Periodontitis is a chronic inflammatory disease, initiated by the presence of a bacterial biofilm, called dental plaque, which affects both the periodontal ligaments and bone surrounding teeth. In the last decades, several lines of evidence have supported the existence of a relationship between periodontitis and systemic health. For instance, as periodontitis acts within the same chronic inflammatory model seen in cardiovascular disease (CVD), or other disorders, such as diabetes, several studies have suggested the existence of a bi-directional link between periodontal health and these pathologies. For instance, people with diabetes are more susceptible to infections and are more likely to suffer from periodontitis than people without this syndrome. Analogously, it is now evident that cardiac disorders are worsened by periodontitis, both experimentally and in humans. For all these reasons, it is very plausible that preventing periodontitis has an impact on the onset or progression of CVD and diabetes. On these grounds, in this review, we have provided an updated account on the current knowledge concerning periodontal disease and the adverse effects exerted on the cardiovascular system health and diabetes, informing readers on the most recent preclinical studies and epidemiological evidence.

The association between muscle strength and z scores of pulmonary function

Z scores and percent predicted values of spirometry parameters are widely used for the evaluation of pulmonary function and detection of respiratory diseases. Decreased pulmonary function should correlate with reduction of muscle strength although it is unclear whether z scores of respiratory parameters are associated with muscle strength. In a cohort of 398 adults, pulmonary function assessment and muscle strength measurement by handgrip strength and physical activity level were evaluated. Probable sarcopenia was defined by muscle strength reduction. Univariate and multivariate regression analysis and ROC curve were applied. The AUC for FEV1 and FVC were significantly greater than the z FEV1 and z FVC (

= 0.0004 and

= 0.0008) respectively. Multivariate regression analysis adjusted for confounders also reported a significant and independent relationship between z FEV1; z FVC and HGS measurements. FEV

and FVC in absolute values have an important role in the detection of sarcopenia status. Predicted values and z scores also show a significant association suggesting the pathophysiological connection between lung function and muscle strength.

Inter-relationships between Gender, Frailty and 10-Year Survival in Older Italian Adults: an observational longitudinal study

Aim of the present study was to assess the impact of gender on the relationship between long-term mortality and clinical frailty. In an observational, longitudinal study on 10-year mortality, we examined 1284 subjects. The Frailty Staging System was used to assess frailty. The Cox model was employed to assess variables independently associated with survival using a backward stepwise algorithm. To investigate the possible interactions between gender and the selected variables, an extension of the multivariable fractional polynomial algorithm was adopted. Women were more likely to be older, have a higher disability, present with more comorbidities, consume more drugs, be frail and have a higher rate of survival at the follow-up than were men. At the Cox multivariate analysis only age (HR 2.26), female gender (HR 0.43), and number of drugs (HR 1.57) were significant and independent factors associated with all-cause mortality. In the survival analyses, only frailty (
vs
no frailty) showed significant interaction with gender (p < 0.001, HR = 1.92). While the presence of frailty reduced the survival rate in women, no effect was observed in men. Importantly, frail women showed higher survival rates than did both frail and no frail men. The main finding of the present study is that gender shapes up the association between frailty and long-term survival rates.

Technologies to produce electric energy from renewable geothermal source are gaining increasing attention, due to their ability to provide a stable output suitable for baseload production. Performing life cycle assessment (LCA) of geothermal systems has become essential to evaluate their environmental performance. However, so far, no documented nor reliable information has been made available for developing robust LCA studies. This work provides a comprehensive inventory of the Italian Bagnore geothermal power plants system. The inventory is based exclusively on primary data, accounting for every life cycle stage of the system. Data quality was assessed by means of a pedigree matrix. The calculated LCA results showed, with an overall low level of uncertainty (2–3%), that the commissioning and operational phases accounted for more than 95% of the environmental profile. Direct emissions to atmosphere were shown to be the major environmental impact, particularly those released during the operational phase (84%). The environmental performances comparison with the average Italian electricity mix showed that the balance is always in favor of geothermal energy production, except in the climate change impact category. The overall outcome confirms the importance, for flash technology employing fluid with a high concentration of gas content, of using good quality primary data to obtain robust results.

Heat production from a geothermal energy source is gaining increasing attention due to its potential contribution to the decarbonization of the European energy sector. Obtaining representative results of the environmental performances of geothermal systems and comparing them with other renewables is of utmost importance in order to ensure an effective energy transition as targeted by Europe. This work presents the outputs of a Life Cycle Assessment (LCA) performed on the Rittershoffen geothermal heat plant applying guidelines that were developed within the H2020 GEOENVI project. The production of 1 kWhth from the Rittershoffen heat plant was compared to the heat produced from natural gas in Europe. Geothermal heat production performed better than the average heat production in climate change and resource use, fossil categories. The LCA identified the electricity consumption during the operation and maintenance phase as a hot spot for several impact categories. A prospective scenario analysis was therefore performed to assess the evolution of the environmental performances of the Rittershoffen heat plant associated with the future French electricity mixes. The increase of renewable energy shares in the future French electricity mix caused the impact on specific categories (e.g., land use and mineral and metals resource depletion) to grow over the years. However, an overall reduction of the environmental impacts of the Rittershoffen heat plant was observed.

