El compromiso autonómico en el diseño jurídico del juego responsable

La prevención, la intervención y la reparación en el sector del juego se convierten en basamentos sobre los que se vienen trazando los bocetos de nuevas normas, que en conjunto se prevé que ofrezcan un escenario de armonía en un ámbito cada día más complejo. Nuevos modelos de juegos y apuestas, el uso de medios tecnológicos y la proliferación de establecimientos en los que se desarrollan, han llevado a que desde enero de 2019 la Junta de Extremadura comenzase un proceso intenso de adaptación de las normas sobre el sector, tarea que se fundamenta en principios de juego responsable. La delimitación de qué se entiende por juego responsable, así como de la adopción de medidas de ordenación de las políticas de juego responsable darán protagonismo a las nuevas directrices normativas. Una labor que ha comenzado y requiere un arduo esfuerzo por parte de todos los agentes del sector en la que el papel de las Administraciones públicas se dibuja esencial. Antes, durante y después de la puesta en marcha de cada juego y de las distintas modalidades de apuestas se activarán las funciones de una Administración autonómica que orientará su actuación hacia objetivos de prevención y protección de los grupos vulnerables, en definitiva, de responsabilidad.

Effectiveness–implementation hybrid-2 randomised trial of a collaborative Shared Care Model for Detecting Neurodevelopmental Impairments after Critical Illness in Young Children (DAISY): pilot study protocol

IntroductionIn Australia, while paediatric intensive care unit (PICU) mortality has dropped to 2.2%, one in three survivors experience long-term neurodevelopmental impairment, limiting their life-course opportunities. Unlike other high-risk paediatric populations, standardised routine neurodevelopmental follow-up of PICU survivors is rare, and there is limited knowledge regarding the best methods. The present study intends to pilot a combined multidisciplinary, online screening platform and general practitioner (GP) shared care neurodevelopmental follow-up model to determine feasibility of a larger, future study. We will also assess the difference between neurodevelopmental vulnerability and parental stress in two intervention groups and the impact of child, parent, sociodemographic and illness/treatment risk factors on child and parent outcomes.Methods and analysisSingle-centre randomised effectiveness–implementation (hybrid-2 design) pilot trial for parents of children aged ≥2 months and <4 years discharged from PICU after critical illness or injury. One intervention group will receive 6 months of collaborative shared care follow-up with GPs (supported by online outcome monitoring), and the other will be offered self-directed screening and education about post-intensive care syndrome and child development. Participants will be followed up at 1, 3 and 6 months post-PICU discharge. The primary outcome is feasibility. Secondary outcomes include neurodevelopmental vulnerability and parental stress. An implementation evaluation will analyse barriers to and facilitators of the intervention.Ethics and disseminationThe study is expected to lead to a full trial, which will provide much-needed guidance about the clinical effectiveness and implementation of follow-up models of care for children after critical illness or injury. The Children’s Health Queensland Human Research Ethics Committee approved this study. Dissemination of the outcomes of the study is expected via publication in a peer-reviewed journal, presentation at relevant conferences, and via social media, podcast presentations and open-access medical education resources.Registration detailsThe trial was prospectively registered with the Australian New Zealand Clinical Trials Registry as ‘Pilot testing of a collaborative Shared Care Model for Detecting Neurodevelopmental Impairments after Critical Illness in Young Children’ (the DAISY Pilot Study).Trial registration numberACTRN12621000799853.

There is a current consensus on teacher training as a key element to promote the quality of higher education. Despite that, a lot of teacher preparation programs are short-term training efforts enmeshed in a politically endorsed ‘social market’ perspective that emphasized instructional skill training and adaptation to the existing curricular organization. In this context, training processes based on an innovative alternative vision framed in a dynamic learning system were context plays a determining role are crucial. This article presents a formative experience of teacher-researchers in the university context, theoretically oriented from systems thinking and characterized by a holistic approach based on contextual learning. The research was designed using Grounded Theory with a biographical-narrative approach. Participant observation, sociodemographic questionnaire, and semi-structured interviews were used as data collection instruments. The results show a formative process in which the pedagogical practice is the training core, and highlight the strengths of systems thinking to favor the co-construction of a teacher identity that integrates biographies and personal interests of teachers, as well as the configuration of teacher training processes oriented to collaborative learning. Additionally, it was found that the process shows strengths in several relevant aspects in teacher education such as reflective teaching as a rejection of top-down forms of educational programs, the importance of reflective thinking and reflective feeling and the importance of collaborative learning to contribute to the solution of contextual problems.

En la actualidad, el turismo educativo internacional va creciendo muy positivamente cada año, sobre todo entre los estudiantes internacionales universitarios y más concretamente entre los estudiantes procedentes del programa Erasmus. Éste es el caso de España, concretamente la ciudad de Córdoba, donde podemos hablar de la existencia de una demanda real y creciente de estudiantes internacionales universitarios. En este estudio se analizarán las motivaciones de los estudiantes internacionales universitarios de la ciudad de Córdoba que dan lugar al turismo educativo internacional en dicha ciudad con el fin de crear otras vías de fomento para atraer a más estudiantes universitarios internacionales y así poder desarrollar otros tipos de turismo. Entre las razones para elegir Córdoba como ciudad de destino, cabe destacar el idioma como la principal razón para venir a Córdoba a estudiar. Además del motivo idiomático, los estudiantes internacionales universitarios eligen Córdoba como ciudad de destino turístico por su situación geográfica.

