Diagnosis of Feline Infectious Peritonitis: A Review of the Current Literature

Feline infectious peritonitis (FIP) is a fatal disease that poses several challenges for veterinarians: clinical signs and laboratory changes are non-specific, and there are two pathotypes of the etiologic agent feline coronavirus (FCoV), sometimes referred to as feline enteric coronavirus (FECV) and feline infectious peritonitis virus (FIPV) that vary fundamentally in their virulence, but are indistinguishable by a number of diagnostic methods. This review focuses on all important steps every veterinary practitioner has to deal with and new diagnostic tests that can be considered when encountering a cat with suspected FIP with the aim to establish a definitive diagnosis. It gives an overview on all available direct and indirect diagnostic tests and their sensitivity and specificity reported in the literature in different sample material. By providing summarized data for sensitivity and specificity of each diagnostic test and each sample material, which can easily be accessed in tables, this review can help to facilitate the interpretation of different diagnostic tests and raise awareness of their advantages and limitations. Additionally, diagnostic trees depict recommended diagnostic steps that should be performed in cats suspected of having FIP based on their clinical signs or clinicopathologic abnormalities. These steps can easily be followed in clinical practice.

Pilot study: In vitro reduction of hemoglobin from canine blood with hemoperfusion using the Cytosorb® adsorber

Cell-free hemoglobin (cfHb) can be toxic and lead to kidney injury. This study assessed the in vitro reduction of cfHb from canine hemolyzed blood using hemoperfusion with a Cytosorb® cytokine adsorber. Canine whole blood was processed in linear and circular setups, with three runs each, at 100 mL/min. Hemolysis and osmolarity adjustments were performed with distilled water and hypertonic saline. Anticoagulation was optimized with heparin (10,000 IU/L). A median of 3.38 L of hemoglobin solution was processed in the linear setup. Samples were collected after the adsorber and from the waste bag. In the circular setup, a median of 2.09 L was processed, with samples taken before and after the adsorber. CfHb concentration was measured using the XT-2000iV® hematology analyzer (Sysmex). A control setup without an adsorber was run for 24 hours to assess cfHb stability. In the linear setup, cfHb concentration decreased by a median of 17.8% (14.7–26.8%), from 1.7 mmol/L to a minimum of 0.9 mmol/L, with 12.1 g (11.9–23.5 g) of cfHb removed. The median cfHb concentration after the adsorber displayed a logarithmic increase from 0.9 mmol/L (0.8–1.2 mmol/L) to 1.6 mmol/L (1.4–2.1 mmol/L). After processing 2.4 L, no further reduction occurred. In the circular setup, cfHb was reduced by a median of 41.3% (46.1–45.0%), representing 17.4 g (14.6–19.0 g) removed after 13.0 L (13.0–14.0 L). The reduction plateau was reached after 13 L. The hemoglobin reduction ratio in the circular setup at 3 L processed cfHb-solution was 25.0% (23.1–33.3%) and was not different from the linear setup (p = 0.400). The cfHb clearance decreased in both setups over time. CfHb concentration in the control setup was stable for 24 hours. Hemoperfusion with the Cytosorb ® adsorber reduced cfHb in vitro from a canine blood solution.

Feline coronavirus (FCoV) is a ubiquitous RNA virus of cats, which is transmitted faeco-orally. In these guidelines, the European Advisory Board on Cat Diseases (ABCD) presents a comprehensive review of feline infectious peritonitis (FIP). FCoV is primarily an enteric virus and most infections do not cause clinical signs, or result in only enteritis, but a small proportion of FCoV-infected cats develop FIP. The pathology in FIP comprises a perivascular phlebitis that can affect any organ. Cats under two years old are most frequently affected by FIP. Most cats present with fever, anorexia, and weight loss; many have effusions, and some have ocular and/or neurological signs. Making a diagnosis is complex and ABCD FIP Diagnostic Approach Tools are available to aid veterinarians. Sampling an effusion, when present, for cytology, biochemistry, and FCoV RNA or FCoV antigen detection is very useful diagnostically. In the absence of an effusion, fine-needle aspirates from affected organs for cytology and FCoV RNA or FCoV antigen detection are helpful. Definitive diagnosis usually requires histopathology with FCoV antigen detection. Antiviral treatments now enable recovery in many cases from this previously fatal disease; nucleoside analogues (e.g., oral GS-441524) are very effective, although they are not available in all countries.

Treatment of Leishmania infantum Infections in Dogs

Dogs are reservoir hosts of the zoonotic parasite Leishmania infantum, the causative agent of canine leishmaniosis. Antiparasitic drugs that are commonly used in dogs include allopurinol, miltefosine, and meglumine antimoniate. Treatment success is characterized by an improvement of disease signs, reduction in parasite load, as well as prevention of relapse. However, despite treatment, infections in dogs can usually not be cleared and often lead to (recurrent) signs of disease. Since most of the drugs used in dogs are also applied in human medicine, the prevention of treatment-induced drug-resistant Leishmania strains is a major one-health concern. This review article provides an overview of current treatment options for Leishmania-infected dogs with allopurinol, meglumine antimoniate, and miltefosine, related adverse effects, and drug resistance potential.

