Anti-microbial efficacy of Vanilla planifolia leaf extract against common oral micro-biomes: A comparative study of two different antibiotic sensitivity tests
Introduction: Over the past century, several antibiotics have been discovered and used to treat various microbial diseases. However, over the past few decades, with the emergence of anti-microbial resistant strains of microbiomes, it has become increasingly necessary to discover and develop alternative anti-microbial agents. Herbal formulations have shown promising results in the past decade. However, many herbal formulations remain unexplored. The present study aims to explore the anti-microbial properties of a newly prepared Vanilla planifolia extract. Methodology: Vanilla planifolia leaves were collected, shade-dried, and then powdered. The powdered leaves of Vanilla planifolia (100 gm) were extracted by the cold percolation method with 300 ml ethanol at room temperature for 72 hours. The extracts were then tested for its constituent anti-microbial activity by the agar well method and disk diffusion method against different commonly found oral micro-biomes. The zones of inhibition were noted and measured, and the results were derived. Statistical analysis was performed using the Student t-test (P <_ 0.001). Based on the statistical analysis, conclusions were drawn. Results: The ethanolic extracts of Vanilla planifolia on the agar plates showed considerable anti-microbial activity in both the test methods against Staphylococcus aureus, Streptococcus mutans, and Enterococcus. However, no effect was found against Candida albicans. There was no significant difference in the results obtained by the two test methods (P >_ 0.001). Conclusions: This experimental study presents a medicinal plant, an orchid Vanilla planifolia, which demonstrates the presence of essential anti-microbial agents in it, making it a potent, potential dental biomaterial with a positive and benefitting effect on the oral micro-environment.
Chemical (Alkali) Burn-Induced Neurotrophic Keratitis Model in New Zealand Rabbit Investigated Using Medical Clinical Readouts and In Vivo Confocal Microscopy (IVCM)
Purpose: Chemical eye injury is an acute emergency that can result in vision loss. Neurotrophic keratitis (NK) is the most common long-term manifestation of chemical injury. NK due to alkali burn affects ocular surface health and is one of its most common causes. Here, we established a rabbit model of corneal alkali burns to evaluate the severity of NK-associated changes. Material methods: Alkali burns were induced in NZ rabbits by treating the cornea with (i) a 5 mm circular filter paper soaked in 0.75 N NaOH for 10 s (Mild NK) and (ii) trephination using a guarded trephine (5 mm diameter and 150-micron depth), followed by alkali burn, with a 5 mm circular filter paper soaked in 0.75 N NaOH for 10 s (a severe form of NK). Immediately after, the cornea was rinsed with 10 mL of normal saline to remove traces of NaOH. Clinical features were evaluated on Day 0, Day 1, Day 7, Day 15, and Day 21 post-alkali burn using a slit lamp, Pentacam, and anterior segment optical coherence tomography (AS-OCT). NK-like changes in epithelium, sub-basal nerve plexus, and stroma were observed using in vivo confocal microscopy (IVCM), and corneal sensation were measured using an aesthesiometer post alkali injury. After 21 days, pro-inflammatory cytokines were evaluated for inflammation through ELISA. Results: Trephination followed by alkali burn resulted in the loss of epithelial layers (manifested using fluorescein stain), extensive edema, and increased corneal thickness (550 µm compared to 380 µm thickness of control) evaluated through AS-OCT and increased opacity score in alkali-treated rabbit (80 compared to 16 controls). IVCM images showed complete loss of nerve fibers, which failed to regenerate over 30 days, and loss of corneal sensation—conditions associated with NK. Cytokines evaluation of IL6, VEGF, and MMP9 indicated an increased angiogenic and pro-inflammatory milieu compared to the milder form of NK and the control. Discussion: Using clinical parameters, we demonstrated that the alkali-treated rabbit model depicts features of NK. Using IVCM in the NaOH burn animal model, we demonstrated a complete loss of nerve fibers with poor self-healing capability associated with sub-basal nerve degeneration and compromised corneal sensation. This pre-clinical rabbit model has implications for future pre-clinical research in neurotrophic keratitis.