The sphingosine kinases 1 and 2 (SphK1 and 2) catalyze the phosphorylation of the lipid, sphingosine, generating the signal transmitter, sphingosine 1-phosphate (S1P). The activation of such kinases and the subsequent S1P generation and secretion in the blood serum of mammals represent a major checkpoint in many cellular signaling cascades. In fact, activating the SphK/S1P system is critical for cell motility and proliferation, cytoskeletal organization, cell growth, survival, and response to stress. In the cardiovascular system, the physiological effects of S1P intervene through the binding and activation of a family of five highly selective G protein-coupled receptors, called S1PR
. Importantly, SphK/S1P signal is present on both vascular and myocardial cells. S1P is a well-recognized survival factor in many tissues. Therefore, it is not surprising that the last two decades have seen a flourishing of interest and investigative efforts directed to obtain additional mechanistic insights into the signaling, as well as the biological activity of this phospholipid, and of its receptors, especially in the cardiovascular system. Here, we will provide an up-to-date account on the structure and function of sphingosine kinases, discussing the generation, release, and function of S1P. Keeping the bull's eye on the cardiovascular system, we will review the structure and signaling cascades and biological actions emanating from the stimulation of different S1P receptors. We will end this article with a summary of the most recent, experimental and clinical observations targeting S1PRs and SphKs as possible new therapeutic avenues for cardiovascular disorders, such as heart failure.

Dietary Phytochemicals in Neuroimmunoaging: A New Therapeutic Possibility for Humans?

Although several efforts have been made in the search for genetic and epigenetic patterns linked to diseases, a comprehensive explanation of the mechanisms underlying pathological phenotypic plasticity is still far from being clarified. Oxidative stress and inflammation are two of the major triggers of the epigenetic alterations occurring in chronic pathologies, such as neurodegenerative diseases. In fact, over the last decade, remarkable progress has been made to realize that chronic, low-grade inflammation is one of the major risk factor underlying brain aging. Accumulated data strongly suggest that phytochemicals from fruits, vegetables, herbs, and spices may exert relevant immunomodulatory and/or anti-inflammatory activities in the context of brain aging. Starting by the evidence that a common denominator of aging and chronic degenerative diseases is represented by inflammation, and that several dietary phytochemicals are able to potentially interfere with and regulate the normal function of cells, in particular neuronal components, aim of this review is to summarize recent studies on neuroinflammaging processes and proofs indicating that specific phytochemicals may act as positive modulators of neuroinflammatory events. In addition, critical pathways involved in mediating phytochemicals effects on neuroinflammaging were discussed, exploring the real impact of these compounds in preserving brain health before the onset of symptoms leading to inflammatory neurodegeneration and cognitive decline.

Infective endocarditis (IE) is an inflammatory disease usually caused by bacteria entering the bloodstream and settling in the heart lining valves or blood vessels. Despite modern antimicrobial and surgical treatments, IE continues to cause substantial morbidity and mortality. Thus, primary prevention and enhanced diagnosis remain the most important strategies to fight this disease. In this regard, it is worth noting that for over 50 years, oral microbiota has been considered one of the significant risk factors for IE. Indeed, among the disparate recommendations from the American heart association and the European Society of Cardiology, there are good oral hygiene and prophylaxis for high-risk patients undergoing dental procedures. Thus, significant interest has grown in the role of oral microbiota and it continues to be a subject of research interest, especially if we consider that antimicrobial treatments can generate drug-resistant mutant bacteria, becoming a severe social problem. This review will describe the current knowledge about the relationship between oral microbiota, dental procedures, and IE. Further, it will discuss current methods used to prevent IE cases that originate from oral pathogens and how these should be focused on improving oral hygiene, which remains the significant persuasible way to prevent bacteremia and systemic disorders.

Myocardial pathology induced by aldosterone is dependent on non-canonical activities of G protein-coupled receptor kinases

Hyper-aldosteronism is associated with myocardial dysfunction including induction of cardiac fibrosis and maladaptive hypertrophy. Mechanisms of these cardiotoxicities are not fully understood. Here we show that mineralocorticoid receptor (MR) activation by aldosterone leads to pathological myocardial signalling mediated by mitochondrial G protein-coupled receptor kinase 2 (GRK2) pro-death activity and GRK5 pro-hypertrophic action. Moreover, these MR-dependent GRK2 and GRK5 non-canonical activities appear to involve cross-talk with the angiotensin II type-1 receptor (AT
1
R). Most importantly, we show that ventricular dysfunction caused by chronic hyper-aldosteronism
in vivo
is completely prevented in cardiac
Grk2
knockout mice (KO) and to a lesser extent in
Grk5
KO mice. However, aldosterone-induced cardiac hypertrophy is totally prevented in
Grk5
KO mice. We also show human data consistent with MR activation status in heart failure influencing GRK2 levels. Therefore, our study uncovers GRKs as targets for ameliorating pathological cardiac effects associated with high-aldosterone levels.

Over the last decades, life expectancy has significantly increased although several chronic diseases persist in the population, with aging as the leading risk factor. Despite improvements in diagnosis and treatment, many elderlies suffer from cardiovascular problems that are much more frequent in an older, more fragile organism. In the long term, age-related cardiovascular diseases (CVDs) contribute to the decline of quality of life and ability to perform normal activities of daily living. microRNAs (miRNAs) are a class of small non-coding RNAs that regulate gene expression at the posttranscriptional level in both physiological and pathological conditions. In this review, we will focus on the role of miRNAs in aging and age-related CVDs as heart failure, hypertension, atherosclerosis, atrial fibrillation, and diabetes mellitus. miRNAs are key regulators of complex biological mechanisms, representing an exciting potential therapeutic target in CVDs. Moreover, one major challenge in geriatric medicine is to find reliable biomarkers for diagnosis, prognosis, and prediction of the response to specific drugs. miRNAs represent a very promising tool due to their stability in the circulation and unique signature in CVDs. However, further studies are needed to investigate their translational potential in the real clinical practice.
