromboembolism risk and prophylaxis in the acute hospital care setting (ENDORSE study): a multinational cross-sectional study
Information about the variation in the risk for venous thromboembolism (VTE) and in prophylaxis practices around the world is scarce. The ENDORSE (Epidemiologic International Day for the Evaluation of Patients at Risk for Venous Thromboembolism in the Acute Hospital Care Setting) study is a multinational cross-sectional survey designed to assess the prevalence of VTE risk in the acute hospital care setting, and to determine the proportion of at-risk patients who receive effective prophylaxis. All hospital inpatients aged 40 years or over admitted to a medical ward, or those aged 18 years or over admitted to a surgical ward, in 358 hospitals across 32 countries were assessed for risk of VTE on the basis of hospital chart review. The 2004 American College of Chest Physicians (ACCP) evidence-based consensus guidelines were used to assess VTE risk and to determine whether patients were receiving recommended prophylaxis. 68 183 patients were enrolled; 30 827 (45%) were categorised as surgical, and 37 356 (55%) as medical. On the basis of ACCP criteria, 35 329 (51·8%; 95% CI 51·4–52·2; between-country range 35·6–72·6) patients were judged to be at risk for VTE, including 19 842 (64·4%; 63·8–64·9; 44·1–80·2) surgical patients and 15 487 (41·5%; 41·0–42·0; 21·1–71·2) medical patients. Of the surgical patients at risk, 11 613 (58·5%; 57·8–59·2; 0·2–92·1) received ACCP-recommended VTE prophylaxis, compared with 6119 (39·5%; 38·7–40·3; 3·1–70·4) at-risk medical patients. A large proportion of hospitalised patients are at risk for VTE, but there is a low rate of appropriate prophylaxis. Our data reinforce the rationale for the use of hospital-wide strategies to assess patients' VTE risk and to implement measures that ensure that at-risk patients receive appropriate prophylaxis.
Effects of combined GIP and GLP-1 infusion on energy intake, appetite and energy expenditure in overweight/obese individuals: a randomised, crossover study
Aims/hypothesisGlucagon-like peptide 1 (GLP-1) reduces appetite and energy intake in humans, whereas the other incretin hormone, glucose-dependent insulinotropic polypeptide (GIP), seems to have no effect on eating behaviour. Interestingly, studies in rodents have shown that concomitant activation of GIP and GLP-1 receptors may potentiate the satiety-promoting effect of GLP-1, and a novel dual GLP-1/GIP receptor agonist was recently shown to trigger greater weight losses compared with a GLP-1 receptor agonist in individuals with type 2 diabetes. The aim of this study was to delineate the effects of combined GIP and GLP-1 receptor activation on energy intake, appetite and resting energy expenditure in humans.MethodsWe examined 17 overweight/obese men in a crossover design with 5 study days. On day 1, a 50 g OGTT was performed; on the following 4 study days, the men received an isoglycaemic i.v. glucose infusion (IIGI) plus saline (154 mmol/l NaCl; placebo), GIP (4 pmol kg−1 min−1), GLP-1 (1 pmol kg−1 min−1) or GIP+GLP-1 (4 and 1 pmol kg−1 min−1, respectively). All IIGIs were performed in a randomised order blinded for the participant and the investigators. The primary endpoint was energy intake as measured by an ad libitum meal after 240 min. Secondary endpoints included appetite ratings and resting energy expenditure, as well as insulin, C-peptide and glucagon responses.ResultsEnergy intake was significantly reduced during IIGI+GLP-1 compared with IIGI+saline infusion (2715 ± 409 vs 4483 ± 568 kJ [mean ± SEM, n = 17], p = 0.014), whereas there were no significant differences in energy intake during IIGI+GIP (4062 ± 520 kJ) or IIGI+GIP+GLP-1 (3875 ± 451 kJ) infusion compared with IIGI+saline (p = 0.590 and p = 0.364, respectively). Energy intake was higher during IIGI+GIP+GLP-1 compared with IIGI+GLP-1 infusion (p = 0.039).Conclusions/interpretationWhile GLP-1 infusion lowered energy intake in overweight/obese men, simultaneous GIP infusion did not potentiate this GLP-1-mediated effect.Trial registrationClinicalTrials.gov NCT02598791FundingThis study was supported by grants from the Innovation Fund Denmark and the Vissing Foundation.
MACC regional multi-model ensemble simulations of birch pollen dispersion in Europe
This paper presents the first ensemble modelling experiment in relation to birch pollen in Europe. The seven-model European ensemble of MACC-ENS, tested in trial simulations over the flowering season of 2010, was run through the flowering season of 2013. The simulations have been compared with observations in 11 countries, all members of the European Aeroallergen Network, for both individual models and the ensemble mean and median. It is shown that the models successfully reproduced the timing of the very late season of 2013, generally within a couple of days from the observed start of the season. The end of the season was generally predicted later than observed, by 5 days or more, which is a known feature of the source term used in the study. Absolute pollen concentrations during the season were somewhat underestimated in the southern part of the birch habitat. In the northern part of Europe, a record-low pollen season was strongly overestimated by all models. The median of the multi-model ensemble demonstrated robust performance, successfully eliminating the impact of outliers, which was particularly useful since for most models this was the first experience of pollen forecasting.
