Background:
Mucormycosis (black fungus) is an aggressive, life-threatening infectious disease-causing infiltration and by destroying the surrounding bone and soft tissue through vascular thrombosis and subsequent tissue infarction that may reach the brain with fatal complications. Its outbreak has been assessed around the clock during the recent pandemic as post COVID-19 sequelae.
Aims:
To assess the risk factors, oral signs and symptoms, investigations, treatment and rehabilitation strategies amongst COVID-19 associated mucormycosis patients in a tertiary care hospital.
Methods and Material:
An observational, pilot study was carried out amongst 30 active or recent COVID-19 associated mucormycosis patients reporting in the Department of Dentistry of a tertiary care hospital in Bhopal over a period of 6 months. Statistical analysis was done using Kolmogorov-Smirnov test using the Statistical Package for Social Sciences, Version 16 software.
Results:
Study was done on 30 patients (80% males and 20% females) and their median age was 46 years. Predominant risk factor present was diabetes mellitus (90%) and steroid therapy (60%). Prime oral manifestations were toothache (90%), mobile teeth (83%), draining sinus (53%), palatal ulceration (43%) and para-sinusal pain (37%).
Conclusions:
Mucormycosis can cause serious oro-facial morbidity and mortality in COVID-19 patients. The current management strategy requires early diagnosis, prompt treatment and oral rehabilitation.

Exploring the Domain of Interpretive Structural Modelling (ISM) for Sustainable Future Panorama: A Bibliometric and Content Analysis

Interpretive Structural Modelling (ISM) is one of the most widely used techniques in identifying the complex structural relationship between various elements. It is commonly used in multiple disciplines but hardly explored its all-encompassing scientific productivities. Hence, this paper has endeavoured to scrutinize 1480 documents using the ISM technique from 2000 to 2020 in the Scopus database. A systematic two-tier approach comprising bibliometric analysis and visualization review has been made with VOSviewer and Biblioshiny software. Extensive data mining has been done to collect required information with certain filters containing document type, language, author, subject, publication status, source title, affiliation, country, and source type. The study has generated information regarding ISM documents, their types, publications, citations, and predictions. The citation analysis is used to ascertain the most prolific and dominant authors, sources, articles, countries, and organizations. The author-keywords, index-keywords, and text data content analysis is conducted to find ISM's hotspots and progress trends. The study has found a rapid exponential pace in annual publications using the ISM technique since 2000. The most prolific and dominant articles based on total citation include Diabat and Govindan, 2011; the top source is the Journal of Cleaner Production. The chief author is Shankar R., the leading organization IIT New Delhi, India, and the leading country is India. The study has explored many research hotspots and less explored areas using ISM techniques. The present research is the first paper that has utilized bibliometric analysis to analyze the ISM publications widely. This bibliometric review has further contributed to the ISM technique, usability, and exploitation areas and future scope for scholars working in this area through its research hotspots and valuable findings.

Comparison of Recurrence Risk Prediction by the CanAssist Breast Test and the PREDICT Online Tool in Early Breast Cancer Patients

Introduction Several methods, such as multi-marker (both gene/protein) tests and online/free tools, are available for the prognostication of early breast cancer (EBC) patients. This article compares the risk assessment between the immunohistochemistry-based (IHC) CanAssist Breast (CAB) test and the online/free prognostic tool PREDICT in EBC patients treated at Rajiv Gandhi Cancer Institute and Research Centre (RGCIRC) between May 2017 and June 2022. Methodology The current study cohort comprises 130 patients. Risk proportions were assessed by CAB and the online/free tool PREDICT. Concordance between the risk groups of CAB and PREDICT was assessed by the kappa coefficient, which was used to evaluate the significance. Results The low-risk (LR) and high-risk (HR) proportions for CAB and PREDICT were 61:39 and 46:54 percent, respectively. CAB stratified a significantly (
=0.027) higher number of patients as LR compared to PREDICT. Interestingly, in the subgroup analysis of the age and clinicopathological parameters CAB stratified 28% of patients with grade 3 tumors as LR, whereas PREDICT stratified all grade 3 tumors as HR. The overall treatment compliance using CAB was 89%. Conclusion CAB, a relatively new multi-marker prognostic test entailing five relevant protein biomarkers provides augmented and relevant prognostic information over PREDICT in all patients. Thus, CAB helps optimize treatment for HR+/HER2- EBC patients as its risk stratification is independent of age and clinical parameters and assigns recurrence risk based on tumor biology.

VLSI implementations of retimed high speed adaptive filter structures for speech enhancement

This paper presents FPGA implementation of retimed high speed adaptive filter structures for speech enhancement. In this work, various high speed adaptive filtering structures for noise cancellation are implemented for Xilinx Spartan-6 series and Virtex-4 series FPGA platforms. It has been observed that various VLSI implementations vary considerably in clock speed, hardware requirements, latency and cost. For instance, for the Spartan-6 series FPGA platform implementation, the clock speed of retimed DF-RDLMS implementation is found to be 98.309 MHz whereas that of conventional unretimed DF-LMS structure is 85.485 MHz, thereby having an improvement of 15% in clock speed. Similarly, for the Virtex-4 series FPGA platform implementation, the clock speed of retimed DF-RDLMS implementation is found to be 88.176 MHz whereas that of conventional unretimed DF-LMS structure is 75.855 MHz, thereby having an improvement of 16.5% in clock speed. The VLSI implementation and the performance analysis provides crucial information about an algorithm structure such as hardware requirements, power consumption and real-time performance. Performance of the implemented structures have been checked in terms of operating frequency, maximum combinational path delay, latency, and power consumption.

