Systematic review: emotion dysregulation and challenging behavior interventions for children and adolescents on the autism spectrum with graded key evidence-based strategy recommendations
Challenging behavior, such as aggression, is highly prevalent in children and adolescents on the autism spectrum and can have a devastating impact. Previous reviews of challenging behavior interventions did not include interventions targeting emotion dysregulation, a common cause of challenging behavior. We reviewed emotion dysregulation and challenging behavior interventions for preschoolers to adolescents to determine which evidence-based strategies have the most empirical support for reducing/preventing emotion dysregulation/challenging behavior. We reviewed 95 studies, including 29 group and 66 single case designs. We excluded non-behavioral/psychosocial interventions and those targeting internalizing symptoms only. We applied a coding system to identify discrete strategies based on autism practice guidelines with the addition of strategies common in childhood mental health disorders, and an evidence grading system. Strategies with the highest quality evidence (multiple randomized controlled trials with low bias risk) were Parent-Implemented Intervention, Emotion Regulation Training, Reinforcement, Visual Supports, Cognitive Behavioral/Instructional Strategies and Antecedent-Based Interventions. Regarding outcomes, most studies included challenging behavior measures, while few included emotion dysregulation measures. This review highlights the importance of teaching emotion regulation skills explicitly, positively reinforcing replacement/alternative behaviors, using visuals and metacognition, addressing stressors proactively, and involving parents. It also calls for more rigorously designed studies and for including emotion dysregulation as an outcome/mediator in future trials.
Advancing SAR Target Recognition Through Hierarchical Self-Supervised Learning with Multi-Task Pretext Training
Synthetic Aperture Radar (SAR) Automatic Target Recognition (ATR) systems face significant challenges due to limited labeled data availability and persistent domain gaps between synthetic and measured imagery. This paper presents a comprehensive self-supervised learning (SSL) framework that eliminates dependency on synthetic data while achieving state-of-the-art performance through multi-task pretext training and extensive downstream classifier evaluation. We systematically evaluate our SSL framework across diverse downstream classifiers spanning different computational paradigms and architectural families. Our study encompasses traditional machine learning approaches (SVM, Random Forest, XGBoost, Gradient Boosting), deep convolutional neural networks (ResNet, U-Net, MobileNet, EfficientNet), and a generative adversarial network. We conduct extensive experiments using the SAMPLE dataset with rigorous evaluation protocols. Results demonstrate that SSL significantly improves SAR ATR performance, with SVM achieving 99.63% accuracy, ResNet18 reaching 97.40% accuracy, and Random Forest demonstrating 99.26% accuracy. Our multi-task SSL framework employs nine carefully designed pretext tasks, including geometric invariance, signal robustness, and multi-scale analysis. Cross-validation experiments validate the generalizability and robustness of our findings. Rigorous comparison with SimCLR baseline validates that task-based SSL outperforms contrastive learning for SAR ATR. This work establishes a new paradigm for SAR ATR that leverages inherent radar data structure without synthetic augmentation, providing practical guidelines for deploying SSL-based SAR ATR systems and a foundation for future domain-specific self-supervised learning research in remote sensing applications.
The Gut Microbiota (Microbiome) in Cardiovascular Disease and Its Therapeutic Regulation
In the last two decades, considerable interest has been shown in understanding the development of the gut microbiota and its internal and external effects on the intestine, as well as the risk factors for cardiovascular diseases (CVDs) such as metabolic syndrome. The intestinal microbiota plays a pivotal role in human health and disease. Recent studies revealed that the gut microbiota can affect the host body. CVDs are a leading cause of morbidity and mortality, and patients favor death over chronic kidney disease. For the function of gut microbiota in the host, molecules have to penetrate the intestinal epithelium or the surface cells of the host. Gut microbiota can utilize trimethylamine, N-oxide, short-chain fatty acids, and primary and secondary bile acid pathways. By affecting these living cells, the gut microbiota can cause heart failure, atherosclerosis, hypertension, myocardial fibrosis, myocardial infarction, and coronary artery disease. Previous studies of the gut microbiota and its relation to stroke pathogenesis and its consequences can provide new therapeutic prospects. This review highlights the interplay between the microbiota and its metabolites and addresses related interventions for the treatment of CVDs.
Comprehensive Review on Phytochemicals, Pharmacological and Clinical Potentials of Gymnema sylvestre
Gymnema sylvestre is a plant included in Apocynaceae family and is located in many regions of Asia, Africa and Australia. This plant is widely used as a traditional therapy for different purposes. Even now it is being used as a dietary supplement due to its numerous therapeutic uses. It is known to have blood glucose lowering potential and, thus, is widely used in traditional and Ayurvedic systems of medicine. It renders glucose lowering activity due to the presence of phytochemicals, such as gurmarin, gymnemic acid as well as gymnemasaponins. Gymnema sylvestre is also known to have anti-oxidant, antibiotic, anti-inflammatory, antiviral, gastro and hepatoprotective, anticancer and lipid-lowering activities. This review discusses in details on different pharmacological and clinical potentials of Gymnema sylvestre and its chemical constituents associated with its therapeutic potentials.
