Surgical management of patient with left ventricular aneurysm and ventricular tachycardia

The management of patients with dilated cardiomyopathy and large anterior ventricular aneurysm presenting with ventricular tachycardia is not well-described. We report the case of 45-year-old male who presented with recurrent episodes of prolonged polymorphic ventricular tachycardia and previously failed medical management and endocardial and epicardial transcatheter ablation. We performed a Dor procedure to exclude the left ventricular aneurysm in conjunction with cryoablation to terminate his ventricular tachycardia. This surgical approach was found to be successful with conversion of the patient into normal sinus rhythm and restoration of the patient’s left ventricular morphology and function. We also propose a methodology for the surgical management of patients with left ventricular aneurysm and intractable ventricular tachycardia focused on a discussion with the patient and the cardiac team about the options for treatment, including surgery or continuing pharmacological and electrical cardioversion therapy, choosing the surgical technique that would exclude the most diseased and akinetic myocardial segment, and being more liberal with the use of cryoablation.

The Evolution of Minimally Invasive Cardiac Surgery Literature: A 50‐Year North American Bibliometric Review and Future Trends

Objective This study provides the first bibliometric analysis of minimally invasive cardiac surgery (MICS) in North America, mapping its evolution, collaboration networks, and research trends while forecasting future scientific trajectories through 2050. Methods We performed a bibliometric analysis of North American MICS literature (1975–2025) using Scopus and PubMed. Data processed using R (bibliometrix and biblioshiny) enabled network mapping, keyword analysis, and predictive modeling. Metrics included publication growth, document types, authorship, collaboration, citations, h‐index, journals, and research topics. Collaboration networks and thematic evolution were visualized. Future MICS output (2030–2050) was forecasted using linear extrapolation with 95% confidence intervals. Results We analyzed 1433 publications (United States: 76.3%; Canada: 23.7%). Original articles comprised 48.7%, and case reports comprised 28.7%, with an 8.51% annual growth rate. Among 4800 authors, 89.9% were male, averaging 5.65 coauthors per paper and 22.2% international collaborations. First authors were mainly faculty–surgeons (67.5%) and fellows (15.3%). Top topics are mitral valve (41.7%) and CABG (28.7%). Leading journals are Innovations (16.0%), Annals of Thoracic Surgery (12.6%), Journal of Cardiac Surgery (5.7%), and JTCVS (5.0%). Key affiliations are Cleveland Clinic (13.8%) and Western University (12.5%). Collaborations were strongest with Italy (43.5%) and Germany (32.8%). Forecasting models predicted an annual output of 79 publications by 2030, 93 by 2040, and 107 by 2050 (95% CI 58–146). Conclusions This inaugural bibliometric analysis of North American MICS outlines five decades of growth, projects continued expansion to 2050, and offers a strategic foundation for future research, funding, and collaboration.

Radix Ilicis Pubescentis total flavonoids combined with mobilization of bone marrow stem cells to protect against cerebral ischemia/reperfusion injury

Previous studies have shown that Radix Ilicis Pubescentis total flavonoids have a neuroprotective effect, but it remains unclear whether Radix Ilicis Pubescentis total flavonoids have a synergistic effect with the recombinant human granulocyte colony stimulating factor-mobilized bone marrow stem cell transplantation on cerebral ischemia/reperfusion injury. Rat ischemia models were administered 0.3, 0.15 and 0.075 g/kg Radix Ilicis Pubescentis total flavonoids from 3 days before modeling to 2 days after injury. Results showed that Radix Ilicis Pubescentis total flavonoids could reduce pathological injury in rats with cerebral ischemia/reperfusion injury. The number of Nissl bodies increased, Bax protein expression decreased, Bcl-2 protein expression increased and the number of CD34-positive cells increased. Therefore, Radix Ilicis Pubescentis total flavonoids can improve the bone marrow stem cell mobilization effect, enhance the anti-apoptotic ability of nerve cells, and have a neuroprotective effect on cerebral ischemia/reperfusion injury in rats.

