Determination of Side Effects and Medication Adherence in Major Depression Patients Utilized Antidepressants

The aim of the study is to determine side effects, and medication adherence in patients who were diagnosed with major depression utilized antidepressants. This study was conducted in outpatient psychiatry clinic between February 2013 and May 2013. 'Toronto Side Effects Scale' and 4-item Morisky-Green- Levine Medication Adherence Scale were evaluated. Fifty-six major depression patients (mean of age: 40.82 ± 14.28 years old; male/female: 13/43) were included in this study. Patients were treated with escitalopram (46.4%), sertraline (26.8%), venlafaxine (10.7%), citalopram (8.9%) and paroxetine (7.1%). The most common side effects that patients reported were drowsiness/ daytime somnolence (57.1%), dry mouth (46.4%) and increased sleep (46.4%), increased appetite (37.5%) and malaise or fatigue (30.4%). The most severity side effects that patients experienced were drowsiness/ daytime somnolence (25%), increased sleep (19.6%) and decreased libido (14.3%). Only fifteen (26.8%) patients were found high adherent to their medication. It was concluded that although the frequency and severity of side effects have been found high, the percentage of patients' medication adherence has been obtained low. Patient education and monitoring regarding side effects and medication adherence providing by pharmacists would be contributed to prevent possible drug induced problems in these patients besides the routine services they are taken from outpatient clinic.

Türkçe Ders Kitaplarında Kabul ve Ret Bildiren İfadeler

Kabul ve ret bildiren ifadeleri günlük hayatta sıkça kullanılmaktadır. Bu ifadelerin ders kitaplarında nasıl kullanıldığına dair daha önce yapılmış herhangi bir çalışma bulunmamaktadır. Günlük hayatta kabul bildiren ifadelerin ret bildiren ifadelere oranla daha çok kullanılması onaylamanın reddetmeye göre daha kolay olmasından kaynaklandığı söylenebilir. Bu çalışmada, öncelikle Türk dili alanı uzmanlarıyla görüşmeler yapılmış, sözlükler taranarak kabul ve ret bildiren ifadeler tespit edilmiştir. Daha sonra belirlenen ifadeler Türkçe ders kitaplarında aranmıştır. Bu çalışmanın amacı Türkçe ders kitaplarında, kabul ve ret bildiren ifadelerin ne ölçüde kullanıldığını tespit etmektir. Çalışmada 2018-2019 Eğitim-Öğretim yılında kullanılan 5, 6, 7 ve 8. Sınıf Türkçe ders kitapları incelenmiş, elde edilen verilerin analizinde, nitel araştırma yöntemlerinden biri olan betimsel analiz kullanılmıştır. Elde edilen veriler çalışmanın amacı doğrultusunda sınıflandırılmıştır. Çalışmanın sonucunda ders kitaplarında kabul bildiren ifadelerin ret bildiren ifadelerden daha fazla karşımıza çıktığı görülmüştür. Hayır diyebilmek insan olmanın gereklerinden biridir. Aileler ve eğitimcilerin önemli amaçlarından biri olan çocuklara hayır demeyi öğretme sürecinde eğitim ortamındaki en kolay ulaşılır materyal olan ders kitabının da yeri oldukça önemlidir. Türkçe dersi, çocuklara kendi haklarını savunma ve istemedikleri durumlara karşı çıkabilme becerisi kazandırmada diğer derslere göre daha avantajlıdır. Bu nedenle Türkçe ders kitaplarına metin seçilirken, kişinin hayır deme hakkını işleyen, hayır diyebilmenin olağan ve bazı durumlarda gerekli olduğunu benimseten ayrıca hayır diyebilme becerisini pekiştirmesine imkân tanıyan metinlerin seçilmesine dikkat edilmelidir.

