The role of inflammatory biomarkers in the development and progression of pre-eclampsia: a systematic review and meta-analysis

Pre-eclampsia (PE) is a pregnancy complication associated with maternal and fetal morbidity and mortality. Among the potential pathogenesis discussed, inflammation is considered an essential initiator of PE. Previous studies have compared the levels of various inflammatory biomarkers that indicate the existence of PE; however, the relative levels of pro-inflammatory and anti-inflammatory biomarkers and their dynamic changes during PE progression remain unclear. This knowledge is essential to explain the occurrence and progression of the disease.
We aimed to identify the relationship between inflammatory status and PE using inflammatory biomarkers as indicators. We also discussed the underlying mechanism by which inflammatory imbalance contributes to PE by comparing the relative levels of pro-inflammatory and anti-inflammatory biomarkers. Furthermore, we identified additional risk factors for PE.
We reviewed PubMed, Embase, and the Cochrane Library for articles published until 15
September 2022. Original articles that investigated inflammatory biomarkers in PE and normal pregnancy were included. We selected healthy pregnant women as controls. The inflammatory biomarkers in the case and control groups were expressed as standardized mean differences and 95% confidence intervals using a random-effects model. Study quality was assessed using the Newcastle-Ottawa Scale. Publication bias was assessed using Egger's test.
Thirteen articles that investigated 2,549 participants were included in this meta-analysis. Patients with PE had significantly higher levels of C-reactive protein (CRP), interleukin (IL)-4, IL-6, IL-8, IL-10, and tumor necrosis factor (TNF) than the controls. CRP and pro-inflammatory cytokine levels were higher than those of anti-inflammatory cytokines. Patients with gestational age > 34 weeks had significantly higher IL-6 and TNF levels. Patients with higher systolic blood pressure had significantly higher IL-8, IL-10, and CRP levels.
Inflammatory imbalance is an independent risk factor for PE development. Impairment of the anti-inflammatory system is a crucial initiating factor for PE development. Failed autoregulation, manifested as prolonged exposure to pro-inflammatory cytokines, leads to PE progression. Higher levels of inflammatory biomarkers suggest more severe symptoms, and pregnant women after 34 weeks of gestation are more susceptible to PE.

LiDAR reveals the process of vision-mediated predator-prey relationships

Exploring the processes of interspecific relationships is crucial to understanding the mechanisms of biodiversity maintenance. Visually detecting interspecies relationships of large mammals is limited by the reconstruction accuracy of the environmental structure and the timely detection of animal behavior. Hence, we used backpack laser scanning (BLS) to reconstruct the high-resolution three-dimensional environmental structure to simulate the process of a predator approaching its prey, indicating that predator tigers would reduce their visibility by changing their behavior. Wild boars will nibble off about 5m of branches around the nest in order to create better visibility around the nest, adopting an anti-predation strategy to detect possible predators in advance. Our study not only points out how predator–prey relationships are affected by visibility as the environment mediates it, but also provides an operable framework for exploring interspecific relationships from a more complex dimension. Finally, this study provides a new perspective for exploring the mechanisms of biodiversity maintenance.

This article studies the geological characteristics analysis and prospecting direction of the triangle top gold mine in the Saishiteng area, so as to realize accurate and massive gold mining in this area in the future. Taking the triangle top area of Saishiteng area located in the northern margin of Qaidam Basin from Saishiteng Mountain to Tanjianshan Mountain as the research object, the technical route of combining field investigation with indoor comprehensive research is adopted, and the observation of field geological characteristics is emphasized, and the geophysical characteristics are analyzed, and finally the characteristics of regional geological structure are obtained. The controlled source audio-frequency magnetotelluric method is used in combination with geological characteristics, and the distribution of gold deposits in the study area is determined by calculating and detecting electromagnetic anomalies, and the prospecting direction of gold deposits is clear. The test results show that the stratum structure in the study area is rich, the folds and faults are very complex, the primary colors of metals are distributed in all strata, and the elements of Au are more distributed. According to the results of electromagnetic analysis, there are magnetic anomalies in M15 and M18-1 areas, and these two prospecting target areas are clearly identified through detection.

