Anticipating the international spread of Zika virus from Brazil
With no vaccine or antiviral therapy available, possible interventions include: personal protection (ie, repellent use) and daytime avoidance of mosquito bites (especially pregnant women until more is known about the association between Zika virus infection and microcephaly); daytime avoidance of mosquito bites among Zika virus-infected individuals to disrupt human to mosquito to human transmission cycles (80% of infected individuals are asymptomatic and the remainder have clinical syndromes overlapping with dengue and chikungunya);5 and community-level mosquito surveillance and control measures.
The Promise of Science, Knowledge Mobilization, and Rapid Learning Systems for COVID-19 Recovery
The health, economic, and social crises created by the coronavirus disease 2019 (COVID-19) pandemic have been global in scope and inequitable in impact. The global road to recovery can be enhanced with robust, relevant, and timely scientific evidence. This commentary seeks to illustrate the power of science, scientific collaboration, and innovative research funding programs to inform pandemic recovery and inspire transformational changes for a more equitable, resilient, and sustainable future. Specifically, this commentary provides an introduction to the United Nations (UN) Research Roadmap for the COVID-19 Recovery that was published in November 2020. It introduces 5 scoping reviews that helped inform the UN Research Roadmap and that are now available open access within this series of special papers, and it provides an overview of an innovative research funding program that facilitated rapid mobilization and collaboration to produce the scoping reviews. The publication of the scoping reviews in this journal series will help complement and amplify the UN Research Roadmap by furthering knowledge mobilization efforts and informing COVID-19 recovery around the world, to ensure a more equitable, resilient, and sustainable postpandemic future.
Zika virus in the Americas: Early epidemiological and genetic findings
Brazil has experienced an unprecedented epidemic of Zika virus (ZIKV), with ~30,000 cases reported to date. ZIKV was first detected in Brazil in May 2015, and cases of microcephaly potentially associated with ZIKV infection were identified in November 2015. We performed next-generation sequencing to generate seven Brazilian ZIKV genomes sampled from four self-limited cases, one blood donor, one fatal adult case, and one newborn with microcephaly and congenital malformations. Results of phylogenetic and molecular clock analyses show a single introduction of ZIKV into the Americas, which we estimated to have occurred between May and December 2013, more than 12 months before the detection of ZIKV in Brazil. The estimated date of origin coincides with an increase in air passengers to Brazil from ZIKV-endemic areas, as well as with reported outbreaks in the Pacific Islands. ZIKV genomes from Brazil are phylogenetically interspersed with those from other South American and Caribbean countries. Mapping mutations onto existing structural models revealed the context of viral amino acid changes present in the outbreak lineage; however, no shared amino acid changes were found among the three currently available virus genomes from microcephaly cases. Municipality-level incidence data indicate that reports of suspected microcephaly in Brazil best correlate with ZIKV incidence around week 17 of pregnancy, although this correlation does not demonstrate causation. Our genetic description and analysis of ZIKV isolates in Brazil provide a baseline for future studies of the evolution and molecular epidemiology of this emerging virus in the Americas.
Risk factors for tuberculosis in a cohort of recent immigrants to Ontario
The World Health Organization estimates that one-third of the world's population is infected with TB. Eighty percent of TB in Ontario is among the foreign-born. Before effective targeted testing programs can be developed to control TB in Ontario local epidemiological profiles to identify high risk groups among foreign-form populations are needed. The objective of this study was to describe the effect of time since arrival on TB risk and to analyze determinants of risk for TB over time after immigrating to Ontario. The group contributing to this increased risk were young males from high endemic countries. Targeted TB testing activities among migrant populations should be directed to high risk groups only, which include people between 16–30 years, over the age of 65 and from countries with endemic TB. These people may be at significantly increased risk even 8 years after arrival. Studies should investigate increases in risk due to reunification of families or visiting the country of origin.
