To develop and validate a new risk-of-bias tool for nonrandomized studies (NRSs).
We developed the Risk of Bias Assessment Tool for Nonrandomized Studies (RoBANS). A validation process with 39 NRSs examined the reliability (interrater agreement), validity (the degree of correlation between the overall assessments of RoBANS and Methodological Index for Nonrandomized Studies [MINORS], obtained by plotting the overall risk of bias relative to effect size and funding source), face validity with eight experts, and completion time for the RoBANS approach.
RoBANS contains six domains: the selection of participants, confounding variables, the measurement of exposure, the blinding of the outcome assessments, incomplete outcome data, and selective outcome reporting. The interrater agreement of the RoBANS tool except the measurement of exposure and selective outcome reporting domains ranged from fair to substantial. There was a moderate correlation between the overall risks of bias determined using RoBANS and MINORS. The observed differences in effect sizes and funding sources among the assessed studies were not correlated with the overall risk of bias in these studies. The mean time required to complete RoBANS was approximately 10min. The external experts who were interviewed evaluated RoBANS as a “fair” assessment tool.
RoBANS shows moderate reliability, promising feasibility, and validity. The further refinement of this tool and larger validation studies are required.

On-site manual therapy for firefighters in Seoul: six-month utilization and outcome from a retrospective service analysis and web-based survey

Background
In 2023, firefighters in Seoul responded to over two million emergency calls, exposing them to substantial physical and psychological strain. This working environment increases the risk of musculoskeletal disorders (MSDs) and occupational stress. To address these issues, the Seoul Metropolitan Fire & Disaster Headquarters (SMFDH) piloted a manual therapy program provided at fire stations. To our knowledge, this manuscript is the first report describing the feasibility of such an intervention in fire stations.
Methods
From 11 June to 6 December 2024, one Korean Medicine (KM) doctor rotated through five fire stations, providing free one-on-one treatment sessions during duty hours. This retrospective report analyzed detailed clinical records regarding manual therapies. Baseline characteristics, diagnoses according to ICD-10, and visit frequency were compared between firefighters with ≤ 10 years and > 10 years of service. A web-based survey (12–18 November 2024) assessed self-reported pain reduction and satisfaction.
Results
A total of 376 firefighters received 808 sessions; 66.8% had a history of herniated disc or rotator cuff syndrome. Manual therapies—primarily myofascial release, joint mobilization, and muscle energy techniques—were applied in 94.8% of visits. No serious adverse events were reported. Among 148 survey respondents (39.4%), self-reported NRS pain scores were lower after the program in most cases (82.4%). Firefighters with longer tenure reported a lower proportion of pain improvement but higher satisfaction scores than those with shorter tenure. Most respondents (97.3%) recommended the service, with on-site accessibility as the key advantage.
Conclusions
Station-based manual therapy appeared feasible and well accepted, with participants reporting pain reduction and high satisfaction. Their working tenure influenced utilization and perceived benefit. In 2025, SMFDH extended the program to a nine-month schedule across fifteen stations. Controlled studies with objective functional outcomes and long-term follow-up are warranted to confirm its occupational health benefits.

Utilization of traditional medicine in primary health care in low- and middle-income countries: a systematic review

This paper reports the findings from the first systematic review of the utilization of traditional medicine (TM) in primary health care (PHC) in low- and middle-income countries (LMICs). PHC is an important component of health care and essential for achieving universal health coverage (UHC). For countries where there is a gap in PHC, TM plays a vital role. It is widely used and has the potential to increase the coverage of PHC and UHC. Hence in situations where TM is recognized in a considerable magnitude, there are scarce evidence and minimal regulation on it and TM practitioners (TMPs). This study aims to identify the current situation in the utilization of TM in PHC or UHC in LMICs. A systematic review and thematic synthesis of qualitative and quantitative studies have been conducted. A total of 56 articles met the criteria and were included in the review. In all, 14 analytic themes have been developed including the current use of TM in PHC, higher accessibility of TM, medical pluralism, national health system, national health policy and national health insurance to include TM, including TMPs in the referral system, utilizing TMPs as community health workers, the needs of scientific research on TM and the need for training both TMPs and conventional medical staffs for better collaboration. The study concluded that it is necessary to further focus on TM in the macro level on strengthening the referral system by including TM to establish a comprehensive service delivery network under UHC and in the micro level to focus on training the TMPs and conventional medicine health workers on both areas to attain more in-depth understanding of each other, which can lead to better collaboration and quality patient care.

