Association between blood lead levels and metabolic syndrome considering the effect of the thyroid-stimulating hormone based on the 2013 Korea National health and nutrition examination survey
Imbalances in thyroid-stimulating hormone (TSH) levels are associated with metabolic syndrome (MetS), and the underlying mechanism is partly in alignment with that of lead exposure causing MetS. Many studies have reported the association between lead exposure and MetS, but no study has considered the possibility of TSH mediating lead's effect on MetS. Therefore, we aimed to examine the association between lead exposure and MetS considering TSH as a partial mediator. The data of 1,688 adults (age ≥19 years) from the Korea National Health and Nutrition Examination Survey in 2013 were analyzed. The prevalence of MetS in the Korean population was 21.9%, and the geometric mean of blood lead and serum TSH levels were 1.96 μg/dL and 2.17 μIU/mL, respectively. The associations between blood lead levels, serum TSH levels, and MetS were determined through a multiple logistic regression analysis. Blood lead levels were positively associated with high TSH levels (upper 25%) with an odds ratio (OR) and 95% confidence interval (CI) of 1.79 (1.24, 2.58) per doubled lead levels. The increase in blood lead and serum TSH levels both positively increased the odds of developing MetS. The OR of MetS per doubling of blood lead level was 1.53 (1.00, 2.35), and was not attenuated after adjusting for TSH levels. These findings suggest that higher levels of blood lead are positively associated with serum TSH levels and MetS. By exploring the role of TSH as a partial mediator between lead and MetS, we verified that lead exposure has an independent relationship with MetS, regardless of TSH levels.
Disease Latency according to Asbestos Exposure Characteristics among Malignant Mesothelioma and Asbestos-Related Lung Cancer Cases in South Korea
Korea was one of the major consumers of asbestos in the late 1900s, and asbestos-related disease patients have been reported continuously to date, owing to long disease latency. Several studies have been conducted to predict the future incidence of malignant mesothelioma and lung cancer in Korea, but little is understood about the latency time. Therefore, the aim of this study is to estimate the latency period of malignant mesothelioma and asbestos-related lung cancer in Korea and its determinants. We obtained information from the Environmental Health Centers for Asbestos in Korea on the history of asbestos exposure and demographic characteristics of 1933 patients with malignant mesothelioma and asbestos-related lung cancer. In our study, the latency periods for malignant mesothelioma and lung cancer were 33.7 and 40.1 years, respectively. Regardless of the disease type, those with a history of exposure related to the production of asbestos-containing products or asbestos factories had the shortest latency period. In addition, we observed that those who worked in or lived near asbestos mines tended to have a relatively long disease latency. Smoking was associated with shorter latency, but no linear relationship between the lifetime smoking amount (expressed in pack years) and latent time was observed. In addition, the age of initial exposure showed a negative linear association with the latency period for mesothelioma and lung cancer.
Evaluation of effectiveness and improvement factors of occupational health and safety management system in the Republic of Korea Navy based on AHP-entropy and IPA
The Republic of Korea Navy (ROKN) has applied Occupational Health and Safety Management System (OHSMS), but the effectiveness of OHSMS is being questioned due to continuous industrial accidents that continue to occur. Although OHSMS, which has been generally applied in enterprises, has more potential for improper implementation in the military, there are few studies on OHSMS in the military. Therefore, this study verified the effectiveness of OHSMS in the ROKN and derived improvement factors. This study was conducted in a two-step process. First, we surveyed 629 workers at the ROKN workplaces to confirm the effectiveness of OHSMS by comparing occupational health and safety (OHS) efforts according to whether OHSMS was applied and the period of application. Second, 29 naval OHSMS experts evaluated the factors for improving OHSMS using two decision-making tools: Analytic Hierarchy Process (AHP)-entropy and Importance-Performance Analysis (IPA). The study results indicate that the OHS efforts of OHSMS-applied workplaces were similar to that of unapplied. Also, no better OHS efforts were identified in workplaces with more extended OHSMS application periods. There were five improvement factors of OHSMS applied to the ROKN workplaces, with the highest weight in the following order: consultation and participation of workers; resources; competence; hazard identification and risk assessment; and organizational roles, responsibilities, and authorities. The effectiveness of OHSMS in the ROKN was insufficient. Therefore, the ROKN needs focused improvement efforts on the five requirements to implement OHSMS practically. These results can be helpful information for the ROKN to apply OHSMS more effectively for industrial safety.
