Associations between life’s essential 8 and non-alcoholic fatty liver disease among US adults
Background Nonalcoholic fatty liver disease (NAFLD) is closely associated with Cardiovascular disease (CVD). We aim to examine the association of Life’s Essential 8 (LE8), the recently updated measurement of cardiovascular health (CVH), with the presence of NAFLD among US adults. Methods This population-based cross-sectional study used data from the National Health and Nutrition Examination Survey in 2017–2018 and included adults 20 years or older. LE8 score (range 0–100) was measured according to American Heart Association definitions and was categorized into low (0–49), moderate (50–79), and high (80–100) CVH. NAFLD was determined by transient elastography measured hepatic steatosis in the absence of other liver diseases and excess alcohol use. Multivariable logistic and restricted cubic spline models were used to assess the associations. Results Among 3588 participants included (weighted mean age, 48.0 years; 95% confidence interval [CI] 46.4–49.7 years), 1839 were female (weighted percentage, 51.6%; 95% CI 49.0–54.2%) and 1483 were determined to have NAFLD (weighted percentage, 36.5%; 95% CI 33.3–39.7%). The weighted mean LE8 score of the study population was 67.9 (95% CI 66.6–69.2). After the adjustment of potential confounders, higher LE8 scores were associated with reduced odds of NAFLD (odds ratio [OR] for per 10 score increase, 0.67; 95% CI 0.59–0.76) and a nonlinear dose–response relationship was observed. Similar patterns were also identified in the association of health behavior and health factor scores with NAFLD. The inversed association of LE8 score and NAFLD was significantly stronger among younger, Asian, and participants with higher education and income level. Conclusions LE8 and its subscales scores were negatively associated with the presence of NAFLD in non-linear fashions. Promoting adherence to optimal CVH levels may be beneficial to reduce the burden of NAFLD as well as CVD.
Association between 5-year change in cardiovascular risk and the incidence of atherosclerotic cardiovascular diseases: a multi-cohort study
Background The influence of the historical cardiovascular risk status on future risk of atherosclerotic cardiovascular disease (ASCVD) is poorly understood. We aimed to investigate the association between 5-year changes in cardiovascular risk and ASCVD incidence. Methods We analyzed pooled data from seven community-based prospective cohort studies with up to 20 years of follow-up data. The study populations included White or Black participants aged 40–75 years without prevalent ASCVD. Cardiovascular risk was assessed using the pooled cohort equation and was categorized into non-high (< 20%) or high risk (≥ 20%). Changes in cardiovascular disease (CVD) risk over a 5-year interval were recorded. The main outcome was incident ASCVD. Results Among 11,026 participants (mean [SD] age, 60.0 [8.1] years), 4272 (38.7%) were female and 3127 (28.4%) were Black. During a median follow-up period of 9.9 years, 2560 (23.2%) ASCVD events occurred. In comparison with individuals showing a consistently high CVD risk, participants whose CVD risk changed from non-high to high (hazard ratio [HR], 0.67; 95% confidence interval [CI] 0.59–0.77) or high to non-high (HR, 0.57; 95% CI 0.41–0.80) and those with a consistently non-high risk (HR, 0.33; 95% CI 0.29–0.37) had a lower risk of incident ASCVD. In comparison with individuals showing a consistently non-high CVD risk, participants whose CVD risk changed from high to non-high (HR, 1.74; 95% CI 1.26–2.41) or from non-high to high risk (HR, 2.04; 95% CI 1.84–2.27) and those with a consistently high risk (HR 3.03; 95% CI 2.69–3.42) also showed an increased risk of incident ASCVD. Conclusions Individuals with the same current CVD risk status but different historical CVD risks exhibited varying risks of future ASCVD incidents. Dynamic risk evaluation may enable more accurate cardiovascular risk stratification, and decision-making regarding preventive interventions should take the historical risk status into account. Engineering Vehicles Detection Based on Modified Faster R-CNN for Power Grid Surveillance
Engineering vehicles intrusion detection is a key problem for the security of power grid operation, which can warn of the regional invasion and prevent external damage from architectural construction. In this paper, we propose an intelligent surveillance method based on the framework of Faster R-CNN for locating and identifying the invading engineering vehicles. In our detection task, the type of the objects is varied and the monitoring scene is large and complex. In order to solve these challenging problems, we modify the network structure of the object detection model by adjusting the position of the ROI pooling layer. The convolutional layer is added to the feature classification part to improve the accuracy of the detection model. We verify that increasing the depth of the feature classification part is effective for detecting engineering vehicles in realistic transmission lines corridors. We also collect plenty of scene images taken from the monitor site and label the objects to create a fine-tuned dataset. We train the modified deep detection model based on the technology of transfer learning and conduct training and test on the newly labeled dataset. Experimental results show that the proposed intelligent surveillance method can detect engineering vehicles with high accuracy and a low false alarm rate, which can be used for the early warning of power grid surveillance.
