Gene therapy: progress and predictions
The first clinical gene delivery, which involved insertion of a marker gene into lymphocytes from cancer patients, was published 25 years ago. In this review, we describe progress since then in gene therapy. Patients with some inherited single-gene defects can now be treated with their own bone marrow stem cells that have been engineered with a viral vector carrying the missing gene. Patients with inherited retinopathies and haemophilia B can also be treated by local or systemic injection of viral vectors. There are also a number of promising gene therapy approaches for cancer and infectious disease. We predict that the next 25 years will see improvements in safety, efficacy and manufacture of gene delivery vectors and introduction of gene-editing technologies to the clinic. Gene delivery may also prove a cost-effective method for the delivery of biological medicines.
Linking 'toxic outliers' to environmental justice communities
Several key studies have found that a small minority of producers, polluting at levels far exceeding group averages, generate the majority of overall exposure to industrial toxics. Frequently, such patterns go unnoticed and are understudied outside of the academic community. To our knowledge, no research to date has systematically described the scope and extent of extreme variations in industrially based exposure estimates and sought to link inequities in harm produced to inequities in exposure. In an analysis of all permitted industrial facilities across the United States, we show that there exists a class of hyper-polluters-the worst-of-the-worst-that disproportionately expose communities of color and low income populations to chemical releases. This study hopes to move beyond a traditional environmental justice research frame, bringing new computational methods and perspectives aimed at the empirical study of societal power dynamics. Our findings suggest the possibility that substantial environmental gains may be made through selective environmental enforcement, rather than sweeping initiatives.
The Role of Google Scholar in Evidence Reviews and Its Applicability to Grey Literature Searching
Google Scholar (GS), a commonly used web-based academic search engine, catalogues between 2 and 100 million records of both academic and grey literature (articles not formally published by commercial academic publishers). Google Scholar collates results from across the internet and is free to use. As a result it has received considerable attention as a method for searching for literature, particularly in searches for grey literature, as required by systematic reviews. The reliance on GS as a standalone resource has been greatly debated, however, and its efficacy in grey literature searching has not yet been investigated. Using systematic review case studies from environmental science, we investigated the utility of GS in systematic reviews and in searches for grey literature. Our findings show that GS results contain moderate amounts of grey literature, with the majority found on average at page 80. We also found that, when searched for specifically, the majority of literature identified using Web of Science was also found using GS. However, our findings showed moderate/poor overlap in results when similar search strings were used in Web of Science and GS (10-67%), and that GS missed some important literature in five of six case studies. Furthermore, a general GS search failed to find any grey literature from a case study that involved manual searching of organisations' websites. If used in systematic reviews for grey literature, we recommend that searches of article titles focus on the first 200 to 300 results. We conclude that whilst Google Scholar can find much grey literature and specific, known studies, it should not be used alone for systematic review searches. Rather, it forms a powerful addition to other traditional search methods. In addition, we advocate the use of tools to transparently document and catalogue GS search results to maintain high levels of transparency and the ability to be updated, critical to systematic reviews.
Dendritic cell entrapment within the pregnant uterus inhibits immune surveillance of the maternal/fetal interface in mice
Embryo implantation induces formation of the decidua, a stromal cell-derived structure that encases the fetus and placenta. Using the mouse as a model organism, we have found that this tissue reaction prevents DCs stationed at the maternal/fetal interface from migrating to the lymphatic vessels of the uterus and thus reaching the draining lymph nodes. Strikingly, decidual DCs remained immobile even after being stimulated with LPS and exhibiting responsiveness to CCL21, the chemokine that drives DC entry into lymphatic vessels. An analysis of maternal T cell reactivity toward a surrogate fetal/placental antigen furthermore revealed that regional T cell responses toward the fetus and placenta were driven by passive antigen transport and thus the tolerogenic mode of antigen presentation that predominates when there is negligible input from tissue-resident DCs. Indeed, the lack of involvement of tissue-resident DCs in the T cell response to the fetal allograft starkly contrasts with their prominent role in organ transplant rejection. Our results suggest that DC entrapment within the decidua minimizes immunogenic T cell exposure to fetal/placental antigens and raise the possibility that impaired development or function of the human decidua, which unlike that of the mouse contains lymphatic vessels, might lead to pathological T cell activation during pregnancy.
Hazardous air pollutant exposure as a contributing factor to COVID-19 mortality in the United States
To date, COVID-19 has claimed more than 100 000 American lives. Early inquiry suggests preexisting conditions are key risk factors contributing to COVID-19 mortality and air pollution exposure could exacerbate this relationship. Building on prior research linking deaths from respiratory viruses to air pollution exposures, we investigate how 2014 National Air Toxics Assessment hazardous air pollutants (HAPs) respiratory hazard quotient and respiratory hazard index are related to COVID-19 mortality. Our focus on HAPs builds upon the knowledge base linking poor air quality to COVID-19 mortality, since most (if not all) earlier studies only include criteria pollutants. Herein, we examine the relationship between HAP exposure and US-based COVID-19 mortality, while controlling for socioeconomic status, population health indicators, and exposure to PM2.5 and ozone. We fit county-level negative binomial mixed models, predicting COVID-19 mortality as a function of HAP respiratory toxicity levels and relevant covariates. We include models for combined exposure to HAPs, as well as for specific pollutants. We find that an increase in the respiratory hazard index is associated with a 9% increase in COVID-19 mortality. Although differing in magnitude, this association holds for individual HAPs acetaldehyde, and diesel PM. These findings help us to understand variation in US-based COVID-19 mortality rates, reinforce existing research linking air pollution to mortality, and emphasize the importance of regulatory efforts to limit air pollution exposure risk.