Addition of rituximab to fludarabine and cyclophosphamide in patients with chronic lymphocytic leukaemia: a randomised, open-label, phase 3 trial
On the basis of promising results that were reported in several phase 2 trials, we investigated whether the addition of the monoclonal antibody rituximab to first-line chemotherapy with fludarabine and cyclophosphamide would improve the outcome of patients with chronic lymphocytic leukaemia. Treatment-naive, physically fit patients (aged 30–81 years) with CD20-positive chronic lymphocytic leukaemia were randomly assigned in a one-to-one ratio to receive six courses of intravenous fludarabine (25 mg/m 2 per day) and cyclophosphamide (250 mg/m 2 per day) for the first 3 days of each 28-day treatment course with or without rituximab (375 mg/m 2 on day 0 of first course, and 500 mg/m 2 on day 1 of second to sixth courses) in 190 centres in 11 countries. Investigators and patients were not masked to the computer-generated treatment assignment. The primary endpoint was progression-free survival (PFS). Analysis was by intention to treat. This study is registered with ClinicalTrials.gov, number NCT00281918. 408 patients were assigned to fludarabine, cyclophosphamide, and rituximab (chemoimmunotherapy group) and 409 to fludarabine and cyclophosphamide (chemotherapy group); all patients were analysed. At 3 years after randomisation, 65% of patients in the chemoimmunotherapy group were free of progression compared with 45% in the chemotherapy group (hazard ratio 0·56 [95% CI 0·46–0·69], p<0·0001); 87% were alive versus 83%, respectively (0·67 [0·48–0·92]; p=0·01). Chemoimmunotherapy was more frequently associated with grade 3 and 4 neutropenia (136 [34%] of 404 vs 83 [21%] of 396; p<0·0001) and leucocytopenia (97 [24%] vs 48 [12%]; p<0·0001). Other side-effects, including severe infections, were not increased. There were eight (2%) treatment-related deaths in the chemoimmunotherapy group compared with ten (3%) in the chemotherapy group. Chemoimmunotherapy with fludarabine, cyclophosphamide, and rituximab improves progression-free survival and overall survival in patients with chronic lymphocytic leukaemia. Moreover, the results suggest that the choice of a specific first-line treatment changes the natural course of chronic lymphocytic leukaemia. F Hoffmann-La Roche.
Loading of the knee joint during activities of daily living measured in vivo in five subjects
Detailed knowledge about loading of the knee joint is essential for preclinical testing of implants, validation of musculoskeletal models and biomechanical understanding of the knee joint. The contact forces and moments acting on the tibial component were therefore measured in 5 subjects in vivo by an instrumented knee implant during various activities of daily living. Average peak resultant forces, in percent of body weight, were highest during stair descending (346% BW), followed by stair ascending (316% BW), level walking (261% BW), one legged stance (259% BW), knee bending (253% BW), standing up (246% BW), sitting down (225% BW) and two legged stance (107% BW). Peak shear forces were about 10–20 times smaller than the axial force. Resultant forces acted almost vertically on the tibial plateau even during high flexion. Highest moments acted in the frontal plane with a typical peak to peak range −2.91% BWm (adduction moment) to 1.61% BWm (abduction moment) throughout all activities. Peak flexion/extension moments ranged between −0.44% BWm (extension moment) and 3.16% BWm (flexion moment). Peak external/internal torques lay between −1.1% BWm (internal torque) and 0.53% BWm (external torque). The knee joint is highly loaded during daily life. In general, resultant contact forces during dynamic activities were lower than the ones predicted by many mathematical models, but lay in a similar range as measured in vivo by others. Some of the observed load components were much higher than those currently applied when testing knee implants.
Modelling future changes in surface ozone: a parameterized approach
This study describes a simple parameterization to estimate regionally averaged changes in surface ozone due to past or future changes in anthropogenic precursor emissions based on results from 14 global chemistry transport models. The method successfully reproduces the results of full simulations with these models. For a given emission scenario it provides the ensemble mean surface ozone change, a regional source attribution for each change, and an estimate of the associated uncertainty as represented by the variation between models. Using the Representative Concentration Pathway (RCP) emission scenarios as an example, we show how regional surface ozone is likely to respond to emission changes by 2050 and how changes in precursor emissions and atmospheric methane contribute to this. Surface ozone changes are substantially smaller than expected with the SRES A1B, A2 and B2 scenarios, with annual global mean reductions of as much as 2 ppb by 2050 vs. increases of 4–6 ppb under SRES, and this reflects the assumptions of more stringent precursor emission controls under the RCP scenarios. We find an average difference of around 5 ppb between the outlying RCP 2.6 and RCP 8.5 scenarios, about 75% of which can be attributed to differences in methane abundance. The study reveals the increasing importance of limiting atmospheric methane growth as emissions of other precursors are controlled, but highlights differences in modelled ozone responses to methane changes of as much as a factor of two, indicating that this remains a major uncertainty in current models.
