Operationalisation of health equity principles in physiotherapy hospital triage policies
Background Healthcare triage policies are vital for allocating limited resources fairly and equitably. Despite extensive studies of healthcare equity, consensus on the applied definition of equity in triage remains elusive. This study aimed to investigate how the principles of equity are operationalised in Australian hospital physiotherapy triage tools to guide resource distribution. Methods A retrospective, qualitative content analysis of 13 triage policies from 10 hospitals across Australia was conducted. Triage policies from both inpatient and outpatient settings were sourced. Data were coded deductively using the five discrete domains of the multi-faceted operational definition of health equity posited by Lane et al. (2017): 1) point of equalisation in the health service supply/access/outcome chain, 2) need or potential to benefit, 3) groupings of equalisation, 4) caveats to equalisation, 5) close enough is good enough. Descriptive summative statistics were used to analyse and present the frequency of reported equity domains. Results Within the included triage tools, four out of five domains of equity were evident in the included documents to guide decision making. Allocation based on perceived patient need and overall health outcomes were the central guiding principles across both inpatient and outpatient settings. Equal provision of service relative to patient need and reducing wait times were also prioritised. However, explicit inclusion of certain equity domains such as discrimination, ensuring equal capability to be healthy and other patient factors was limited. Conclusions Physiotherapy triage policies consider various domains of equity to guide resource allocation decisions. Policymakers and service providers can use the insights gained from this study to review the application and operationalisation of equity principles within their healthcare systems through mechanisms such as patient triage tools.
The effectiveness of preoperative oral carbohydrate loading on postoperative nausea and vomiting in adults receiving total intravenous anaesthesia compared to inhalational anaesthesia: A systematic review and meta-analysis
Preoperative oral carbohydrate loading is thought to reduce postoperative nausea and vomiting (PONV). However, it is unknown if the benefit of carbohydrate loading is maintained in the presence of total intravenous anaesthesia (TIVA). The aim of this systematic review was to determine whether oral carbohydrate loading reduced PONV compared to overnight fasting between adult elective surgery patients receiving TIVA or inhalational general anaesthesia. A search of seven databases was conducted until March 2024. Randomised controlled trials conducted with patient aged 18 years or older were included. Two reviewers independently screened titles, abstracts and full texts, and assessed risk of bias using the Cochrane ROB-2 Tool. Study data was pooled using random effects meta-analyses. We included 26 studies in this review, and 25 in the meta-analyses (n = 2491). Preoperative oral carbohydrate loading reduced the overall risk (log RR: –0.35, 95 % CI: −0.62 to −0.08, I2 = 51.11 %) and severity (SMD: -0.46, 95 % CI: −0.68 to −0.24, I2 = 62.09 %) of PONV, and pain severity (MD: -0.69, 95 % CI: −1.13 to −0.25, I2 = 83.98 %) compared to prolonged fasting when pooled across both anaesthesia approaches. The risk of PONV was reduced in patients receiving inhalational anaesthesia, but not in those receiving TIVA, while the reduction in severity was more significant in TIVA. Oral carbohydrate loading reduces the severity of PONV and pain, regardless of the anaesthesia approach, compared to prolonged fasting. These findings support the clinical advantages of oral carbohydrate loading for postoperative outcomes, regardless of anaesthesia approach. •Prolonged fasting is still a prevalent issue, even in the allowance of liberal fluid intake.•Meta-analysis of 25 RCTs (n = 2491) supports clinical benefits of OCL, comparing TIVA with IA.•PONV risk is reduced with IA, and PONV severity is reduced in either type of anaesthesia approach, more significantly in TIVA.•Findings reinforce the clinical benefits of preoperative oral carbohydrate loading for PONV regardless of anaesthesia approach.

Dietary Protein and Amino Acid Intake: Links to the Maintenance of Cognitive Health
With the rapid growth in the aging population, there has been a subsequent increase in the rates of Alzheimer’s disease and related dementias (ADRD). To combat these increases in ADRD, scientists and clinicians have begun to place an increased emphasis on preventative methods to ameliorate disease rates, with a primary focus area on dietary intake. Protein/amino acid intake is a burgeoning area of research as it relates to the prevention of ADRD, and consumption is directly related to a number of disease-related risk factors as such low-muscle mass, sleep, stress, depression, and anxiety. As a result, the role that protein/amino acid intake plays in affecting modifiable risk factors for cognitive decline has provided a robust area for scientific exploration; however, this research is still speculative and specific mechanisms have to be proven. The purpose of this review is to describe the current understanding of protein and amino acids and the preventative roles they play with regard to ADRD, while providing future recommendations for this body of research. Additionally, we will discuss the current recommendations for protein intake and how much protein older adults should consume in order to properly manage their long-term risk for cognitive decline.