Evaluation of the re-mineralization capacity of a gold nanoparticle-based dental varnish: An in vitro study
Background: Dental caries is an infectious microbial disease caused by acidogenic bacteria. It leads to the dissolution of enamel, dentin, and cementum. Enamel demineralization is often appreciated as' 'White Chalky lesions or Chalky enamel''. Standard procedures for protection of these teeth are fissure sealing and topical fluoride application. A varnish is generally a material in which a resin such as copal is dissolved within an organic solvent such as ethanol. Gold is one of the most biocompatible dental materials. Gold nanoparticles were biosynthesised using aspartic acid in previous studies. Aim and Objectives: To prepare a gold nanoparticle based dental varnish and to evaluate its re-mineralizing capacity. Materials and Methods: Gold nanoparticle dental varnish was prepared using all the necessary constituents. This newly prepared dental varnish was compared with G.C Fuji/SnF2 dental varnish. Demineralizing capacity of the dental varnishes were analysed. The tooth specimens were prepared according to methodology and mounted on resin blocks. They were subjected to demineralization remineralization cycles. ICP-OES and Knoop Hardness tests were performed. Results: AuNP dental varnish had a satisfactory remineralization effect on demineralised enamel. For calcium analysis, the AuNP group showed significantly more total calcium loss when compared to the SnF2 group (P < 0.05) and was statistically significant. For phosphorus analysis, AuNP group showed significantly greater net phosphorus loss compared to the SnF2 group (P < 0.05) and was statistically significant. It was also observed that the KHN increased more significantly in Group A (SnF2) as compared to Group B (AuNP) and was also statistically significant (P < 0.05). Conclusion: AuNP dental varnish showed considerable re mineralizing property; however, it was not superior to dental varnishes like SnF2 dental varnish. Further research needs to be done in vitro to better modify the AuNP dental varnish before proceeding to in vivo studies. Effectiveness of comprehensive oral care on Bradburn scale of psychological well-being among young adult smokers and tobacco chewers: A prospective clinical study
Aim: To assess the benefits of comprehensive oral care in improving the psychological well-being of young adult smokers and tobacco chewers. Materials and Methods: A prospective, clinical study of 60 young adult smokers/tobacco chewers or both were carried out. This study was conducted to assess the psychological well-being at the first dental visit, and then after the completion of comprehensive oral care using Bradburn scale of psychological well-being (Affect Balance Scale). Comprehensive oral care included oral prophylaxis, simple restorative care, preventive procedures, and health education. Health education was given before the appointment, during the stage of diagnosis and treatment planning, in the operatory during the various stages of treatment. Results: A total of 60 males were participated in this clinical study, with a minimum age of 18 years to the maximum age of 25 years. A total of 20 (33.3%) were using both smokeless and smoking tobacco. The majority of them were school educated (74%) and semi-skilled (86%). The overall oral hygiene status was fair. The mean decayed, missing, and filled teeth (DMFT) ranged from 0 to 9. The Bradburn scale of psychological well-being score was 1.55 ± 2.54 at the first visit to the dentist and after completing the comprehensive oral care there was an improvement in the score (3.20 ± 1.29). This was statistically significant. Conclusion: This study findings showed improvement in the psychological well-being of young adult smokers and tobacco chewers after the completion of comprehensive oral care.

Clinical quantitative antibacterial potency of garlic-lemon against sodium hypochlorite in infected root canals: A double-blinded, randomized, controlled clinical trial
Introduction: Sodium hypochlorite for endodontic treatment has been shown to exhibit significant antimicrobial properties, with adverse effects such as sodium hypochlorite accidents. Natural irrigants have shown significant antimicrobial action and the added advantage of being biocompatible. This study proposes an alternative intracanal irrigant made from Garlic-Lemon (Ga-Li) extract. Aim: To evaluate the antimicrobial action of 1.8% Garlic-Lemon (Ga-Li) mixture in contrast to 3% sodium hypochlorite in a tooth diagnosed with asymptomatic apical periodontitis. Materials and Methods: Thirty patients were randomly allocated into two groups: Group A, 3% sodium hypochlorite and Group B, 1.8% Garlic-Lemon. Single- or multirooted teeth root canals were instrumented and prepared by using ProTaper Gold. Root canal samples were taken both pre- and postinstrumentation. These samples were subjected to DNA extraction, amplification, and quantification by using a real-time polymerase chain reaction (qPCR). Results: Samples before preparation (S1) were tested positive for microbial presence, with mean numbers of 7.0 ×107 and 12.4 ×107 bacterial cells for the sodium hypochlorite and Garlic-Lemon groups, respectively. Postpreparation (S2), in sodium hypochlorite and Garlic-Lemon groups, bacterial counts were still present with mean counts seen at 27.4 ×105 and 7.7 ×105 bacterial cells, respectively. Intergroup comparison resulted in a statistically insignificant difference (P > 0.05). Conclusion: Garlic-Lemon has shown microbial load reduction that is as effective as sodium hypochlorite, with the highest mean bacterial reduction percentage. The results of the present randomized, controlled clinical trial suggest that Garlic-Lemon is a potential new alternative as an endodontic irrigant.
