Pese a la abundante bibliografía sobre estas piezas, hasta el momento no se había realizado un análisis arqueométrico y arqueometalúrgico de las mismas. Los resultados permiten inferir observaciones interesantes acerca de su composición, manufactura y secuencia tecnológica de elaboración, así como sobre la posible procedencia del oro, confirmando, con los paralelos reconocidos para la decoración de estas placas, de una clara raigambre atlántica.

Se presenta un estudio detallado de dos discos de oro de procedencia desconocida que permanecen depositados desde mediados del s. xx en el Museo Arqueológico de Asturias. Pese a la abundante bibliografía sobre estas piezas, hasta el momento no se había realizado un análisis arqueométrico y arqueometalúrgico de las mismas. Los resultados permiten inferir observaciones interesantes acerca de su composición, manufactura y secuencia tecnológica de elaboración, así como sobre la posible procedencia del oro, confirmando, con los paralelos reconocidos para la decoración de estas placas, de una clara raigambre atlántica. Los ejemplares de las Islas Británicas se asocian con inhumaciones campaniformes. Para los objetos en estudio se consideró una cronología más moderna dada la ausencia de evidencias similares en Asturias. Sin embargo, la reciente identificación de las típicas cerámicas en espacios de transformación de mineral de cobre en el territorio central asturiano posibilita una nueva consideración, reforzada con los más recientes hallazgos áureos de similar cronología en la Península Ibérica que se reconocen como parte del ajuar de inhumaciones campaniformes. Todos los argumentos refuerzan la propuesta de la existencia de redes de intercambio entre las Islas Británicas y el continente a lo largo de aquel periodo.

Background and purpose Levodopa–entacapone–carbidopa intestinal gel (LECIG) infusion is a recently developed device‐aided therapy for advanced Parkinson disease (PD) patients. The aim of this study was to report real‐world evidence about the effectiveness, tolerability, and safety of LECIG in PD patients. Methods A multicenter observational retrospective study of the first patients who initiated LECIG in Spain was performed. All neurologists with an experience of at least two patients treated until 30 March 2024 were invited to participate. Data about effectiveness and safety from the medical records (V0, pre‐LECIG; V1, initiation of LECIG; V2, post‐LECIG follow‐up) with a total of 246 variables were collected. Results Seventy‐three PD patients (61.6% males, 70.1 ± 9.1 years old) from 21 Spanish centers with a mean disease duration of 14.4 ± 6.3 years (range = 5–31) were included. Twenty‐six patients (35.6%) were switched directly from levodopa–carbidopa intestinal gel. The mean exposure to LECIG was 177.3 ± 110.5 days (range = 7–476). The mean daily OFF time decreased from 5.2 ± 3 (pre‐LECIG) to 1.9 ± 1.8 (post‐LECIG; n = 66, p < 0.0001). Global improvement was observed in >85% of the patients. No significant change was detected in the levodopa equivalent daily dose from V0 to V2. Only 7% received 24‐h infusion, and 24.7% required more than one cartridge per day at V2. Thirty‐four patients (46.6%) had at least one adverse event related to LECIG and/or the device system. Five patients (6.8%) discontinued LECIG. Conclusions LECIG was safe and effective in advanced PD patients.

Crimean-Congo Hemorrhagic Fever, Spain, 2013–2021

Crimean-Congo hemorrhagic fever (CCHF) is a viral infectious disease for which distribution of the main vector, Hyalomma spp. ticks, is expanding. We analyzed all 10 cases of CCHF diagnosed in Spain during 2013-2021; case-patient median age was 56.5 years, and 7 were men. We identified CCHF virus genotypes III and V. Six case-patients acquired the infection in urban areas. Sixty percent of patients were infected in summer and 40% in spring. Two patients met criteria for hemophagocytic syndrome. Seven patients survived. The epidemiologic pattern of CCHF in Spain is based on occasional cases with an elevated mortality rate. Genotype III and, to a less extent also genotype V, CCHF circulates in humans in a common geographic area in Spain. Those data suggest that the expansion pathways are complex and may change over time. Physicians should remain alert to the possibility of new CCHF cases.

