The relationship between a plant-based diet and mental health: Evidence from a cross-sectional multicentric community trial (LIPOKAP study)
Dietary patterns emphasizing plant foods might be neuroprotective and exert health benefits on mental health. However, there is a paucity of evidence on the association between a plant-based dietary index and mental health measures. This study sought to examine the association between plant-based dietary indices, depression and anxiety in a large multicentric sample of Iranian adults. This cross-sectional study was performed in a sample of 2,033 participants. A validated food frequency questionnaire was used to evaluate dietary intakes of participants. Three versions of PDI including an overall PDI, a healthy PDI (hPDI), and an unhealthy PDI (uPDI) were created. The presence of anxiety and depression was examined via a validated Iranian version of the Hospital Anxiety and Depression Scale (HADS). PDI and hPDI were not associated to depression and anxiety after adjustment for potential covariates (age, sex, energy, marital status, physical activity level and smoking). However, in the crude model, the highest consumption of uPDI approximately doubled the risk of depression (OR= 2.07, 95% CI: 1.49, 2.87; P<0.0001) and increased the risk of anxiety by almost 50% (OR= 1.56, 95% CI: 1.14, 2.14; P= 0.001). Adjustment for potential confounders just slightly changed the associations (OR for depression in the fourth quartile= 1.96; 95% CI: 1.34, 2.85, and OR for anxiety in the fourth quartile= 1.53; 95% CI: 1.07, 2.19). An unhealthy plant-based dietary index is associated with a higher risk of depression and anxiety, while plant-based dietary index and healthy plant-based dietary index were not associated to depression and anxiety.
Relationship of the Prime Diet Quality Score (PDQS) and Healthy Eating Index (HEI-2015) with depression and anxiety: a cross-sectional study
Background Previous studies have suggested a significant association between diet quality and mental health. However, a limited number of studies have utilized the Prime Diet Quality Score (PDQS) to examine this association. Additionally, no study has yet compared the PDQS and Healthy Eating Index-2015 (HEI-2015) in terms of their association with depression and anxiety. Aim This cross-sectional study aimed to investigate the association between diet quality, measured by the PDQS and HEI-2015, and the odds of depression and anxiety in adults. Methods This study utilized data from the LIPOKAP study, which was conducted between February 2018 and July 2019 in five cities in Iran. We included 1994 adults aged 18 and above who were selected through a multistage cluster sampling method. Participants completed a validated semiquantitative food frequency questionnaire (FFQ) to evaluate their dietary intake. The FFQ was used to calculate the PDQS and HEI-2015. Depression and anxiety levels were determined using the Hospital Anxiety and Depression Scale (HADS). Results The participants had a mean age of 39.79 ± 13.87 years, with females accounting for 1,041 (52.2%) of the total population. PDQS showed a significant inverse association with the odds of depression (OR = 0.45, 95% CI: 0.28–0.71) and anxiety (OR = 0.40, 95% CI: 0.25–0.62) in the fully adjusted model. Similarly, the highest quartile of HEI-2015 showed a significantly lower odds of depression (OR = 0.60, 95% CI: 0.40–0.90) and anxiety (OR = 0.62, 95% CI: 0.42–0.92) compared to the lowest quartile. Conclusion Both PDQS and HEI-2015 were associated with a reduced risk of depression and anxiety. However, the PDQS demonstrated a stronger inverse association with these risks compared to the HEI-2015. This suggests that the PDQS could be a more beneficial dietary pattern for preventing depression and anxiety compared to HEI-2015. Further large-scale studies are required to confirm these findings.
Association between ultra-processed foods consumption and micronutrient intake and diet quality in Iranian adults: a multicentric study
To identify ultra-processed foods (UPF) contribution to daily energy and nutrient intake in Iranians and examine whether UPF intake is associated with nutrient profile and diet quality. In this cross-sectional study, a validated FFQ was used to evaluate usual dietary intake over the preceding year. NOVA system was applied to categorise foods based on their levels of processing. Diet quality was evaluated using the nutrient adequacy ratio (NAR), Nutrient Rich Food Index (NRF) and hybrid nutrient density. The LIPOKAP study conducted in five cities of Iran (Isfahan, Birjand, Bandar Abbas, Kermanshah and Shahrekord). A total of 1994 adults aged ≥18 years were recruited using stratified multistage random cluster sampling method. UPF were responsible for 8·5 % of daily energy intake. In the adjusted model, UPF consumption was inversely associated with carbohydrate, protein, refined and whole grains, fibre, fruit and meat, but was positively linked to energy, total fat, saturated and trans fatty acids and cholesterol. Compared with those in the lowest tertile, individuals in the highest tertile of UPF had smaller NAR for Ca, Mg, Zn, Fe, phosphorus, thiamin, niacin, folate and vitamin C. Both NRF and hybrid nutrient density decreased when the share of daily energy intake from UPF increased. The higher consumption of UPF is associated with poorer diet quality and lower nutrient intake. It is recommended that UPF be replaced with minimally processed foods to improve diet quality and nutrient profile.
