Multitarget Stool RNA Test for Noninvasive Detection of Colorectal Neoplasia in a Multicenter, Prospective, and Retrospective Cohort

INTRODUCTION:Effective colorectal cancer (CRC) prevention and screening requires sensitive detection of all advanced neoplasias (CRC and advanced adenomas [AA]). However, existing noninvasive screening approaches cannot accurately detect adenomas with high sensitivity.METHODS:Here, we describe a multifactor assay (RNA-FIT test) that combines 8 stool-derived eukaryotic RNA biomarkers, patient demographic information (smoking status), and a fecal immunochemical test (FIT) to sensitively detect advanced colorectal neoplasias and other non-advanced adenomas in a 1,305-patient, average-risk, prospective cohort. This cohort was supplemented with a 22-patient retrospective cohort consisting of stool samples obtained from patients diagnosed with AA or CRC before treatment or resection. Participants within these cohorts were evaluated with the RNA-FIT assay and an optical colonoscopy. RNA-FIT test results were compared with colonoscopy findings.RESULTS:Model performance was assessed through 5-fold internal cross-validation of the training set (n = 939) and by using the model on a hold out testing set (n = 388). When used on the hold out testing set, the RNA-FIT test attained a 95% sensitivity for CRC (n = 22), 62% sensitivity for AA (n = 52), 25% sensitivity for other non-AA (n = 139), 80% specificity for hyperplastic polyps (n = 74), and 85% specificity for no findings on a colonoscopy (n = 101).DISCUSSION:The RNA-FIT assay demonstrated clinically relevant detection of all grades of colorectal neoplasia, including carcinomas, AAs, and ONAs. This assay could represent a noninvasive option to screen for both CRC and precancerous adenomas.

Brown marmorated stink bug, Halyomorpha halys (Stål), genome: underpinnings of polyphagy, insecticide resistance potential and biology of a top worldwide pest

Halyomorpha halys (Stål), the brown marmorated stink bug, is a highly invasive insect species that is also a nuisance due to overwintering in human-made structures. It has caused significant agricultural losses in recent years along the Atlantic seaboard of North America and in Continental Europe. Analysis of the 1.15 Gb genome assembly in a highly collaborative manner has identified a wide variety of genetic elements responsible for the biological peculiarities of this formidable pest species, including a highly expanded gustatory receptor repertoire likely responsible for H. halys' exceptionally high levels of polyphagy. Many of these genes may have potential for biomolecular pesticide applications. Availability of the H. halys genome sequence will be useful for the development of environmentally friendly biomolecular pesticides for use in concert with more traditional, synthetic chemical-based controls.

Cardiovascular morbidity and mortality display a distinct time dependence also known as circadian variation. Whether such time dependence extends to the risk of procedural-related mortality after percutaneous coronary intervention (PCI) is presently unknown.
Inhospital mortality was analyzed in 6347 patients with PCI start times from 8:00
am to 6:59
pm (“usual” workday). The sample was divided into 3 evenly populated groups (morning start 8:00-10:59
am, midday start 11:00
am-1:59
pm, afternoon start 2:00-6:59
pm). The association between procedural start time and mortality was assessed using multivariable analysis including a propensity score accounting for factors associated with procedural start time.
There was a significant, nonlinear relationship between procedural-related mortality and start time (
P = .03). Afternoon start patients were at higher adjusted risk of mortality compared with midday start patients (OR 2.03, 95% CI 1.07-3.83,
P = .03 ). Morning start patients were also at higher risk compared with midday start patients although the association was not statistically significant (OR 1.73, 95% CI 0.89-3.39,
P = .11).
There is a significant time-dependent variation in the risk of inhospital PCI-related mortality during usual working hours. The highest risk period, taking into account numerous factors that confound this association, is the latter part of the workday. A second period of apparent greater risk occurs during the early part of the workday and is consistent with our present understanding of circadian variation in cardiovascular disease processes.

Human immunodeficiency virus (HIV) case-based surveillance (CBS) systematically and continuously collects available demographic and health event data (sentinel events) about persons with HIV infection from diagnosis and, if available, throughout routine clinical care until death, to characterize HIV epidemics and guide program improvement. Surveillance signals such as high viral load, mortality, or recent HIV infection can be used for rapid public health action. To date, few standardized assessments have been conducted to describe HIV CBS systems globally. For this assessment, a survey was disseminated during May-July 2019 to all US President's Emergency Plan for AIDS Relief (PEPFAR)--supported countries with CDC presence (46) to describe CBS implementation and identify facilitators and barriers. Despite these limitations, this is the first comprehensive global assessment of CBS implementation in PEPFAR-supported countries. CBS is an effective system for countries to monitor their HIV epidemics in real time and to better inform responses.
