Impact of living with bipolar patients: caregivers’ mental health and quality of life

IntroductionBipolar disorder is a common chronic illness with progressive intermittent. The new health policy, which advocates the deinstitutionalization of patient’s mental illnesses, caused, for a part, a transfer of the load of these patients from the specialized care services to natural caregivers, which can involve a great burden for family members as well as an altered quality of life, anxiety and depressive symptoms.ObjectivesThe aim of the study was to evaluate the rate of anxiety, depression and the repercussions on life quality in natural caregivers of patients with bipolar disorder.MethodsThis is a descriptive cross-sectional study that involved a group of 50 caregivers of patients with bipolar disorder treated at psychiatry department ‘A’ at Razi Hospital. We applied a questionnaire recording the different socio-demographic data. To determine the impact on the caregivers mental health and life quality we used the WHOQOL-brief (World Health Organization Quality Of Life-abbreviated version) and the HAD (Hospital Anxiety and Depression) Scale.ResultsOur sample consisted of 32 women and 18 men. The mean age was 52.12 years with extremes ranging from 28 to 79 years. A majority were parents (60%), 18% spouses, 16% siblings and 6% descendants.The repercussions of management of patients with bipolar disorder on the life and health of the caregivers were significant. Indeed, more than half of the caregivers (52%) (n=26) had a definite anxiety symptomatology on the Hospital Anxiety and Depression Scale (HAD). Twenty-two percent (n=11) presented definite depressive symptomatology and thirty percent (n=15) doubtful depressive symptomatology on this same scale. The most impaired domains on the World Health Organization Quality of Life Scale-version abbreviated (WHOQOL) were first: the environment domain with an average of 25.9 and second: the physical health domain with an average of 23.9.ConclusionsBeing a caregiver for a patient with bipolar disorder is associated with a great burden that can be the cause of anxiety-depressive complications and an alteration in the caregiver’s quality of life. It is important to assess this burden and its repercussions in order to preserve good family dynamics and ensure the proper functioning of the helping relationship and consequently improve the prognosis.Disclosure of InterestNone Declared

Factors associated with poor medication adherence in patients with Bipolar Disorders

IntroductionTreatment adherence in patients living with Bipolar Disorders can influence prognosis and quality of life. It is associated with an increased morbidity and healthcare costs.ObjectivesThe aim of our study was to evaluate treatment adherence in a sample of patients living with Bipolar disorders and to determine factors associated with poor adherence.MethodsWe conducted a cross sectional study where we included bipolar patients being treated in psychiatry department A. We developed a survey containing sociodemographic and clinical features. We used the medical adherence rating scale to evaluate treatment adherence.ResultsOur sample consisted of 100 patients with a mean age of 47,5 years old. Sixty seven patients were being treated for bipolar disorder type 1. Medication adherence rate was 64%.Factors associated with poor medication adherence were being single, an early age of onset, comorbid substance abuse disorder, severe treatment side effects and poor insight.ConclusionsPoor medication adherence is a major issue for people living with Bipolar Disorders. Clinicians should pay more attention to sociodemographic and clinical factors to predict and enhance treatment adherence.Disclosure of InterestNone Declared

Digital mental health: Perceptions and opinions of Tunisian patients

Introduction With the rapid advancement of modern technology, many countries have adopted mental health care systems supplemented by digital means of communication. Are Tunisian patients “ready” for the “digital revolution”? Objectives The aim of our study was to assess perceptions of people living with mental illness on digital mental health. Methods We developed a cross-sectional study where we randomly included patients who were treated for a psychiatric disorder in a public or a private practice. Inclusion criteria were: subject 18 years old or older, clinical remission for at least three months. We developed a questionnaire on sociodemographic and clinical variables. We also included questions on patients’ level of interest in using digital mental health services such as teleconsultation and mental health smartphone apps. Perceived obstacles in using digital mental health by patients were also evaluated. Results Our sample size was 260 patients. The mean age of our population was 36,4 years old with. The mean distance from the household to the mental health care provider was 17,3 km. Two thirds of the sample had access to a wifi connection at home (172 patients). When asked about the content of internet searches, 66% have already looked for information on their mental health or mental disorders on web pages. Patients were very interested in video teleconsultation with their therapists (72%), psychoeducation apps (68%) and online mood journals (61%). They expressed little to no interest in online exchanges with other patients and medication reminder apps. The most reported obstacles in implementing digital mental health as noted by patients were : lack of perceived effectiveness, virtual communication with their therapist and confidentiality issues. Conclusions Mental health patients in Tunisia expressed a great interest in teleconsultations and online psychoeducation programs. Further research on the willingness of mental health professionals to adopt digital mental health services are needed. Disclosure of Interest None Declared

