Experiences of general practice of children with complex and palliative care needs and their families: a qualitative study
ObjectivesTo investigate the views and experiences of general practice of children with life-limiting and life-threatening conditions, and their family members, through secondary analysis of a qualitative serial interview study. Thematic analysis was conducted on all interview data relating to experiences of primary care.SettingWest Midlands, UK.ParticipantsA total of 31 participants (10 children with life-limiting and life-threatening conditions and 21 family members) from 14 families.Study design and settingSecondary thematic analysis of qualitative interview data from a study carried out in the West Midlands, UK.Method41 serial interviews with 31 participants from 14 families: 10 children aged 5–18 years with life-limiting and life-threatening conditions, and 21 of their family members.ResultsThree key themes emerged: (1) poor experiences of general practice cause children and families to feel isolated, (2) children and families value support from general practice, and (3) there are practical ways through which general practice has the potential to provide important aspects of care. Children and families reported benefits from fostering their relationship with their general practice in order to access important aspects of care, including the assessment and management of acute illness, chronic disease and medication reviews, and holistic support.ConclusionChildren with life-limiting and life-threatening conditions and their families value the involvement of general practice in the care, alongside their paediatric specialists. Ways of developing and providing such support as part of an integrated system of care need to be developed.
Daptomycin dosing in obese patients: analysis of the use of adjusted body weight versus actual body weight
Background: Food and Drug Administration–approved daptomycin dosing uses actual body weight, despite limited dosing information for obese patients. Studies report alterations in daptomycin pharmacokinetics and creatine phosphokinase elevations associated with higher weight-based doses required for obese patients. Limited information regarding clinical outcomes with alternative daptomycin dosing strategies in obesity exists. Objective: This study evaluates equivalency of clinical and safety outcomes in obese patients with daptomycin dosed on adjusted body weight versus a historical cohort using actual body weight. Methods: This retrospective, single center study compared equivalency of outcomes with two one-sided tests in patients with body mass index ⩾30 kg/m2 who received daptomycin dosed on actual body weight versus adjusted body weight. The primary outcome was clinical failure. Secondary outcomes included 90-day readmission and 90-day mortality. A combined safety endpoint included creatine phosphokinase elevation, patient-reported myopathy, and rhabdomyolysis. Results: A total of 667 patients were screened for inclusion; 101 patients were analyzed with 50 in the actual body weight cohort and 51 in the adjusted body weight cohort. The two regimens were statistically equivalent for clinical failure (2% actual body weight versus 4% adjusted body weight; p < 0.001 for equivalency). The two regimens were also statistically equivalent for 90-day mortality (6% actual body weight versus 4% adjusted body weight; p = 0.0014 for equivalency). Limitations include single center, retrospective design, and sample size. Daptomycin dosing intensified throughout the study period. Conclusion: The two daptomycin dosing cohorts were statistically equivalent for both clinical failure and 90-day mortality. More data are needed to assess outcomes with higher (⩾8 mg/kg/day) daptomycin doses in this patient population.
Recalling My Afghanistan Experience
MANY OF YOU MIGHT remember the picture from the cover of Women-Police magazine in 2009: I was in Mazar e Sharif in northern Afghanistan, and my interpreter, Ali, was standing next to me, assisting with translation to an Afghan Police Sergeant while on the firearms range. Ali was by my side every day, either in the base camp or \"outside the wire\" on the shooting range. Because affiliate employees of the U.S. government - like Ali - would unfortunately be left behind to fend for themselves.

Kris Arneson Retires After 32 Years in Policing
Reaping the Rewards of a Career in Service Kris stated that one of the most rewarding positions she held was as a community police officer working with \"Little Earth,\" a community of Native Americans. The results of the Safety Center helped to form a bond with the residents and the local police, where residents would feel safe reporting crimes and talking with law enforcement. MPD Chief of Police, Janee' Harteau, asked her to step up and take over. Since Kris had previously been the Vice President of the Minnesota Association of Women Police, she had a little familiarity with police conferences.

Afghan Women's Police Training
The lack of female ANP instructors to go handson during the physical training classes proved to be especially challenging* But these were minor blips in the road compared to the overwhelming respect anj apport they received from the ANP | high command and all the assisting NATO countries. There currently is a backlog in excess of 100 female officers that have been hired but have not yet attended a basic academy training class.
