Human influence over 150 years of coastal evolution in the Volturno delta system (southern Italy)
This paper focused on the past shoreline change rates along the coastal plain of the Volturno River, in southern Italy, western Mediterranean. A wide database comprising historical maps, aerial photographs, topographic sheets, bathymétrie data was used to extract the spatial and temporal information of the coastlines at seven time points. Coastline displacement was calculated for two successive time points and relative surface variation (accretion and erosion) was estimated as well as minimum and maximum accretion/erosion linear values and rates. The surface variation analysis has revealed that the studied coast can be considered homogeneous since the 1970s, whereas it exhibits a variety of shoreline evolutionary trends after that time period. Timing and causes of trends and rates of variation were detected. Based on the estimated shoreline change rates, an appropriate morhodynamic one-line model was applied to predict evolutionary scenarios also in presence of port and defence works. The results obtained strongly emphasize that a successful coastal management requires a constant monitoring of the human-induced changes to account for the variability of rates over time.
Hydrodynamic Dispersion of Pollutant Discharge by Means of Submarine Pipelines

Anthropic activities have an important influence on the ecological state of seawater, so innovative systems able to guarantee water quality and minimize human impact are required. In this paper the pollution of Litorale Domitio coastal waters is investigated. The analyzed coastal areas represent the final discharge of treated wastewater and drainage water. Therefore an evaluation of a technical solution able to make the quality of such waters compatible with the tourism is needed. For this reason, the integration of the depurative scheme with a system of submarine pipelines is proposed. An application of the proposed method is shown with reference to the case of Regi Lagni in Campania. Using numerical models by DHI dedicated software, it is verified that the suggested method contributes to a meaningful reduction of near-shore pollution.
Urban Water Consumption at Multiple Spatial and Temporal Scales. A Review of Existing Datasets
Over the last three decades, the increasing development of smart water meter trials and the rise of demand management has fostered the collection of water demand data at increasingly higher spatial and temporal resolutions. Counting these new datasets and more traditional aggregate water demand data, the literature is rich with heterogeneous urban water demand datasets. They are characterized by heterogeneous spatial scales—from urban districts, to households or individual water fixtures—and temporal sampling frequencies—from seasonal/monthly up to sub-daily (minutes or seconds). Motivated by the need of tracking the existing datasets in this rapidly evolving field of investigation, this manuscript is the first comprehensive review effort of the state-of-the-art urban water demand datasets. This paper contributes a review of 92 water demand datasets and 120 related peer-review publications compiled in the last 45 years. The reviewed datasets are classified and analyzed according to the following criteria: spatial scale, temporal scale, and dataset accessibility. This research effort builds an updated catalog of the existing water demand datasets to facilitate future research efforts end encourage the publication of open-access datasets in water demand modelling and management research.
Long-term continuous N-carbamylglutamate treatment in frequently decompensated propionic acidemia: a case report
Background Propionic acidemia is a rare autosomal recessive inherited metabolic disorder that can inhibit the synthesis of N -acetylglutamate, the obligatory activator in urea synthesis, leading to hyperammonemia. N -carbamylglutamate ameliorates hyperammonemia in decompensated propionic acidemia. The effects of long-term continuous N -acetylglutamate administration in such patients are unknown. We report our clinical experience with continuous administration of N -acetylglutamate for 6 years in a patient with propionic acidemia frequently presenting with hyperammonemia. Case presentation A male Caucasian patient with frequently decompensated propionic acidemia and hyperammonemia was admitted 78 times for acute attacks during the first 9 years of his life. Continuous daily treatment with oral N -carbamylglutamate 100 mg/kg (50 mg/kg after 6 months) was initiated. During 6 years of treatment, he had a significant decrease in his mean plasma ammonia levels (75.7 μmol/L vs. 140.3 μmol/L before N -carbamylglutamate therapy, p < 0.005 [normal range 50–80 μmol/L]) and fewer acute episodes (two in 6 years). Conclusion Our results suggest a benefit of N -acetylglutamate administration outside the emergency setting. If this observation is confirmed, future studies should aim to optimize the dosage and explore effects of the dosage requirements on other drugs and on protein tolerance.