Feline Panleukopenia Outbreaks and Risk Factors in Cats in Animal Shelters

(1) Background: This study aimed to determine the risk factors for outbreaks of feline panleukopenia in shelters. (2) Methods: Four shelters (A−D) with 150 cats were included. Fecal samples were analyzed by parvovirus real-time polymerase chain reaction (qPCR), including culture and sequencing of qPCR-positive samples. Information on cats, husbandry, hygiene, and infection management was evaluated to determine risk factors for feline panleukopenia and parvovirus shedding by logistic regression. (3) Results: Feline panleukopenia occurred in 28.0% (42/150) of cats (0 in shelter D). Shedding was found in 48.7% (73/150) (A: 21/73; B: 29/73; C: 7/73; D: 16/73). Of 73 qPCR-positive fecal samples, 65.8% (48/73) were culture-positive; sequencing revealed feline panleukopenia virus (FPV) isolates in 34/48 samples and vaccine virus isolate in 14/48; canine parvovirus was not detected. Presence of feline panleukopenia was significantly more likely in cats from shelter A (p < 0.05), unvaccinated cats (p < 0.001), and young cats (4 weeks to 2 years; p = 0.008). Parvovirus shedding was significantly more common in young cats (p < 0.001), cats with feline panleukopenia (p = 0.033), and group-housed cats (p = 0.025). (4) Conclusions: Vaccination is the most important measure to reduce the risk of feline panleukopenia in shelters. Risk of parvovirus shedding is especially high in young, group-housed cats.

Effect of probiotic treatment on the clinical course, intestinal microbiome, and toxigenic Clostridium perfringens in dogs with acute hemorrhagic diarrhea

The impact of probiotics on dogs with acute hemorrhagic diarrhea syndrome (AHDS) has not been evaluated so far. The study aim was to assess the effect of probiotic treatment on the clinical course, intestinal microbiome, and toxigenic Clostridium perfringens in dogs with AHDS in a prospective, placebo-controlled, blinded trial.
Twenty-five dogs with AHDS with no signs of sepsis were randomly divided into a probiotic (PRO; Visbiome, ExeGi Pharma) and placebo group (PLAC). Treatment was administered for 21 days without antibiotics. Clinical signs were evaluated daily from day 0 to day 8. Key bacterial taxa, C. perfringens encoding NetF toxin and enterotoxin were assessed on days 0, 7, 21.
Both groups showed a rapid clinical improvement. In PRO a significant clinical recovery was observed on day 3 (p = 0.008), while in PLAC it was observed on day 4 (p = 0.002) compared to day 0. Abundance of Blautia (p<0.001) and Faecalibacterium (p = 0.035) was significantly higher in PRO on day 7 compared to day 0, while in PLAC the abundance of Faecalibacterium was not significantly higher on any study day and Blautia (p = 0.016) was only significantly higher on day 21 compared to day 0. Abundance of C. perfringens was significantly lower on day 7 (p = 0.011) compared to day 0 in PRO but not in PLAC. Enterotoxin genes were significantly lower in PRO on day 21 (p = 0.028) compared to PLAC. Fecal samples of 57% of all dogs were positive for netF toxin genes on day 0 and the abundance was significantly lower on day 7 compared to day 0 in PRO (p = 0.016) and PLAC (p = 0.031).
The probiotic treatment was associated with an accelerated normalization of the intestinal microbiome. Dogs with aseptic AHDS showed a rapid decrease of netF toxin genes and fast clinical recovery in both groups under symptomatic treatment without antibiotics.

Canine vaccination in Germany: A survey of owner attitudes and compliance

Vaccination is the most important preventive measure for protection against infectious diseases in humans and companion animals. Nevertheless, scepticism about the safety and importance of vaccines is increasing in human and in veterinary medicine. Although owner attitudes towards vaccination have been investigated in cats, there are no similar studies in dogs. The goals of this study were therefore to investigate the vaccination status of dogs in Germany, to determine owner compliance with vaccination and to identify factors that play a role in owners' decisions to have their dogs vaccinated. Data were collected from August 2018 to February 2019 using an online survey targeting dog owners in Germany. A total of 3,881 questionnaires were evaluated, and factors associated with the vaccination status of dogs were determined by a linear logistic regression model using Akaike information criterion. Cohen's kappa statistic was used to evaluate agreement between questionnaire and 340 vaccination passports submitted voluntarily by owners. A total of 46.8% (n = 1,818/3,881) of dogs were vaccinated with core vaccines according to current guidelines with the lowest vaccination rate for leptospirosis (50.1%; n = 1,941/3,874). Dog's age (16 weeks to 15 months) (odds ratio (OR): 3.08; 95% CI: 2.05-4.68), type (working dog) (OR: 2.06; 95% CI: 1.22-3.53) and travelling abroad within previous 36 months (OR: 1.82; 95% CI: 1.12-2.96) had the strongest 'positive' association with the vaccination status. Recommendation from a veterinarian not to vaccinate against leptospirosis had the strongest 'negative' association (OR: 0.08; 95% CI: 0.04-0.18). The study revealed a need for improvement in vaccination compliance because of inadequate vaccination coverage, especially for leptospirosis, in dogs. Factors influencing owner compliance were numerous. Vaccination recommendations made by the veterinarian had a strong association with the vaccination status and should be used to increase canine vaccination rates.

