Olympic combat sports and mental health in children and adolescents with disability: A protocol paper for systematic review

Mental health is important for children and adolescents, particularly those with disabilities. While the mental health advantages of sports participation are well-documented, the specific type of sport may have heightened relevance for children and adolescents with disabilities. The objective of this systematic review protocol is to outline the rationale and methodology for investigating how participation in Olympic combat sports influences the mental health outcomes of this unique population, which is more susceptible to developing mental health issues than their neurotypical counterparts.
A comprehensive search will be conducted across academic databases, including the Cochrane Library, ERIC, PsycINFO, PubMed, Scopus, SPORTDiscus, and Web of Science. The focus will be on identifying randomized and non-randomized controlled trials (RCTs and non-RCTs, respectively), and observational studies with control groups that explore the impact of Olympic combat sports on the mental health of children and adolescents with disabilities. To assess the risk of bias, the Rob 2.0 tool will be employed for RCTs, and the ROBINS-I tool for CTs. For longitudinal and cross-sectional studies, the National Institute of Health's Study Quality Assessment Tool for Observational Cohort and Cross-sectional Studies will be used. The review process will be conducted using Covidence, possibly utilizing JASP software for meta-analysis if the retrieved studies exhibit sufficient homogeneity. Data that cannot be included in the meta-analysis will be synthesized using the Synthesis without Meta-Analysis (SWiM) tool. Furthermore, the Consolidated Framework for Implementation Research (CFIR) will provide a framework consisting of five broad domains: intervention characteristics, outer setting, inner setting, characteristics of individuals, and the process of implementation.

The lifelong education of coaches is one of the priorities of the European Union. Therefore, the purpose of this study was to investigate Italian elite coaches’ motivation to engage in a sport-related academic education, and its demands, barriers, support, and relocation issues in relation to their dual career (DC) path. Sixteen Italian elite coaches (e.g., certified fourth-level national team coaches, senior team coaches) enrolled in a specifically tailored Bachelor’s degree in sports sciences at the University of Rome Foro Italico (Italy) volunteered for this study. A qualitative approach integrating inductive and deductive reasoning, and thematic analysis was applied to participants’ responses to an open-ended item survey. Independently from relocation, student-coaches’ DC perceptions resulted in 15 lower-order themes further organized in 5 high-er-order themes (e.g., Benefit, Challenge, Expectation, Organization, and Support), each related to the contexts (e.g., Personal, Academic, Sport), the DC dimension (e.g., micro, meso, macro, and policy), and the DC push (e.g., facilitating) /pull (e.g., hindering) factors. The elite coaches’ insights emphasize the complexity of the coach lifelong education at university level, and provide valuable information for promoting European and National (e.g., Italian) DC recommendations for elite sportspersons through a cooperation between sport bodies and higher educational institutions.

Relative Age Effect in Olympic Karate: Evidence from Tokyo 2020

Background: The Relative Age Effect (RAE) refers to the advantage conferred to athletes born earlier within a selection year. In karate, particularly at the highest level, evidence is lacking. This study aimed to examine the presence of RAE among male and female karate athletes competing at the Tokyo 2020 Olympic Games, in two competitive disciplines: kata and kumite. Methods: Data from 81 athletes (42 males, 39 females) were retrieved from open-access databases. Birthdates were grouped into quartiles (Q1–Q4) and semesters (S1–S2). Chi-squared tests and odds ratios (ORs) were used to assess deviations from a uniform distribution, while binary logistic regression examined the association between semester of birth and medal attainment. Results: The overall distribution of birth quartiles significantly deviated from a uniform distribution (χ2(3) = 9.81, p = 0.020), indicating a higher proportion of athletes born in Q1 (38%) compared with Q4 (19%; OR = 2.07). RAE was particularly evident in kumite (χ2(3) = 17.87, p < 0.001; OR = 3.50 for Q1 vs. Q4) and among female athletes (χ2(3) = 9.92, p = 0.019), whereas no significant effect was found in kata or among males. Logistic regression revealed no significant association between semester of birth and medal success (OR = 0.49, 95% CI [0.20–1.21], p = 0.125). Conclusions: This study provides the first evidence of RAE in Olympic karate, especially among females and in kumite. However, relative age did not predict performance outcomes, suggesting that once athletes reach the Olympic level, technical and tactical factors outweigh birthdate advantages.