CMV Infection Is Directly Related to the Inflammatory Status in Chronic Heart Failure Patients

High levels of inflammation play an important role in chronic heart failure (CHF). Patients with CHF have elevated levels of pro-inflammatory cytokines circulating systemically, mainly TNF and IL-6. However, there are almost no studies that relate these levels to the functional status of patients in CHF, much less to their CMV serostatus. In this study, patients with CHF (n=40; age=54.9 ± 6.3; New York Heart Association functional classification (NYHA, I-III) and healthy controls (n=40; age=53.5 ± 7.1) were analyzed. The serum concentrations of nine pro- and anti-inflammatory cytokines were measured by Luminex ® xMap Technology and the basal level of mRNA expression of some immune molecules was quantified by TaqMan™ Array in CD4+ T-lymphocytes. The concentration of these cytokines in culture supernatants in response to anti-CD3 and LPS was also measured. The percentage of CD28null T-cells was determined, as well as the antibody titer against CMV. We found a higher concentration of all cytokines studied in CHF serum compared to healthy controls, as well as a direct correlation between functional status in CHF patients and levels of inflammatory cytokines. Moreover, the highest cytokine concentrations were found in patients with higher concentrations of lymphocytes lacking CD28 molecule. The cytokine production was much higher in CMV+ patients, and the production of these cytokines was found mainly in the T-lymphocytes of CMV+ patients in response to anti-CD3. Anti-CMV antibody levels were positively correlated with cytokine levels. The baseline expression of specific mRNA of the main molecules involved in the Th1 response, as well as molecules related to the CD4+CD28 null subset was higher in CMV+ patients. The cytokine concentrations are higher in CHF CMV+ patients and these concentrations are related to the production of antibodies against CMV. These high levels of cytokines are also associated with the more differentiated CD28null lymphocyte populations. All this, together with the dynamics of the pathology itself, makes CMV+ patients present a worse functional status and possibly a worse evolution of the pathology.

Lower ototoxicity and absence of hidden hearing loss point to gentamicin C1a and apramycin as promising antibiotics for clinical use

Spread of antimicrobial resistance and shortage of novel antibiotics have led to an urgent need for new antibacterials. Although aminoglycoside antibiotics (AGs) are very potent anti-infectives, their use is largely restricted due to serious side-effects, mainly nephrotoxicity and ototoxicity. We evaluated the ototoxicity of various AGs selected from a larger set of AGs on the basis of their strong antibacterial activities against multidrug-resistant clinical isolates of the ESKAPE panel: gentamicin, gentamicin C1a, apramycin, paromomycin and neomycin. Following local round window application, dose-dependent effects of AGs on outer hair cell survival and compound action potentials showed gentamicin C1a and apramycin as the least toxic. Strikingly, although no changes were observed in compound action potential thresholds and outer hair cell survival following treatment with low concentrations of neomycin, gentamicin and paromomycin, the number of inner hair cell synaptic ribbons and the compound action potential amplitudes were reduced. This indication of hidden hearing loss was not observed with gentamicin C1a or apramycin at such concentrations. These findings identify the inner hair cells as the most vulnerable element to AG treatment, indicating that gentamicin C1a and apramycin are promising bases for the development of clinically useful antibiotics.

Novel Agents as Main Drivers for Continued Improvement in Survival in Multiple Myeloma

(1) Background: New therapeutic strategies have improved the prognosis of multiple myeloma (MM), changing the accepted view of this disease from being incurable to treatable. (2) Methods: We studied 1001 patients with MM between 1980 and 2020, grouping patients into ten-year periods by diagnosis 1980–1990, 1991–2000, 2001–2010 and 2011–2020. (3) Results: After 65.1 months of follow-up, the median OS of the cohort was 60.3 months, and OS increased significantly over time: 22.4 months in 1980–1990, 37.4 months in 1991–2000, 61.8 months in 2001–2010 and 103.6 months in 2011–2020 (p < 0.001). Using novel agents in the front-line setting for myeloma patients yielded a significantly better OS than in those treated with conventional therapies, especially when combinations of at least two novel agents were used. The median OS of patients treated with the combination of at least two novel agents in induction was significantly prolonged compared to those treated with a single novel agent or conventional therapy in induction: 143.3 vs. 61.0 vs. 42.2 months (p < 0.001). The improvement was apparent in all patients regardless of age at diagnosis. In addition, 132 (13.2%) patients were long-term survivors (median OS ≥ 10 years). Some independent clinical predictors of long-term survival were identified: ECOG < 1, age at diagnosis ≤ 65 years, non-IgA subtype, ISS-1 and standard-risk cytogenetic. Achieving CR and undergoing ASCT were positively associated with >10 years of survival. (4) Conclusions: The combination of novel agents appears to be the main factor for the improvement in survival in MM, which is becoming a chronic and even curable disease in a subtype of patients without high-risk features.
