Background Androgenetic alopecia (AGA) is a common cause of hair loss in both genders that may be associated with disturbed systemic metabolism. Irisin is a hormone‐like myokine that greatly influences systemic metabolism and is linked to cardiovascular diseases. Aim To detect irisin role in AGA and its associated metabolic syndrome (MetS) and cardiovascular risk. Patients/Methods This case–control study included 44 AGA patients of both genders and 22 healthy individuals. Serum irisin level was measured using ELISA and scalp biopsy was taken to detect irisin immunohistochemically. Carotid Doppler ultrasonography was performed to measure carotid intima media thickness (CIMT). Results Higher serum irisin was significantly detected in AGA patients (p ˂ 0.001), and in males (p = 0.01) particularly severe cases (p ˂ 0.001). It was significantly higher in AGA patients presenting with MetS and those suffering from dyslipidemia (p ˂ 0.001 for both). Multivariate regression analysis proved BMI (p = 0.01) and serum irisin (p = 0.02) as independent predictors of CIMT abnormality among AGA patients. Regarding cutaneous irisin expression, the epidermal H‐score was significantly higher in AGA patients with MetS compared to those without (p = 0.04). Epidermal H‐score ˃100 was significantly associated with male gender (p = 0.05), severe AGA (p = 0.02), MetS (p = 0.03), dyslipidemia (p = 0.03), and abnormal CIMT (p = 0.03). Conclusion High serum irisin and upregulated epidermal irisin expression are associated with the incidence of MetS, dyslipidemia, and CIMT abnormality among AGA patients. This may indicate resistance to irisin, which hinders its favorable cardiometabolic actions. Further studies are warranted to investigate the concept of irisin resistance in AGA patients, which was uniquely discussed in the present study.

Background: We investigated the anti-cancer effect of carnosine-loaded niosomes (Car-NIO) and melittin-loaded niosomes (Mel-NIO) with olaparib in breast cancer cell lines (MCF-7 and MDA-MB-231). Methods: The thin film method was used for preparing the niosomes and characterized in terms of morphology, size, and polydispersity index (PDI). We further evaluated the impact of these peptides on breast cancer cells viability, RT-qPCR assays, malondialdehyde (MDA) activity, and cell cycle progression, to determine if these are linked to carnosine and melittin’s anti-proliferative properties. Results: Car-NIO and Mel-NIO in vitro study inhibited cancer cell viability. They have also upregulated the expression of protein 53 (P53), BCL2-Associated X Protein (Bax), caspase-9, caspase-3, programmed cell death 4 (PDCD4), and Forkhead box O3 (FOXO3), while downregulated the expression of B-cell lymphoma 2 (Bcl2), poly (ADP-ribose) polymerase (PARP 1), and MicroRNA-183 (miRNA-183). The MCF-7 cells were arrested at the G2/M phase in Car-NIO, on the other hand, the MDA-MB-231 cells were arrested at the S phase. While the Mel-NIO and olaparib arrested the MCF-7 and MDA-MB-231 cells at the G0/1 phase. Conclusion: Our study successfully declared that Mel-NIO had more anti-cancer effects than Car-NIO in both MCF-7 and MDA-MB-231 breast cancer cells.

Water and soil polluted by heavy metals (HMs) are the primary problem due to rapidly increasing urbanization and industrialization. For the treatment of polluted soil, phytoremediation turns into a cost-effective and eco-friendly technique. The current research aimed to examine the load of pollution, specifically HMs, in sediment and wastewater (WW) of the GadoonAmazai Industrial Estate (GAIE), Pakistan and compare the ability of native grass species Cynodon dactylon and Digiteria sanguinalis for the phytoaccumulation of HMs. The industrially polluted soil was analysed for HMs using atomic absorption spectrophotometry (AAS) and compared with healthy soil (irrigated by freshwater), which served as a control. The HM accumulation was considerably higher in the soil irrigated with WW than in control soil samples. The most substantial metal pollutant was manganese (Mn), which accumulated up to 2491.7 mg/kg in the WW irrigated soil. For assessing the bioremoval efficiency of grass species, pot experimentation was performed for 90 days. Soil samples and grasses were collected from the pots to examine the HM removal efficiency. A significant reduction was noted in physicochemical characteristics of the soil, such as electrical conductivity, total organic matter, phosphorus, potassium, and saturation. The grasses removed up to 59.0% of the Cu and 59.9% of Mn from the soil. The highest bioconcentration factor (BCF) and translocation factor (TF) of Cu were observed for D. sanguinalis. While the highest BCF and TF of Mn were obtained for C. dactylon. The research showed that the grass significantly (p ≤ 0.05) reduced HM in soil samples. Moreover, the selected grasses found a higher capability to accumulate HM in the roots than in the shoot. The maximum Cu removal was obtained by D. sanguinalis and Mn by C. dactylon. The research study concluded that phytoremediation using D. sanguinalis and C. dactylon is an eco-friendly and cost-effective method that can be utilized for soil remediation.

