Community-directed vector control to supplement mass drug distribution for onchocerciasis elimination in the Madi mid-North focus of Northern Uganda
Onchocerciasis a neglected tropical disease that historically has been a major cause of morbidity and an obstacle to economic development in the developing world. It is caused by infection with Onchocerca volvulus, which is transmitted by black flies of the genus Simulium. The discovery of the potent effect of Mectizan (ivermectin) on O. volvulus microfilariae and the decision by its manufacturer to donate the drug for onchocerciasis spurred the implementation of international programs to control and, more recently, eliminate this scourge. These programs rely primarily on mass distribution of ivermectin (MDA) to the afflicted populations. However, MDA alone will not be sufficient to eliminate onchocerciasis where transmission is intense and where ivermectin MDA is precluded by co-endemicity with Loa loa. Vector control will likely be required as a supplemental intervention in these situations. Because biting by the black fly vectors is often a major nuisance in onchocerciasis afflicted communities, we hypothesized that community members might be mobilized to clear the breeding sites of the vegetation that represents the primary black fly larvae attachment point. We evaluated the effect of such a community based \"slash and clear\" intervention in multiple communities in Northern Uganda. Slash and Clear resulted in 89-99% declines in vector biting rates. The effect lasted up to 120 days post intervention. Slash and clear might represent an effective, inexpensive, community- based tool to supplement ivermectin distribution as a contributory method to eliminate onchocerciasis and prevent recrudescence.
GradFreeBits: Gradient-Free Bit Allocation for Mixed-Precision Neural Networks
Quantized neural networks (QNNs) are among the main approaches for deploying deep neural networks on low-resource edge devices. Training QNNs using different levels of precision throughout the network (mixed-precision quantization) typically achieves superior trade-offs between performance and computational load. However, optimizing the different precision levels of QNNs can be complicated, as the values of the bit allocations are discrete and difficult to differentiate for. Moreover, adequately accounting for the dependencies between the bit allocation of different layers is not straightforward. To meet these challenges, in this work, we propose GradFreeBits: a novel joint optimization scheme for training mixed-precision QNNs, which alternates between gradient-based optimization for the weights and gradient-free optimization for the bit allocation. Our method achieves a better or on par performance with the current state-of-the-art low-precision classification networks on CIFAR10/100 and ImageNet, semantic segmentation networks on Cityscapes, and several graph neural networks benchmarks. Furthermore, our approach can be extended to a variety of other applications involving neural networks used in conjunction with parameters that are difficult to optimize for.
Plain radiographic indices are reliable indicators for quantitative bone mineral density in male and female patients before total hip arthroplasty
Osteoporosis is underdiagnosed in patients undergoing total hip arthroplasty (THA). Bone mineral density measurement by dual-energy X-ray absorptiometry (DXA) is the gold standard, but indices on plain hip radiographs also seemed to be reliable screening tools in female or Asian ethnicities in previous studies. Given the lack of knowledge about male patients and Caucasian ethnicities, this study was conducted to evaluate plane hip radiographic indices as a screening tool for osteopenia and osteoporosis in Caucasian female and also male patients before undergoing THA. A retrospective analysis of 216 elderly patients with pre-existing DXA before hip arthroplasty was performed and four indices were calculated on plain hip radiographs: Canal-Flare-Index (CFI), Canal-Calcar-Ratio (CCR), Canal-Bone-Ratio (CBR) 7 and 10 cm below the lesser trochanter. They were correlated with femoral neck DXA T-scores by Pearson’s correlation and intraclass correlation coefficient, and a ROC analysis was performed. A total of 216 patients (49.5% male) were included. CBR-7 and -10 were highly correlated (p < 0.001) with femoral neck T-score in males (Pearson’s correlation CBR-7 r = − 0.60, CBR-10 r = − 0.55) and females (r = − 0.74, r = − 0.77). CBR-7 and -10 also showed good diagnostic accuracy for osteoporosis in the ROC analysis in males (CBR-7: AUC = 0.75, threshold = 0.51; CBR-10: 0.63; 0.50) and females (CBR-7: AUC = 0.87, threshold = 0.55; CBR-10: 0.90; 0.54). Indices such as the Canal Bone Ratio (CBR) 7 or 10 cm below the lesser trochanter on plain hip radiographs are a good screening tool for osteopenia and osteoporosis on plain hip radiographs and can be used to initiate further diagnostics like the gold standard DXA. They differ between male and female patients.
In vitro additive effects of dalbavancin and rifampicin against biofilm of Staphylococcus aureus
Dalbavancin is a novel glycopeptide antibiotic approved for the treatment of acute bacterial skin and skin structure infections (ABSSSIs). It is characterized by a potent activity against numerous Gram-positive pathogens, a long elimination half-life and a favorable safety profile. Most recently, its application for the treatment of periprosthetic joint infections (PJIs) was introduced. The aim of this study was to proof our hypothesis, that dalbavancin shows superior efficacy against staphylococcal biofilms on polyethylene (PE) disk devices compared with vancomycin and additive behavior in combination with rifampicin. Staphylococcus aureus biofilms were formed on PE disk devices for 96 h and subsequently treated with dalbavancin, vancomycin, rifampicin and dalbavancin-rifampicin combination at different concentrations. Quantification of antibacterial activity was determined by counting colony forming units (CFU/ml) after sonification of the PE, serial dilution of the bacterial suspension and plating on agar-plates. Biofilms were additionally life/dead-stained and visualized using fluorescence microscopy. Dalbavancin presented superior anti-biofilm activity compared to vancomycin. Additive effects of the combination dalbavancin and rifampicin were registered. Dalbavancin combined with rifampicin presents promising anti-biofilm activity characteristics in vitro . Further in vivo studies are necessary to establish recommendations for the general use of dalbavancin in the treatment of PJIs.