Asymptotic analysis of large antenna arrays for communications and radar applications
In recent years there has been a growing interest in using antenna arrays at both ends of a wireless communication link. Such multiple input multiple output (MIMO) systems are beneficial both in terms of providing greatly improved data rates, as well as in terms of robustness in combating errors compared to systems which use only one antenna. These benefits are obtained without requiring extra transmit power or spectral bandwidth, but come at the cost of additional processing power. In radar, multiple antenna arrays have been in use for several decades. Even so, the idea of measuring the full received electro-magnetic (EM) wave for parameter estimation has been a recent one. In this dissertation, we address two issues through asymptotics: in MIMO systems, we develop insights into finite NAMO array performance by deriving precise results for asymptotically large MIMO arrays, and in radar we derive the gain from measuring the complete field over a spherical surface versus measuring only one polarization component using an equal number of sensors. First, we consider the distribution of the mutual information of a MIMO system with an uncorrelated Rayleigh fading channel. We show that, as the transmit and receive array sizes tend to infinity while maintaining their ratio constant, the mutual information distribution tends to Gaussian distribution at all signal to noise ratios (SNRs), and give a closed-form expression for its mean and variance. Through simulations, we observe that the mutual information distribution of a finite MIMO system with as few as 4 array elements at either end has a variance which depends only on the ratio of the two arrays and is also closely approximated by the asymptotic distribution variance. We show that the mean of the distribution can also be approximated much closer than previously shown, and hence combined with the asymptotic variance, this yields close approximations for outage capacities. We next consider the problem of determining the best possible tradeoff between diversity and multiplexing gains in an uncorrelated Rayleigh fading channel. Zheng and Tse have characterized this tradeoff in the large signal to noise ratio (SNR) limit. (Abstract shortened by UMI.)
Soft tissue coverage in open fractures of tibia
Background The treatment of Gustilo Anderson type 3B open fracture tibia is a major challenge and it needs aggressive debridement, adequate fixation, and early flap coverage of soft tissue defect. The flaps could be either nonmicrovascular which are technically less demanding or microvascular which has steep learning curve and available only in few centers. An orthopedic surgeon with basic knowledge of the local vascular anatomy required to harvest an appropriate local or regional flap will be able to manage a vast majority of open fracture tibia, leaving the very few complicated cases needing a free microvascular flap to be referred to specialized tertiary center. This logical approach to the common problem will also lessen the burden on the higher tertiary centers. We report a retrospective study of open fractures of leg treated by nonmicrovascular flaps to analyze (1) the role of nonmicrovascular flap coverage in type 3B open tibial fractures; (2) to suggest a simple algorithm of different nonmicrovascular flaps in different zones and compartment of the leg, and to (3) analyze the final outcome with regards to time taken for union and complications. Materials and Methods One hundered and fifty one cases of Gustilo Anderson type 3B open fracture tibia which needed flap cover for soft tissue injury were included in the study. Ninety four cases were treated in acute stage by debridement; fracture fixation and early flap cover within 10 days. Thirty-eight cases were treated between 10 days to 6 weeks in subacute stage. The rest 19 cases were treated in chronic stage after 6 weeks. The soft tissue defect was treated by various nonmicrovascular flaps depending on the location of the defect. Results All 151 cases were followed till the raw areas were covered. In seven cases secondary flaps were required when the primary flaps failed either totally or partially. Ten patients underwent amputation. Twenty-two patients were lost to followup after the wound coverage. Out of the remaining 119 patients, 76 achieved primary acceptable union and 43 patients went into delayed or nonunion. These 43 patients needed secondary reconstructive surgery for fracture union. Conclusion open fracture of the tibia which needs flap coverage should be treated with high priority of radical early debridement, rigid fixation, and early flap coverage. A majority of these wounds can be satisfactorily covered with local or regional nonmicrovascular flaps.
