Self-reported health status and mortality from all-causes of death, cardiovascular disease and cancer in an older adult population in Spain
To assess the association between self-reported health (SRH) and mortality from all-causes, cardiovascular disease (CVD) and cancer, in adults 65 years and older in Spain. We analysed data of 894 adults (504 women, 390 men) aged 65 years and above from two population-based studies, the EUREYE-Spain study and the Valencia Nutritional Survey (VNS). SRH was assessed at baseline using a single question which is widely used in epidemiological studies: \"Overall, how would you consider your health at present?\" and the response options were: 1. Very good, 2. Good, 3. Fair, 4. Poor, 5. Very poor. Deaths were ascertained during a 12-year follow-up period, and we used Cox proportional hazards regression models to obtain adjusted hazard ratios (HR). During the 12 years of follow-up (8566.2 person-years), we observed 400 deaths, 158 (39.5%) due to CVD and 89 (22.3%) due to cancer. Fair and poor/very poor SRH were significantly associated with higher all-cause mortality after 12-years of follow-up, HR = 1.29 (95% CI, 1.03-1.61) and HR 1.53 (95% CI, 1.09-2.15), respectively. We observed evidence of higher CVD mortality among those who reported fair and poor/very poor SRH, although the association was attenuated and lost statistical significance in the fully adjusted models. This study suggests that a poor SRH status is associated with a higher all-cause mortality risk among older adults in Spain. Checking SHR status may be useful to plan health care in older adults.
Occupational therapy interventions for adults with severe mental illness: a scoping review
ObjectiveTo identify the occupational therapy (OT) interventions in adults with severe mental illness (SMI) most investigated in intervention studies and to describe their characteristics.DesignScoping review.Data sourcesOn 17 January 2020, we searched the following electronic databases: MEDLINE, Scopus, Web of Science and EMBASE. We also performed a manual search of TESEO doctoral thesis database and of the journals indexed in the first quartile of OT according to the SCImago Journal Rank. We updated our search on 10 March 2021, performing a complementary search on ProQuest database and repeating the search in all sources. The terms included in the search strategy were: schizophrenia, schizotypal personality, delusional, schizoaffective, psychotic, bipolar, major depression, obsessive–compulsive, severe mental, OT and intervention.Study selectionThe study screening was peer-reviewed. Inclusion criteria were: (1) OT intervention studies in SMI: experimental, randomised, non-randomised and pilot/exploratory studies; (2) adult population with SMI: schizophrenia, schizotypal personality disorder, delusional disorder, obsessive–compulsive disorder, schizoaffective disorder, psychotic disorder, bipolar disorder, major depressive disorder; (3) OT identified as a discipline involved in the intervention; (4) English or Spanish language and (5) studies with full text available.ResultsThirty-five studies met the inclusion criteria. OT interventions were classified in psychosocial, psychoeducational, cognitive and exercise interventions. The most used OT intervention was psychosocial intervention.ConclusionPsychosocial intervention was the most investigated OT intervention in SMI, followed by psychoeducational, cognitive and exercise interventions. These interventions are usually group interventions in patients with schizophrenia, performed by a multidisciplinary team (in which an occupational therapist collaborates), with 2–3 weekly 60 min sessions and a duration of 3–6 months.
Interventions to Promote Fruit and Vegetable Consumption in Children with Neurodevelopmental Disorders: A Scoping Review
Background/Objectives: Children with neurodevelopmental disorders (NDDs) frequently present with feeding challenges that can lead to inadequate fruit and vegetable consumption, which can increase their risk of nutritional deficiencies and related health issues. The aim of this scoping review was to describe the main interventions focused on promoting fruit and vegetable consumption in children with NDDs. Methods: Two authors carried out a search strategy in PubMed, Scopus, EMBASE, Web of Science, and PsycINFO using the following terms: “autism OR autistic OR asperger OR rett OR pervasive OR disintegrative OR ‘neurodevelopmental disorder’ OR ‘attention deficit disorder with hyperactivity’”; and the outcome (O): “fruit OR vegetable”. We included all randomized or non-randomized studies, published in English or Spanish, which assessed children’s fruit and vegetable consumption. Results: We included seven articles that applied different interventions, such as a mobile health and behavioral modification program (n = 1), repeated food exposure programs (n = 2), a play-based program with parental training (n = 1); the implementation of Dietary Approaches to Stop Hypertension diet (n = 1); an intensive interdisciplinary mealtime behavioral treatment (n = 1); and restrictive diets (n = 1). Conclusions: Parent-led behavior changes supported by multidisciplinary teams using play, positive reinforcement, and food modification strategies were the main interventions used to increase fruit and vegetable consumption in children with NDDs. This review supports designing evidence-based approaches to improve feeding challenges in this population.
