From Semantic Modeling to Precision Radiotherapy: An AI Framework Linking Radiobiology, Oncology, and Public Health Integration

Background/Objectives: Radiotherapy, radiobiology, and oncology have evolved rapidly over the past six decades. This progress has generated vast but fragmented bodies of scientific evidence. The present study aimed to systematically map and interpret their conceptual and temporal development using artificial intelligence (AI)-based methods. It highlights the integration between molecular mechanisms, clinical applications, and technological innovation within a precision radiotherapy framework. Methods: A corpus of 3343 unique articles (1964–2025) was retrieved from Scopus, PubMed, and Web of Science. Records were harmonized through deduplication, lemmatization, and metadata normalization. Topic modeling using Latent Dirichlet Allocation (LDA) and co-occurrence network analysis were applied to identify dominant research axes. Semantic and temporal analyses were conducted to reveal patterns, emerging trends, and translational connections across decades. Results: Three historical phases were identified. The first was a period of limited production (1964–1990). The second showed moderate growth (1991–2010). The third, from 2011 to 2024, represented exponential expansion, with publication peaks in 2020 and 2023. LDA revealed two principal axes. The first, a clinical–anatomical axis, focused on cancer sites, treatment modalities, and prognosis. The second, a mechanistic–molecular axis, centered on DNA repair, radiosensitivity, and biomarkers. Case synthesis from 2014–2025 defined five operational classes: DNA repair and molecular response; precision oncology and genomic modeling; individual radiosensitivity; mechanisms of radioresistance; and advanced technologies such as FLASH radiotherapy and optimized brachytherapy. Conclusions: AI-driven semantic and temporal analyses showed that radiotherapy has matured into an interconnected and interdisciplinary domain. The derived Precision Radiotherapy Implementation Plan translates molecular and computational insights into clinically actionable strategies. These approaches can enhance survival, reduce toxicity, and inform equitable health policies for advanced cancer care.

A Once-in-a-Generation Transformation at an Academic Public Hospital During Federal Funding Uncertainty

Fiscal and regulatory pressures on public hospitals and academic medical centers (AMC) will likely worsen in coming years, given looming changes to Medicaid and federal research funding. Recently, such pressures threatened to disrupt the 140-year affiliation between the Los Angeles General Medical Center (LA General) and the University of Southern California (USC). We sought to describe lessons learned during a two-year restructuring of this relationship, and to describe the impact of this transformation on patient care. Hospital-wide mortality metrics were extracted from the Vizient Clinical Database and Leapfrog hospital safety scores were retrieved from public releases. Citing increasing fiscal pressure, in March of 2023, USC noticed LA County that it was terminating the existing $170 million per year medical school affiliation agreement (MSAA) that funded >90% of the clinical staffing at LA General. In response, LA General began a national hiring campaign to ensure sufficient staffing irrespective of the outcome of ongoing negotiations. In 21 months, LA General hired 273 physicians, dentists, and allied health professionals across all 19 clinical departments. Meantime, LA General and USC evolved a shared understanding that a blended model of employment best served both organizations. Such a dramatic transformation of workforce in such a short period of time raised concerns about risks to patients. We therefore monitored hospital-wide mortality rate and Leapfrog patient safety scores during the hiring process. However, hospital-wide observed and observed/expected mortality rates significantly declined the quarter after hiring began, and Leapfrog safety scores rose during the hiring period. Public hospitals and AMCs can strengthen their relationships despite ongoing fiscal and regulatory pressures via intentional organization restructuring and cultural transformation. Such transformational efforts may be all the more important at a time of great flux and uncertainty in government funding for clinical care and academic research.

Second Primary Malignant Neoplasms in Survivors of Retinoblastoma in a Single Ocular Oncology Practice

A retrospective review of patients treated for retinoblastoma who developed a non-pineoblastoma second primary malignant neoplasm (SPMN) was performed.
The demographics, clinical features and treatments for retinoblastoma, pathologic types of non-pineoblastoma second primary malignant neoplasm (SPMN), intervals between the retinoblastoma diagnosis and treatment and diagnosis of non-pineoblastoma SPMN, treatment provided for the SPMN, and the survival outcomes of the patients were evaluated.
Of 550 patients treated initially for retinoblastoma, this series used the 15 (2.7) that developed a non-pineoblastoma SPMN, 14 of which (93.3%) had been treated for bilateral retinoblastoma. All patients had carried a germline mutations in the
gene. The median time from retinoblastoma diagnosis to SPMN diagnosis was 19.0 years (extremes 3.4 and 39.4 years). Six of the fifteen patients died during the follow-up of their SPMN. The median interval between initial retinoblastoma diagnosis and death in the 6 patients who died of their SPMN was 18.8 years (extremes 6.2 and 34.6 years) and between diagnosis of the SPMN and death was 1.2 years (extremes 0.25 and 4 years).
Of the patients who had been treated with External Beam Radiotherapy (EBRT), 13 developed a SPMN within the previously irradiated field.

The city and the coming climate : climate change in the places we live
\"In the first decade of this century, for the first time in history, the majority of the planet's population resided in cities. We are an urban planet. If ongoing changes in climate are to have an impact on the human species, most of these impacts will play out in cities. This fact was brought into full relief in the summer of 2003, when more than 70,000 residents of Europe perished in one of the most prolonged and intense heat waves in human history. The final death toll would exceed that associated with any Western European or American conflict since World War II, or any other natural disaster to have ever struck a region of the developed world, and the vast majority of these deaths occurred in cities. Studies in the aftermath of the heat wave would show that not only had global warming increased the likelihood of such an extreme event, but that the intensity of the heat had been greatly enhanced by the physical design of the cities themselves, exposing residents of cities to a much greater risk of illness or death than others. This book is the first to explore the dramatic amplification of global warming underway in cities and the range of actions that can be taken to slow the pace of warming. Molecular characterization and phylogenetic analysis of transmissible gastroenteritis virus HX strain isolated from China

BACKGROUND: Porcine transmissible gastroenteritis virus (TGEV) is the major etiological agent of viral enteritis and severe diarrhea in suckling piglets. In China, TGEV has caused great economic losses, but its role in epidemic diarrhea is unclear. This study aims to reveal the etiological role of TGEV in piglet diarrhea via molecular characterization and phylogenetic analysis. RESULTS: A TGEV-HX strain was isolated from China, and its complete genome was amplified, cloned, and sequenced. Sequence analysis indicated that it was conserved in the 5′ and 3′-non-translated regions, and there were no insertions or deletions in nonstructural genes, such as ORF1a, ORF1b, ORF3a, ORF3b, and ORF7, as well as in genes encoding structural proteins, such as the envelope (E), membrane (M), and nucleoprotein (N) proteins. Furthermore, the phylogenetic analysis indicated that the TGEV-HX strain was more similar to the TGEV Purdue cluster than to the Miller cluster. CONCLUSIONS: The present study described the isolation and genetic characterization of a TGEV-HX strain. The detailed analysis of the genetic variation of TGEVs in China provides essential information for further understanding the evolution of TGEVs.