Sustainable Career Transitions and Mental Health Support in Elite Sport: A Systematic Review of Evidence and Practices

The present systematic review aims to provide a comprehensive synthesis of evidence and practices regarding sustainable career transitions in elite sport. Following PRISMA guidelines, an extensive literature search was conducted in SPORTDiscus (EBSCOhost), PsycINFO, Scopus, Web of Science, and Google Scholar databases, resulting in 117 manuscripts, published from January 2015 to May 2025, and meeting the defined inclusion criteria. The review focused on mental health, dual-career pathways, transition readiness, and identity-related issues among elite athletes, Olympians, and Paralympians. Methodologies included qualitative, quantitative, and mixed-methods designs, with multisport and mixed-gender samples prevailing. The most commonly used instruments were semi-structured interviews and surveys. The main findings highlighted the centrality of mental health support, the role of dual-career planning, and the importance of proactive identity negotiation. Despite growing research interest, significant gaps persist in access to psychological support, structured transition planning, and dual-career strategies, with notable inconsistencies across countries and sports. The review emphasizes the necessity for integrated, multidimensional guidance, culturally sensitive psychological services, and flexible educational pathways to promote athlete well-being and sustainable post-sport careers. These insights are intended to inform the implementation of the ERASMUS+ funded PORTAL project, supporting evidence-based interventions and the development of resources such as an online platform and Real-Life Transition Officers to enhance the transition experiences of elite athletes.

Previous physical activity interventions for children (5-12yrs) have aimed to change determinants associated with self-report physical activity behaviour (PAB) and/or sedentary behaviour (SB), however, the associations between these determinants and PAB/SB in different settings are uncertain. The present study aimed to identify modifiable determinants targeted in previous PAB/SB interventions for children. Intervention effects on the determinants and their associations with self-report PAB/SB were assessed across settings. Search of relevant interventions from pre-defined databases was conducted up to July 2023. Randomized and non-randomized controlled trials with modifiable determinants were included. Data extraction and risk of bias assessments were conducted by two independent researchers. Where data could be pooled, we performed Robust Bayesian meta-analyses. Heterogeneity, publication bias and certainty of evidence were assessed. Fifteen studies were deemed eligible to be included. Thirty-seven unique determinants within four settings were identified–school, family, school with family/home, and community with(out) other settings. Ninety-eight percent of determinants belonged to individual/interpersonal determinant categories. Narratively, intervention effects on student perception of teachers’ behaviour (school), self-management, perceived barriers, external motivation, exercise intention, parental modeling on SB (school with family/home) and MVPA expectations (community) were weak to strong, however, corresponding PAB/SB change was not evident. There were negligible effects for all other determinants and the corresponding PAB/SB. Meta-analyses on self-efficacy, attitude, subjective norm and parental practice and PAB/SB in two settings showed weak to strong evidence against intervention effect, while the effect on knowledge could not be determined. Similarly, publication bias and heterogeneity for most analyses could not be ascertained. We found no concrete evidence of association between the modifiable determinants and self-report PAB/SB in any settings. This is presumably due to intervention ineffectiveness. Design of future interventions should consider to follow the systems-based approach and identify determinants unique to the context of a setting, including policy and environmental determinants.