Circulating immune complexes in dogs with Leishmania infantum infection in a non-endemic country

Circulating immune complexes (CIC) cause different organ lesions in canine leishmaniosis. This study aimed to measure CIC in dogs infected with Leishmania (L.) infantum in a non-endemic country and to analyze associations with disease parameters. Measurement of Leishmania -specific CIC was performed by a polyethylene glycol ELISA (cut-off: 0.274 optical density (OD)) every three months during a one-year study period in 52 L. infantum -infected dogs. Each appointment included a physical examination, complete blood count, serum biochemistry including C-reactive protein, urinalysis, L. infantum PCR and antibody ELISA. Statistical analyses included Mann-Whitney U tests, a multivariable robust linear regression, Spearman correlation, univariable logistic regression, and a receiver operating characteristic (ROC) curve. CIC levels differed significantly between dogs with and without lymphadenopathy ( p < 0.01; β =−0.38; 95% CI: −0.52, −0.24), uveitis ( p < 0.01; β=− 0.52; 95% CI: −0.89, −0.15), seborrhea/hypotrichosis ( p = 0.04; β =−0.11; 95% CI: −0.21, −0.01), and skin nodules ( p < 0.01; β = 0.22; 95% CI: 0.13, 0.31). Significant moderate to strong correlations with CIC levels were found for L. infantum antibodies ( p < 0.01; r s = 0.65; 95% CI: 0.57, 0.72), globulin ( p < 0.01; r s = 0.60; 95% CI: 0.51, 0.68), albumin-to-globulin ratio (p < 0.01; r s =−0.56; 95% CI: −0.65, −0.47) and total protein ( p < 0.01; r s = 0.46; 95% CI: 0.35, 0.56). CIC levels were significantly higher in dogs with positive lymph node PCR ( p = 0.04) as well as in dogs with disease relapses ( p < 0.01). The ROC curve analysis revealed one highly specific cut-off value at 1.668 OD for differentiating between dogs with and without disease relapse (98% specificity; 60% sensitivity). Measurement of Leishmania -specific CIC is useful for monitoring dogs with L. infantum infections and for indicating disease relapse.

Short Treatment of 42 Days with Oral GS-441524 Results in Equal Efficacy as the Recommended 84-Day Treatment in Cats Suffering from Feline Infectious Peritonitis with Effusion—A Prospective Randomized Controlled Study

In the past, feline infectious peritonitis (FIP) caused by feline coronavirus (FCoV) was considered fatal. Today, highly efficient drugs, such as GS-441524, can lead to complete remission. The currently recommended treatment duration in the veterinary literature is 84 days. This prospective randomized controlled treatment study aimed to evaluate whether a shorter treatment duration of 42 days with oral GS-441524 obtained from a licensed pharmacy is equally effective compared to the 84-day regimen. Forty cats with FIP with effusion were prospectively included and randomized to receive 15 mg/kg of GS-441524 orally every 24h (q24h), for either 42 or 84 days. Cats were followed for 168 days after treatment initiation. With the exception of two cats that died during the treatment, 38 cats (19 in short, 19 in long treatment group) recovered with rapid improvement of clinical and laboratory parameters as well as a remarkable reduction in viral loads in blood and effusion. Orally administered GS-441524 given as a short treatment was highly effective in curing FIP without causing serious adverse effects. All cats that completed the short treatment course successfully were still in complete remission on day 168. Therefore, a shorter treatment duration of 42 days GS-441524 15 mg/kg can be considered equally effective.

Feline calicivirus (FCV) is a common pathogen in domestic cats that is highly contagious, resistant to many disinfectants and demonstrates a high genetic variability. FCV infection can lead to serious or even fatal diseases. In this review, the European Advisory Board on Cat Diseases (ABCD), a scientifically independent board of experts in feline medicine from 11 European countries, presents the current knowledge of FCV infection and fills gaps with expert opinions. FCV infections are particularly problematic in multicat environments. FCV-infected cats often show painful erosions in the mouth and mild upper respiratory disease and, particularly in kittens, even fatal pneumonia. However, infection can be associated with chronic gingivostomatitis. Rarely, highly virulent FCV variants can induce severe systemic disease with epizootic spread and high mortality. FCV can best be detected by reverse-transcriptase PCR. However, a negative result does not rule out FCV infection and healthy cats can test positive. All cats should be vaccinated against FCV (core vaccine); however, vaccination protects cats from disease but not from infection. Considering the high variability of FCV, changing to different vaccine strain(s) may be of benefit if disease occurs in fully vaccinated cats. Infection-induced immunity is not life-long and does not protect against all strains; therefore, vaccination of cats that have recovered from caliciviral disease is recommended.