An integrated model of self-service technology (SST) usage in a retail context

Purpose
– The purpose of this paper is to propose and test a comprehensive model that captures individual, system and situational drivers of customers’ intention to use and actual use of self-service technologies (SSTs).
Design/methodology/approach
– This paper is based on a survey conducted among 143 users and 150 non-users of SSTs at the exit of a grocery store. The proposed model was analysed using structural equation modelling and a logistic regression.
Findings
– The results demonstrate that: first, in addition to previous usage behaviour (i.e. usage frequency), situational factors (time pressure, basket size, coupons and queue length at the SSTs and staffed checkouts) influence customers’ decisions to use SSTs during a specific shopping trip; and second, perceived behavioural control is the most important determinant of behavioural intention, followed by perceived usefulness, need for interaction and perceived ease of use and enjoyment.
Originality/value
– Although an abundance of research has investigated the adoption of SSTs, little is known about what drives real usage. This study considers the actual usage of SSTs in a specific context, as well as the situational factors that influence the choice of SSTs over traditional checkouts. In addition, this paper provides an integrative model including actual usage, use frequency and behavioral intention and its antecedents by extending the Technology Acceptance Model 3.

Customer responses to waits for online banking service delivery

Purpose
– This study aims to examine the effect of situational factors, related to transactions and waiting, on customers' cognitive and affective responses to service delivery time, including their service evaluations.
Design/methodology/approach
– A web-based survey included customers of an online credit company.
Findings
– The authors' results demonstrate that disconfirmation influences cognitive but not affective responses. Both transaction importance and opportunity waiting cost influence wait acceptability, and transaction importance moderates the relationship between disconfirmation and wait acceptability. Delivery time worry and transaction worry increase negative affective responses; affective and cognitive responses then determine service evaluation. Opportunity waiting costs increase the positive effect of wait acceptability on service evaluation.
Research limitations/implications
– The main limitations pertain to the small sample size and the restriction of the survey to customers of only one financial company.
Practical implications
– The derived guidelines suggest how banks can improve service evaluations by modifying consumer time expectations, as well as reducing opportunity waiting cost, delivery time worry, and transaction worry.
Originality/value
– The conceptualization of service delivery time refers to waiting outside the company's premises, as opposed to in-line waiting on company premises. This study considers the influence of situational factors, such as transaction importance, transaction worry, waiting cost, and delivery time worry, on customers' reactions to service delivery time in the context of online banking services.

L’objectif de cette recherche est d’étudier la collaboration entreprise—consommateurs dans un processus d’innovation via la pratique du crowdsourcing interactif. Ce type de crowdsourcing s’inscrit dans une démarche de co-création et se caractérise par des interactions étroites entre les deux catégories d’acteurs. Pour répondre à cet objectif, le concept d’objet-frontière, développé en sociologie, a été mobilisé. L’objet-frontière est susceptible de faciliter la compréhension des dynamiques d’interactions et d’actions collectives entre acteurs disparates. Les résultats de cette recherche mettent en évidence que les objets-frontières et artefacts de négociation permettent de matérialiser la production des échanges de connaissance et de compétence entre le monde de l’entreprise et celui des consommateurs. Ils soulignent également le rôle de ces objets en tant que révélateurs de jeux d’influence et de pouvoir susceptibles de générer des déceptions et des tensions entre acteurs. Cette recherche apporte un regard nouveau sur les intérêts, limites et défis d’une démarche de co-création.