The influential role of parents’ socioeconomic status and diet quality on their children’s dietary behavior: results from the LIPOKAP study among the Iranian population
Purpose There is a paucity of evidence regarding the influential factors on Iranian children’s diet quality. To assess this issue, we explore the relationship between parental diet quality, socioeconomic status (SES), and nutritional knowledge with their children’s diet quality using a cross-sectional sample of the Iranian population. Methods In this study, paired parents along with one of their children (aged 6–18 years old) who lived with them were included. Dietary intake was assessed using a food frequency questionnaire (FFQ) and diet quality was determined using an Alternative Healthy Eating Index-2010 (AHEI). Nutritional knowledge and SES were also explored using validated questionnaires. Multiple linear regression analysis was used and beta (β) and corresponding 95% confidence interval (CI) were reported. Results After controlling for potential confounders, the parents’ AHEI was significantly associated with their children’s diet quality (β = 12.34, 95% CI: 10.75, 13.93; P < 0.001). The nutritional knowledge of parents was significantly associated with children’s AHEI after controlling for potential confounders (β = 1.63, 95% CI: 0.14, 3.11; P = 0.032). Moreover, the parents’ SES was inversely associated with the AHEI of children (β=-3.76, 95% CI: -5.40, -2.11; P < 0.001); however, further adjustment for confounders attenuated this relationship (Model 3: β = 0.87, 95% CI: -0.76, 2.34; P = 0.269). Conclusions We found that the children’s diet quality could be influenced by their parents’ SES, nutritional knowledge, and diet quality. Our findings suggest that improving the nutritional knowledge of parents not only may improve the healthy eating pattern of parents but also could influence their children’s diet quality.
Unusual and very rare presentation of wrist pain in a man with acute anterior myocardial infarction, a case report and literature of review
Background As acute myocardial infarction (AMI) prevalence is increasing because of lifestyle changes, the incidence of atypical symptoms in acute coronary syndrome (ACS) is rising and making misdiagnosing of this fatal event more probable. To better approach the patients with atypical symptoms, we tend to present a rare case of AMI with wrist pain. Case report A 41-year-old man presented to the emergency room (ER) with severe both-hand wrist pain and mild epigastric pain. His electrocardiogram (ECG) showed anterior ST-elevation myocardial infarction (MI) with an ejection fraction of 35–40%. His angiography showed severe left anterior descending artery (LAD), and first obtuse marginal artery (OM1) artery stenosis. He underwent Primary percutaneous coronary intervention (PCI). The patient recovered without serious complications and was discharged the day after PCI. Discussion In this rare case of AMI with wrist pain, it is important to know that atypical symptoms can be present at various levels of symptoms, which prevents future misdiagnosis.
National program for cardiovascular disease and hypertension in primary healthcare: WHO-PEN program initial scale-up in Iran (IraPEN)
Introduction We evaluated the IraPEN program, a localized adaptation of the WHO PEN initiative in Iran, with respect to cardiovascular disease and hypertension screening and management. Methods A cross-sectional ecological study utilizing the Input-Process-Output-Outcome framework was conducted during 2020–2021. Aggregated data from non-communicable disease (NCD) facilities were collected and analyzed by urban/rural primary healthcare settings. Results The study evaluated 610 NCD facilities, serving 1.8 million adults aged ≥ 30, of whom 515,488 underwent 10-year WHO/ISH CVD risk scoring and hypertension assessment, indicating coverage of 30%. Input/process showed all NCD facilities had functional blood pressure devices, 87% had POCT for measurement of glucose and cholesterol and 14% experienced medication stock-outs in the past three months. All staff received CVD risk scoring training, with 60% re-trained during the last two years. Output/outcome indicators revealed 3% of individuals were at moderate risk (CVD risk 10–19%), 0.6% at high risk (20–29%), 0.9% at very high risk (≥ 30%), and 2.7% had a prior CVD history. Prevalence of hypertension was 17%, compared with a national survey, indicating an observed-to-expected ratio of 55%. 80% of patients with hypertension received blood pressure-lowering medications, and 74% of them achieved blood pressure control; 38% of CVD patients received antiplatelet/anticoagulant therapy, and 66% of eligible patients took statins. Conclusion IraPEN demonstrated low coverage for CVD risk scoring and hypertension detection but acceptable treatment and control rates. Management of patients with CVD seems suboptimal. The WHO/ISH CVD risk scoring may lack in detecting high and very high-risk individuals.
