Delayed presentation of a malpositioned central venous access device (CVAD) in a neonate
Position was confirmed on a single antero-posterior (AP) abdominal radiograph per local practice, with projection of the tip adjacent to L4/L5 vertebrae at the level of the common iliac vein (figure 2). Table 1 Blood results Name Level Reference range Units pH 7.19 7.35–7.45 Carbon dioxide (CO2) 50 35–46 mm Hg Base excess −9 −2 to +3 mmol/L Bicarbonate (HCO3) 19 22–28 mmol/L Lactate 1.3 0.4–2 mmol/L Sodium (Na) 128 135–145 mmol/L On day 18, left leg and flank oedema evolved with difficulty infusing via the PICC line. While PICC lines deliver essential, lifesaving treatment to neonates, they also carry substantial risks.1 2 It is precisely because of these risks that so many national and regional centres have guidelines and frameworks of practice designed for clinicians who use central venous access devices (CVADs).3–7 PICCs inserted into the lower limbs should ideally thread without resistance, follow a smooth course without kinks or wiggles and sit to the right of the spine, above the level of the renal vessels at L2 but below the level of the cardiac silhouette at T9.1 4–6 On retrospective review, the single AP radiograph taken following insertion of the PICC line did not adhere to all of these principles, and the line malposition was not recognised. Neonatal presentation of PHACE syndrome
Large fluctuations in blood pressure complicated this treatment, most likely due to a combination of factors involving medication effects, systemic steal phenomenon and compression of both the superior vena cava and brainstem. Table 1 Summary of medical treatment to reduce mass effect of capillary haemangiomas from day 116 of life Drug, route Dose, frequency Duration of treatment (days of life) Propranolol PO 1 mg/kg three times a day 116—ongoing, aged 10 months Methylprednisolone intravenously 30 mg/kg OD 118–121 Interferon alpha 2a S/C 3 million subunits OD 119–156 Prednisolone intravenously 2 mg/kg OD 121–134 Hydrocortisone PO 3 mg/kg OD 135–160 OD, once daily; PO, orally; S/C, subcutaneous. Southside Voices Readers' Letters Tell Us What You Think Water Authority polluted
I asked at the Delta international counter if they knew where Presidential's ticket booth was located. The very polite representative said that it was in the North Terminal. We made our way to the north side through much construction and a maze of plastic sheets and finally reached the North Terminal ticket counters. We walked the length of the counter and saw no sign of Presidential Airlines. As members of the General Assembly debate this year's budget, we citizens need to take note of and applaud the efforts of state leaders to make a significant investment in Georgia's future with their support for education. Gov. Zell Miller's proposal provides vital support for preschool, elementary and secondary education and strengthens our higher- education system in ways that will help Georgia to compete successfully for economic development and ensure a quality work force. I own a business in Conyers. Shortly after I moved to my present location in 1994, a nail shop opened up next door to me. Since that time, I have had numerous customer complaints and have personally experienced health problems related to these fumes. I have had sinus infections, bronchitis and walking pneumonia. I never experienced these problems prior to being placed next to the nail shop, where I and my staff are constantly bombarded with these fumes.
Brain atlas for glycoprotein hormone receptors at single-transcript level
There is increasing evidence that anterior pituitary hormones, traditionally thought to have unitary functions in regulating single endocrine targets, act on multiple somatic tissues, such as bone, fat, and liver. There is also emerging evidence for anterior pituitary hormone action on brain receptors in mediating central neural and peripheral somatic functions. Here, we have created the most comprehensive neuroanatomical atlas on the expression of TSHR, LHCGR, and FSHR. We have used RNAscope, a technology that allows the detection of mRNA at single-transcript level, together with protein level validation, to document Tshr expression in 173 and Fshr expression in 353 brain regions, nuclei and subnuclei identified using the Atlas for the Mouse Brain in Stereotaxic Coordinates . We also identified Lhcgr transcripts in 401 brain regions, nuclei and subnuclei. Complementarily, we used ViewRNA, another single-transcript detection technology, to establish the expression of FSHR in human brain samples, where transcripts were co-localized in MALAT1 -positive neurons. In addition, we show high expression for all three receptors in the ventricular region—with yet unknown functions. Intriguingly, Tshr and Fshr expression in the ependymal layer of the third ventricle was similar to that of the thyroid follicular cells and testicular Sertoli cells, respectively. In contrast, Fshr was localized to NeuN-positive neurons in the granular layer of the dentate gyrus in murine and human brain—both are Alzheimer’s disease-vulnerable regions. Our atlas thus provides a vital resource for scientists to explore the link between the stimulation or inactivation of brain glycoprotein hormone receptors on somatic function. New actionable pathways for human disease may be unmasked through further studies.
Marine Snow Aggregates are Enriched in Polycyclic Aromatic Hydrocarbons (PAHs) in Oil Contaminated Waters: Insights from a Mesocosm Study
Marine snow was implicated in the transport of oil to the seafloor during the Deepwater Horizon oil spill, but the exact processes remain controversial. In this study, we investigated the concentrations and distributions of the 16 USEPA priority polycyclic aromatic hydrocarbons (PAHs) in marine snow aggregates collected during a mesocosm experiment. Seawater only, oil in a water accommodated fraction (WAF), and Corexit-enhanced WAF (DCEWAF) were incubated for 16 d. Both WAF and DCEWAF aggregates were enriched in heavy molecular weight PAHs but depleted in naphthalene. DCEWAF aggregates had 2.6 times more total 16 PAHs than the WAF (20.5 vs. 7.8 µg/g). Aggregates in the WAF and DCEWAF incorporated 4.4% and 19.3%, respectively of the total PAHs in the mesocosm tanks. Our results revealed that marine snow sorbed and scavenged heavy molecular weight PAHs in the water column and the application of Corexit enhanced the incorporation of PAHs into the sinking aggregates.