Validity of a Short Food Frequency Questionnaire for Toddlers of NELA Birth Cohort Study
Background/Objectives: Our aim was to evaluate the reproducibility and validity of a semi-quantitative food frequency questionnaire (FFQ) for the assessment of usual nutrient and food intakes in children of 18 months old. Methods: We included 103 toddlers aged 18 months from the Nutrition in Early Life and Asthma (NELA) birth cohort study. A 47-item FFQ was administered twice to parents with a 3-month interval. During that period, we also performed three non-consecutive 24 h dietary recalls (24hDRs) and oral mucosa samples for determining the fatty acid profile (glycerophospholipids). We estimated correlation coefficients of reproducibility for nutrient and food group intakes by comparing both FFQs and validity coefficients by comparing nutrient intakes between the second FFQ and the average of the three 24hDRs. We also explored biochemical validity by comparing the intake of fish from the FFQ with the percentage of fatty acids in oral mucosa cells. Results: The average of correlation coefficients for FFQ reproducibility was r = 0.48 for energy-adjusted nutrient intakes (ranging from r = 0.28 for Na to r = 0.62 for Mg and Zn) and r = 0.35 for the intake of energy-adjusted food groups. The average correlation coefficient for FFQ validity on nutrient intakes was r = 0.48, ranging from r = 0.16 for α-carotene to r = 0.75 for vitamin E. We also found a positive correlation between total omega-3 fatty acids and docosahexaenoic acid percentages in oral mucosa cells and the intake of total and white fish, r = 0.31 and r = 0.34, respectively. Conclusions: This study shows that our short FFQ demonstrated moderate reproducibility (mean r = 0.48) and validity (mean r = 0.48) for dietary assessment of most nutrients and foods in 18-month-old children in Spain. This FFQ provides an efficient and minimally invasive method for evaluating toddler dietary intake, particularly in Mediterranean contexts.
Use of Robotic Devices in Upper Limb Rehabilitation for Adult Stroke Patients: Protocol for an Umbrella Review
Background and Aims The sequelae of stroke have a profound impact on individuals' independence. Among these, motor limitations of the upper limb significantly impair activities of daily living. Currently, robotic therapies have emerged as an innovative therapeutic approach; however, the existing evidence is heterogeneous and fails to provide conclusive data regarding their effectiveness. Therefore, the objective of this umbrella review is to describe the effectiveness of robotic devices for upper limb rehabilitation in adults who have had a stroke. In addition, to identify the most commonly used robotic devices and examine the benefits of their use in combination with other therapeutic modalities. Methods We will conduct an umbrella review with a systematic search in the following databases: PubMed (MEDLINE), Scopus, EMBASE, Web of Science, and the Cochrane Library. We will limit the search to the last 5 years in order to include the most recent literature. Search results will be independently screened by two authors in two phases: by title and , and then by full text. We will include systematic reviews with and without meta‐analysis published in English, French, or Spanish aimed at exploring the use of robotic devices in upper limb motor rehabilitation for adults' post‐stroke. Results Two authors will extract data into pre‐designed tables. We will include variables such as strength, joint range of motion, upper limb and hand motor function, spasticity, and dependence in activities of daily living. The AMSTAR‐2 scale will be used to assess the methodological quality of the included systematic reviews. Results will be presented narratively and in tables. Conclusion This protocol will facilitate the development of an umbrella review that synthesizes current evidence on the effectiveness of robotic devices for upper limb rehabilitation after stroke, specifically identifying device classifications, therapeutic approaches, and clinical outcomes.
Occupational Therapy Interventions in Mental Health During Lockdown: A Scoping Review
Lockdown derived from the COVID-19 pandemic posed significant challenges to mental health care, prompting the adaptation of therapeutic practices. The objective of this study was to describe the characteristics and objectives of occupational therapy (OT) interventions conducted in the field of mental health during the COVID-19 lockdown. A scoping review was conducted following PRISMA-ScR guidelines. A systematic search was carried out in the following databases: PubMed, Scopus, Embase, OTSeeker, PsycINFO, and Web of Science. We included randomized or non-randomized intervention studies, published in English or Spanish, that explored OT interventions in mental health during the COVID-19 period and/or lockdown. Data were extracted using pre-designed tables in accordance with the recommendations of the Cochrane Handbook. We included seven articles. OT interventions were conducted via video call (n = 4) and in person (n = 3). These were carried out with adults, adolescents, and children, lasting from 1 to 32 weeks, with the number of sessions ranging from 7 to 22 and lasting 20 to 90 min. The most frequently addressed outcomes were quality of life (n = 4), anxiety or depression (n = 4), and sleep (n = 4). During lockdown, OT interventions were mainly applied via telerehabilitation with the aim of increasing activity participation and addressing emotional issues. These results could help occupational therapists to implement mental health interventions when in-person application is compromised.