Dengue Fever: Therapeutic Potential of Carica papaya L. Leaves
Dengue, a very widespread mosquito-borne infectious disease caused by Aedes aegypti virus, has been occurring during the monsoons every year. The prevalence and incidence of dengue fever and death due to its complications have been increased drastically in these recent years in Bangladesh, Philippines, Thailand, Brazil, and India. Recently, dengue had spread in an epidemic form in Bangladesh, Thailand, and Philippines. Although the infection affected a large number of people around the world, there is no established specific and effective treatment by synthetic medicines. In this subcontinent, Malaysia could effectively control its incidences and death of patients using alternative medication treatment mainly prepared from Carica papaya L . leaves along with proper care and hospitalization. Papaya leaves, their juice or extract, as well as their different forms of preparation have long been used traditionally for treating dengue fever and its complications to save patients’ lives. Although it is recommended by traditional healers, and the general public use Papaya leaves juice or their other preparations in dengue fever, this treatment option is strictly denied by the physicians offering treatment in hospitals in Bangladesh as they do not believe in the effectiveness of papaya leaves, thus suggesting to patients that they should not use them. In Bangladesh, 1,01,354 dengue patients have been hospitalized, with 179 deaths in the year 2019 according to information from the Institute of Epidemiology, Disease Control, and Research as well as the Directorate General of Health Services of Bangladesh. Most of the patients died because of the falling down of platelets to dangerous levels and hemorrhage or serious bleeding. Therefore, this paper aims to critically review the scientific basis and effectiveness of Carica papaya L. leaves in treating dengue fever based on preclinical and clinical reports. Thrombocytopenia is one of the major conditions that is typical in cases of dengue infection. Besides, the infection and impairment of immunity are concerned with dengue patients. This review summarizes all the scientific reports on Carica papaya L. for its ability on three aspects of dengue: antiviral activities, prevention of thrombocytopenia and improvement of immunity during dengue fever.
Emerging Role of Neuron-Glia in Neurological Disorders: At a Glance
Based on the diverse physiological influence, the impact of glial cells has become much more evident on neurological illnesses, resulting in the origins of many diseases appearing to be more convoluted than previously happened. Since neurological disorders are often random and unknown, hence the construction of animal models is difficult to build, representing a small fraction of people with a gene mutation. As a result, an immediate necessity is grown to work within in vitro techniques for examining these illnesses. As the scientific community recognizes cell-autonomous contributions to a variety of central nervous system illnesses, therapeutic techniques involving stem cells for treating neurological diseases are gaining traction. The use of stem cells derived from a variety of sources is increasingly being used to replace both neuronal and glial tissue. The brain’s energy demands necessitate the reliance of neurons on glial cells in order for it to function properly. Furthermore, glial cells have diverse functions in terms of regulating their own metabolic activities, as well as collaborating with neurons via secreted signaling or guidance molecules, forming a complex network of neuron-glial connections in health and sickness. Emerging data reveals that metabolic changes in glial cells can cause morphological and functional changes in conjunction with neuronal dysfunction under disease situations, highlighting the importance of neuron-glia interactions in the pathophysiology of neurological illnesses. In this context, it is required to improve our understanding of disease mechanisms and create potential novel therapeutics. According to research, synaptic malfunction is one of the features of various mental diseases, and glial cells are acting as key ingredients not only in synapse formation, growth, and plasticity but also in neuroinflammation and synaptic homeostasis which creates critical physiological capacity in the focused sensory system. The goal of this review article is to elaborate state-of-the-art information on a few glial cell types situated in the central nervous system (CNS) and highlight their role in the onset and progression of neurological disorders.
Illness-related suffering and need for palliative care in Rohingya refugees and caregivers in Bangladesh: A cross-sectional study
Despite recognition that palliative care is an essential component of any humanitarian response, serious illness-related suffering continues to be pervasive in these settings. There is very limited evidence about the need for palliative care and symptom relief to guide the implementation of programs to alleviate the burden of serious illness-related suffering in these settings. A basic package of essential medications and supplies can provide pain relief and palliative care; however, the practical availability of these items has not been assessed. This study aimed to describe the illness-related suffering and need for palliative care in Rohingya refugees and caregivers in Bangladesh. Between November 20 and 24, 2017, we conducted a cross-sectional study of individuals with serious health problems (n = 156, 53% male) and caregivers (n = 155, 69% female) living in Rohingya refugee camps in Bangladesh, using convenience sampling to recruit participants at the community level (i.e., going house to house to identify eligible individuals). The serious health problems, recent healthcare experiences, need for medications and medical supplies, and basic needs of participants were explored through interviews with trained Rohingya community members, using an interview guide that had been piloted with Rohingya individuals to ensure it reflected the specificities of their refugee experience and culture. The most common diagnoses were significant physical disabilities (n = 100, 64.1%), treatment-resistant tuberculosis (TB) (n = 32, 20.5%), cancer (n = 15, 9.6%), and HIV infection (n = 3, 1.9%). Many individuals with serious health problems were experiencing significant pain (62%, n = 96), and pain treatments were largely ineffective (70%, n = 58). The average age was 44.8 years (range 2-100 years) for those with serious health problems and 34.9 years (range 8-75 years) for caregivers. Caregivers reported providing an average of 13.8 hours of care per day. Sleep difficulties (87.1%, n = 108), lack of appetite (58.1%, n = 72), and lack of pleasure in life (53.2%, n = 66) were the most commonly reported problems related to the caregiving role. The main limitations of this study were the use of convenience sampling and closed-ended interview questioning. In this study we found that many individuals with serious health problems experienced significant physical, emotional, and social suffering due to a lack of access to pain and symptom relief and other essential components of palliative care. Humanitarian responses should develop and incorporate palliative care and symptom relief strategies that address the needs of all people with serious illness-related suffering and their caregivers.