Thermocouple and Infrared Sensor-Based Measurement of Temperature Distribution in Metal Cutting

In metal cutting, the magnitude of the temperature at the tool-chip interface is a function of the cutting parameters. This temperature directly affects production; therefore, increased research on the role of cutting temperatures can lead to improved machining operations. In this study, tool temperature was estimated by simultaneous temperature measurement employing both a K-type thermocouple and an infrared radiation (IR) pyrometer to measure the tool-chip interface temperature. Due to the complexity of the machining processes, the integration of different measuring techniques was necessary in order to obtain consistent temperature data. The thermal analysis results were compared via the ANSYS finite element method. Experiments were carried out in dry machining using workpiece material of AISI 4140 alloy steel that was heat treated by an induction process to a hardness of 50 HRC. A PVD TiAlN-TiN-coated WNVG 080404-IC907 carbide insert was used during the turning process. The results showed that with increasing cutting speed, feed rate and depth of cut, the tool temperature increased; the cutting speed was found to be the most effective parameter in assessing the temperature rise. The heat distribution of the cutting tool, tool-chip interface and workpiece provided effective and useful data for the optimization of selected cutting parameters during orthogonal machining.

H3B2 Scoring Validation and Comparing of the Other Scoring Systems in Patients with Upper Gastrointestinal Bleeding: A Retrospective Study

Objective:We had validation of H3B2 scoring on Turkish patients in this study. In addition, it was compared with Glasgow-Blatchford and AIMS65 scoring.Method:This study was conducted retrospectively and single centered. It was continued by scanning of tertial education hospital datum in 07-2021 to 07-2022. Patients were the adults who was made endoscopic intervention during initial 24 hours. Glasgow-Blatchford, AIMS65 and H3B2 scoring were calculated according to initial parameters.Results:The study included 116 patients. Median age was 60 (45,53) years. H3B2, AIMS65 and Glasgow-Blatchford scoring were significantly higher in non-survivor group than survivor group (p=0.005, <0.001, 0.013. respectively). With the addition of lactate and albumin to H3B2, the area under the curve value reached 0.910 and gained a stronger predictive ability.Conclusion:H3B2 was successful in predicting short-term mortality in Turkish patients, we recommend adding lactate and albumin to the H3B2 for stronger predictivity.

A Comparison of the First 60 Enucleation Cases Using a Thulium Fiber Laser without a Mentor to a Transurethral Resection of the Prostate (TURP) and Open Prostatectomy, and the Learning Curve

Background and Objectives: In the surgical treatment of benign prostatic hyperplasia (BPH), laser enucleation of the prostate is recommended as an alternative to transurethral resection (TURP) and open prostatectomy (OP). The thulium fiber laser, with its superficial penetration depth, can offer a rapid learning process by causing less heat injury and capsule damage. This study compares the first 60 cases of an endourologist performing thulium fiber enucleation of the prostate (ThuFLEP) without a mentor to the results of OP and TURP performed by experienced surgeons. It also identifies the case number at which the operation time for ThuFLEP starts to plateau. Materials and Methods: Between 1 November 2021 and 1 November 2023, the initial 60 ThuFLEP cases of an endourologist with no prior enucleation experience were compared with TURP and OP operations performed by experienced surgeons. Since the first 60 ThuFLEP cases involved 80–120 cc prostates, TURP and OP operations within this size range performed during the same period were included in the study. The groups were assessed for age, preoperative and postoperative prostate volume, PSA levels, the IPSS, the IPSS Quality of Life (QoL), and maximum urinary flow (Qmax). The 60 consecutive ThuFLEP cases were divided into three groups of 20 (Groups 1, 2, and 3) and compared for operation time, IPSS, and Qmax. Results: The operation time for TURP was shorter than for ThuFLEP and OP (p < 0.001). There was no significant difference between ThuFLEP and OP in postoperative Qmax and IPSS, while TURP had lower values than the other two methods. For ThuFLEP, the operation time was longer in the first 20 cases but plateaued in groups 2 and 3 (p < 0.001). Postoperative Qmax and IPSS values showed no significant differences among the three ThuFLEP groups (p > 0.05). Conclusions: For large prostates, ThuFLEP provides better postoperative results than TURP and offers shorter catheterization and hospital stay times than OP. Its short learning curve makes it a preferable method for treating BPH compared to other laser techniques.