Developing a Chitosan/polyvinyl alcohol hydrogel for gastro-retentive release of ranitidine and enhanced anti-ulcerative properties

Ranitidine is widely used to treat gastrointestinal conditions, but recent studies have revealed severe potential side effects, including a link to cancer. Therefore, this study aims to develop a new gastro-retentive formulation of ranitidine by utilizing the biocompatibility and biodegradability of Chitosan, along with the strength and hydrophilicity of polyvinyl alcohol (PVA). A chitosan/PVA/ranitidine hydrogel was created using the freeze-thaw method and evaluated for stability, ranitidine release behavior, and efficacy in treating ulcers in rats compared to a commercial formulation. The hydrogel demonstrates an average particle size of 69 nm, a polydispersity index of 0.344, and a zeta potential of + 38 mV. Transmission Electron Microscopy confirmed the spherical shape of the formulation, while X-ray diffraction verified its crystalline structure. Additionally, the study observed an impressive encapsulation efficiency of 98.66% ± 1.01 and a high drug content of 49.82% ± 1.29, as confirmed by Fourier transform infrared analysis. The prepared hydrogel controls the release of ranitidine over 12 h, with an average release of 87.98% ± 4.01%. The hydrogel exhibits minimal degradation over 15 days, greater thermal stability than ranitidine, and adequate stability in acidic gastric conditions. Furthermore, the cytotoxicity assay demonstrated that the hydrogel is biocompatible and promotes cell growth. The study discovered that the hydrogel formulation enhances the effects of ranitidine, particularly its antioxidant and anti-inflammatory properties. In vivo studies illustrated the hydrogel’s promising ulcer-healing properties, suggesting potential use in treating peptic ulcers. Hence, the chitosan/PVA hydrogel can be used as a possible drug delivery system for the sustained release of ranitidine.

Bioremediation of Metal-Polluted Industrial Wastewater with Algal-Bacterial Consortia: A Sustainable Strategy

Aquatic pollution is a burning issue nowadays due to urbanization and industrialization. Industrial wastewater (IWW) contains pollutants that pose a great risk to the environment and human beings and is a big challenge for industries. The remediation of IWW by microorganisms is an environmentally friendly technique. This study was carried out to evaluate the pollution of IWW and to use consortia of Bacillus pakistanensis, Lysinibacillus composti, and Cladophora glomerata for bioremediation. The IWW was obtained from the Hayatabad Industrial Estate and was evaluated for physicochemical parameters and metal concentration. A pot experiment was carried out for two weeks to assess the efficiency of the developed consortia. The IWW and tap water (control) were treated with three different consortia (Bacillus pakistanensis-Cladophora glomerata (CT1, E1), Lysinibacillus composti-Cladophora glomerata (CT2, E2), and Bacillus pakistanensis-Lysinibacillus composti-Cladophora glomerata (CT3, E3). The three pots (CT1, CT2, and CT3) serving as the control were provided with tap water, and the three experimental pots (E1, E2, and E3) were provided with IWW. After treatment, substantial reductions were obtained in the following parameters and percentages: colour 85.7%, electrical conductivity (EC) 40.8%, turbidity 69.6%, sulphide 78.5%, fluoride 38.8%, chloride 62.9%, biological oxygen demand (BOD) 66%, chemical oxygen demand (COD) 81.8%, total suspended solids (TSSs) 82.7%, total dissolved solids (TDSs) 24.6%, Ca hardness 37.2%, Mg hardness 50%, and total hardness 39%. The samples of water were also examined for metal concentrations using atomic absorption spectrophotometry. The selected species removed 98.2% of Mn, 94% of Cu, 97.7% of Cr, 91.6% of Cd, 92.8% of Co, 79.6% of Ag, 82.6% of Ni, 98% of Ca, 90% of Mg, and 82.1% of Pb. The BCF values showed by the consortia for Mn, Cu, Cr, Cd, Co, Ag, Ni, Ca, Mg, and Pb were 91.8, 67, 97.5, 83.3, 85.7, 48.1, 80.4, 84.3, 82.5, and 80.3%, respectively. The t-test analysis showed that the treatment with the selected species significantly decreased the metal concentrations in the IWW (p ≤ 0.05). Overall, the study concludes that metal concentration in the water was decreased significantly by the consortia of algae-bacteria.