IntroductionPeripheral intravenous catheters (PIVCs) are frequently used in hospitals. However, PIVC complications are common, with failures leading to treatment delays, additional procedures, patient pain and discomfort, increased clinician workload and substantially increased healthcare costs. Recent evidence suggests integrated PIVC systems may be more effective than traditional non-integrated PIVC systems in reducing phlebitis, infiltration and costs and increasing functional dwell time. The study aim is to determine the efficacy, cost–utility and acceptability to patients and professionals of an integrated PIVC system compared with a non-integrated PIVC system.Methods and analysisTwo-arm, multicentre, randomised controlled superiority trial of integrated versus non-integrated PIVC systems to compare effectiveness on clinical and economic outcomes. Recruitment of 1560 patients over 2 years, with randomisation by a centralised service ensuring allocation concealment. Primary outcomes: catheter failure (composite endpoint) for reasons of: occlusion, infiltration/extravasation, phlebitis/thrombophlebitis, dislodgement, localised or catheter-associated bloodstream infections. Secondary outcomes: first time insertion success, types of PIVC failure, device colonisation, insertion pain, functional dwell time, adverse events, mortality, cost–utility and consumer acceptability. One PIVC per patient will be included, with intention-to-treat analysis. Baseline group comparisons will be made for potentially clinically important confounders. The proportional hazards assumption will be checked, and Cox regression will test the effect of group, patient, device and clinical variables on failure. An as-treated analysis will assess the effect of protocol violations. Kaplan-Meier survival curves with log-rank tests will compare failure by group over time. Secondary endpoints will be compared between groups using parametric/non-parametric techniques.Ethics and disseminationEthical approval from the Royal Brisbane and Women’s Hospital Human Research Ethics Committee (HREC/16/QRBW/527), Griffith University Human Research Ethics Committee (Ref No. 2017/002) and the South Metropolitan Health Services Human Research Ethics Committee (Ref No. 2016–239). IntroductionAround 30% of peripherally inserted central catheters (PICCs) fail from vascular, infectious or mechanical complications. Patients with cancer are at highest risk, and this increases morbidity, mortality and costs. Effective PICC dressing and securement may prevent PICC failure; however, no large randomised controlled trial (RCT) has compared alternative approaches. We designed this RCT to assess the clinical and cost-effectiveness of dressing and securements to prevent PICC failure.Methods and analysisPragmatic, multicentre, 2×2 factorial, superiority RCT of (1) dressings (chlorhexidine gluconate disc (CHG) vs no disc) and (2) securements (integrated securement dressing (ISD) vs securement device (SED)). A qualitative evaluation using a knowledge translation framework is included. Recruitment of 1240 patients will occur over 3 years with allocation concealment until randomisation by a centralised service. For the dressing hypothesis, we hypothesise CHG discs will reduce catheter-associated bloodstream infection (CABSI) compared with no CHG disc. For the securement hypothesis, we hypothesise that ISD will reduce composite PICC failure (infection (CABSI/local infection), occlusion, dislodgement or thrombosis), compared with SED. Secondary outcomes: types of PICC failure; safety; costs; dressing/securement failure; dwell time; microbial colonisation; reversible PICC complications and consumer acceptability. Relative incidence rates of CABSI and PICC failure/100 devices and/1000 PICC days (with 95% CIs) will summarise treatment impact. Kaplan-Meier survival curves (and log rank Mantel-Haenszel test) will compare outcomes over time. Secondary end points will be compared between groups using parametric/non-parametric techniques; p values <0.05 will be considered to be statistically significant.Ethics and disseminationEthical approval from Queensland Health (HREC/15/QRCH/241) and Griffith University (Ref. El Colegio de México, a través del Centro de Estudios Demográficos, Urbanos y Ambientales (CEDUA), organizó una Celebración a la Trayectoria de Manuel Ángel Castillo. El homenaje se llevó a cabo en la Sala Alfonso Reyes de la institución, el 5 de abril de 2022. Participaron Gustavo Garza (Programa de Estudios Interdisciplinarios, Colmex), Martha Rojas (El Colegio de la Frontera Sur), Jéssica Nájera (CEDUA, Colmex), Leticia Argüelles (editora de Estudios Demográficos y Urbanos, CEDUA, Colmex), Silvia Giorguli (presidenta de El Colegio de México), María Isabel Molina (esposa del homenajeado) y, por último, Manuel Ángel Castillo, quien pronunció unas palabras de agradecimiento. Educación ambiental en el bachillerato: De la escuela a la familia

El documento tiene como objetivo mostrar cómo los estudiantes del nivel medio superior (NMS) involucran y concientizan a su familia, a través de acciones prácticas y viables para conservar y cuidar el entorno escolar y comunitario. La investigación se desarrolla en el marco de la metodología cualitativa. La información se recolectó utilizando el método de observación directa o participante; tareas, bitácoras, asimismo se les aplicó un cuestionario con preguntas para respuestas en forma abierta y cerrada. Esta investigación fue realizada en el Centro de Bachillerato Tecnológico Industrial y Servicios 212 (CBTis) de Tetla de la Solidaridad, Tlaxcala, México, en el año 2017. Los sujetos de estudio fueron 85 estudiantes y 282 familiares, vinculados para operar su Programa Ambiental Familiar (PAF) como estrategia operativa. Los datos resultantes del trabajo de campo se sistematizaron y procesaron para facilitar su análisis e interpretación. Los estudiantes aplicaron el PAF, con sus familias, así como enseñar a cada uno de sus integrantes para preservar el medio ambiente. Se concluye que la educación ambiental debe ser de padres a hijos y de hijos a padres, esto permite reforzar su propia conciencia, desarrollar, fomentar valores y conductas amigables con el medio ambiente, para el cuidado y conservación de los recursos naturales. La población mexicana necesita fomentar la cultura ambiental.