Reproducibility and Validity of a Food Frequency Questionnaire for Dietary Assessment in Adolescents in a Self-Reported Way
Tools to assess diet in a reliable and efficient way are needed, particularly in children and adolescents. In this study, we assess the reproducibility and validity of a semiquantitative food frequency questionnaire (FFQ) among adolescents in Spain. We analyzed data of 51 male adolescents aged 15–17 years from a prospective birth cohort study. Participants answered the FFQ twice in a self-administered way over a 12-month period. Reproducibility was assessed by comparing nutrient and food intakes from the FFQs, and validity by comparing nutrient intakes from the average of two FFQs and the average of two 24-Hour Dietary Recalls obtained in the period. Pearson correlation coefficients were calculated. The average of reproducibility correlation coefficients for food group intakes was 0.33, with the highest correlation for vegetable intake (r = 0.81); and the average for nutrient intake was 0.32, with the highest coefficients for α- and β-carotene (r = 0.65). Validity correlation coefficients ranged from 0.07 for carbohydrates to 0.53 for dietary fiber. The average of the validity correlation coefficients was r = 0.32. This study suggests that our FFQ may be a useful tool for assessing dietary intake of most nutrient and food groups among Spanish male adolescents in a self-administered way, despite reproducibility and, particularly validity, being low for some nutrients and food groups.
Manual Dexterity Training and Cognitive Function in Adults with Stroke: A Scoping Review
Background: Acquired brain injury (ABI) affects manual dexterity (MD) and cognitive functions, limiting daily activity performance. Occupational therapy aims to improve functionality and quality of life. Objective: To examine and describe the available evidence on the impact of MD training on cognitive processes and functional performance in adults with stroke, as well as to identify the most commonly used assessment tools and intervention techniques. Methods: Scoping review. A systematic literature search was conducted in PubMed and Scopus to identify experimental studies from the last 10 years involving adults with ABI who participated in interventions targeting upper-limb, MD, and cognitive function. A three-phase screening was carried out by two authors with duplicates removed using Zotero version 7.0. Results: Ten articles published between 2016 and 2023 were included. The most frequent interventions involved robotics and virtual reality. Eight studies were conducted by occupational therapists or included occupational therapy involvement, while two were conducted by physiotherapists. Training MD and upper-limb motor skills led to improvements in attention, memory, and executive functions. Conclusions: Findings support combined motor–cognitive interventions carried out by occupational therapists or physiotherapists to optimize rehabilitation outcomes, although further research is needed to strengthen the evidence.
Occupational Therapy Interventions for Dementia Caregivers: Scoping Review
Background and purpose: caregivers of people with dementia (PwD) often experience a significant caregiver burden. Occupational Therapy (OT) is a helpful discipline for improving quality of life and other health factors for these caregivers. We conducted a Scoping Review to describe OT interventions for caregivers of PwD. Methods: two authors searched PubMed, Scopus, EMBASE and Web of Science databases and OT journals indexed in the Journal Citation Reports. Terms included in the search strategy were: dementia, Alzheimer, Parkinson, caregivers and OT. We included articles with experimental design in which an OT intervention in caregivers of PwD was carried out, written in Spanish or English and with the full text available. Results: a total of 2121 articles were obtained, 31 of which were included; 22 of them described home-based OT interventions: Tailored Activity Program (TAP) (n = 5), Environmental Skill-Building Program (ESP) (n = 4) and Advancing Caregiver Training (ACT) (n = 3) and other household interventions (n = 10); the remaining studies described OT interventions in other settings (n = 9). Conclusions: OT interventions for caregivers of PwD were mainly carried out at home. The most commonly used interventions were TAP focused on caregivers of people with Alzheimer’s disease, aimed at lessening the burden, depression and stress experienced by caregivers.
Total and different types of olive oil consumption, gut microbiota, and cognitive function changes in older adults
Background Over the past decade, emerging evidence has shed light on the role of the gut microbiota in the interface between diet and brain health. Olive oil, particularly virgin olive oil, a key component and major fat source in the Mediterranean diet, has exhibited widespread healthful benefits, including improvements in gut microbiota and cognitive health. Despite insights from preclinical studies into the relationship between virgin olive oil consumption, gut microbiota, and cognitive function, human research in this area remains limited. Therefore, our study aims to investigate the interplay between total olive oil consumption and its subtypes, gut microbiota, and changes in cognitive function in older adults who were cognitively healthy at baseline but at high risk of cognitive decline. Methods In this prospective cohort study, we assessed a total of 656 participants aged 55 to 75y (mean age 65.0 ± 4.9y, 47.9% women) with overweight/obesity and metabolic syndrome who provided stool samples and completed a validated semi-quantitative food frequency questionnaire at baseline and a comprehensive battery of neuropsychological tests at baseline and at a 2-y follow-up. Results Results from the multivariable linear regression models showed that higher consumption of virgin olive oil was associated with improved cognitive function over a 2-y follow-up, and a more diverse gut microbiota overall structure at baseline. Conversely, increased consumption of common olive oil is linked to lower alpha diversity of the microbial communities, and accelerated cognitive decline. Mediation analysis suggests that gut microbiota and particularly the Adlercreutzia , may serve as a mediator taxon in the association between virgin olive oil consumption and positive changes in general cognitive function. Conclusions Higher consumption of virgin olive oil was associated with cognitive preservation, possibly mediated by favorable alterations in gut microbiota composition. Our study provides novel insights into the complex interplay between different types of olive oil consumption, gut microbiota, and changes in cognitive function. These findings underscore the potential of microbiota-targeted dietary strategies to promote cognitive health in aging populations, though further high-quality and clinical cohort studies are required.