Multiphase aqueous soggy sand electrolyte for zinc metal batteries applications at elevated temperatures
Aqueous zinc metal batteries often encounter uncontrollable failure or performance decay under thermal environments, primarily due to the severe side reactions caused by aqueous electrolytes. Here, an organic-inorganic-H 2 O multiphase aqueous soggy sand electrolyte is reported to enhance high-temperature performance of aqueous zinc metal batteries. Soggy sand components form strong interactions between the heterogeneous phases, reconstructing hydrogen-bond networks and forming water-deficient solvation structures, which greatly limits water activity and improves thermal stability, eliminating parasitic reactions and inspiring a uniform Zn deposition at elevated temperatures. As a result, multiphase aqueous soggy sand electrolyte enables aqueous zinc metal batteries to achieve an operating temperature of 140 °C, a lifespan of 1700 cycles, and a current density of 8 A g −1 . More importantly, the application of this electrolyte allows the stable cycling of Zn metal pouch cells under continuous operation at elevated temperatures. This work paves the path towards electrolyte design that enables aqueous batteries to cycle stably under thermal environments. Aqueous zinc metal batteries often face performance decay under thermal environments, originating from side reactions caused by aqueous electrolytes. Differential diagnosis of pancreatic serous cystadenoma and mucinous cystadenoma: utility of textural features in combination with morphological characteristics
Background Texture analysis of medical images has been reported to be a reliable method for differential diagnosis of neoplasms. This study was to investigate the performance of textural features and the combined performance of textural features and morphological characteristics in the differential diagnosis of pancreatic serous and mucinous cystadenomas. Methods We retrospectively reviewed 59 patients with pancreatic serous cystadenoma and 32 patients with pancreatic mucinous cystadenoma at our hospital. A three-dimensional region of interest (ROI) around the margin of the lesion was drawn manually in the CT images of each patient, and textural parameters were retrieved from the ROI. Textural features were extracted using the LifeX software. The least absolute shrinkage and selection operator (LASSO) method was applied to select the textural features. The differential diagnostic capabilities of morphological features, textural features, and their combination were evaluated using receiver operating characteristic (ROC) analysis, and the area under the receiver operating characteristic curve (AUC) was used as the main indicator. The diagnostic accuracy based on the AUC value is defined as follows: 0.9–1.0, excellent; 0.8–0.9, good; 0.7–0.8, moderate; 0.6–0.7, fair; 0.5–0.6, poor. Results In the differential diagnosis of pancreatic serous and mucinous cystadenomas, the combination of morphological characteristics and textural features (AUC 0.893, 95% CI 0.816–0.970) is better than morphological characteristics (AUC 0.783, 95% CI 0.665–0.900) or textural features (AUC 0.777, 95% CI 0.673–0.880) alone. Conclusions In conclusion, our preliminary results highlighted the potential of CT texture analysis in discriminating pancreatic serous cystadenoma from mucinous cystadenoma. Furthermore, the combination of morphological characteristics and textural features can significantly improve the diagnostic performance, which may provide a reliable method for selecting patients with surgical intervention indications in consideration of the different treatment principles of the two diseases.
Proteomic profiling identifies molecular subtypes and unveils mechanistic insights into clinical features of hypertrophic cardiomyopathy
Background Hypertrophic cardiomyopathy (HCM) is the most common inherited heart disease, shows significant genetic and clinical heterogeneity, yet its molecular basis remains unclear. This study aims to uncover molecular insights behind HCM based on proteomic analysis. Methods We performed proteomic analysis on formalin-fixed paraffin-embedded septal tissue samples from 105 patients with obstructive HCM undergoing myectomy, including 35 with MYBPC3 and 35 with MYH7 mutations. The primary outcome was major adverse cardiovascular events (MACE), comprising all-cause mortality and other major cardiovascular events. Results We identified four molecular subtypes: 39 patients with subtype-I (S-I), 38 with S-II, 27 with S-III, and 1 with S-IV. During a median follow-up of 6.8 years, 28.6% of patients developed MACE. Among the subtypes, S-III exhibited the minimal fibrosis and the best clinical outcomes. S-II presented the most severe phenotype and poorest prognosis, characterized by activation of inflammatory and fibrotic pathways, along with downregulation of multiple metabolic processes. Compared to S-II, S-I displayed opposing pathway patterns and better outcomes, with a predominance of MYBPC3 mutation carriers. Beisides, we identified protein modules that were associated with genotypes and clinical features. MYH7-related modules were positively correlated with fibrosis and MACE, primarily enriched in inflammatory and fibrotic pathways. In contrast, MYBPC3-related modules were linked to better clinical outcomes, enriched in energy metabolism pathways. Conclusions This proteomic study revealed molecular mechanisms linking genotype, fibrosis and prognosis in HCM, and identified potential drivers of high-risk subtypes with severe phenotypes. These findings may guide future risk stratification and therapeutic target development. Graphical Abstract
The phenotypic characteristic observed by cardiac magnetic resonance in a PLN-R14del family
Phospholamban (PLN) is an important regulator for sarcoendoplasmic reticulum (SR) calcium transport ATPase (SERCA), which uptakes Ca 2+ to SR during the diastolic phase of cardiomyocytes to maintain intracellular calcium homeostasis. Mutations on PLN result in intracellular calcium disorder, myocardial contraction defect, and eventually heart failure and/or malignant ventricular arrhythmia. Since 2003, several kinds of PLN mutations have been identified in familial dilated cardiomyopathy (DCM) patients, illustrating a few clinical characteristics that differs from classical DCM patients. Herein, we report a large PLN-R14del family with typical clinical characteristics reported including relatively late-onset clinical symptoms, low-voltage in ECG, as well as frequent ventricular arrythmias. Moreover, members underwent cardiac magnetic resonance (CMR) examination showed a strikingly similar pattern of late gadolinium enhancement (LGE)—Sub-epicardial involvement in the left ventricular (LV) lateral wall with or without linear mid-wall enhancement in the interventricular septum. The former one can also present in younger PLN-R14del carriers despite completely normal LV structure and function. Meanwhile, T1 mapping also found significantly increased extracellular volume (ECV) in PLN-R14del carriers. These findings highlight the special role of CMR to phenotyping PLN-induced cardiomyopathy patients and distinguish them from other types of cardiomyopathy.