Dental Students’ Perception, Awareness and Knowledge About HPV Infection, Vaccine, and Its Association with Oral Cancer: A Multinational Study

Background: Human papilloma virus (HPV) infection forms a major etiological factor for oropharyngeal cancer (OPC), which has exhibited increased global incidence. Aim: To compare the knowledge regarding HPV, its association with OPC, and HPV vaccine among students from different countries, years of the undergraduate program, and gender. Methods: The current multinational cross-sectional study was conducted in 886 undergraduate dental students from Egypt, India, Pakistan, Saudi Arabia, UAE, and Sudan through Google survey forms from July 2021 to September 2021. The survey form comprised 27 items divided into four sections. The answers to the questionnaire were compared among students from different countries, different years of the undergraduate program, and males and females. Chi-square test was used to evaluate the correlation between the demographic characteristics of students and their knowledge regarding HPV and OPC. Results: Females exhibited a better knowledge regarding knowledge and perception on HPV vaccine, whereas males exhibited a better knowledge regarding HPV and its correlation with OPC, and these differences were statistically significant (P < 0.05). The third- and fourth-year undergraduate students displayed a higher awareness of OPC and its connection with HPV than other year students, and this variance was found to be statistically significant (P < 0.001). Third-year and internship students exhibited a more positive attitude and comfort regarding the vaccine and discussing the same with patients than the other educational- level students. Students from India exhibited better knowledge about HPV and its association with OPC than the students from other countries, and this difference was statistically significant (P < 0.001). Conclusion: Disparities in knowledge regarding HPV-related oral cancer have been detected among the female and male participants among different nations. From the entire study population, Indian students exhibited better knowledge regarding HPV. Females from all the nations exhibited a more positive attitude and comfort regarding the vaccine and discussing the same with patients than males. The results of this necessitate intervention measures including training workshops and awareness campaigns. Improving their knowledge regarding the same may increase their awareness, resulting in better patient care. PurposeDespite evidence of myocardial infarct size reduction in animal studies, remote ischaemic conditioning (RIC) failed to improve clinical outcomes in the large CONDI-2/ERIC-PPCI trial. Potential reasons include that the predominantly low-risk study participants all received timely optimal reperfusion therapy by primary percutaneous coronary intervention (PPCI). Whether RIC can improve clinical outcomes in higher-risk STEMI patients in environments with poor access to early reperfusion or PPCI will be investigated in the RIC-AFRICA trial.MethodsThe RIC-AFRICA study is a sub-Saharan African multi-centre, randomized, double-blind, sham-controlled clinical trial designed to test the impact of RIC on the composite endpoint of 30-day mortality and heart failure in 1200 adult STEMI patients without access to PPCI. Randomized participants will be stratified by whether or not they receive thrombolytic therapy within 12 h or arrive outside the thrombolytic window (12–24 h). Participants will receive either RIC (four 5-min cycles of inflation [20 mmHg above systolic blood pressure] and deflation of an automated blood pressure cuff placed on the upper arm) or sham control (similar protocol but with low-pressure inflation of 20 mmHg and deflation) within 1 h of thrombolysis and applied daily for the next 2 days. STEMI patients arriving greater than 24 h after chest pain but within 72 h will be recruited to participate in a concurrently running independent observational arm.ConclusionThe RIC-AFRICA trial will determine whether RIC can reduce rates of death and heart failure in higher-risk sub-optimally reperfused STEMI patients, thereby providing a low-cost, non-invasive therapy for improving health outcomes.