We performed a meta-analysis to compare the impact of extracorporeal cardiopulmonary resuscitation (ECPR) to that of conventional cardiopulmonary resuscitation (CCPR) in adult patients who experience cardiac arrest of cardiac origin. A literature search was performed using criteria set forth in a predefined protocol. Report inclusion criteria were that ECPR was compared to CCPR in adult patients with cardiac arrest of cardiac origin, and that survival and neurological outcome data were available. Exclusion criteria were reports describing non-cardiac origin arrest, review articles, editorials, and nonhuman studies. The efficacies of ECPR and CCPR were compared in terms of survival and neurological outcome. A total of 38,160 patients from 7 studies were ultimately included. ECPR showed similar survival (odds ratio [OR] 2.26, 95% confidence interval [CI] 0.45–11.20) and neurologic outcomes (OR 3.14, 95% CI 0.66–14.85) to CCPR in out-of-hospital cardiac arrest patients. For in-hospital cardiac arrest (IHCA) patients, however, ECPR was associated with significantly better survival (OR 2.40, 95% CI 1.44–3.98) and neurologic outcomes (OR 2.63, 95% CI 1.38–5.02) than CCPR. Hence, ECPR may be more effective than CCPR as an adjuvant therapy for survival and neurologic outcome in cardiac-origin IHCA patients.

The delta neutrophil index (DNI) as a prognostic marker for mortality in adults with sepsis: a systematic review and meta-analysis

We performed a meta-analysis to seek evidence for the usefulness of the delta neutrophil index (DNI) as a prognostic blood biomarker for mortality in the early stage of sepsis in adults. A literature search was performed using criteria set forth in a predefined protocol. Studies of adults with sepsis that provided a DNI measurement and that had mortality as the outcome, were included. Review articles, editorials, and non-human studies were excluded. The methodological quality of identified studies was assessed independently by two authors using the Quality in Prognosis Studies (QUIPS) tool. A total of 1,822 patients from eleven studies were ultimately included. Standardized mean differences between non-survivors and survivors were compared. An elevated DNI was associated with mortality in patients with sepsis (standardized mean difference [SMD] 1.22; 95% confidence interval 0.73–1.71; I

= 91%). After excluding two studies—one that included paediatric patients and one with a disproportionately low mortality rate—heterogeneity was minimized (SMD 0.74, 95% confidence interval 0.53–0.94; I

= 43%). Overall, the findings suggest that high DNI values are associated with mortality in septic patients.

A systematic review and meta-analysis on the efficacy of postoperative radiotherapy after gross total resection of intracranial solitary fibrous tumors

The efficacy of postoperative radiotherapy (PORT) after gross total resection (GTR) for intracranial solitary fibrous tumors (SFT) remains unclear due to the inconsistent results of previous studies, with some studies suggesting improved outcomes in progression-free survival (PFS) and overall survival (OS), while others report no significant benefit. Therefore, by evaluating and synthesizing data from relevant studies, we aimed to investigate the role of PORT, as compared with surgery alone, in survival outcomes after GTR of intracranial SFT. A systematic literature search, adhering to PRISMA guidelines and using Medline, Embase, and the Cochrane Library to identify relevant literature. The outcomes of interest included progression-free survival (PFS), overall survival (OS), and metastasis-free survival (MFS) at 3, 5, and 10 years, respectively. Differences between the two cohorts (GTR + PORT vs. GTR only) were estimated by calculating the hazard ratios. Twelve studies, including data from 419 patients (GTR + PORT, n = 225 vs. GTR, n = 194), were selected for meta-analysis. Pooled hazard ratios revealed that the PORT cohort showed sustained superiority in both PFS and OS compared with the surgery-only cohort after GTR of the tumor. These results were consistent with those of a subgroup analysis that focused on grade 2 and 3 intracranial SFT. However, no significant improvement was observed in MFS with PORT addition. This study underscores the importance of PORT in enhancing the PFS and OS of patients with intracranial SFT after GTR. These findings suggest that PORT should be considered an effective treatment strategy for all patients with intracranial SFT, irrespective of the extent of resection.

Changes in lymph node surgery in breast cancer and preoperative drug prescription analysis for postoperative pain management: A retrospective, cross-sectional study

This study aimed to investigate the changes in lymph node surgery types and prescription patterns of postoperative medications for pain management in patients with breast cancer using national health insurance claim data from South Korea. The study population comprised patients with at least one record of a principal diagnosis of breast cancer (ICD-10 code: C50) from the national health insurance claim database between 2010 and 2019. Patients who underwent mastectomy or lumpectomy only once were selected for the analysis. Patients who underwent axillary lymph node dissection (ALND) with mastectomy or lumpectomy on the day of surgery were included in the ALND group, whereas those who underwent sentinel lymph node biopsy (SLNB) were included in the SLNB group. Prescription records of opioids before, after and on the date of breast cancer surgery were collected and categorized according to the opioid type. Multivariate logistic regression modeling was used to compare postoperative opioid prescriptions. The proportion of those undergoing ALND among 3,080 patients decreased consistently after 2014, while the proportion undergoing SLNB increased. Although the rate of pain medication prescription on the day of surgery was similar between the two groups, the rate of prescription of postoperative pain medication and anticancer agents was lower in the SLNB group than in the ALND group. Logistic regression modeling showed that the SLNB group had lower odds of receiving opioids than did the ALND group (Odds ratio (OR) = 0.727, Confidence Interval (CI) = 0.546–0.970). A consistent trend was observed when the model was adjusted for neoadjuvant chemotherapy and the use of preoperative pain medications (OR = 0.718, CI = 0.538–0.959). To manage postoperative pain and prevent chronic pain with minimal side effects, sufficient discussion among clinicians, patients, and other healthcare professionals is imperative, along with adequate treatment planning.