Green Human Resources Management (GHRM) is an alignment of traditional human resource practices such as strategies, policies, procedures, and rules to the latest green and sustainable environment responsive practices. Unlike corporates, the adoption of GHRM in educational institutes (EIs) is still in its infancy stage. Through an extensive literature survey, this study identified eleven challenges in adopting the GHRM “strategy”, ten on “policy”, nine on the “procedures”, and eight on framing the GHRM “rules” aspect. The aim is to identify and develop the relationship between major and minor challenges in adopting GHRM practices in EIs. Hence, this study has applied the Interpretive Structural Modelling (ISM) technique to each GHRM practice and developed interrelation among such challenges and results verified with Analytic Hierarchy Process (AHP). The rankings of both techniques have been statistically verified with the Spearman Rank Correlation technique. The study concludes the lacking or insufficient considered four main pillars: clear vision and top management cooperation among these EIs to ensure sustainable GHRM practices from the strategy aspect perspective, benchmark in fixing accountability considering the policy aspect, suitable course curricula in universities focused on GHRM practices from procedural viewpoint, and transparency in EIs from the perspective of rules as significant challenges in GHRM adoption. The findings of the reported results can be further extended in cross-sectional and cross-cultural studies in further studies.

Genetic disorders are showing an upward trend. The social and economic impact of genetic disorders on individual, family and society is enormous. There is an urgent need to explore alternate strategies to mitigate the burden of genetic disorders. This is especially true with regard to developing countries such as India where there is a shortage of health personnel adequately trained in genetic counseling at present. Dental professionals have a unique opportunity to observe the development of preadolescent and adolescent patients during periods when important growth and development changes occur. The objective of this study was to review the existing literature on the role of dentist in genetic counseling with a critical appraisal on the current practices among dentists on genetic services in India, the need, scope, and future requirements. The literature on genetic services and genetic counseling was identified by searching the biomedical databases for primary research material by one investigator over a period of 8 weeks. The articles related to dentist's role in genetic counseling were assessed and discussed in the present review. A total of 239 resource materials were retrieved in the initial search. The literature from these sources was thoroughly scrutinized by the authors, and the literature (review articles, descriptive studies, or any form of study) focusing on role of dentist in genetic counseling was finally considered for critical appraisal in the present review. The role of genetics in health and oral health care has not received due attention of the dental practitioners who otherwise are in a crucial position in identifying the patients with genetic disorders and offer requisite counseling and referral to designated genetic centers. The short training courses for practicing dentists, faculty members, and a small change in dental curriculum to make provision for teaching genetics to undergraduate students may go a long way in filling the void created by these obstacles.

In engineering applications, such as automobile, marine, aerospace, and railway, lightweight alloys of aluminum (Al) and magnesium (Mg) ensure design fitness for fuel economy, better efficiency, and overall cost reduction. Friction stir welding (FSW) for joining dissimilar materials has been considered better than the conventional fusion welding process because of metallurgical concerns. In this study, dissimilar joints were made between the AA6061 (A), AZ31B (B), and AZ91D (C) combinations based on the varying advancing side (AS) and retreating side (RS). The dissimilar joints prepared by the FSW process were further characterized by tensile testing, impact testing, corrosion testing, fracture, and statistical and cost analysis. The results revealed a maximum tensile strength of 192.39 MPa in AZ91 and AZ31B, maximum yield strength of 134.38 MPa in a combination of AA6061 and AZ91, maximum hardness of 114 Hv in AA6061 and AZ31B, and lowest corrosion rate of 7.03 mV/A in AA6061 and AZ31B. The results of the properties were supported by photomicrographic fracture analysis by scanning electron microscopy (SEM) observations. Further, the performance of dissimilar joints was statistically analyzed and prioritized for preference by similarity to the ideal solution (TOPSIS) method.

Electronic equipment used on a daily basis now frequently includes active noise cancellation. The adaptive filters, which are positioned within, are essential for noise cancellation. An essential component to take into account for the overall performance is the structural and computational complexity of the filter. The filter’s structure has an impact on this. The amount of taps determines the structure. Active noise cancellation filters often have set tap lengths and are lengthy, which causes sluggish convergence and delay. As a result, a trade-off between the filter’s length and convergence is required. This is conceivable if there is a flexible filter with a tap length that adapts to the environment while still ensuring acceptable convergence. This study proposes a novel Minimum Mean M-estimate method with changeable tap length and uses a sub-band adaptive filtering technique to shorten the filter’s length. In order to maximize the filter’s efficiency, the advantages of three approaches are specifically merged in this work. They are the proposed algorithm, the proposed method’s variable tap length variant, and the sub-band adaptive filtering. The simulation’s findings and recommendations are supported.

The present cross-sectional study following the STROBE guidelines was conducted to assess the prevalence of oral mucosal lesions among males, females, and eunuchs residing in Bhopal city, Madhya Pradesh India.
Based on convenient non-probability snowball sampling technique, all the self-identified eunuchs residing in the city of Bhopal who were present at the time of examination and who fulfilled the selection criteria were examined. A cross section of the general population (males and females) residing in the same locality where these eunuchs live was also examined. The World Health Organization (WHO) oral health assessment proforma (1997) was used to collect the information on oral mucosal lesions. All the obtained data were analyzed by using a Statistical Package for Social Sciences version 20.
Overall prevalence of oral mucosal lesions was 127 (19.9%) among the study subjects. Fifty-nine (28.5%) eunuchs, 56 (25.7%) males, and 12 (5.6%) females were observed to have some oral mucosal lesions. Oral submucous fibrosis (6.4%), leukoplakia (5.5%), and traumatic ulceration (4.2%) were the major oral mucosal conditions observed.
The information presented in this study adds to our understanding of the common oral mucosal lesions occurring in the eunuch population. Efforts to increase patient awareness of the oral effects of tobacco use and to eliminate the habit are needed to improve the oral and general health of eunuchs.
