The Use of Artificial Intelligence to Analyze the Exposome in the Development of Chronic Diseases: A Review of the Current Literature

The “Exposome” is a concept that indicates the set of exposures to which a human is subjected during their lifetime. These factors influence the health state of individuals and can drive the development of Noncommunicable Diseases (NCDs). Artificial Intelligence (AI) allows one to analyze large amounts of data in a short time. As such, several authors have used AI to study the relationship between exposome and chronic diseases. Under such premises, this study reviews the use of AI in analyzing the exposome to understand its role in the development of chronic diseases, focusing on how AI can identify patterns in exposure-related data and support prevention strategies. To achieve this, we carried out a search on multiple databases, including PubMed, ScienceDirect, and SCOPUS, from 1 January 2019 to 31 May 2023, using the MeSH terms (exposome) and (‘Artificial Intelligence’ OR ‘Machine Learning’ OR ‘Deep Learning’) to identify relevant studies on this topic. After completing the identification, screening, and eligibility assessment, a total of 18 studies were included in this literature review. According to the search, most authors used supervised or unsupervised machine learning models to study multiple exposure factors’ role in the risk of developing cardiovascular, metabolic, and chronic respiratory diseases. In some more recent studies, authors also used deep learning. Furthermore, the exposome analysis is useful to study the risk of developing neuropsychiatric disorders or evaluating pregnancy outcomes and child growth. Understanding the role of the exposome is pivotal to overcome the classic concept of a single exposure/disease. The application of AI allows one to analyze multiple environmental risks and their combined effects on health conditions. In the future, AI could be helpful in the prevention of chronic diseases, providing new diagnostic, therapeutic, and follow-up strategies.

Background: Traditionally, Eosinophilic Granulomatosis with Polyangiitis (EGPA) has been treated with systemic corticosteroids and immunosuppressants. In recent years, therapeutic efforts have been directed towards targeting eosinophils which represent a major player in the pathogenesis of EGPA. In 2017 the Food and Drug Administration (FDA) approved mepolizumab, a humanized monoclonal antibody targeting interleukin 5 (IL-5) which reduces the production and survival of eosinophils, already used to treat severe eosinophilic asthma, for the management of EGPA. Benralizumab is a humanized monoclonal antibody that targets the IL-5 receptor and is indicated in the treatment of severe eosinophilic asthma. Case description: We describe the case of a young female with a positive history of severe eosinophilic asthma associated with EGPA, treated successfully with benralizumab.

Immunological Markers Associated with Skin Manifestations of EGPA

Eosinophilic Granulomatosis with Polyangiitis (EGPA) is a rare systemic vasculitis with eosinophilic inflammation and variable clinical presentations. Although skin manifestations are frequent, current classification criteria do not include them, which may underestimate their diagnostic value. This prospective observational study aimed to assess systemic and skin involvement as well as eosinophilia, anti-neutrophil cytoplasmic antibody (ANCA), and Anti-nuclear antibodies (ANA) serum levels in 20 EGPA patients followed for one year at the University Hospital of Messina, Italy, before starting Mepolizumab, 300 mg. Eosinophilia, ANCA status, systemic and skin involvement were also evaluated at 6 and 12 months; a literature review on these data supplements our findings. Skin involvement was present in 55% of patients, including purpura, urticarial vasculitis, angioedema, maculopapular rash, and nodules, mostly in ANCA-negative patients, though purpura was more frequent in ANCA-positive cases but without any statistically significant correlation. ANAs were present in 50% of patients, together with ANCA in two subjects and without in eight. Mepolizumab significantly reduced eosinophil levels, BVASs, and corticosteroid dependence, with notable improvement in skin symptoms. In conclusion, skin manifestations are common in EGPA and may represent useful indicators of disease activity. Their integration with ANCA status, eosinophil counts, and positivity to other autoantibodies could enhance diagnostic and monitoring strategies identifying different clusters of EGPA patients even if the small sample size limits the generalizability of the findings.