The Mediterranean Diet: An Update of the Clinical Trials
The Mediterranean Diet (MedDiet) is a term used to identify a dietary pattern originating from the unique multi-millennial interplay between natural food resources and the eating practices of people living in the Mediterranean basin. Scientific evidence has described the healthy properties of the MedDiet and its beneficial role in several pathological conditions. Nevertheless, current socio-economic trends have moved people away from this healthy lifestyle. Thus, clinical and biological evidence supporting the benefits of the MedDiet is needed to overcome these limitations. Clinical nutrition research examines the effects of dietary interventions on biological or health-related outcomes in a determined study population. The evidence produced by these studies is useful for dietary guidance and public health messaging. We provided an update of the clinical trials registered on the database clinicaltrials.gov evaluating the effects of the MedDiet on health and specific diseases. Our findings revealed an increased number of clinical trials in the last decade and found that most disease-related studies focused on cardiovascular diseases, metabolic diseases, and cancer. The majority of MedDiet’s beneficial effects could be primarily related to its anti-inflammatory and anti-oxidant properties as well as the effectiveness of this dietary pattern in controlling waist circumference and obesity. Moreover, strict and long-lasting adherence to the MedDiet as well as the beneficial effects of specific components (e.g., olive oil or its polyphenols) seem to emerge as useful insights for interventional improvements. These findings present further insights into the MedDiet’s resources and how it could strengthen overall public health.
Short Physical Performance Battery and all-cause mortality: systematic review and meta-analysis
Background The Short Physical Performance Battery (SPPB) is a well-established tool to assess lower extremity physical performance status. Its predictive ability for all-cause mortality has been sparsely reported, but with conflicting results in different subsets of participants. The aim of this study was to perform a meta-analysis investigating the relationship between SPPB score and all-cause mortality. Methods Articles were searched in MEDLINE, the Cochrane Library, Google Scholar, and BioMed Central between July and September 2015 and updated in January 2016. Inclusion criteria were observational studies; >50 participants; stratification of population according to SPPB value; data on all-cause mortality; English language publications. Twenty-four articles were selected from available evidence. Data of interest (i.e., clinical characteristics, information after stratification of the sample into four SPPB groups [0–3, 4–6, 7–9, 10–12]) were retrieved from the articles and/or obtained by the study authors. The odds ratio (OR) and/or hazard ratio (HR) was obtained for all-cause mortality according to SPPB category (with SPPB scores 10–12 considered as reference) with adjustment for age, sex, and body mass index. Results Standardized data were obtained for 17 studies ( n  = 16,534, mean age 76 ± 3 years). As compared to SPPB scores 10–12, values of 0–3 (OR 3.25, 95%CI 2.86–3.79), 4–6 (OR 2.14, 95%CI 1.92–2.39), and 7–9 (OR 1.50, 95%CI 1.32–1.71) were each associated with an increased risk of all-cause mortality. The association between poor performance on SPPB and all-cause mortality remained highly consistent independent of follow-up length, subsets of participants, geographic area, and age of the population. Random effects meta-regression showed that OR for all-cause mortality with SPPB values 7–9 was higher in the younger population, diabetics, and men. Conclusions An SPPB score lower than 10 is predictive of all-cause mortality. The systematic implementation of the SPPB in clinical practice settings may provide useful prognostic information about the risk of all-cause mortality. Moreover, the SPPB could be used as a surrogate endpoint of all-cause mortality in trials needing to quantify benefit and health improvements of specific treatments or rehabilitation programs. Burst-like swarms in the Campi Flegrei caldera accelerating unrest from 2021 to 2024
Since early 2021, peculiar Volcano-Tectonic (VT) seismic sequences with very short inter-event times have become increasingly frequent and evident in the Campi Flegrei caldera (Italy), which has been experiencing a long-term unrest since 2005. During the same period the ground deformation (uplift), seismicity and gas emission that characterize the current unrest showed an acceleration. Within this type of seismic sequence, we identify burst-like swarms, characterized by inter-event times so short that they are often not easily recognizable. Here we show that these sequences are located in an area encompassing the main hydrothermal field and a zone affected by a geodetic anomaly (a region that uplifts less rapidly than the surroundings), which became evident in 2021. This type of seismicity has been associated with phreatic explosions and critical phases of unrest in other volcanoes and currently characterizes the state of activity of Campi Flegrei caldera. During the long-term unrest of the Campi Flegrei caldera, burst-like seismic swarms are observed and associated with an increase in hydrothermal activity and an anomaly in the ground deformation pattern recorded since 2021.