Efficacy and safety profile of biotechnological agents and Janus kinase inhibitors in VEXAS syndrome: data from the international AIDA Network VEXAS registry

VEXAS syndrome, a recently identified systemic autoinflammatory disorder, poses new diagnostic and management challenges. Based on experience with other autoinflammatory diseases, anti-interleukin (IL)-1, anti-IL-6, anti-tumor necrosis factor (TNF) biotechnological agents, and Janus kinase inhibitors (JAKis) have been widely employed in VEXAS patients. The aim of this study is to evaluate the global effectiveness and safety of biotechnological agents and JAKis using data from the real-world context.
Clinical, laboratory, and therapeutic data from VEXAS patients were obtained from the international AIDA Network VEXAS registry.
In total, 69 VEXAS patients were enrolled in the study. Among them, 12 patients (13 treatment courses) received IL-1 inhibitors, 12 patients (13 treatment courses) were administered anti-IL-6 agents, 8 patients (9 treatment courses) were treated with anti-TNF agents, and 16 patients (17 treatment courses) were treated with JAKis. A complete response was observed in 3 patients (23%) treated with anti-IL-1 agents, 2 patients (15%) receiving IL-6 inhibitors, 1 patient (11%) receiving TNF inhibitors, and 4 patients (23.5%) treated with JAKis. The mean prednisone (or equivalent) dosage significantly decreased during anti-IL-1 treatment (p = 0.01), while glucocorticoids changed during anti-IL-6, anti-TNF, and JAKi treatment in a non-significant fashion. A total of 21 patients experienced adverse events, 3 of which led to death (gut perforation, Legionnaires' disease, and infectious pneumonia) while on JAKis; treatment withdrawal was required for 8 out of 21 patients.
IL-1 and IL-6 inhibitors, along with JAKis, represent promising therapeutic options for VEXAS patients, albeit careful monitoring is mandatory to control disease activity and ensure safety.

The impact of
LRRK2
variants on the risk of Parkinson’s disease in Egyptians remains unknown. We examined 1210 Egyptians (611 PD patients and 599 controls) from 16 governorates across Egypt for 12
LRRK2
pathogenic variants. The p.Gly2019Ser was the only variant detected, with a prevalence of 4.1% in sporadic cases, 6.5% in familial cases, and 0.68% in controls. Among p.Gly2019Ser carriers, all were heterozygous bar one homozygous patient, and all shared the common haplotype 1. Demographics and UPDRS scores did not differ between carriers and non-carriers, with most patients being males and developed PD in their fifties. Young and Early-onset PD prevalence was 37.5% in carriers and 33% in non-carriers. Familial cases were 16.6% in carriers and 11.5% in non-carriers. This study affirms that like other Mediterranean populations, Egyptians with PD have a higher prevalence of the p.Gly2019Ser variant compared to the global average.
LRRK2
inhibitors could be promising therapeutic options for further exploration in this population.

Ophthalmic involvement in VEXAS syndrome and its influence on mortality: insights from the international AIDA network registry

Orbital inflammation is the most common presentation of VEXAS, with any orbital structure potentially affected. Non-sight-threatening and uncomplicated anterior non-granulomatous uveitis and anterior diffuse scleritis follow in frequency, along with episcleritis. Ophthalmic involvement was significantly associated with relapsing polychondritis (
= 0.014), with an increased chance of a fatal outcome (RR 5.87,
= 0.016) and independently predicts a higher mortality rate (OR 3.72,
= 0.026). Treatment of ophthalmic involvement showed full or partial response to glucocorticosteroids alone in 66.7% of cases. Ophthalmic involvement is common in VEXAS syndrome and signals a poorer prognosis in terms of mortality, highlighting the need for close monitoring.