Natural Bioactive Molecules: An Alternative Approach to the Treatment and Control of COVID-19
Several coronaviruses (CoVs) have been associated with serious health hazards in recent decades, resulting in the deaths of thousands around the globe. The recent coronavirus pandemic has emphasized the importance of discovering novel and effective antiviral medicines as quickly as possible to prevent more loss of human lives. Positive-sense RNA viruses with group spikes protruding from their surfaces and an abnormally large RNA genome enclose CoVs. CoVs have already been related to a range of respiratory infectious diseases possibly fatal to humans, such as MERS, SARS, and the current COVID-19 outbreak. As a result, effective prevention, treatment, and medications against human coronavirus (HCoV) is urgently needed. In recent years, many natural substances have been discovered with a variety of biological significance, including antiviral properties. Throughout this work, we reviewed a wide range of natural substances that interrupt the life cycles for MERS and SARS, as well as their potential application in the treatment of COVID-19.
Palliative care needs and health seeking behavior of people in two rural districts of nepal: a house-to-house survey
Background Palliative care is integral to Universal Health Coverage. In Nepal 80% of the population live in rural often mountainous areas. Despite a national commitment to palliative care close to home, services in rural areas remain underdeveloped. We report on the palliative care needs and health seeking behaviour in two rural districts. Methods A cross-sectional house to house survey was conducted across four municipalities in two rural districts. People in need of palliative care were identified using the general indicators of Supportive and Palliative Care Indicators Tool for Low Income Settings (SPICT-LIS) and recruited. They were questioned about diagnosis and health seeking behaviour and also about symptoms and quality of life using the Nepali Palliative Care Outcome Scale (N-POS). For those unable to respond, the main carer was asked to provide data. Data were analyzed descriptively using MS Excel 2016. Results A total of 587 households with 2320 residents were surveyed. Fifty-eight (2.5%) were identified as needing palliative care, ranging from 0.8 to 3.4% between municipalities. Only one child was identified and excluded from this study because their needs are not comparable with adults. The median age of those requiring palliative care was 79 years (Range 21–98); 50% were female. Forty-four/58(76%) had a diagnosis of at least one chronic illness, including hypertension and diabetes, but in only 32/58 (55%) was this a condition potentially leading to palliative care need. Difficulty in walking (58/58;100%), weakness (56/58;97%) and pain (44/58;76%) were the most common physical complaints. Fifty-one/58(93%) reported at least one psychosocial or spiritual need. Of those with a chronic illness diagnosis, 25/44(57%) were receiving ongoing medical care: 16/25(64%) at tertiary hospitals and 5/25(20%) at a local community hospital. Although 41/58(71%) had enrolled in government health insurance, cost remained a commonly reported barrier to accessing health care. Conclusion Significant numbers of adults with palliative need in rural Nepal exist with a high burden of physical, psychosocial and spiritual suffering. Those receiving follow up mainly travel to distant tertiary hospitals with local services being underutilized. Despite health insurance, costs prevented access to health care. Our findings are informing the development of a rural palliative care model for Nepal.
Mechanical Behavior of Polysaccharide Based Biopolymer Synthesized from the Seed Kernel of Tamarindus Indica L
Biopolymer carboxymethyl tamarind seed kernel polysaccharide (CMTSP) was synthesized by the reaction of tamarind kernel powder (TKP) of L. with monochloroacetic acid by an improved method. The synthesis was conducted in presence of sodium hydroxide at optimized conditions of time, temperature, concentrations of TKP, MA, sodium hydroxide. Tamarind seed polysaccharide (TSP) was also extracted from TKP by boiling distilled water. The chemical structure of TKP, TSP and CMTSP were analyzed by the ATRFTIR. When TKP, TSP, and CMTSP’s comparative physico-mechanical properties were examined and compared, CMTSP performed better due to increase in viscosity, water solubility and tensile properties.
