Evaluation of neuromuscular blocker use in myasthenia gravis patients undergoing rapid sequence intubation
Myasthenia gravis (MG) pathophysiology and drug-drug interactions with cholinesterase inhibitors can make patients both relatively sensitive and resistant to neuromuscular blocking agents (NMBAs). NMBA dose adjustments in MG patients undergoing rapid sequence intubation (RSI) are described in anesthesia literature but not outside of the operating room (OR). This study sought to characterize NMBA use in MG patients undergoing RSI outside of the OR.
Retrospective, observational, multicenter study of MG patients undergoing RSI outside of the OR from January 2011–September 2023. Patient demographics, NMBA used and dose, and post-intubation monitoring were collected. Data are reported descriptively.
Forty-four cases were included; median age 74 years (IQR 59.8–81.0), 31.8% receiving pyridostigmine, 88.6% intubated for respiratory failure, 81.8% received rocuronium. Only 11 cases (25%) had an NMBA dose adjustment. No cases experienced inadequate paralysis. Of the 24 cases (54.5%) with monitoring within the expected NMBA recovery timeframe, 75% had documented recovery. Of these, only one case had an NMBA dose adjustment. Five cases (11.4%) experienced possible prolonged paralysis based on study definitions; one case with NMBA dose adjustment.
Most cases did not receive an NMBA dose adjustment for MG. There were limited neurologic and neuromuscular assessments available post-intubation. Cases of possible prolonged paralysis were identified. The necessity of adjusting NMBA doses in MG patients undergoing RSI remains unclear and warrants further study. Clinicians should be aware of the potential for unpredictable NMBA response in MG and need for timely monitoring and sedation and analgesia in the immediate post-intubation period.

Sexual selection leads to positive allometry but not sexual dimorphism in the expression of horn shape in the blue wildebeest, Connochaetes taurinus
Sexual selection is thought to be an important driver of adaptation, speciation and extinction. Empirically testing these predictions across macroevolutionary timescales first requires an understanding of the morphology of secondary sexual traits in extant taxa. We used three-dimensional geometric morphometrics to analyse a large sample of the skull of the blue wildebeest,
Connochaetes taurinus
, in which horns are found in both sexes but only used in intrasexual competition in males. We show that the horns fit several predictions of secondary sexual traits; overall skull shape is significantly correlated with size (R
2
= 0.38,
p
= 0.001), and the sexually selected horns show drastically higher growth rates and variation than any other skull element, supporting previous findings. We also find that despite showing significant sexual dimorphism in shape and size (R
2
= 0.21,
p
= 0.001), allometric growth trajectories of sexes are identical (R
2
= 0.01,
p
= 0.635) and dimorphism is not readily detectable without prior knowledge of sex, and is not possible when shape is corrected for size. Our results show that even with strong sexual selection operating in only one sex, the expression of secondary sexual traits may show characteristic and indistinguishable patterns of growth and variance in both sexes.

Reports from a Scholar's Life
Tryggve N. D. Mettinger, emeritus professor of Hebrew Bible at
Lund University, has long been one of the best known and most
admired voices in biblical studies. His eight (English-language)
books and countless articles, published in a career spanning over
four decades, have changed the field in many ways. Mettinger is
renowned not as an iconoclast, but as one working within mainstream
scholarship who is nevertheless willing to challenge cherished
ideas and who takes nothing for granted. For example, in one of his
earliest works, A Farewell to the Servant Songs -published
in its entirety in this volume-Mettinger, with his trademark
brevity and articulation, drew attention to the shaky ground on
which this consensus idea was built and called fellow scholars to
reexamine this notion taken for granted by so many for so long. For
Mettinger, the Bible is sacred literature, but in biblical
interpretation there are no sacred cows.
Reports from a Scholar's Life: Select Papers on the Hebrew
Bible collects 16 studies (one short monograph, twelve
articles, and three reviews), originally published between 1977 and
2008, into one volume, along with a new reflective essay. The
papers included provide not only Mettinger's most groundbreaking
publications, but also glimpses into several of the areas of study
that occupied the author. Mettinger's work ranged far and wide in
the Hebrew Bible, and here one finds examples of his contributions
to the study of, among other things:
• the notions of God, the Gottesbild , in ancient Israel
;
the theology of \"YHWH Sabaoth\" in the monarchic period ;
• the development of the story of David in 1-2 Samuel;
• aniconism in ancient Israel;
the motif of the \"dying and rising god\" in the ancient world ;•
narrative criticism of the book of Job;
• the development and structure of Second Isaiah
The entire volume is opened by the titular essay, published for
the first time here, \"Report from a Scholar's Life.\" This article
was originally delivered as the farewell address upon his
retirement from Lund University, and it provides a retrospective on
his entire life and career.

Online Instruction to Measure Axial Alignment with the Bonesetter App
Background and objectives: Alignment of the lower extremity is important when treating congenital deformities, fractures, and joint replacement. During the COVID-19 pandemic, AO North America offered an online course on deformity measurement and planning. The Bonesetter app is a deformity planning tool that is freely available online. The purpose of this study was to see how effective an online course was in teaching axial alignment measurement and to assess that skill using an online digital planning tool, the Bonesetter app. Materials and Methods: An online module on axial alignment was provided during the AONA osteotomy course as well as a tutorial on how to use an online digital planning tool (Bonesetter app). The tools within the Bonesetter app allow users to draw digital lines directly on the CT images and measure the exact angle between two planes. Participants in this study were directed to perform these measurements in four different cases that tested different variations of deformity. Results: The measurements were completed correctly in case 1 = 56%, case 2 = 61%, case 3 = 84%, and case 4 = 76%. The standard deviation of angular digital measurements between individuals was ±4.26 degrees. Measuring the angle directly vs. drawing angles to a horizontal line had smaller standard deviations per case (p < 0.005) and less incidents outside 1 standard deviation for each measurement. Errors in adding and subtracting were the most common errors, particularly in relation to femoral anteversion or retroversion. Conclusions: The online course successfully instructed a group of orthopedic surgeons to measure alignment and malalignment of lower limb axial deformities. The Bonesetter app helped participants to learn this skill and identify errors in measurement. The inability to differentiate between anteversion and retroversion of the femur is a common source of error when it occurs and should be a focus of instruction.

The Sitz im Leben of Esarhaddon's Apology
Since the publication of Hayim Tadmor's seminal 1983 essay, “Autobiographical Apology in the Royal Assyrian Literature,” scholars have almost universally accepted that the imminent nomination of Ashurbanipal as crown prince provided the impetus for the composition of Esarhaddon's apology (i 8–ii 11 of his Nineveh A inscription). In this article, I reassess this conclusion. I suggest instead that the defeat of the Assyrian army in Egypt in 674 BCE raised concerns about Esarhaddon's legitimacy, concerns fueled by his assumption of his murdered father's throne in dubious circumstances. Esarhaddon therefore commissioned the apology as one part of a greater propagandistic campaign to reinforce his legitimacy. I conclude by showing that although Tadmor's work is often cited to support connecting the apology's composition exclusively to Ashurbanipal's nomination, the eminent historian also considered the Egyptian debacle to have undermined Esarhaddon's position on the throne, providing him with impetus to commission the Nineveh A inscription.
