From policy to practice: An assessment of biosecurity practices in cattle, sheep and goats production, marketing and slaughter in Baringo County, Kenya
Globally, biosecurity is instrumental in prevention, control and management of livestock diseases and protection of human health. It is defined, prescribed, adopted and enforced through global, regional and national frameworks, laws, policies and strategies. There is more biosecurity practice research conducted in developed countries than developing ones. Consequently, the gap between the ideals recommended in biosecurity frameworks and what is practical in under-resourced rural settings is poorly understood. This anthropological study sought to assess adoption of biosecurity practices across a cattle, sheep and goat value chains continuum to demonstrate where risks lie. The cross-sectional mixed-methods study took place in Baringo County, Kenya. Qualitatively, it utilized 26 focus group discussions with community members and 10 observational interviews with slaughter facility workers. Quantitatively, it included a household survey with 560 community members and a separate survey with 231 livestock traders. Results show that producers, traders and slaughter facility workers did observe some biosecurity practices but not others due but not limited to personal preference, limitations in veterinary service delivery and enforcement of some biosecurity measures, and lack of requisite infrastructure. The study concludes that the implementation of biosecurity measures in rural settings is more complex than envisioned in biosecurity policies and frameworks. It can be hampered by resource limitations, poor enforcement, and contestations with cultural practices. The study recommends that further studies on willingness to adopt biosecurity measures targeting community members in under-resourced settings be conducted to identify possible critical points of intervention at county and national levels.
Mapping the risk of Rift Valley fever in Uganda using national seroprevalence data from cattle, sheep and goats
Uganda has had repeated outbreaks of Rift Valley fever (RVF) since March 2016 when human and livestock cases were reported in Kabale after a long interval. The disease has a complex and poorly described transmission patterns which involves several mosquito vectors and mammalian hosts (including humans). We conducted a national serosurvey in livestock to determine RVF virus (RVFV) seroprevalence, risk factors, and to develop a risk map that could be used to guide risk-based surveillance and control measures. A total of 3,253 animals from 175 herds were sampled. Serum samples collected were screened at the National Animal Disease Diagnostics and Epidemiology Centre (NADDEC) using a competition multispecies anti-RVF IgG ELISA kit. Data obtained were analyzed using a Bayesian model that utilizes integrated nested Laplace approximation (INLA) and stochastic partial differential equation (SPDE) approaches to estimate posterior distributions of model parameters, and account for spatial autocorrelation. Variables considered included animal level factors (age, sex, species) and multiple environmental data including meteorological factors, soil types, and altitude. A risk map was generated by projecting fitted (mean) values, from a final model that had environmental factors onto a spatial grid that covered the entire domain. The overall RVFV seroprevalence was 11.3% (95% confidence interval: 10.2–12.3%). Higher RVFV seroprevalences were observed in older animals compared to the young, and cattle compared to sheep and goats. RVFV seroprevalence was also higher in areas that had (i) lower precipitation seasonality, (ii) haplic planosols, and (iii) lower cattle density. The risk map generated demonstrated that RVF virus was endemic in several regions including those that have not reported clinical outbreaks in the northeastern part of the country. This work has improved our understanding on spatial distribution of RVFV risk in the country as well as the expected burden of the disease in livestock.
“Without a man’s decision, nothing works”: Building resilience to Rift Valley fever in pastoralist communities in Isiolo Kenya
Rift Valley Fever (RVF) is a zoonotic disease that affects both livestock and humans. Men and women in pastoralist communities are vulnerable to RVF risk exposure because of their different roles and reliance on livestock products. This study sought to understand how ownership and decision-making in pastoralist male and female-headed households influence coping mechanisms and resilience to Rift Valley fever (RVF), using the three resilience capacities of absorptive, adaptive, and transformative. This study was conducted in two sub-counties (Garbatulla) and Merti), Isiolo County, Kenya. Data were collected through 16 focus group discussions and 13 key informant interviews with pastoralists and animal and human health stakeholders. The findings indicate that traditionally, men have the final say on decisions related to livestock ownership and make overall household decisions. Pastoralist men and women employ different approaches, including hygiene practices and mosquito nets, community knowledge dissemination, establishment of new businesses, utilization of healthcare, and indigenous medicines, to reduce the effects of RVF in both humans and livestock. They also collaborated with community disease surveillance initiatives to strengthen disease surveillance networks and gain access to county government support. This process fosters resilience, community empowerment, and transformative and sustainable adaptation responses to RVF.
