The economic burden of infertility treatment and distribution of expenditures overtime in France: a self-controlled pre-post study
Background Recent cost studies related to infertility treatment have focused on assisted reproductive technologies. None has examined lower-intensity infertility treatments or analyzed the distribution of infertility treatment expenditures over time. The Purpose of the study was to analyse the size and distribution of infertility treatment expenditures over time, and estimate the economic burden of infertility treatment per 10,000 women aged 18 − 50 in France from a societal perspective. Methods We used French National individual medico-administrative database to conduct a self-controlled before-after analytic cohort analysis with 556 incidental women treated for infertility in 2014 matched with 9,903 controls using the exact matching method. Infertility-associated expenditures per woman and per 10,000 women over the 3.5-year follow-up period derived as a difference-in-differences. Results The average infertility related expenditure per woman is estimated at 6,996 (95% CI: 5,755–8,237) euros, the economic burden for 10,000 women at 70.0 million (IC95%: 57.6–82.4) euros. The infertility related expenditures increased from 235 (IC95%: 98–373) euros in semester 0, i.e. before treatment, to 1,509 (IC95%: 1,277–1,741) euros in semester 1, mainly due to ovulation stimulation treatment (47% of expenditure), to reach a plateau in semesters 2 (1,416 (IC95%: 1,161–1,670)) and 3 (1,319 (IC95%: 943–1,694)), where the share of expenses is mainly related to hospitalizations for assisted reproductive technologies (44% of expenditure), and then decrease until semester 6 (577 (IC95%: 316–839) euros). Conclusion This study informs public policy about the economic burden of infertility estimated at 70.0 million (IC95%: 57.6–82.4) euros for 10,000 women aged between 18 and 50. It also highlights the importance of the share of drugs in infertility treatment expenditures. If nothing is done, the increasing use of infertility treatment will lead to increased expenditure. Prevention campaigns against the preventable causes of infertility should be promoted to limit the use of infertility treatments and related costs.
The NIKA2 Instrument, A Dual-Band Kilopixel KID Array for Millimetric Astronomy
New IRAM KID array 2 (NIKA2) is a camera dedicated to millimeter-wave astronomy based upon kilopixel arrays of kinetic inductance detectors [ 1 ] (KID). The pathfinder instrument, NIKA [ 2 ], has already shown state-of-the-art detector performance. NIKA2 builds upon this experience but goes one step further, increasing the total pixel count by a factor ∼ 10 while maintaining the same per pixel performance. For the next decade, this camera will be the resident photometric instrument of the Institut de Radio Astronomie Millimetrique (IRAM) 30 m telescopes in Sierra Nevada (Spain). In this paper, we give an overview of the main components of NIKA2 and describe the achieved detector performance. The camera has been permanently installed at the IRAM 30 m telescope in October 2015. It will be made accessible to the scientific community at the end of 2016, after a 1-year commissioning period. When this happens, NIKA2 will become a fundamental tool for astronomers worldwide.
NIKA2 observations of 3 low-mass galaxy clusters at z ~ 1: Pressure profile and YSZ – M relation
Three galaxy clusters selected from the XXL X-ray survey at high redshift and low mass (z ~ 1 and M500 ~ 1 – 2 × 1014 M⊙) were observed with NIKA2 to image their Sunyaev-Zel'dovich effect (SZ) signal. They all present an SZ morphology, together with the comparison with X-ray and optical data, that indicates dynamical activity related to merging events. Despite their disturbed intracluster medium, their high redshifts, and their low masses, the three clusters follow remarkably well the pressure profile and the SZ flux-mass relation expected from standard evolution. This suggests that the physics that drives cluster formation is already in place at z ~ 1 down to M500 ~ 1014 M⊙.
Constraining millimeter dust emission in nearby galaxies with NIKA2: The case of NGC2146 and NGC2976
This study presents the first millimeter continuum mapping observations of two nearby galaxies, the starburst spiral galaxy NGC2146 and the dwarf galaxy NGC2976, at 1.15 mm and 2 mm using the NIKA2 camera on the IRAM 30m telescope, as part of the Guaranteed Time Large Project IMEGIN. These observations provide robust resolved information about the physical properties of dust in nearby galaxies by constraining their FIR-radio SED in the millimeter domain. After subtracting the contribution from the CO line emission, the SEDs are modeled spatially using a Bayesian approach. Maps of dust mass surface density, temperature, emissivity index, and thermal radio component of the galaxies are presented, allowing for a study of the relations between the dust properties and star formation activity (using observations at 24μm as a tracer). We report that dust temperature is correlated with star formation rate in both galaxies. The effect of star formation activity on dust temperature is stronger in NGC2976, an indication of the thinner interstellar medium of dwarf galaxies. Moreover, an anti-correlation trend is reported between the dust emissivity index and temperature in both galaxies.
