The Deservingness Heuristic and the Politics of Health Care

Citizens' social policy opinions are strongly influenced by a simple heuristic: Are the recipients of social benefits deserving or not? Adding to this growing literature, we provide evidence that the deservingness heuristic does not treat all social benefits alike. Already at the level of preconsciousprocessing, the heuristic displays a bias toward tagging the recipients of health care—that is, sick individuals—as deserving. This powerful, implicit effect overrides other opinion factors and produces broad-based support among the public for health care—across levels of self-interest, media frames, ideological divides, and national cultures. In contrast, when the deservingness heuristic is utilized for reasoning about unemployment benefits, implicit psychological constraints are fewer and political conflict erupts depending on differences in interest and worldviews. Using a variety of methodologies, we track this fundamental difference between the politics of health care and unemployment benefits from the level of implicit processing to the level of political attitudes.

Holy War in the Baltic and the Battle of Lyndanise 1219

In June 1219 Danish crusaders fought a vicious battle against local pagan warriors at the place in northern Estonia where Tallinn now lies. The battle—known then as the Battle of Lyndanise—was a narrow victory for the crusaders and eventually turned out to be a pivotal event in the national histories of both Denmark and Estonia as a milestone in the overall military conquest of the entire region by (mostly) western military powers. The main scope of this book is to present a study of this military conquest of Estonia around 1200 with a special focus on the Scandinavian involvement, enabling us to better understand the intense political, military, and religious changes that came to influence the region and its many people from the early high medieval period onwards.
Reforming the welfare state

Life Cycle Risks and the Politics of the Welfare State

Life Cycle Risks and the Politics of the Welfare State presents the dual risk model of the welfare state. Previous research in the field has predominantly studied the role of modernization and the associated labor market risks; this book gives equal weight to a different class of social risks, namely those related to the life cycle. Labor market and life cycle risks each have profound, but distinct consequences for the political process of the welfare state, including public opinion formation, party competition, and public policy-making. The dual risk model helps us to understand why some social programs are prioritized over others in terms of political attention and public spending - and how this prioritization leads to mounting economic inequalities in modern-day societies.
Patients with suspected allergic reactions to COVID‐19 vaccines can be safely revaccinated after diagnostic work‐up

Background When initiating the Danish vaccination program against COVID‐19, the incidence of anaphylaxis was estimated to be 10 times higher compared to other virus‐based vaccines. In this study, we present data on patients referred with suspected allergic reactions to COVID‐19 vaccines. The main purpose of the study is to investigate the incidence and severity of the allergic reactions, and to evaluate the safety of revaccination. Methods All patients in the region of Southern Denmark with case histories of allergic reactions to COVID‐19 vaccines in a defined period are included in this study. Diagnostic work up consisted of a detailed case history, evaluation of Brighton level of diagnostic certainty and World Allergy Organization grade of anaphylaxis and skin prick testing‐ and basophil histamine release testing with COVID‐19 vaccines and relevant drug excipients. Patients were revaccinated at the Allergy Center when possible. Results Sixty‐one patients are included in this study. In 199,377 doses administered, nine patients fulfilled the criteria of anaphylaxis when using the Brighton Criteria (incidence being 45 per million). Of 55 patients with reactions to the first dose, 52 patients were revaccinated without adverse reactions. We found no proven cases of immediate anaphylaxis due to COVID‐19 vaccines. By skin prick test, we diagnosed three patients with drug excipient allergy and further a patient with mastocytosis was found. Conclusions Anaphylactic reactions to COVID‐19 vaccines are rare and the incidence is similar to what is seen with other virus‐based vaccines. Revaccination is safe in the majority of patients; however, allergological evaluation is important since some prove to have drug excipient allergy.

The Gait Deviation Index Is Associated with Hip Muscle Strength and Patient-Reported Outcome in Patients with Severe Hip Osteoarthritis—A Cross-Sectional Study

The Gait Deviation Index summarizes overall gait 'quality', based on kinematic data from a 3-dimensional gait analysis. However, it is unknown which clinical outcomes may affect the Gait Deviation Index in patients with primary hip osteoarthritis. The aim of this study was to investigate associations between Gait Deviation Index as a measure of gait 'quality' and hip muscle strength and between Gait Deviation Index and patient-reported outcomes in patients with primary hip osteoarthritis.
Forty-seven patients (34 males), aged 61.1 ± 6.7 years, with BMI 27.3 ± 3.4 (kg/m2) and with severe primary hip osteoarthritis underwent 3-dimensional gait analysis. Mean Gait Deviation Index, pain after walking and maximal isometric hip muscle strength (flexor, extensor, and abductor) were recorded. All patients completed the 'Physical Function Short-form of the Hip disability and Osteoarthritis Outcome Score (HOOS-Physical Function) and the Hip disability and Osteoarthritis Outcome Score subscales for pain (HOOS-Pain) and quality-of-life (HOOS-QOL).
Mean Gait Deviation Index was positively associated with hip abduction strength (p<0.01, r = 0.40), hip flexion strength (p = 0.01, r = 0.37), HOOS-Physical Function (p<0.01, r = 0.41) HOOS-QOL (p<0.01, r = 0.41), and negatively associated with HOOS-Pain after walking (p<0.01, r = -0.45). Adjusting the analysis for walking speed did not affect the association.
Patients with the strongest hip abductor and hip flexor muscles had the best gait 'quality'. Furthermore, patients with higher physical function, quality of life scores and lower pain levels demonstrated better gait 'quality'. These findings indicate that interventions aimed at improving hip muscle strength and pain management may to a moderate degree improve the overall gait 'quality' in patients with primary hip OA.

The severity of an allergic reaction can range from mild local symptoms to anaphylactic shock. To score this, a number of instruments have been developed, although heterogeneous in design and purpose. Severity scoring algorithms are therefore difficult to compare, but are frequently used beyond their initial purpose. Our objective was to compare the most used severity scoring instruments by a data-driven approach on both milder reactions and anaphylaxis.
Methods
All positive challenges to foods or drugs (n = 2828) including anaphylaxis (n = 616) at Odense University Hospital, Denmark from 1998 to 2016 were included and severity was scored according to Sampson5. Based on recommendations from an expert group, the symptoms and values from Sampson5 were for all reactions and anaphylaxis only translated and compared by kappa statistics with 22 instruments, ranging from 3 to 6 steps.
Results
For milder reactions, there was a significant correlation between the number of steps in an instrument and the number of challenges that could be translated, whereas all instruments were good to identify food anaphylaxis. Some instruments scored reactions more severely than Sampson5, other scored them milder and some scored food and drug challenges differently. Instruments for hymenoptera reactions were difficult to apply on food and drug reactions, and thus distributed severity differently. Algorithms hampered the translation between instruments, and 7 instruments were poor concerning drug anaphylaxis, including the only instrument developed specifically for drug reactions.
Conclusion
The distributions of severity differed between the 23 instruments in both food and drug allergy, and thus rendering translation especially between scoring systems with 3 and 5 grades difficult. Fine-graded and simple instruments are preferred for comparison especially among milder reactions, and instruments applied to non-intended situations may not reflect a true severity picture.