The incidence of respiratory and cutaneous allergic disorders may be variable if we consider work activity. These disorders are reported in the current literature to have a relevant psychosomatic involvement. The aim of the study was to submit a survey on the self-reported occurrence of allergic respiratory and cutaneous diseases in outdoor and indoor workers to verify the onset or exacerbations of such pathologies, encourage surveillance, and suggest the need for further studies. Two groups of workers were compared when exposed and not exposed to air pollutants. An outdoor population of motorway toll collectors (153 workers; M: 58.03 years old, SD: 6.1; and female prevalence of 66.01%) and an indoor group of office employees (59 workers; mean age 54.44 years, SD: 8.50; and male prevalence of 61.02%) entered the study. The results of three multivariate analyses of the obtained data, investigating contributions of external factors, including age, sex, smoking habits, working type, and seniority, on allergic conditions were significant in both workers’ groups. The findings highlighted that age and smoking habits were significant contributors to allergic conditions, both independently and in combination with other factors, such as sex and working type. The data suggest the presence of phenomena related to different etiological domains, as already reported in the literature. Through the collection of these data, it was possible to highlight the need to analyze clinical signs from different perspectives.

A spotlight on lime: a review about adverse reactions and clinical manifestations due to Citrus aurantiifolia

Citrus aurantiifolia
) is a plant belonging to the family of Rutaceae and to the genus Citrus. The fruit is widely used in the United States, Mexico, Southeast Asia, Latin America, but is increasingly widespread all over the world. It is used as a fresh fruit, in the preparation of foods, sweets and drinks and its oils are used in the cosmetic and pharmaceutical industry. The main adverse reactions to lime seem to be represented by contact dermatitis, allergic and phototoxic type. In the context of allergic forms, several allergens have been identified in the citrus family, the main one being limonene, but no noteworthy cross-reactivity has been identified. However, a case of fruit protein contact dermatitis has been described, showing sensitization to other fruits, such as kiwi, avocado, pineapple and apple. There are several molecules responsible for phototoxic reactions and mainly belonging to the coumarin and furocoumarins families. Reactions related to ingesting the fruit or inhaling pollen from the tree appear to be rare, as there are no known cases reported in the literature. The increasing diffusion of lime in Europe must pay attention to possible adverse reactions due to contact with this fruit, which seem destined to increase in future years. Further importance must be placed on patch tests and on the possibility of using alternative extracts to classic fragrance mixes.

Charon, Pluto’s largest moon, has been extensively studied, with research focusing on its primitive composition and changes due to radiation and photolysis. However, spectral data have so far been limited to wavelengths below 2.5 μm, leaving key aspects unresolved. Here we present the detection of carbon dioxide (CO
2
) and hydrogen peroxide (H
2
O
2
) on the surface of Charon’s northern hemisphere, using JWST data. These detections add to the known chemical inventory that includes crystalline water ice, ammonia-bearing species, and tholin-like darkening constituents previously revealed by ground- and space-based observations. The H
2
O
2
presence indicates active radiolytic/photolytic processing of the water ice-rich surface by solar ultraviolet and interplanetary medium Lyman-
α
photons, solar wind, and galactic cosmic rays. Through spectral modeling of the surface, we show that the CO
2
is present in pure crystalline form and, possibly, in intimately mixed states on the surface. Endogenically sourced subsurface CO
2
exposed on the surface is likely the primary source of this component, with possible contributions from irradiation of hydrocarbons mixed with water ice, interfacial radiolysis between carbon deposits and water ice, and the implantation of energetic carbon ions from the solar wind and solar energetic particles.
Due to the limited wavelength coverage of measurements to date, some aspects of the composition of Pluto’s largest moon, Charon, remain unresolved. Here, the authors detect carbon dioxide and hydrogen peroxide on the surface of Charon’s northern hemisphere using JWST data.

Cytoskeleton Remodeling-Related Proteins Represent a Specific Salivary Signature in PSC Patients

Primary sclerosing cholangitis (PSC) and Primary biliary cholangitis (PBC) are chronic inflammatory biliary diseases characterized by progressive damage of the bile ducts, resulting in hepatobiliary fibrosis and cirrhosis. Currently, specific biomarkers that allow to distinguish between PSC and PBC do not exist. In this study, we examined the salivary proteome by carrying out a comprehensive and non-invasive screening aimed at highlighting possible quali-quantitative protein deregulations that could be the starting point for the identification of effective biomarkers in future. Saliva samples collected from 6 PBC patients were analyzed using a liquid chromatography–tandem mass spectrometry technique, and the results were compared with those previously obtained in the PSC group. We identified 40 proteins as significantly deregulated in PSC patients compared to the PBC group. The Gene Ontology and pathway analyses highlighted that several proteins (e.g., small integral membrane protein 22, cofilin-1, macrophage-capping protein, plastin-2, and biliverdin reductase A) were linked to innate immune responses and actin cytoskeleton remodeling, which is a critical event in liver fibrosis and cancer progression. These findings provide new foundations for a deeper understanding of the pathophysiology of PSC and demonstrate that saliva is a suitable biological sample for obtaining proteomic fingerprints useful in the search for biomarkers capable of discriminating between the two cholestatic diseases.