Spatial analysis and risk mapping of Crimean-Congo hemorrhagic fever (CCHF) in Sub-Saharan Africa
Crimean Congo hemorrhagic fever (CCHF) is a re-emerging tick-borne zoonosis that is caused by CCHF virus (CCHFV). The geographical distribution of the disease and factors that influence its occurrence are poorly known. We analysed historical records on its outbreaks in various countries across the Sub-Saharan Africa (SSA) to identify hotspots and determine socioecological and demographic factors associated with these outbreaks. We used data from historical outbreaks that were reported between 1981 and 2022 in various countries in SSA. To develop a common framework for merging the outbreak data and potential explanatory variables, we generated a common shapefile that combined Level 2 administrative units in all the countries. Several climatic, environmental, socioecological data were obtained from on-line GIS databases and extracted using the shapefile. The data were analysed using an approximate Bayesian hierarchical model using the R-INLA package. The outcome was a Boolean variable which indicated whether an administrative unit in the shapefile was affected in a given year or not. A neighborhood structure was also generated and used to account for spatial autocorrelation in the analysis. The final model that was obtained from the analysis was used to build a CCHF risk map. A total of 54 CCHF outbreaks were compiled across 414 districts in nine SSA countries. Factors that were positively associated with CCHF outbreaks included human population density, land area under grassland, bare soil cover and shrub cover. Conversely, high precipitation during wet months, elevated mean temperature and slope had negative effects. The risk map generated shows that CCHF occurrence risk is higher in arid and semi-arid land (ASAL) of West Africa, the Sahelian region, Central Africa, and the Eastern and Southern Africa region. The analysis identified ecological and demographic factors that are associated with CCHF outbreaks in SSA. This finding suggests the need to improve surveillance for the disease especially in the grasslands where the human population is increasing.
“We do not bury dead livestock like human beings”: Community behaviors and risk of Rift Valley Fever virus infection in Baringo County, Kenya
Rift Valley Fever (RVF), is a viral zoonotic disease transmitted by Aedes and Culex mosquitoes. In Kenya, its occurrence is associated with increased rains. In Baringo County, RVF was first reported in 2006-2007 resulting in 85 human cases and 5 human deaths, besides livestock losses and livelihood disruptions. This study sought to investigate the county's current RVF risk status. A cross-sectional study on the knowledge, attitudes and practices of RVF was conducted through a mixed methods approach utilizing a questionnaire survey (n = 560) and 26 focus group discussions (n = 231). Results indicate that study participants had little knowledge of RVF causes, its signs and symptoms and transmission mechanisms to humans and livestock. However, most of them indicated that a person could be infected with zoonotic diseases through consumption of meat (79.2%) and milk (73.7%) or contact with blood (40%) from sick animals. There was a statistically significant relationship between being male and milking sick animals, consumption of milk from sick animals, consuming raw or cooked blood, slaughtering sick livestock or dead animals for consumption (all at p≤0.001), and handling sick livestock with bare hands (p = 0.025) with more men than women engaging in the risky practices. Only a few respondents relied on trained personnel or local experts to inspect meat for safety of consumption every time they slaughtered an animal at home. Sick livestock were treated using conventional and herbal medicines often without consulting veterinary officers. Communities in Baringo County engage in behaviour that may increase their risk to RVF infections during an outbreak. The authors recommend community education to improve their response during outbreaks.
The future of zoonotic risk prediction
In the light of the urgency raised by the COVID-19 pandemic, global investment in wildlife virology is likely to increase, and new surveillance programmes will identify hundreds of novel viruses that might someday pose a threat to humans. To support the extensive task of laboratory characterization, scientists may increasingly rely on data-driven rubrics or machine learning models that learn from known zoonoses to identify which animal pathogens could someday pose a threat to global health. We synthesize the findings of an interdisciplinary workshop on zoonotic risk technologies to answer the following questions. What are the prerequisites, in terms of open data, equity and interdisciplinary collaboration, to the development and application of those tools? What effect could the technology have on global health? Who would control that technology, who would have access to it and who would benefit from it? Would it improve pandemic prevention? Could it create new challenges? This article is part of the theme issue 'Infectious disease macroecology: parasite diversity and dynamics across the globe'.
