Immigration Policy as Foreign Policy

Immigration policy has taken centre stage in the social sciences over the past 20 years. Despite the proliferation of articles and books in this field, very little attention has been paid to immigration policy as foreign policy. It is domestic policy that prevails in the literature, most notably about the effects of immigration on destination societies. This article distinguishes the domestic and foreign policy aspects of immigration policy, acknowledging as it does so that foreign policy is virtually always an expression of national self-interest. It concludes with observations on the realist and idealist/liberal approaches to international relations theory including with respect to the recently adopted United Nations Global Compact for Safe, Orderly, and Regular Migration and the United Nations Global Compact on Refugees. Its purpose is to draw attention to this neglected aspect of immigration policy and to encourage others to explore it in greater detail, from the perspectives of both individual states and the world's international institutions.
Göç politikası son 20 yılda sosyal bilimlerde merkezi bir konuma ulaştı. Konuyla ilgili olarak yayınlanan makalelerin ve kitapların sayısının büyük oranda artmasına rağmen, göç politikasının dış politikanın konusu olmasına çok az dikkat çekildi. Literatür göçle ilgili daha çok iç politikaya ağırlık vermekte, özellikle göçün hedef ülkelerdeki etkilerine yoğunlaşmaktadır. Bu çalışma, göç politikasının iç ve dış politika unsurlarını ayırdetmekte ve dış politikanın fiiliyatta herzaman ulusal çıkarın ifadesi olduğunu açıklamaktadır. Makale sonuç bölümünde yakın zaman önce kabul edilen BM Güvenli, Düzenli ve Kurallı Göç İçin Küresel Mutabakat ve BM Mültecilere İlişkin Küresel Mutabakat bağlamında realist ve idealist/liberal uluslararası ilişkiler teorileri çerçevesinde konuyu analiz etmektedir. Çalışmanın amacı, göç politikasının ihmal edilen bu alanına dikkat çekmek ve bu konuyla ilgili hem devletler hem de uluslararası kurumlar perspektifinden yapılacak çalışmaları teşvik etmektir.

Risk Profile of the RET A883F Germline Mutation: An International Collaborative Study

The A883F germline mutation of the rearranged during transfection (RET) proto-oncogene causes multiple endocrine neoplasia 2B. In the revised American Thyroid Association (ATA) guidelines for the management of medullary thyroid carcinoma (MTC), the A883F mutation has been reclassified from the highest to the high-risk level, although no well-defined risk profile for this mutation exists.
To create a risk profile for the A883F mutation for appropriate classification among the ATA risk levels.
Retrospective analysis.
International collaboration.
Included were 13 A883F carriers.
The intervention was thyroidectomy.
Earliest age of MTC, regional lymph node metastases, distant metastases, age-related penetrance of MTC and pheochromocytoma (PHEO), overall and disease-specific survival, and biochemical cure rate.
One and three carriers were diagnosed at age 7 to 9 years (median, 7.5 years) with a normal thyroid and C-cell hyperplasia, respectively. Nine carriers were diagnosed with MTC at age 10 to 39 years (median, 19 years). The earliest age of MTC, regional lymph node metastasis, and distant metastasis was 10, 20, and 20 years, respectively. Fifty percent penetrance of MTC and PHEO was achieved by age 19 and 34 years, respectively. Five- and 10-year survival rates (both overall and disease specific) were 88% and 88%, respectively. Biochemical cure for MTC at latest follow-up was achieved in 63% (five of eight carriers) with pertinent data.
MTC of A883F carriers seems to have a more indolent natural course compared with that of M918T carriers. Our results support the classification of the A883F mutation in the ATA high-risk level.

'Feeling like a deer in the headlights': A qualitative study of registrar experience in early general practice training

Background and objective: The journey of becoming a competent general practitioner is, anecdotally, a bumpy ride. The aim of this study was to explore general practice registrars' experiences in their first six-month term as they learn the science, art and trade of general practice. Methods: This study explored the experience of 12 registrars undertaking their first general practice term using a qualitative narrative inquiry approach. Registrars kept a fortnightly audio-diary reflection, supplemented by a semi-structured interview at the start and end of the term. Results: The start of the general practice term was marked by stress and anxiety. The steep learning curve of the registrars' educational journey was matched by an emotional journey from the initial stress to becoming more 'comfortable'. Discussion: The stress of the early days was experienced by all registrars and managed in a number of ways. The findings may help facilitate the journey for future registrars.

Becoming a general practitioner in Australia: Transformative learning and threshold concepts
A previous article presented stories of registrars' journeys through early general practice training. This paper aims to explore in detail two registrars' stories using the lenses of transformative learning theory and threshold concept theory to seek fresh insights into becoming a general practitioner.
This study explored the experience of 12 registrars undertaking their first general practice term using a qualitative narrative enquiry approach. Two stories are examined in more detail to see if they resonate with the transformative learning and threshold concept theories.
Both theories appear to have resonance with the experiences, as described by the two registrars.
The transformative experience of early general practice training can be usefully described by transformative learning theory as a holistic change of 'meaning perspective', and also as learning key threshold concepts. This might help inform future registrars' learning and supervisors' teaching.

Exploring the introduction of remote external clinical teaching visits into Australian general practice

Background and objective: The COVID-19 pandemic changed the way education was delivered. In Australian general practice training, external clinical teaching visits (ECTVs) had to be done remotely. This research explored that phenomenon from the point of view of the participants to answer questions about feasibility, acceptability and effectiveness.
Methods: The experiences of ECTV visitors, registrars, supervisors and practice managers were invited initially by survey and then through interview and focus groups. Data were analysed using a constant comparative method giving four major themes and several subthemes. Results Remote ECTVs were well accepted by all participants. They are different to ECTVs done face to face, with both advantages and disadvantages.
Discussion: Remote ECTVs are feasible and effective and are likely to become part of the ongoing Australian general practice teaching program.