Unveiling Asymptotic Behavior in Precipitation Time Series: A GARCH-Based Second Order Semi-Parametric Autocorrelation Framework for Drought Monitoring in the Semi-Arid Region of India
This study evaluated ten drought indices focusing on their ability to monitor drought events in Marathwada, a semi-arid region of India. High-resolution gridded monthly total precipitation data for 75 years (1950–2024) from the European Centre for Medium-Range Weather Forecasts (ECMWF) were used to evaluate the drought indices. These indices were computed across six timescales: 1, 3, 4, 6, 9, and 12 months. A Generalized Autoregressive Conditional Heteroscedastic (GARCH) model was employed to detect temporal volatility in precipitation, followed by a second-order geospatial autocorrelation eigenfunction eigendecomposition using Global Moran’s Index statistics to geolocate both aggregated and non-aggregated precipitation locations. The performance of drought indices was assessed using non-parametric Spearman’s correlation to identify the strength, direction, and similarity of regional-specific drought events. The temporal lag interdependence between meteorological and agricultural droughts was assessed using a non-parametric Spearman’s cross correlation function (SCCF). The findings revealed that the GARCH model with a skewed Student’s t distribution effectively captured conditional temporal volatility and asymptotic behavior in the precipitation series. The model’s sensitivity enabled the incorporation of temporal fluctuations related to droughts and extreme meteorological events. The Bhalme and Mooley Drought Index (BMDI-6) and Z-Score Index (ZSI-6) were the most applicable indices for drought monitoring. Spearman’s cross-correlation analysis revealed that meteorological droughts influenced agricultural droughts with a time lag of up to 4 months.
Understanding the Impact of Bathymetric Changes in the German Bight on Coastal Hydrodynamics: One Step Toward Realistic Morphodynamic Modeling
The hydrodynamic response to morphodynamic variability in the coastal areas of the German Bight was analyzed via numerical experiments using time-referenced bathymetric data for the period 1982–2012. Time-slice experiments were conducted for each year with the Semi-implicit Cross-scale Hydroscience Integrated System Model (SCHISM). This unstructured-grid model resolves small-scale bathymetric features in the coastal zone, which are well-resolved in the high-resolution time-referenced bathymetric data (50 m resolution). Their analysis reveals the continuous migration of tidal channels, as well as rather complex change of the depths of tidal flats in different periods. The almost linear relationship between the cross-sectional inlet areas and the tidal prisms of the intertidal basins in the East Frisian Wadden Sea demonstrates that these bathymetric data describe a consistent morphodynamic evolutionary trend. The numerical experiment results are streamlined to explain the hydrodynamic evolution from 1982 to 2012. Although the bathymetric changes were mostly located in a relatively small part of the model area, they resulted in substantial changes in the M2 tidal amplitudes, i.e., larger than 5 cm in some areas. The hydrodynamic response to bathymetric changes largely exceeded the response to sea level rise. The tidal asymmetry estimated from the model appeared very sensitive to bathymetric evolution, particularly between the southern tip of Sylt Island and the Eider Estuary along the eastern coast. The peak current asymmetry weakened from 1982 to 1995 and even reversed within some tidal basins to become flood-dominant. This would suggest that the flushing trend in the 1980s was reduced or reversed in the second half of the studied period. Salinity also appeared sensitive to bathymetric changes; the deviations in the individual years reached ~22 psu in the tidal channels and tidal flats. One practical conclusion from the present numerical simulations is that wherever possible, the numerical modeling of near-coastal zones must employ time-referenced bathymetry data. The second, perhaps even more important conclusion, is that the progress of morphodynamic modeling in realistic ocean settings with multiple scales and varying bottom forms is strongly dependent on the availability of bathymetric data with appropriate temporal and spatial resolution.
A Multidimensional Perspective on Resilience in Later Life: A Systematic Literature Review of Protective Factors and Adaptive Processes in Ageing
Background: With the global population rapidly aging, resilience has emerged as a critical determinant of healthy aging. While many factors are associated with resilience, a comprehensive synthesis is needed to inform targeted interventions and policy. Objectives: This systematic review aimed to identify and synthesize the conceptual models and key protective factors associated with resilience in older people. Methods: Following PRISMA guidelines, a systematic literature search was conducted in Web of Science, PubMed, PsycNet, and JSTOR for studies published between 2017 and 2025. Search terms included (including synonyms and closely related words) “resilience,” “older people,” and “models.” Studies were screened based on relevance to resilience models, measurement tools, and associated factors. Included studies underwent a formal risk of bias assessment. Results: From 7109 initial records, 54 studies met the inclusion criteria. Ten studies explored conceptual models, while 44 investigated contributing factors. Resilience was predominantly assessed using standardized psychometric tools. Findings were synthesized by mapping key determinants across Macro-Environmental, Meso-Social, Micro-Individual and Bio-Physiological domains. Conclusions: Resilience in later life is a dynamic and multifactorial process, not a fixed trait. The evidence suggests a range of modifiable factors at various levels that can be targeted to support wellbeing. An integrated, systems-based perspective is essential for guiding future research and developing effective interventions to promote resilience across the aging trajectory.