Intergenerational Judo: Synthesising Evidence- and Eminence-Based Knowledge on Judo across Ages

In alignment with European policies regarding intergenerational sports, this study seeks to integrate existing evidence- (i.e., umbrella review) and eminence- (i.e., international validated educational courses for coaches) based knowledge on judo training across the lifespan. For evidence knowledge, searches of the Google Scholar, ISI-WoS, PubMed, and Scopus databases (PROSPERO ID: CRD42024535420) for studies focused on training youth and older judo practitioners, with no time restriction up to April 2024, were conducted. This resulted in 16 systematic reviews meeting the inclusion criteria, with an “excellent” (n = 6), “good” (n = 7), and “fair” (n = 3) quality according to the NIH tool. With a large sample size (n = 372,655), the reviews focused on wellbeing (n = 9; bone status, injuries, and psychophysical effects) and performance (n = 7; athlete success, combat time, rapid weight loss, and the Special Judo Fitness Test), examining athletic levels (novices to Olympics), sex, and age (children to older adults). For eminence knowledge, the International Judo Federation Academy courses encompassed the “Undergraduate Certificate as Judo Instructor” (UCJI), which provides a comprehensive structure for teaching judo, including techniques, moral code, roles, responsibilities, refereeing, safety, and age-specific belt examination requirements and progression, and the “Educating Judo Coaches for Older Practitioners” (EdJCO) curricula, which provide theoretical and applied modules (e.g., ageing, health, and organisation) to train older judo practitioners. The findings were summarised in a framework, highlighting the most relevant aspects of intergenerational judo programmes (i.e., aims, key characteristics, coaches’ roles, barriers, and facilitators). To develop a comprehensive educational intergenerational judo methodology, future research should examine coaches’ and practitioners’ opinions on intergenerational judo activities carried out in different national contexts.

Risks and Benefits of Judo Training for Middle-Aged and Older People: A Systematic Review

This systematic overview aimed to review studies investigating the benefits and risks of judo training in older people, and to explore practical methodological applications (Registration ID: CRD42021274825). Searches of EBSCOhost, ISI-WoS, and Scopus databases, with no time restriction up to December 2022, resulted in 23 records meeting the inclusion criteria. A quality assessment was performed through the following tools: ROBINS-I for 10 experimental studies, NIH for 7 observational studies, and AGREE-II for 6 methodological studies. A serious risk of bias emerged for 70% of the experimental studies, whereas 100% of the observational and 67% of the methodological studies presented a “fair” quality. When involving 1392 participants (63 ± 12 years; females: 47%), the studies investigated novice (n = 13), amateur/intermediate (n = 4), expert (n = 4), and unknown (n = 3) level judoka by means of device-based, self-reported, and visual evaluation measures. Mean training encompassed 2 ± 1 sessions. week−1 of 61 ± 17 min for 7 ± 6 months. In relation to judo training exposure and outcomes, three main themes emerged: (i) health (56% of studies; e.g., bones, anthropometry, quality of life); (ii) functional fitness (43%; e.g., balance, strength, walking speed); and iii) psychosocial aspects (43%; e.g., fear of falling, cognition, self-efficacy). Although the included studies presented relevant methodological weaknesses, the data support the positive effects of judo training with advancing age. Future research is needed to help coaches plan judo programs for older people.

Stress-Related Hormonal and Psychological Changes to Simulated and Official Judo Black Belt Examination in Older Tori and Adult Uke: An Exploratory Observational Study

This study investigated the psycho-physiological impact of a black belt examination. Older brown-belt judoka (Tori, F = 2, M = 4; age = 75.6 ± 4.5 yrs) and their 2nd–5th Dan black-belt coaches (Uke; M = 6; age = 36.5 ± 10.8 yr) were evaluated during a simulated and official examination and a resting day. Participants’ trait anxiety (STAI-Y2) was recorded prior to the study. State anxiety (STAI-Y1), ratings of perceived exertion (RPE), enjoyment (ENJ), and fear of falling (FoF) were collected 15 min before and after the experimental conditions. Saliva samplings at awakening (T0), PRE (T1), and POST (T2) exercise and during the recovery (15 min-T3, 30 min-T4, 60 min-T5) were collected for cortisol (sC), testosterone (sT), and alpha-amylase (sAA). Participants showed normal age-reference population trait anxiety. A difference (p ≤ 0.05) for role emerged for ENJ and sT only. For STAI-Y1, higher PRE values with respect to POST ones emerged (p = 0.005), and the highest values (p = 0.007) for PRE of the examination were with respect to the simulation. For sAA, differences for sampling were found in the examination conditions only, with peak values at T2 (370.3 ± 78.6 U/mL, p = 0.001). For sC, a significant peak value (0.51 ± 0.09 μg/dL, p = 0.012) emerged at T2 in the examination condition. With respect to Tori, Uke showed higher mean sT values in all conditions (p ≤ 0.05) and the highest T2 during examination (712.5 ± 57.2 pg/mL). Findings suggest the relevance of monitoring psycho-physiological stress-related responses in judo for optimizing both coaching effectiveness and sport performance, especially in older judo practitioners.