Effects of highly dispersed Ni nanoparticles on the hydrogen storage performance of MgH2
MgH 2 with a large hydrogen capacity is regarded as a promising hydrogen storage material. However, it still suffers from high thermal stability and sluggish kinetics. In this paper, highly dispersed nano-Ni has been successfully prepared by using the polyol reduction method with an average size of 2.14 nm, which significantly improves the de/rehydrogenation properties of MgH 2 . The MgH 2 —10wt% nano-Ni sample starts releasing H 2 at 497 K, and roughly 6.2wt% H 2 has been liberated at 583 K. The rehydrogenation kinetics of the sample are also greatly improved, and the adsorption capacity reaches 5.3wt% H 2 in 1000 s at 482 K and under 3 MPa hydrogen pressure. Moreover, the activation energies of de/rehydrogenation of the MgH 2 —10wt% nano-Ni sample are reduced to (88 ± 2) and (87 ± 1) kJ·mol −1 , respectively. In addition, the thermal stability of the MgH 2 —10wt% nano-Ni system is reduced by 5.5 kJ per mol H 2 from that of pristine MgH 2 . This finding indicates that nano-Ni significantly improves both the thermodynamic and kinetic performances of the de/rehydrogenation of MgH 2 , serving as a bi-functional additive of both reagent and catalyst.
Pretreatment Inflammatory Indexes as Prognostic Predictors for Survival in Colorectal Cancer Patients Receiving Neoadjuvant Chemoradiotherapy
This study was to evaluate the prognostic value of pretreatment inflammatory indexes including neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), lymphocyte-to-monocyte ratio (LMR), and systemic immune-inflammation index (SII) in colorectal cancer (CRC) patients receiving neoadjuvant chemoradiotherapy (CRT). We enrolled 98 eligible CRC patients and divided them into high or low NLR, PLR, LMR, and SII groups according to their median index value, respectively. Univariate and multivariate analysis were performed to identify the potential predictors of progression-free survival (PFS) and overall survival (OS). In the univariate analysis, ECOG performance status, distant metastasis, NLR, PLR, LMR, and SII were found to be significantly associated with PFS and OS. In the multivariate analysis, ECOG performance status, distant metastasis, and NLR were identified to be independent predictors of PFS (HR 2.487, p = 0.012; HR 2.422, p = 0.042; HR 2.243, p = 0.034, respectively), and OS (HR 2.237, p = 0.018; HR 2.757, p = 0.020; HR 2.336, p = 0.017, respectively). The results of our study revealed that ECOG performance status, distant metastasis and NLR were independent prognostic factors of PFS and OS in CRC patients receiving neoadjuvant CRT.
Nucleus accumbens circuit disinhibits lateral hypothalamus glutamatergic neurons contributing to morphine withdrawal memory in male mice
The lateral hypothalamus (LH) is physiologically critical in brain functions. The LH also plays an important role in drug addiction. However, neural circuits underlying LH involvement of drug addiction remain obscure. In the present study,our results showed that in male mice, during context-induced expression of morphine withdrawal memory, LH glutamatergic neurons played an important role; dopamine D1 receptor-expressing medium spiny neurons (D1-MSNs) projecting from the core of nucleus accumbens (NAcC) to the LH were an important upstream circuit to activate LH glutamatergic neurons; D1-MSNs projecting from the NAcC to the LH activated LH glutamatergic neurons through inhibiting LH local gamma-aminobutyric acid (GABA) neurons. These results suggest that disinhibited LH glutamatergic neurons by neural circuits from the NAcC importantly contribute to context-induced the expression of morphine withdrawal memory. Lateral hypothalamus (LH) plays an role in drug addiction. Here, authors present evidence for disinhibited LH glutamatergic neurons by neural circuits from nucleus accumbens to contribute to context-induced expression of morphine withdrawal memory.