The coronavirus disease 2019 (COVID-19) pandemic caused by severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2) has led to over 770 million cases and >6.9 million deaths worldwide. We identified a panel of human neutralizing monoclonal antibodies (mAbs) targeting the SARS-CoV-2 Spike protein using Harbour H2L2 transgenic mice immunized with Spike receptor-binding domain (RBD) (J. A. Duty, T. Kraus, H. Zhou, Y. Zhang, et al., Med 3:705–721, 2022, https://doi.org/10.1016/j.medj.2022.08.002 ). Representative antibodies from genetically distinct families were evaluated for the inhibition of replication-competent VSV expressing SARS-CoV-2 Spike (rcVSV-S) in the place of VSV-G. One mAb clone denoted FG-10A3 and its therapeutically modified version STI-9167 effectively inhibited infection and in vivo proliferation of early variants of SARS-CoV-2 including Omicron BA.1 and BA.2 and corresponding pseudoviruses and rcVSV-S variants (Duty et al.). To define the epitope of the broadly reactive FG-10A3 mAb, we generated mAb-resistant rcVSV-S virions and performed structural analysis of the antibody/antigen complex using cryo-electron microscopy (EM). FG-10A3/STI-9167 is a Class 1 antibody that prevents Spike-ACE2 binding by engaging a region within the Spike receptor binding motif. Sequencing of mAb-resistant rcVSV-S virions identified F486 as a critical residue for mAb neutralization, with structural analysis revealing that both the variable heavy and light chains of STI-9167 bound the disulfide-stabilized 470–490 loop at the Spike RBD tip. Furthermore, neutralization studies using rcVSV-S F486 point mutants and currently-circulating variants Omicron BA.5, XBB.1.5, and BQ.1.1 that contain a V or P at position 486 further supported the model in which residue 486 is an important residue for FG-10A3 inhibition. This work provides an experimental strategy to define the neutralizing capacity and limitations of mAb therapeutics against emerging SARS-CoV-2 variants. The COVID-19 pandemic remains a significant public health concern for the global population; the development and characterization of therapeutics, especially ones that are broadly effective, will continue to be essential as severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2) variants emerge. Neutralizing monoclonal antibodies remain an effective therapeutic strategy to prevent virus infection and spread so long as they recognize and interact with circulating variants. The epitope and binding specificity of a neutralizing anti-SARS-CoV-2 Spike receptor-binding domain antibody clone against many SARS-CoV-2 variants of concern were characterized by generating antibody-resistant virions coupled with cryo-EM structural analysis and VSV-spike neutralization studies. This workflow can serve to predict the efficacy of antibody therapeutics against emerging variants and inform the design of therapeutics and vaccines.

Intermittent Fasting versus Continuous Calorie Restriction: Which Is Better for Weight Loss?

We conducted a systematic review and meta-analysis of randomized clinical trials and pilot trial studies to compare the effectiveness of intermittent fasting (IF) and continuous calorie restriction (CCR) in overweight and obese people. The parameters included body mass index (BMI), body weight, and other metabolism-related indicators. A systematic search in PubMed, Embase, Cochrane Library, and Web of Science was conducted up to January 2022. Standardized mean differences (SMDs) with 95% confidence intervals (CIs) were used to measure the effectiveness. Publication bias was assessed using Egger’s test. The stability of the results was evaluated using sensitivity analyses. The significance of body weight change (SMD = −0.21, 95% CI (−0.40, −0.02) p = 0.028) was more significant after IF than CCR. There was no significant difference in BMI (SMD = 0.02, 95% CI (−0.16, 0.20) p = 0.848) between IF and CCR. These findings suggest that IF may be superior to CCR for weight loss in some respects.