Occupational and environmental asbestos exposure and the risk of lung cancer in Korea: A case-control study in South Chungcheong Province of Korea
Despite the use of large amounts of asbestos in the 1990s, few studies have been conducted in Korea on occupational and environmental asbestos exposure and lung cancer risk. The main aim of this study was to estimate the risk of lung cancer development caused by occupational and environmental asbestos exposures in residents of South Chungcheong Province, where about half of the asbestos mines in Korea operated. We conducted a case-control study, for which the information on asbestos exposure history and demographic characteristics was provided by the Environmental Health Center for asbestos of Soonchunhyang University Cheonan Hospital. After adjusting for all covariates, the odds ratios for lung cancer tended to increase with higher exposure probability for both occupational as well as environmental asbestos. The relative risk of occupational asbestos exposure was higher than that of environmental exposure; the interaction of co-exposure was not statistically significant. The estimated means of the latency period were significantly shorter in participants who were engaged in the production of asbestos-containing products and in those who lived near asbestos industries as compared to other groups.
Air pollution and survival in patients with malignant mesothelioma and asbestos-related lung cancer: a follow-up study of 1591 patients in South Korea
Background Despite significant advancements in treatments such as surgery, radiotherapy, and chemotherapy, the survival rate for patients with asbestos-related cancers remains low. Numerous studies have provided evidence suggesting that air pollution induces oxidative stress and inflammation, affecting acute respiratory diseases, lung cancer, and overall mortality. However, because of the high case fatality rate, there is limited knowledge regarding the effects of air pollution exposures on survival following a diagnosis of asbestos-related cancers. This study aimed to determine the effect of air pollution on the survival of patients with malignant mesothelioma and asbestos-related lung cancer. Methods We followed up with 593 patients with malignant mesothelioma and 998 patients with lung cancer identified as asbestos victims between 2009 and 2022. Data on five air pollutants—sulfur dioxide, carbon monoxide, nitrogen dioxide, fine particulate matter with a diameter < 10 μm, and fine particulate matter with a diameter < 2.5 μm—were obtained from nationwide atmospheric monitoring stations. Cox proportional hazard models were used to estimate the association of cumulative air pollutant exposure with patient mortality, while adjusting for potential confounders. Quantile-based g-computation was used to assess the combined effect of the air pollutant mixture on mortality. Results The 1-, 3-, and 5-year survival rates for both cancer types decreased with increasing exposure to all air pollutants. The estimated hazard ratios rose significantly with a 1-standard deviation increase in each pollutant exposure level. A quartile increase in the pollutant mixture was associated with a 1.99-fold increase in the risk of malignant mesothelioma-related mortality (95% confidence interval: 1.62, 2.44). For lung cancer, a quartile increase in the pollutant mixture triggered a 1.87-fold increase in the mortality risk (95% confidence interval: 1.53, 2.30). Conclusion These findings support the hypothesis that air pollution exposure after an asbestos-related cancer diagnosis can negatively affect patient survival.
The Joint Effects of Some Beverages Intake and Smoking on Chronic Obstructive Pulmonary Disease in Korean Adults: Data Analysis of the Korea National Health and Nutrition Examination Survey (KNHANES), 2008–2015
Some beverages and smoking cause an inflammatory response in the lungs and airways in a similar way, ultimately affecting chronic obstructive pulmonary disease (COPD) occurrence. Using a nationally representative health survey database, this study investigates the individual and joint effects of consumption of different beverages and smoking on COPD. This study is a cross-sectional analysis of 15,961 Korean adults in the Korea National Health and Nutritional Examination Survey of 2008–2015. COPD was defined as forced expiratory volume in 1 s (FEV1) divided by forced vital capacity (FVC) <0.70. We used multiple linear and logistic regression models to examine the association of beverage consumption and smoking with an FEV1/FVC ratio and odds ratio (OR) for COPD. The mean FEV1/FVC ratio decreased with increasing soda intake (p = 0.016), coffee intake (p = 0.031), and smoking status; however, the mean FEV1/FVC ratio increased with increasing green tea intake frequency (p = 0.029). When soda intake increased to 10 times/month, the OR of having COPD increased to 1.04 times (95% CI: 1.01, 1.07). The positive joint effect of soda intake and smoking on COPD was marginally significant (p = 0.058). We found that soda intake, coffee intake, and smoking increased airflow limitation while green tea intake decreased it. In addition, soda intake and smoking had a positive joint effect on COPD in the Korean population.