Proactive personality and positional competition: exploring the impact on soccer players’ performance and gender differences

Aim
The study aims to explore the relationship between proactive personality and positional competition in soccer players, considering factors like gender, age, position, and years of active participation in the sport.
Method
Quantitative data collection methods were employed, including the Proactive Personality Scale and Positional Competition in Sports Scale, along with a custom personal information form. The study involved 242 soccer players (average age: 20.60 years, 39% female) from semi-amateur soccer clubs in Ankara province, surveyed during the 2022–2023 season.
Results
The study found no significant differences in proactive personality and positional competition among soccer players based on their age and position (
p
> .05). However, gender differences were significant, with male players scoring higher than females in receiving and providing teammate support, communication, and overall positional competition in sports.
Conclusion
Proactive personality can be a determinant of positional competition in sports. Moreover, these findings may provide important contributions to the development of talent identification, athlete development programs and coaching strategies that aim to increase competitiveness and success in positional contexts. In addition, in the present study, it can be concluded that following athletes over time is an important phenomenon for proactive personality and athletic development.

Modifiable determinants of older adults’ physical activity and sedentary behavior in community and healthcare settings: a DE-PASS systematic review and meta-analysis

Objectives
To identify the modifiable determinants targeted in interventions involving older adults, and to determine which of these interventions effectively increased physical activity (PA) and/or reduced sedentary behaviour (SB). Additionally, to explore whether the effects of these interventions vary based on the implementation setting.
Methods
A search of randomized controlled trials (RCTs) and controlled trials (CTs) was performed in Medline, APA PsycArticles, SPORTDiscus, and Web of Science. Risk of bias assessment was performed with Cochrane’s tool. Modifiable determinants were narratively synthesized, and random-effects models were performed to meta-analyse studies reporting device-measured physical activity or sedentary behaviour. Moderator analyses were performed to investigate the role of implementation setting. Standardized between-group mean difference (SMD) with 95% confidence interval (CI) was used to indicate effect sizes.
Results
From 31,727 individual records, 52 eligible studies published between 2012–2022 were identified, 30 and 22 studies from community and health care settings, respectively. Determinants within the category physical health and wellbeing (
n
= 23) were most frequently reported while only one study reported determinants within a social or cultural context. Eighteen studies were included in the meta-analysis. Interventions targeting physical health and wellbeing revealed an increase in steps (SMD = 0.46; 95%CI: 0.15 to 0.77) and minutes of moderate-to-vigorous intensity physical activity (SMD = 0.41; 95%CI: 0.19 to 0.64) among intervention participants compared to controls, whereas interventions targeting psychological or behavioural determinants showed no between-group differences in steps (SMD = 0.10; 95%CI: -0.12 to 0.32) and moderate-to-vigorous intensity physical activity (SMD = 0.26; 95%CI: -0.24 to -0.75). Interventions targeting physical health and wellbeing showed significant heterogeneity (
p

= 73.10%). Subgroup analyses showed a significant effect on device-measured physical activity for the eight community-based interventions (SMD = 0.42; 95%CI: 0.07 to 0.77), while no significant effect was found for the eight studies performed in healthcare settings (SMD = 0.26; 95%CI; -0.10 to 0.62).
Conclusion
Interventions targeting physical health and wellbeing may increase PA in older adults, with community-based studies appearing more effective than studies in healthcare settings. The significant heterogeneity of study findings indicates that further research is needed to fully understand the influence of PA and SB determinants across settings, particularly those related to psychological, behavioural, social, and cultural factors.