Radiological Thoracic Vertebral Fractures are Highly Prevalent in COVID-19 and Predict Disease Outcomes
Abstract Context and Objective COVID-19 has become the most relevant medical issue globally. Despite several studies that have investigated clinical characteristics of COVID-19 patients, no data have been reported on the prevalence of vertebral fractures (VFs). Since VFs may influence cardiorespiratory function and disease outcomes, the aim of this study was to assess VFs prevalence and clinical impact in COVID-19. Design and Patients This was a retrospective cohort study performed at San Raffaele Hospital, a tertiary health care hospital in Italy. We included COVID-19 patients for whom lateral chest x-rays at emergency department were available. VFs were detected using a semiquantitative evaluation of vertebral shape on chest x-rays. Results A total of 114 patients were included in this study and thoracic VFs were detected in 41 patients (36%). Patients with VFs were older and more frequently affected by hypertension and coronary artery disease (P < 0.001, P = 0.007, P = 0.034; respectively). Thirty-six (88%) patients in VFs+ group compared to 54 (74%) in VFs− group were hospitalized (P = 0.08). Patients with VFs more frequently required noninvasive mechanical ventilation compared with those without VFs (P = 0.02). Mortality was 22% in VFs+ group and 10% in VFs− group (P = 0.07). In particular, mortality was higher in patients with severe VFs compared with those with moderate and mild VFs (P = 0.04). Conclusions VFs may integrate the cardiorespiratory risk of COVID-19 patients, being a useful and easy to measure clinical marker of fragility and poor prognosis. We suggest that morphometric thoracic vertebral evaluation should be performed in all suspected COVID-19 patients undergoing chest x-rays.
Markers of Metabolic Abnormalities in Vitiligo Patients
While vitiligo is primarily caused by melanocyte deficiency or dysfunction, recent studies have revealed a notable prevalence of metabolic syndrome (MetS) among patients with vitiligo. This suggests shared pathogenic features between the two conditions. Individuals with vitiligo often exhibit variations in triglyceride levels, cholesterol, and blood pressure, which are also affected in MetS. Given the similarities in their underlying mechanisms, genetic factors, pro-inflammatory signalling pathways, and increased oxidative stress, this study aims to highlight the common traits between vitiligo and metabolic systemic disorders. Serum analyses confirmed increased low-density lipoprotein (LDL) levels in patients with vitiligo, compared to physiological values. In addition, we reported significant decreases in folate and vitamin D (Vit D) levels. Oxidative stress is one of the underlying causes of the development of metabolic syndromes and is related to the advancement of skin diseases. This study found high levels of inflammatory cytokines, such as interleukin-6 (IL-6) and chemokine 10 (CXCL10), which are markers of inflammation and disease progression. The accumulation of insulin growth factor binding proteins 5 (IGFBP5) and advanced glycation end products (AGEs) entailed in atherosclerosis and diabetes onset, respectively, were also disclosed in vitiligo. In addition, the blood-associated activity of the antioxidant enzymes catalase (Cat) and superoxide dismutase (SOD) was impaired. Moreover, the plasma fatty acid (FAs) profile analysis showed an alteration in composition and specific estimated activities of FAs biosynthetic enzymes resembling MetS development, resulting in an imbalance towards pro-inflammatory n6-series FAs. These results revealed a systemic metabolic alteration in vitiligo patients that could be considered a new target for developing a more effective therapeutic approach.