IntroductionFibrosis-4 index (Fib-4i) is a useful and practical indicator of fibrosis risk in chronic liver disease. The study aimed to explore the relationship between Fib-4i and major adverse cardiovascular events (MACEs) within one year in cases of inflammatory myopericardial syndrome (IMPS).MethodsBetween January 2018 and December 2023, 152 patients diagnosed with IMPS were stratified into two groups based on the presence of late gadolinium enhancement (LGE) on cardiac magnetic resonance imaging. The Fib-4i was calculated at admission, and MACEs due to IMPS were recorded at 12 months.ResultsGroup 1 consisted of 36 LGE-negative patients, while group 2 consisted of 116 LGE-positive patients. The Fib-4i was higher in group 2 than in group 1 (p=0.014). Furthermore, it was found that aspartate aminotransferase, high-sensitivity (hs) troponin T, N-terminal pro-brain natriuretic peptide, and C-reactive protein (CRP) were higher in group 2 than in group 1 (respectively, p=0.03, p=0.002, p=0.008, p=0.014). No difference in MACEs between groups was observed at one month, six months, and one year (p=0.581, p=0.558, p=0.665, respectively). However, a positive correlation was observed between the Fib-4i and both hs-troponin T and CRP.ConclusionFib-4i was higher in patients with LGE than in those without LGE. However, its predictive power for MACEs could not be demonstrated.

Introduction: Cervical cancer is one of the most common types of cancer in women worldwide. Recent studies have shown that metabolic programming can support cervical cancer treatment by increasing sensitivity to chemotherapy and radiotherapy. Therefore, the investigation of new treatment agents targeting the metabolism of cervical cancer is of great importance in terms of improving treatment outcomes and developing new strategies. In our previous studies, we determined that Sulfanomide-E derivative has an apoptotic effect on HELA cells. In this study, we aimed to investigate the effects of Sulfanomide-E on carnitine metabolism in HELA cells. Method: HELA cells were used in our study and the cytotoxic and apoptotic effects of Sulfanomide-E were investigated. A dose of 25 μg/ml Sulfanomide-E was applied to HELA cells and the cells were incubated for 24 hours. In order to determine the changes in carnitine metabolism, a total of 27 carnitine and acylcarnitine derivatives were analyzed using LC-MS/MS. The obtained data were analyzed with SPSS 25.0 program. Findings: As a result of the analyses, a significant increase was observed in all carnitine and acylcarnitine derivatives in the Sulfanomide-E applied groups compared to the control group. These findings indicate that Sulfanomide-E has an effect on carnitine metabolism and these metabolic changes may be important in terms of response to treatment. Conclusion: This study reveals that Sulfanomide-E affects carnitine metabolism in HELA cells and exhibits apoptotic effects. The obtained data suggest that these changes in carnitine metabolism may provide a new understanding of the potential benefits of metabolic programming in the treatment of cervical cancer. Therefore, the role of agents such as Sulfanomide-E in the treatment of cervical cancer should be supported by more detailed studies.

The Effect of Early Diuretic Treatment on Clinical Outcomes in Patients with Acute Heart Failure: Door to Diuretic

Conclusion:Early initiation of diuretic treatment has no effect on short-term mortality. However, delayed initiation of diuretic treatment may affect hospitalization rates. According to our results, early initiation of diuretic treatment may reduce hospitalization rates.Results:A total of 325 patients were included. The median age of the patients was 76.0 years (interquartile range 69.0-83.0). The mortality rate of our study population was %14.4 (47). Systolic blood pressure and diastolic blood pressure were significantly higher in the survivor group (p=0.018, 0.033, respectively). Age, troponin-I, and pro-brain natriuretic peptide were significantly higher in the non-survivor group (p<0.001, <0.001, <0.001, respectively). For hospitalization, the area under the curve for the high door to diuretic time was 0.570, the cut-off value was 99 minutes, and the odds ratio was 1.75 (95% confidence interval: 1.09-2.82).Method:Our study was carried out as a retrospective, single-centered, observational study in the department of emergency a tertiary training and research hospital. The study population was the patients presenting with signs and symptoms of acute heart failure. The primary outcome was all-cause 30-day mortality and hospitalization. Univariant tests and ROC analysis were used for analysis.Objective:It was aimed to investigate the effect of early diuretic treatment on hospitalization and short-term mortality in patients with acute heart failure.