Homeostatic circuits selectively gate food cue responses in insular cortex
Physiological needs bias perception and attention to relevant sensory cues. This process is ‘hijacked’ by drug addiction, causing cue-induced cravings and relapse. Similarly, its dysregulation contributes to failed diets, obesity, and eating disorders. Neuroimaging studies in humans have implicated insular cortex in these phenomena. However, it remains unclear how ‘cognitive’ cortical representations of motivationally relevant cues are biased by subcortical circuits that drive specific motivational states. Here we develop a microprism-based cellular imaging approach to monitor visual cue responses in the insular cortex of behaving mice across hunger states. Insular cortex neurons demonstrate food-cue-biased responses that are abolished during satiety. Unexpectedly, while multiple satiety-related visceral signals converge in insular cortex, chemogenetic activation of hypothalamic ‘hunger neurons’ (expressing agouti-related peptide (AgRP)) bypasses these signals to restore hunger-like response patterns in insular cortex. Circuit mapping and pathway-specific manipulations uncover a pathway from AgRP neurons to insular cortex via the paraventricular thalamus and basolateral amygdala. These results reveal a neural basis for state-specific biased processing of motivationally relevant cues. A combination of microprism-based cellular imaging to monitor insular cortex visual cue responses in behaving mice across hunger states with circuit mapping and manipulations reveals a neural basis for state-specific biased processing of motivationally relevant cues. Hunger-induced sensory bias Physiological states can directly bias an animal's attention towards those cues in the environment that are most relevant to its immediate needs. However, our understanding of how the brain drives behaviours directed towards meeting those motivated needs is limited. Here Mark Andermann and colleagues reveal that, during a behavioural task, mice insular cortical neurons were strongly biased towards visual cues representing food when the animals were hungry, but these biases were not apparent following satiation. Insular neural responses to the cues were modulated by the activity levels of hypothalamic AgRP neurons, which drive feeding behaviour. Artificial activation of these neurons over-rode satiety signals and reinstated biased responses by insular neurons to food-related task cues. Neurons driving and responding to specific motivated states can influence and bias cognitive processing of salient cues.
Isolation of adipose tissue derived regenerative cells from human subcutaneous tissue with or without the use of an enzymatic reagent
Freshly isolated, uncultured, autologous adipose derived regenerative cells (ADRCs) have emerged as a promising tool for regenerative cell therapy. The Transpose RT system (InGeneron, Inc., Houston, TX, USA) is a system for isolating ADRCs from adipose tissue, commercially available in Europe as a CE-marked medical device and under clinical evaluation in the United States. This system makes use of the proprietary, enzymatic Matrase Reagent for isolating cells. The present study addressed the question whether the use of Matrase Reagent influences cell yield, cell viability, live cell yield, biological characteristics, physiological functions or structural properties of the ADRCs in final cell suspension. Identical samples of subcutaneous adipose tissue from 12 subjects undergoing elective lipoplasty were processed either with or without the use of Matrase Reagent. Then, characteristics of the ADRCs in the respective final cell suspensions were evaluated. Compared to non-enzymatic isolation, enzymatic isolation resulted in approximately twelve times higher mean cell yield (i.e., numbers of viable cells/ml lipoaspirate) and approximately 16 times more colony forming units. Despite these differences, cells isolated from lipoaspirate both with and without the use of Matrase Reagent were independently able to differentiate into cells of all three germ layers. This indicates that biological characteristics, physiological functions or structural properties relevant for the intended use were not altered or induced using Matrase Reagent. A comprehensive literature review demonstrated that isolation of ADRCs from lipoaspirate using the Transpose RT system and the Matrase Reagent results in the highest viable cell yield among published data regarding isolation of ADRCs from lipoaspirate.
Skyrmions in Twisted Bilayer Graphene: Stability, Pairing, and Crystallization
We study the excitations that emerge upon doping the translationally invariant correlated insulating states in magic-angle twisted bilayer graphene at various integer filling factorsν. We identify parameter regimes where these are excitations associated with skyrmion textures in the spin or pseudospin degrees of freedom, and explore both short-distance pairing effects and the formation of long-range ordered skyrmion crystals. We perform a comprehensive analysis of the pseudospin skyrmions that emerge upon doping insulators at evenν, delineating the regime in parameter space where these are the lowest-energy charged excitations by means of self-consistent Hartree-Fock calculations on the interacting Bistritzer-MacDonald model. We explicitly demonstrate the purely electron-mediated pairing of skyrmions, a key ingredient behind a recent proposal of skyrmion superconductivity. Building upon this, we construct hopping models to extract the effective masses of paired skyrmions, and discuss our findings and their implications for skyrmion superconductivity in relation to experiments, focusing on the dome-shaped dependence of the transition temperature on the twist angle. We also investigate the properties of spin skyrmions about the quantized anomalous Hall insulator atν=+3. In both cases, we demonstrate the formation of robust spin or pseudospin skyrmion crystals upon doping to a finite density away from integer filling.
RiskScape: a flexible multi-hazard risk modelling engine
This paper presents the architecture and features of RiskScape software. RiskScape is an open-source software with a flexible modelling engine for multi-hazard risk analysis. The RiskScape engine implements modeller-defined risk quantification workflows as ‘model pipelines’. Model pipeline steps and functions analyse hazard, exposure, and vulnerability data across different spatio-temporal domains using geoprocessing and spatial sampling operations. The RiskScape engine supports deterministic and probabilistic risk quantification, with several probabilistic-based modes described in this paper. RiskScape advances modelling software for multi-hazard risk analysis through several implementation features. The RiskScape engine operates model pipelines independent of system prescribed model input data classifications or standards. Multiple hazard types, metric intensities, and temporal occurrence information is geometry processed and sampled to create coverage data of simultaneous or sequenced multi-hazard events at object-exposure locations. Escalating multi-hazard event impacts are then determined for object-exposures using scripted conditional or nested statements that apply vulnerability functions in a logical sequence of temporal hazard and impact occurrence. These model features, supported by open geospatial consortium standard geospatial data files and operations, expedite RiskScape for modelling multi-hazard risk at any geographical location or scale.