The Effects of Earphone Use and Environmental Lead Exposure on Hearing Loss in the Korean Population: Data Analysis of the Korea National Health and Nutrition Examination Survey (KNHANES), 2010–2013
Although previous studies have reported that frequent earphone use and lead exposure are risk factors for hearing loss, most of these studies were limited to small populations or animal experiments. Several studies that presented the joint effect of combined exposure of noise and heavy metal on hearing loss were also mainly conducted on occupational workers exposed to high concentration. We investigated both the individual and joint effects of earphone use and environmental lead exposure on hearing loss in the Korean general population. We analyzed data from 7,596 Koreans provided by the Korea National Health and Nutrition Examination Survey (KNHANES) during the period 2010-2013. The pure-tone average (PTA) of hearing thresholds at 2, 3, and 4 kHz frequencies was computed, and hearing loss was defined as a PTA ≥ 25 dB in one or both ears. A dose-response relationship in hearing loss with earphone use time and blood lead level is observed after adjustment for confounding factors. With a 1-hour increase in earphone use time and 1 μg/dL increase in blood lead concentration, the odds of hearing loss increased by 1.19 and 1.43 times, respectively. For hearing loss, the additive and multiplicative effect of earphone use and blood lead level were not statistically significant. Earphone use and environmental lead exposure have an individual effect on hearing loss in the general population. However, the estimated joint effect of earphone use and lead exposure was not statistically significant.
Priority Setting for Management of Hazardous Biocides in Korea Using Chemical Ranking and Scoring Method
Biocides are non-agricultural chemical agents for the prevention of unhygienic pests. The worldwide demand for biocidal products has been rapidly increasing. Meanwhile, biocides have been causing negative health effects for decades, resulting in public health scares. Therefore, governments around the world have tried to strictly control biocides, and it is necessary to prioritize the health risks of biocides for efficient management. Chemical ranking and scoring (CRS) methods have been developed for the effective management of chemicals. However, existing methods do not use suitable variables to evaluate biocides, thus possibly underestimating or overestimating the actual health risks. We developed a new CRS method that reflects the exposure and toxicity characteristics of biocides. Eleven indicators were chosen as appropriate for prioritizing biocides, and scoring based on the globally harmonized system of classification and labeling of chemicals (GHS) improved the efficiency of the method. Correlations between individual indicators in this study were low (−0.151–0.325), indicating that each indicator was independent and well-chosen for prioritizing biocides. The effect of each indicator on the total score showed that carcinogenicity, mutagenicity, and reproductive toxicity (CMR) chemicals ranked high with r = 0.558. This result demonstrated that the most dangerous toxicants should play a more decisive role in the top ranking than the others. We expect that our method can be efficiently used to screen regulated biocides by prioritizing their health hazards, thus leading to better policy decision making about biocide use.
The Epidemiologic Characteristics of Malignant Mesothelioma Cases in Korea: Findings of the Asbestos Injury Relief System from 2011–2015
(1) Background: The purpose of this study was to investigate the epidemiological characteristics of malignant mesothelioma in Korea by investigating cases compensated under the asbestos injury relief system. (2) Methods: A total of 407 compensated cases between 2011 and 2015 were reviewed using medical records and resident registrations in order to investigate the dates of diagnosis and death. Asbestos exposure and patients’ general characteristics were investigated through face-to-face interviews. The standardized incidence ratio was calculated as the number of observations from 2005 to 2014 per exposure region in Korea, using the mid-annual population of each region in 2009 as the standard population. (3) Results: Among the 407 cases, 65.1% were male. The pleura and peritoneum were affected in 76.9% and 23.1% of cases, respectively. For peritoneal mesothelioma, the median survival duration was longer (p = 0.005), and the proportion of affected women was higher than that in pleural mesothelioma. The standardized incidence ratio (95% CI) by province of primary exposure was Chungnam 3.33 (2.51–4.35), Ulsan 1.85 (0.97–3.21), and Seoul 1.32 (1.06–1.63). (4) Conclusions: Although the representativeness of the data is limited, it is sufficient to assume the epidemiologic characteristics of malignant mesothelioma, help improve the compensation system, and contribute to future policies.
Changes in Risk in Medium Business Plating and Paint Manufacturing Plants following the Revision of the Korean Chemical Accident Prevention System
Chemical accidents can occur anywhere. The need for chemical management in Korea was realized following the 2012 Gumi hydrofluoric acid accident in 2012. The Chemicals Control Act was enacted in 2015. This system evaluates the risks (high, medium, low) and consequent safety management at all plants that handle hazardous chemical substances. However, the system was criticized as excessive when most plants were designated high-risk without considering their size. Thus, laboratories and hospitals handling very small quantities were subject to regulation. Accordingly, in 2021 Korea revised the system to include off-site consequence analyses and a Korean-style risk analysis. Plants handling very small quantities, such as laboratories and hospitals, were exempt from regulation. In this study, changes in risk were examined for four medium-sized plating and paint manufacturing plants. Under the previous system, all four factories were judged as high-risk groups. In particular, the paint manufacturing plant A, which has an underground storage tank, received a medium risk like the plating plant C, although the possibility of a chemical accident was lower than that of other plants. However, in the changed system, all plants were changed to the low-risk group. In the Korean-style risk analysis, it is possible to see at a glance what is lacking in the plants, such as cooperation between local residents and local governments and the construction of safety facilities according to the type of accident scenario. The revised system is a reasonable regulation for medium business plants.