Connecting the Dots: Perceived Organization Support, Motive Fulfilment, Job Satisfaction, and Affective Commitment Among Volunteers

In this paper, the influence of organizational factors and the role of individual factors on volunteers’ job satisfaction and affective commitment is examined, in particular, whether volunteers’ motive fulfilment mediates the influence of perceived organizational support for the two outcomes of job satisfaction and affective commitment. A crosssectional survey was used to collect data from 213 volunteers from five non-profit organizations in Queensland, Australia. Perceived organizational support is found to have a significant relationship on volunteers’ job satisfaction and affective commitment. Furthermore, motive fulfilment had a significant effect on the impact of perceived organizational support (POS) on the two outcomes. The results of the study provide new knowledge about the importance of motive fulfilment as a tool for improving volunteers’ positive organizational experiences and explain how motive fulfilment promotes increased job satisfaction and affective commitment.

JWST Detection of Hydrocarbon Ices and Methane Gas on Makemake

JWST/NIRSpec observations of Makemake reveal a chemically complex surface and evidence of gaseous CH4. Our spectral modeling indicates a surface composition consisting of CH4, CH3D, and possibly CH3OH, combined with aggregates of C2H2 and C2H6. The presence of C2H4 is also considered given its expected photochemical origin. Both areal and layered configurations reproduce the observed spectrum, with the latter being preferred. This composition confirms earlier hydrocarbon detections and suggests that CH4 photolysis is either ongoing or occurred recently. The detection of CH3D yields a D/H ratio in CH4 ice of (3.98 ± 0.34) × 10−4, consistent within 2σ with previous estimates. We report the first detection of CH4 fluorescence from Makemake, establishing it as only the second trans-Neptunian object—after Pluto—with confirmed volatile release. We explore two scenarios consistent with the observed CH4 emission, though neither fully reproduces the data: an expanding coma, yielding production rates of (0.2–1.6) × 1028 molecules s−1 and a rovibrational temperature of ∼35 K, possibly originating from a localized plume, and a gravitationally bound atmosphere, which, if adopted, implies gas kinetic temperatures near 40 K and surface pressures of ∼10 pbar—values consistent with stellar occultation constraints and an atmosphere in equilibrium with surface CH4 ice. Discriminating between these scenarios will require higher spectral resolution and improved signal-to-noise observations. Together, the gas-phase CH4, intermediate D/H ratio between that in water and CH4 in comets, and complex surface composition challenge the traditional view of Makemake as a quiescent, frozen body.

Background
The thymic stromal lymphopoietin (TSLP), a key cytokine for development of Th2 immunity, is produced by cancer associated fibroblasts (CAFs) in pancreatic cancer where predominant tumor infiltrating Th2 over Th1 cells correlates with reduced patients’ survival. Which cells and molecules are mostly relevant in driving TSLP secretion by CAFs in pancreatic cancer is not defined.
Methods
We performed in vitro, in vivo and ex-vivo analyses. For in vitro studies we used pancreatic cancer cell lines, primary CAFs cultures, and THP1 cells. TSLP secretion by CAFs was used as a read-out system to identify in vitro relevant tumor-derived inflammatory cytokines and molecules. For in vivo studies human pancreatic cancer cells and CAFs were orthotopically injected in immunodeficient mice. For ex-vivo studies immunohistochemistry was performed to detect ASC (apoptosis-associated speck-like protein containing a caspase recruitment domain) expression in surgical samples. Bioinformatics was applied to interrogate published data sets.
Results
We show in vitro that IL-1α and IL-1β released by pancreatic cancer cells and tumor cell-conditioned macrophages are crucial for TSLP secretion by CAFs. Treatment of immunodeficient mice orthotopically injected with human IL-1 positive pancreatic cancer cells plus CAFs using the IL-1R antagonist anakinra significantly reduced TSLP expression in the tumor. Importantly, we found that pancreatic cancer cells release alarmins, among which ASC, able to induce IL-1β secretion in macrophages. The relevance of ASC was confirmed ex-vivo by its expression in both tumor cells and tumor associated macrophages in pancreatic cancer surgical samples and survival data analyses showing statistically significant inverse correlation between ASC expression and survival in pancreatic cancer patients.
Conclusions
Our findings indicate that tumor released IL-1α and IL-1β and ASC are key regulators of TSLP secretion by CAFs and their targeting should ultimately dampen Th2 inflammation and improve overall survival in pancreatic cancer.