Radix Ilicis Pubescentis total flavonoids ameliorates neuronal damage and reduces lesion extent in a mouse model of transient ischemic attack

Flavonoids are a major component in the traditional Chinese medicine Radix Ilicis Pubescentis.Previous studies have shown that the administration of Radix Ilicis Pubescentis total flavonoids is protective in cerebral ischemia.However,to our knowledge,no studies have examined whether the total flavonoids extracted from Radix Ilicis Pubescentis prevent or ameliorate neuronal damage following transient ischemic attacks.Therefore,Radix Ilicis Pubescentis total flavonoids question and the potential underlying mechanisms.Thus,beginning 3 days before the induction of a mouse model of transient ischemic attack using tert-butyl hydroperoxide injections,mice were intragastrically administered 0.3,0.15,or 0.075 g/kg of Radix Ilicis Pubescentis total flavonoids daily for 10 days.The results of spectrophotometric analyses demonstrated that Radix Ilicis Pubescentis total flavonoids enhanced oxygen free radical scavenging and reduced pathological alterations in the brain.Hematoxylin-eosin staining results showed that Radix Ilicis Pubescentis total flavonoids reduced hippocampal neuronal damage and cerebral vascular injury in this mouse model of transient ischemic attack.These results suggest that the antioxidant effects of Radix Ilicis Pubescentis total flavonoids alleviate the damage to brain tissue caused by transient ischemic attack.

Least-squares Galerkin procedure for second-order hyperbolic equations

This paper proposes the least-squares Galerkin finite element scheme to solve second-order hyperbolic equations. The convergence analysis shows that the method yields the approximate solutions with optimal accuracy in (L2(Ω))2 × L2(Ω) norms. Moreover, the method gets the approximate solutions with second-order accuracy in time increment. A numerical example testifies the efficiency of the novel scheme.

Construction of a human cell landscape at single-cell level

Single-cell analysis is a valuable tool for dissecting cellular heterogeneity in complex systems

. However, a comprehensive single-cell atlas has not been achieved for humans. Here we use single-cell mRNA sequencing to determine the cell-type composition of all major human organs and construct a scheme for the human cell landscape (HCL). We have uncovered a single-cell hierarchy for many tissues that have not been well characterized. We established a ‘single-cell HCL analysis’ pipeline that helps to define human cell identity. Finally, we performed a single-cell comparative analysis of landscapes from human and mouse to identify conserved genetic networks. We found that stem and progenitor cells exhibit strong transcriptomic stochasticity, whereas differentiated cells are more distinct. Our results provide a useful resource for the study of human biology.
Single-cell RNA sequencing is used to generate a dataset covering all major human organs in both adult and fetal stages, enabling comparison with similar datasets for mouse tissues.

Soil-rock mixtures (S-RM) are widely distributed in mountainous regions, and there are different genetic types and scales of S-RM slopes. The stability of these slopes is very important for slope safety during the construction and operation of engineering projects. Since the mechanical properties of an S-RM are mainly dominated by its random component, accurately investigating the structure of an S-RM slope is the fundamental basis for evaluating the mechanism of a landslide. In this paper, in situ direct shear tests are used to obtain the “soil” strength parameters of the S-RM, the microtremor survey method (MSM) is introduced to detect the S-RM slope and the numerical method of FEM is utilised to simulate the failure characteristics and stability of the S-RM slope. Based on the MSM, the strata structure and proportions of solitary rock blocks in the S-RM slope are investigated, and three possible models of the S-RM slope are established. By using the strength reduction method in numerical simulations, the potential failure surface and stability of a slope of homogeneous soil and that with solitary rock blocks are discussed. The study results show that the MSM is an effective method to estimate the proportion of rock blocks in the S-RM slope. The presence of solitary rock blocks makes the slope potential slide surface complicated. The factor of safety of the S-RM slope is higher than that of the homogeneous soil slope, the rock blocks at the toe of the slope are favourable for slope stability and the rock blocks near the surface of the slope squeeze the lower soil and improve the safety factor of the slope.