Nationwide Trends in Healthcare Utilization and Expenditures Among Patients with Cervical Dystonia in Korea: A 7-Year Analysis Using Health Insurance Data

Background/Objectives: Cervical dystonia (CD) is the most common focal dystonia, but nationwide evidence on healthcare use is limited. This study assessed trends in utilization and expenditures in Korea. Methods: Using National Health Insurance Service claims (2017–2023), we identified CD patients (ICD-10 G24.3) and analyzed annual patients, visits, and expenditures by demographics, medical system, service type, and specialty. Results: A total of 6614 patients (33,896 claims) were included. Patient numbers declined until 2021 then slightly rebounded, and total expenditures fluctuated. Women were more prevalent, but men incurred higher costs. Western Medicine (WM) expenditures tended to increase while Korean Medicine (KM) tended to decline. Core botulinum toxin costs remained broadly stable; physiotherapy and diagnostic testing increased, particularly WM outpatient testing. Outpatient injection-related and anesthesia expenditures showed decreasing tendencies, while hospitalization and radiology exhibited modest or minimal changes. Neurology managed the most patients, while neurosurgery generated the highest costs. Conclusions: CD care in Korea showed declining patient numbers alongside overall stable total expenditures, with increasing use of rehabilitation and diagnostic services and continued stable use of botulinum toxin as a core therapy.

Lifetime risks and health impacts of hemorrhagic and ischemic stroke in South Korea

This study is aimed toward estimating the lifetime risks, life expectancy, expected years of life lost (EYLL), and lifetime costs related to different subtypes of stroke in South Korea. We included 13,994 patients diagnosed with stroke (ICD-10, I60-I63) in the National Health Insurance Service-National Sample Cohort of Korea between 2006 and 2015. Lifetime risks were calculated using the cumulative incidence rate for patients aged 18–84. Lifetime survival data were obtained through the Kaplan–Meier method and extrapolated with a rolling-over extrapolation algorithm. The lifetime costs were estimated by multiplying the average monthly expenditures with the survival probabilities and adding the values over lifetime. The lifetime risks of stroke in Korea have been decreasing consistently over the last decade with the exception of subarachnoid hemorrhage in females, which appears to have slightly increased. The EYLL is higher in hemorrhagic stroke than in ischemic stroke (6–9.7 vs. 4.7). Expected lifetime costs reimbursed by the NHIS would amount to about $71,406 accompanied with $14,921 copayment from the patients for hemorrhagic stroke, and $50,551 and $11,666, respectively, for ischemic stroke. Further studies are warranted to combine survival with quality of life and functional disability to obtain a more detailed outcome assessment of the potential impact of the prevention of stroke.

Utilization patterns of traditional medicine in Taiwan and South Korea by using national health insurance data in 2011

The growing popularity of traditional medicine (TM) is reflected in the increasing trend for its use worldwide. Many people are turning to use TM as a complementary or integrative treatment. The aim of this study is to present the first nationwide report describing the use of TM in two countries (South Korea and Taiwan).
To present the TM utilization patterns between South Korea and Taiwan, we analyzed data from the National Health Insurance cohorts in each country, each of which has approximately one million inhabitants.
In total, 261,478 (25.5%) of 1,025,340 people in South Korea and 260,529 (26.8%) of 970,866 people in Taiwan used TM services at least once under the National Health Insurance in 2011. Using multivariable logistic regression, TM users in South Korea were significantly more likely to be female, 61-80 years of age and individuals with a high income, and those in Taiwan were significantly more likely to be female, 21-40 years of age and individuals with a middle income. The two countries showed similar utilization patterns in visit seasons. People visited TM clinics more frequently than TM hospitals in both countries. The most common TM treatment in South Korea was acupuncture, whereas in Taiwan, various powdered Chinese herbal preparations were the most commonly used treatment. The most common diseases for people seeking TM services were musculoskeletal system and connective tissue diseases in South Korea and Symptoms, signs, and ill-defined conditions in Taiwan.
According to the National Health Insurance database, about one fourth of the NHI beneficiaries of South Korea and Taiwan had TM use in 2011. Different TM utilization patterns existed between South Korea and Taiwan, which might be due to the differences in insurance coverage between the two countries.