ICU capacity, ICU staffing, and postcardiotomy ECMO outcomes in China: a multilevel cross-sectional study
Background
As the complexity of cardiac surgeries increases and patient selection criteria expand, the use of veno-arterial extracorporeal membrane oxygenation for high-risk patients has become more prevalent. Despite its critical role in sustaining life, postcardiotomy ECMO (PC-ECMO) is associated with high in-hospital mortality rates. Intensive care unit (ICU) capacity and staffing are crucial in determining patient outcomes. This study aimed to investigate the relationships among ICU capacity, staffing levels, and outcomes in PC-ECMO patients in China.
Methods
A multilevel cross-sectional analysis was conducted using data from 586 hospitals that participated in China’s National Quality Improvement Program in 2018. From these hospitals, we selected those that performed PC-ECMO procedures between April 2016 and December 2021. The novel ICU Capacity Comprehensive Index (ICUCCI) was calculated for each hospital, incorporating medical service capacity, technical ability, quality and safety, and service efficiency. ICU staffing was assessed by patient-to-bed, patient-to-physician, and patient-to-nurse ratios. The primary outcome was in-hospital mortality, with secondary outcomes including complications, length of stay (LOS), and hospitalization costs.
Results
A total of 102 hospitals, encompassing 2,601 patients, were included in the analysis. Higher ICUCCI values were associated with reduced in-hospital mortality (OR: 0.83, 95% CI: 0.70–0.97,

= 0.025) and fewer complications (OR: 0.82, 95% CI: 0.68–0.99,
P
= 0.046). However, higher ICUCCI values correlated with longer LOSs (IRR: 1.14, 95% CI: 1.06–1.22,
P
< 0.001) and increased hospitalization costs (IRR: 1.32, 95% CI: 1.24–1.40,
P
< 0.001). ICU staffing ratios, including patients per bed, physician, and nurse, were protective against mortality, with the ratio of patients per ICU bed showing the most pronounced effect (OR: 0.69, 95% CI: 0.55–0.87,
P
= 0.002). Increased staffing was also associated with longer LOS but did not affect overall complication rates or costs. The ratio of patients per ICU bed was linked to a greater risk of bloodstream infection (OR: 1.96, 95% CI: 1.14–3.46,
P
= 0.022).
Conclusions
This study highlights the critical role of ICU capacity and staffing levels in improving outcomes for patients receiving PC-ECMO. While higher ICU capacity and staffing are associated with reduced mortality, they also correlate with longer hospital stays and/or increased costs, suggesting the need for a balanced approach in resource allocation. Our findings underline the importance of optimizing ICU staffing ratios and enhancing healthcare equity to improve patient care across diverse healthcare institutions.

Validation and modification of the phoenix sepsis score for predicting in-hospital mortality in children with suspected infection admitted to the intensive care unit

The Society of Critical Care Medicine Pediatric Sepsis Definition Task Force recommends the use of the Phoenix Sepsis Score (PSS) to diagnose pediatric sepsis among children with suspected infection. However, the performance of the PSS in different healthcare settings remains unclear. The present study aimed to validate and modify the ability of the PSS to predict in-hospital death in children with suspected infection admitted to the intensive care unit (ICU) in China.
This multicenter retrospective cohort study included children aged ≤18 years with suspected infection admitted to the ICU from five hospitals in China between January 2012 and December 2023. Two children's specialized hospitals and three tertiary general hospitals participated. The primary evaluation of PSS prediction performance was based on in-hospital mortality discrimination through the area under the receiver operating characteristic curve (AUROC). The secondary evaluation was conducted using the area under the precision-recall curve (AUPRC). Given the moderate discriminative performance of the PSS in this cohort, we employed extreme gradient boosting (XGBoost) with SHapley Additive exPlanations (SHAP) to identify key predictors of in-hospital mortality. Predictors selected through this process were incorporated into the original PSS framework while prioritizing clinical feasibility, resulting in a modified score, PSS+, for mortality prediction.
Among 9221 ICU admissions for children with suspected infections (13.4% mortality), the PSS showed moderate discrimination, with an AUROC of 0.60 (95% CI: 0.59-0.62). The modified PSS (PSS+), which selectively incorporated comorbidities, vital signs, and demographic variables identified through model development, outperformed the original PSS-4, PSS-8, and pediatric sequential organ failure assessment (pSOFA) in terms of both internal validation (AUROC of 0.75, 95% confidence interval [CI]: 0.70-0.78) and external validation (AUROC of 0.71, 95% CI: 0.69-0.73).
The PSS demonstrated only moderate ability to predict in-hospital mortality among pediatric ICU patients in the present cohort, indicating that its application in other healthcare settings should be approached with caution. The modified PSS+ scoring system significantly improved mortality prediction performance and may serve as a more reliable and clinically applicable tool for risk assessment in critically ill children.