The relationship between nut consumption and premature coronary artery disease in a representative sample of Iranians: Iran-premature coronary artery disease (IPAD) study
The cardioprotective effects of nuts are well established. However, the positive impacts of nuts in preventing CVD at a younger age, a condition known as premature coronary artery disease (PCAD), is still debated. Therefore, we aim to determine the association between nuts and PCAD occurrence and its severity in different Iranian ethnicities. This case-control study was conducted within the framework of the Iran-premature coronary artery disease (I-PAD) study, an ongoing multi-centric study on Iranian patients of different ethnicities. This multi-centric case-control study was conducted in among 3253 persons under the age of 70 years in women and 60 years in men from different ethnicities in Iran. Information on nut consumption was collected using a validated FFQ. Subjects were selected from among the candidates for angiography. Cases were those whose coronary angiography showed stenosis of more than 75 % in at least one vessel or more than 50 % of the left main artery, while the control group participants had normal angiography results. In the crude model, compared to the first quartile, the highest quartile of nut consumption was significantly associated with a lower risk of PCAD (OR = 0·26, 95 % CI (0·21, 0·32); = 0·001). In the top quartile of nut intake, a substantial decrease in PCAD was observed after controlling for putative confounders (OR = 0·32; 95 % CI (0·24, 0·43); = 0·001). Additionally, a 75 % decrease in the risk of severe PCAD was observed in the participants in the highest quartile of nut intake. A significant inverse association was observed between nut intake and the risk and severity of PCAD in the Iranian population. Large-scale clinical trials are required to confirm these findings.
Antioxidant Properties of Saffron Stigma and Petals: A Potential Therapeutic Approach for Insulin Resistance through an Insulin-Sensitizing Adipocytokine in High-Calorie Diet Rats
Oxidative stress and alteration of lipid profile due to obesity and overweight is a major risk factor for atherosclerotic plaque or coronary artery disease. Because of antioxidant and lipid lowering potential of saffron, this study investigated weight alteration, lipid profiles, and insulin resistance index in high-calorie diet rats treated with aqueous extract of saffron stigma and petal. Forty Sprague-Dawley male rats were randomly divided into 8 groups including healthy control, high-fat diet control, nicotinic acid treated, treated, and saffron stigma and petal treated groups. Rats received a high-calorie diet for 16 weeks. For treatment, aqueous extract of saffron stigma (40 and 80 mg/kg) and petal (50 and 100 mg/kg) was used once daily for 4 weeks. Afterward, lipid profile, oxidative stress status, and insulin and adiponectin levels were measured using desired kits. There was a significant decrease in the mean weight of the groups receiving saffron stigma and petal compared to control group ( < 0.05). The increased level of insulin hormone in obese group was improved in treated groups especially in the case of saffron stigma. Also, the decreased level of adiponectin was recovered in treated groups. An improvement was seen in oxidative stress markers and lipid profiles in treated groups compared to obesity pair. In this study, a remarkable antioxidant and lipid lowering potential was detected for saffron stigma, which could improve insulin resistance in obese rats. Therapeutic and protective effect of saffron is mainly related to its richness in phenolic compounds. Saffron stigma compared with petal had more notable effect, which could and should be mentioned in pharmaceutical studies.
The relationship between cognitive perception of self‐concept and coping styles in heart failure patients
Aim To determine the relationship between self‐concept and coping styles in patients with heart failure in an Iranian population. Design This study had a correlational design. Methods In this study, 100 HF patients hospitalized in the CCU and cardiac ward of Vali‐Asr Hospital of Birjand, Iran were selected by convenience sampling method. Two validated and reliable questionnaires including Cognitive Perception of Self‐Concept and the Coping Styles questionnaires were completed by each patient. Results There were significant associations between emotion‐oriented and threat to self‐concept in total and physical sensation and self‐consistency dimensions. Also, a significant association was observed between avoidance‐oriented and threat to self‐concept in total and body sensation dimension (p < .05). There was no significant association between the problem‐oriented component and the threat to self‐concept in total and none of its dimensions. The challenge to self‐concept and its dimensions were not significantly related to any component of coping styles.
Association of the ANGPTL3 gene polymorphisms and haplotypes with cardiovascular diseases in Birjand longitudinal aging study
Subject Totally, 360 individuals were recruited in baseline of BLAS including 128 patients with CVD and 153 control subjects. DNA extraction of samples and genotyping were performed by Tetra-ARMS PCR (amplification refractory mutation system polymerase chain reaction). The association between three polymorphisms of ANGPTL3 gene (rs1748195, rs11207997, and rs10789117 variants) with CVD and its risk factors were evaluated using multivariate analysis. Univariate and multiple analyses showed that individuals carrying the GG genotype of rs1748195 and those carrying the TT genotype of rs11207997 directly increased the risk of CVD. CC genotype of rs1748195 and rs11207997 polymorphisms had a significant negative relationship with the disease. In addition, the findings of this study indicate a significant difference in LDL, HDL, cholesterol levels between different genotypes of the rs1748195 and rs10789117 in the healthy group. Individuals with haplotypes CAC, CCC and CCT showed a significant positive relationship with CVD, CVA, AMI and CHD. As well as haplotype was associated with a 1.7-fold increase in risk of CVA, AMI, and CHD. We found that polymorphism of ANGPTL3 gene might support to identify individuals with a cardiometabolic and genetic disorders susceptibility. Three haplotypes CAC, CCC and CCT associated with CVD, CVA, AMI and CHD were reported.
