The incorporation of drugs into nanostructured silica has proven to be an effective strategy for delaying drug release, protecting against enzymatic degradation, and enhancing therapeutic efficacy. Specifically, crotoxin, a component derived from the venom of Crotalus durissus terrificus, exhibits notable analgesic and immunomodulatory properties. Previous studies have demonstrated that encapsulating crotoxin within SBA-15 nanostructured mesoporous silica not only reduces its toxicity and enhances its analgesic effects but also enables effective oral administration. Given its promising efficacy and the expanding interest in its application across various experimental models and potential therapeutic uses, this study aimed to conduct a detailed analysis of the physicochemical properties of crotoxin when incorporated into SBA-15 silica. Following characterization, the crotoxin–SBA-15 complex was orally administered to mice in an experimental model of ulcerative colitis (UC). The most widely adopted experimental model for studying UC involves the administration of dextran sodium sulfate (DSS) in drinking water to induce colonic inflammation in susceptible animals. In this study, we hypothesized that crotoxin incorporated into ordered mesoporous silica (SBA-15) could modulate DSS-induced UC. Crotoxin was successfully incorporated into SBA-15 and administered orally, as its physicochemical properties supported this route of delivery. Mice received the crotoxin–SBA-15 complex either at the onset of UC induction or on days 1 and 4 after DSS exposure. Seven days after the start of DSS administration, we observed a substantial reduction (approximately 50%) in Disease Activity Index (DAI) scores, accompanied by marked improvements in the histopathological features of the colon. These findings indicate for the first time that crotoxin incorporated into SBA-15 exhibits significant therapeutic potential in the treatment of experimentally induced ulcerative colitis.

Health teams in primary care play a key role in the eye health of users as they understand that early identification of any visual change can result in satisfactory outcomes and better prognoses, preventing damage that is often irreversible to health. Building an ophthalmological calendar for the therapeutic follow-up of glaucoma in the elderly, this is a methodological study, as the process of constructing the calendar’s content followed the Raymundo theoretical framework. The calendar was built in the following steps: bibliographic survey, content development, transformation of the language of scientific information into easy-to-understand expressions, creation and production of illustrations of the first draft, evaluation of the first draft made by the examining board, diagramming and presentation of the product. The construction of the calendar covers a specific theme for the elderly with glaucoma, which emphasizes the need to invest more in the inclusion of new technologies that will provide greater effectiveness and adherence of the user and the health team for the management of comprehensive care. The implementation of the produced calendar will allow for a better understanding and bond between the team professionals and the user and, consequently, a better monitoring of the therapeutic process of the patient involved.

Atherosclerotic vascular changes can begin during childhood, providing risk for cardiovascular disease (CVD) in adulthood. Identifiable risk factors such as dyslipidemia accelerate this process for some children. The apolipoprotein B (
APOB
) gene could help explain the inter-individual variability in lipid levels among young individuals and identify groups that require greater attention to prevent CVD. A cross-sectional study was conducted with school-aged children and adolescents in Ouro Preto, Minas Gerais. The study evaluated cardiovascular risk factors’ variables and
XbaI
polymorphism in the
APOB
gene for associations with increased total cholesterol (TC). The prevalence of increased TC was notably high, reaching 68.9% in the study population. Carriers of the variant T allele were 1.45 times more likely to develop increased TC in a dominant model (1.09–1.94,
p
= 0.011). After adjustments, excess weight and a family history of dyslipidemia interacted significantly with
XbaI
polymorphism in increased TC, resulting in Odds Ratio of 1.74 (1.11–2.71,
p
= 0.015) and 2.04 (1.14–3.67,
p
= 0.016), respectively. The results suggest that
XbaI
polymorphism in the
APOB
gene may affect the lipid profile of Brazilian children and adolescents and could contribute to the CVD in adulthood.

Introduction: Glaucoma is a public health problem among chronic-degenerative visual diseases due to its high incidence and for causing disability. Thus, the Glaucoma Patient Assistance Program was created to improve follow-up and avoid bleak prognoses. Objectives: to analyze the difficulties of the elderly in the use of eye drops for the treatment of glaucoma. Methods: an exploratory research was carried out where the population was composed of elderly patients with glaucoma treated at the Integrated Unit of Ipiranga linked to Basic Care in District III of João Pessoa-PB, a questionnaire was the instrument for data collection for the 61 participants. Results: it was found that 38 (62.3%) %) of the participants , 53 (87%) received 1-2 minimum wages, and 23 (37.7%) studied only until primary school. As for administration, 37 (60.7%) had no problems remembering if they had already used the eye drops, 39 (63.9%) had no difficulty using it, and 34 (55.7%) were unable to follow its use in the basic assistance. Conclusion: respondents have their obstacles to medication adherence regardless of the disease, the lack of follow-up in basic care only adds another one, so that’s why educating them would enable lower dropout rates and better prognoses.

The care offer to people living with HIV/AIDS must transcend specialized outpatient services and include the participation of the Family Health Strategy. By understanding the importance of integration between these two points in the care network, the study aimed to build a decision support model to assist professionals of specialized health services in identifying behavior patterns in the use of Family Health Strategy services by people living with HIV/AIDS attended in the outpatient clinic. Thus, was proposed a model called decision tree, created from a database of 141 people with AIDS, users of a specialized outpatient clinic. The decision-making variable was the use of Family Health Strategy services by evaluating the integration of care. The model enabled the establishment of 23 rules with 80.1% hit percentage, what may support the decision-making of professionals in identifying situations in which it is necessary to stimulate the use of the Family Health Strategy by users.

Anal imperforation in a Congo village: a challenge for treatment
2021
Most anorectal malformations are identified in the examination of the newborn or are manifested in the first 24 hours by abdominal distension, absence of dejections and bilious vomiting.1 The type of surgery is variable, but immediate evaluation is urgent to avoid serious complications such as intestinal perforation and sepsis.2 3 We report a case of a male newborn, transported by plane on the third day of life from a rural area to a level II hospital in Kinshasa, due to anal imperforation noted at birth, with bilious vomiting in the first 24 hours of life. On examination, he showed signs of moderate dehydration, abdominal distention and absence of anal orifice (figure 1). Meconium emission through the urinary meatus was observed, with no visible fistulas. A nasogastric tube was placed immediately to decompress the abdomen and prevent intestinal perforation. The analytical study was normal and abdominal ultrasound revealed anorectal malformation with a suspected urinary fistula. He was submitted to surgery with terminal colostomy and started antibiotherapy with cefotaxime. He had a fever in the first day after surgery, so metronidazole and amikacin were added. He started breastfeeding with good tolerance, with progressive reduction in fluid therapy. He was discharged after 7 days with reconstructive surgery already scheduled.