Spatial and Temporal Pattern of Rift Valley Fever Outbreaks in Tanzania; 1930 to 2007
Rift Valley fever (RVF)-like disease was first reported in Tanzania more than eight decades ago and the last large outbreak of the disease occurred in 2006-07. This study investigates the spatial and temporal pattern of RVF outbreaks in Tanzania over the past 80 years in order to guide prevention and control strategies. A retrospective study was carried out based on disease reporting data from Tanzania at district or village level. The data were sourced from the Ministries responsible for livestock and human health, Tanzania Meteorological Agency and research institutions involved in RVF surveillance and diagnosis. The spatial distribution of outbreaks was mapped using ArcGIS 10. The space-time permutation model was applied to identify clusters of cases, and a multivariable logistic regression model was used to identify risk factors associated with the occurrence of outbreaks in the district. RVF outbreaks were reported between December and June in 1930, 1947, 1957, 1960, 1963, 1968, 1977-79, 1989, 1997-98 and 2006-07 in 39.2% of the districts in Tanzania. There was statistically significant spatio-temporal clustering of outbreaks. RVF occurrence was associated with the eastern Rift Valley ecosystem (OR = 6.14, CI: 1.96, 19.28), total amount of rainfall of >405.4 mm (OR = 12.36, CI: 3.06, 49.88), soil texture (clay [OR = 8.76, CI: 2.52, 30.50], and loam [OR = 8.79, CI: 2.04, 37.82]). RVF outbreaks were found to be distributed heterogeneously and transmission dynamics appeared to vary between areas. The sequence of outbreak waves, continuously cover more parts of the country. Whenever infection has been introduced into an area, it is likely to be involved in future outbreaks. The cases were more likely to be reported from the eastern Rift Valley than from the western Rift Valley ecosystem and from areas with clay and loam rather than sandy soil texture.
Ecological and subject-level drivers of interepidemic Rift Valley fever virus exposure in humans and livestock in Northern Kenya
Nearly a century after the first reports of Rift Valley fever (RVF) were documented in Kenya, questions on the transmission dynamics of the disease remain. Specifically, data on viral maintenance in the quiescent years between epidemics is limited. We implemented a cross-sectional study in northern Kenya to determine the seroprevalence, risk factors, and ecological predictors of RVF in humans and livestock during an interepidemic period. Six hundred seventy-six human and 1,864 livestock samples were screened for anti-RVF Immunoglobulin G (IgG). Out of the 1,864 livestock samples tested for IgG, a subset of 1,103 samples was randomly selected for additional testing to detect the presence of anti-RVFV Immunoglobulin M (IgM). The anti-RVF virus (RVFV) IgG seropositivity in livestock and humans was 21.7% and 28.4%, respectively. RVFV IgM was detected in 0.4% of the livestock samples. Participation in the slaughter of livestock and age were positively associated with RVFV exposure in humans, while age was a significant factor in livestock. We detected significant interaction between rainfall and elevation's influence on livestock seropositivity, while in humans, elevation was negatively associated with RVF virus exposure. The linear increase of human and livestock exposure with age suggests an endemic transmission cycle, further corroborated by the detection of IgM antibodies in livestock.
Gender roles in ruminant disease management in Uganda: Implications for the control of peste des petits ruminants and Rift Valley fever
There is a distinct division of tasks and roles between men and women in livestock management in the different ruminant production systems in Uganda. Division of roles can influence disease control and prevention. This qualitative study asks what men and women do to prevent or control diseases that affect them and their livestock and what factors influence the choice of disease control measures taken. Discussants represented three production systems (pastoral, agro-pastoral, and mixed crop livestock), selected for the high prevalence of two livestock diseases (peste des petits ruminants and Rift Valley fever). Sex-disaggregated focus group discussions with livestock keepers and key informant interviews with veterinarians and other experts were conducted in six districts in the western, northeastern, and eastern regions of Uganda. Gendered livestock management roles strategically positioned men, women, girls, and boys to observe different clinical manifestations of disease. Livestock keepers mostly reported within-farm prevention and control methods, for which they presumably had more control than between-farm or community-level methods. While livestock keepers embraced disease control options such as the use of drugs, spraying acaracides, and the use of traditional herbs, many had concerns and misconceptions about vaccination as a preventive measure. Although women had fewer concerns about vaccine side effects, they still faced constraints, such as mistrust of animal health workers, limited decision-making powers, domestic workload, and inability to access vaccination points. The study findings can guide appropriate, gender-responsive interventions tailored by production system for controlling ruminant diseases in Uganda.