Unbiased approach to identify and assess efficacy of human SARS-CoV-2 neutralizing antibodies

Coronavirus disease 2019 (COVID-19) continues to significantly impact the global population, thus countermeasure platforms that enable rapid development of therapeutics against variants of SARS-CoV-2 are essential. We report use of a phage display human antibody library approach to rapidly identify neutralizing antibodies (nAbs) against SARS-CoV-2. We demonstrate the binding and neutralization capability of two nAbs, STI-2020 and STI-5041, against the SARS-CoV-2 WA-1 strain as well as the Alpha and Beta variants. STI-2020 and STI-5041 were protective when administered intravenously or intranasally in the golden (Syrian) hamster model of COVID-19 challenged with the WA-1 strain or Beta variant. The ability to administer nAbs intravenously and intranasally may have important therapeutic implications and Phase 1 healthy subjects clinical trials are ongoing.

In this paper, a numerical method with high order accuracy and high resolution was developed to simulate the Richtmyer-Meshkov(RM) instability driven by cylindrical shock waves. Compressible Euler equations in cylindrical coordinate were adopted for the cylindrical geometry and a third order accurate group control scheme was adopted to discretize the equations. Moreover, an adaptive grid technique was developed to refine the grid near the moving interface to improve the resolution of numerical solutions. The results of simulation exhibited the evolution process of RM instability, and the effect of Atwood number was studied. The larger the absolute value of Atwood number, the larger the perturbation amplitude. The nonlinear effect manifests more evidently in cylindrical geometry. The shock reflected from the pole center accelerates the interface for the second time, considerably complicating the interface evolution process, and such phenomena of reshock and secondary shock were studied.

Does the territorial behaviour of the Amur tiger affect the accuracy of occupancy estimation?

Accurate estimates of wildlife distributions and population persistence are essential for conservation programs. Occupancy models that account for detection probability have been used to characterize the occupancy and habitat selection of imperilled species. However, failure to distinguish between true-presence and pseudo-presence associated with territorial behaviour can result in the overestimation of the occupancy probability of target species in unsuitable habitats, and this can have major implications for the development of conservation strategies. For highly territorial wildlife species requiring high-quality habitat for survival, occasional 'Presence' in unsuitable areas might be related to dispersal or migration, but this does not reflect actual occupancy and habitat use. 'True-Presence' and 'Pseudo-Presence' should be distinguished for target species to better reflect their actual occupancy and habitat use. To investigate the effect of 'True-Presence' and 'Pseudo-Presence' on wildlife occupancy estimation, we developed a modified model (Mm) that considers the territorial behaviour of the Amur tiger in northeast China, which distinguished between 'True-Presence' and 'Pseudo-Presence'. We compared two models, Mm and M0 (conventional occupancy model), and assessed model performance using goodness-of-fit evaluation, detection and occupancy probability, and favourable variable selection. We found that Mm, which has fewer favourable variables, is more powerful than M0 for estimating detection and occupancy probability, as well as characterizing the effect of various factors on the habitat use of Amur tigers. Furthermore, Mm significantly reduced the overestimation of occupancy probability outside the home range compared with M0, and detection probability estimates did not significantly differ between M0 and Mm. Finally, Mm provided more empirical habitat selection variables for the Amur tiger. Our results revealed how 'True-Presence' and 'Pseudo-Presence' affect the occupancy probability and habitat selection of Amur tigers. Our method improves the accuracy of occupancy models; it can also be used to characterize the habitat selection and distribution of wildlife species and be applied to other territorial species.Competing Interest StatementThe authors have declared no competing interest.