The NIKA2 Instrument at 30-m IRAM Telescope: Performance and Results
The New IRAM KID Arrays 2 (NIKA2) consortium has just finished installing and commissioning a millimetre camera on the IRAM 30-m telescope. It is a dual-band camera operating with three frequency-multiplexed kilo-pixels arrays of lumped element kinetic inductance detectors (LEKID) cooled at 150 mK, designed to observe the intensity and polarisation of the sky at 260 and 150 GHz (1.15 and 2 mm). NIKA2 is today an IRAM resident instrument for millimetre astronomy, such as intracluster medium from intermediate to distant clusters and so for the follow-up of Planck satellite detected clusters, high redshift sources and quasars, early stages of star formation and nearby galaxies emission. We present an overview of the instrument performance as it has been evaluated at the end of the commissioning phase.
IAS/CEA Evolution of Dust in Nearby Galaxies (ICED): The spatially-resolved dust properties of NGC4254
We present the first preliminary results of the project ICED , focusing on the face-on galaxy NGC4254. We use the millimetre maps observed with NIKA2 at lRAM-30m. as part of the IMEGIN Guaranteed Time Large Program. and of a wide collection of ancillary data (multi-wavelength photometry and gas phase spectral lines) that are publicly available. We derive the global and local properties of interstellar dust grains through infrared-to-radio spectral energy distribution fitting, using the hierarchical Bayesian code HerBIE. which includes the grain properties of the state-of-the-art dust model. THEMIS. Our method allows us to get the following dust parameters: dust mass, average interstellar radiation field, and fraction of small grains. Also, it is effective in retrieving the intrinsic correlations between dust parameters and interstellar medium properties. We find an evident anti-correlation between the interstellar radiation field and the fraction of small grains in the centre of NGC4254. meaning that, at strong radiation field intensities, very small amorphous carbon grains are efficiently destroyed by the ultra-violet photons coming from newly formed stars, through photo-desorption and sublimation. We observe a flattening of the anti-correlation at larger radial distances, which may be driven by the steep metallicity gradient measured in NGC4254.
Latest NIKA Results and the NIKA-2 Project
NIKA (New IRAM KID Arrays) is a dual-band imaging instrument installed at the IRAM (Institut de RadioAstronomie Millimetrique) 30-meter telescope at Pico Veleta (Spain). Two distinct Kinetic Inductance Detectors (KID) focal planes allow the camera to simultaneous image a field-of-view of about 2 arc-min in the bands 125 to 175 GHz (150 GHz) and 200 to 280 GHz (240 GHz). The sensitivity and stability achieved during the last commissioning Run in June 2013 allows opening the instrument to general observers. We report here the latest results, in particular in terms of sensitivity, now comparable to the state-of-the-art Transition Edge Sensors (TES) bolometers, relative and absolute photometry. We describe briefly the next generation NIKA-2 instrument, selected by IRAM to occupy, from 2015, the continuum imager/polarimeter slot at the 30-m telescope.
Exploring the interstellar medium of NGC 891 at millimeter wavelengths using the NIKA2 camera
In the framework of the IMEGIN Large Program, we used the NIKA2 camera on the IRAM 30-m telescope to observe the edge-on galaxy NGC 891 at 1.15 mm and 2 mm and at a FWHM of 11.1” and 17.6”, respectively. Multiwavelength data enriched with the new NIKA2 observations fitted by the HerBIE SED code (coupled with the THEMIS dust model) were used to constrain the physical properties of the ISM. Emission originating from the diffuse dust disk is detected at all wavelengths from mid-IR to mm. while mid-lR observations reveal warm dust emission from compact H II regions. Indications of mm excess emission have also been found in the outer parts of the galactic disk. Furthermore, our SED fitting analysis constrained the mass fraction of the small (< 15 Å) dust grains. We found that small grains constitute 9.5% of the total dust mass in the galactic plane, but this fraction increases up to ~ 20% at large distances (|z| > 3 kpc) from the galactic plane.
Preliminary results on the instrumental polarization of NIKA2-Pol at the IRAM 30m telescope
Clarifying the role of magnetic fields in the star formation process is crucial. Observations have already shown that magnetic fields play an important role in the early stages of star formation. The high spatial resolution (∼0.01 to 0.05 pc) provided by NIKA2-Pol 1.2 mm imaging polarimetry of nearby clouds will help us clarify the geometry of the B-field within dense cores and molecular filaments as part of the IRAM 30m large program B-FUN. There are numerous challenging issues in the validation of NIKA2-Pol such as the calibration of instrumental polarization. The commissioning phase of NIKA2-Pol is underway and is helping us characterize the intensity-to-polarization “leakage” pattern of the instrument. We present a preliminary analysis of the leakage pattern and its dependence with elevation. We also present the current leakage correction made possible by the NIKA2 pipeline in polarization mode based on the NIKA2-Pol commissioning data taken in December 2018. Based on reduced Stokes I, Q, U data we find that the leakage pattern of NIKA2-Pol depends on elevation and is sensitive to the focus of the telescope.
