Cost volume profit analysis for full paying patient services in Malaysia: A study protocol
Dual practice within public hospitals, characterised by the concurrent provision of public and private healthcare services within public hospitals, has become a widespread phenomenon. With the participation of selected public hospitals, dual practice within public hospitals, also known as Full Paying Patient services, was an initiative the Ministry of Health Malaysia took in 2007 to retain senior specialist physicians in Malaysia. The revenue generated from the Full Paying Patient services aims to provide an avenue for public sector specialists to supplement their incomes while alleviating the Government’s burden of subsidising healthcare for financially capable individuals. However, the effectiveness of Full Paying Patient services in recouping service delivery costs and yielding a profit is still uncertain after 16 years of implementation. This study is designed to evaluate the impact of Full Paying Patient inpatient services volume, revenue, and cost on profit versus loss at selected hospitals from 2017 to 2020. From the perspective of healthcare providers, we plan to perform a cost volume profit analysis. This analysis enables us to determine the break-even point, at which total revenues match total costs, along with no-loss and no-profit thresholds for Full Paying Patient services. This study has the potential to provide insights into how variations in service volume, cost, and pricing impact healthcare providers’ profitability. It also offers critical financial information regarding the volume of services required to reach the break-even point. A comprehensive understanding of service volume, cost and pricing is imperative for making informed decisions to fulfil the objectives and ensure the sustainability of the FPP services.
P-248 Stress status of healthcare drivers during the COVID-19 pandemic in Malaysia
IntroductionThe pandemic of COVID-19 has brought a disastrous impact on every single aspect of human life and activities. The economic and health sectors are most affected by restriction on public movement, daily activities, and burden of coronavirus infection through increased infection and hospitalisation rate. Most research focused on front liners but they overlooked ambulance and healthcare drivers.ObjectivesTo determine the stress status of healthcare drivers in Malaysia and its associated factors during the COVID-19 pandemic.MethodsThis cross-sectional study was conducted among randomly selected 163 healthcare drivers in Negeri Sembilan State Health Department, Malaysia, using self-reported validated questionnaires.ResultsA majority of healthcare drivers were male (100%), married (90.1%) with their highest education consisting of a high school certificate (90.1%). Ethnically, they consisted of Malay (95.7%), Indian (3.7%) and Chinese (0.6%). Three out of ten healthcare drivers were ambulance drivers, while the rest were non ambulance drivers. The prevalence of stress among healthcare drivers was 7.4% (95% CI: 3.7, 11.7). Higher prevalence was found among ambulance drivers compared to non ambulance drivers; 10.6% and 6.0% respectively. There was a significant association between stress and smoking status, performing on-call and duration of working hours in a similar unit.ConclusionThe study revealed that there was a low prevalence of stress among healthcare drivers in Malaysia during the pandemic. The reduced life threatening tasks, fewer emergency incidents and lesser assigned tasks throughout the movement control order during the COVID-19 pandemic could have contributed to the low prevalence statistics among the healthcare drivers in Malaysia. In addition, the effort by the Occupational and Environmental Health Unit, Negeri Sembilan State Health Department in providing consistent safety and health training including stress management might have assisted healthcare drivers to cope with the stressed situation both mentally and physically.
Specialists’ Dual Practice within Public Hospital Setting: Evidence from Malaysia
In line with the commitment of the Malaysian government and Ministry of Health to prevent the brain drain of specialists from public hospitals, they have been permitted to perform dual practice within the public hospital setting (DPH) since 2007. DPH allowed them to hold jobs in both public and private practices within the same public hospitals that they are affiliated to, permitting these specialists to treat public and private patients. Nevertheless, the information regarding DPH in Southeast Asia region is still limited. This narrative review provides insight into the implementation of DPH in Malaysia. It highlights that DPH has been well-governed and regulated by the MOH while serving as a means to retain specialists in the public healthcare system by providing them with opportunities to obtain additional income. Such a policy has also reduced the financial burden of the government in subsidizing healthcare. However, as in other countries with similar policies, multiple challenges have arisen from the implementation of DPH in Malaysia despite its positive achievements and potentials. This paper concludes that proactive governance, monitoring, and regulation are key to ensure the success of DPH.
Implications of Dual Practice on Cataract Surgery Waiting Time and Rescheduling: The Case of Malaysia
Background: Dual practice was implemented in selected Ministry of Health Malaysia hospitals to reduce brain drain and provide an alternative for patients willing to pay higher user fees to seek prompt treatment from the specialist of their choice. This study aimed to assess the implications of dual practice on waiting time and rescheduling for cataract surgery. Methods: A retrospective study was conducted in a referral hospital. Inpatient medical records of patients who underwent cataract procedures were used to study the waiting times to surgery and rescheduling between private and public groups. Results: Private patients had a considerably shorter waiting time for cataract surgery, seven times shorter compared to public patients where all surgeries were conducted after hours on weekdays or weekends. Additionally, 14.9% of public patients experienced surgery rescheduling, while all private patients had their surgeries as planned. The main reason for surgery rescheduling was the medical factor, primarily due to uncontrolled blood pressure and upper respiratory tract infection. Conclusion: Private service provision utilizing out-of-office hours slots for cataract surgery optimizes public hospital resources, allowing shorter waiting times and providing an alternative to meet healthcare needs.
